What Are Reactive Oxygen Species (ROS), and How Do They Affect Cancer Cells?

Reactive oxygen species (ROS) are reactive, oxygen-containing molecules made in normal cells. They can take part in cell signaling, but when their level overwhelms a cell’s defenses, the resulting oxidative stress can damage DNA, proteins, and cell membranes. In cancer, ROS can support tumor-cell growth and adaptation at some levels while contributing to cell damage and death at others. Their effect depends on the type, amount, location, and duration of ROS, as well as the tumor’s biology.

What ROS are—and what oxidative stress means

Reactive oxygen species is a collective term for oxygen-containing molecules that react readily with other molecules in a cell. They are part of ordinary cell biology, not substances found only in disease. Cells produce ROS, and the body also has antioxidant systems that help keep their activity in check. In that balanced setting, ROS can participate in signaling: they can influence how cells respond and which processes they carry out.

“Oxidative stress” is not another name for all ROS activity. It describes a state in which ROS are excessive relative to the cell’s ability to manage them. When that balance shifts far enough, ROS can injure important cellular components, including DNA, proteins, and membranes. The distinction matters: a cell can use ROS as signals without undergoing damaging oxidative stress, while sustained or excessive ROS can become harmful.

The National Cancer Institute explains that oxygen-containing free radicals are a common type of free radical produced in living tissue: “Free radicals that contain the element oxygen are the most common type of free radicals produced in living tissue.” Free radicals and ROS are related terms, but the useful point for this topic is that reactive oxygen chemistry is part of normal tissue biology as well as a possible source of cellular injury. The presence of ROS alone does not establish that a cell is being damaged.

Why ROS can have opposite effects in cancer

Cancer cells may produce more ROS than normal cells, in part in association with oncogenic signaling, altered metabolism, and tumor conditions such as limited oxygen or nutrients. Yet increased ROS does not automatically kill a cancer cell. Some ROS can alter signaling proteins and help sustain pathways linked to tumor-cell growth, survival, and adaptation. In that sense, ROS-related activity can assist tumor development.

The other side of the balance is that ROS accumulation can damage cell components and contribute to cell death. Whether ROS helps or harms a cancer cell therefore cannot be answered with a simple “good” or “bad.” A modest, localized signal may have a different effect from a large, persistent buildup. The relevant context includes which ROS are present, where they occur in the cell, how long they remain, the tumor’s origin and stage, and conditions in the surrounding environment.

This two-sided role is often described as the “two faces” of ROS in cancer. Reczek and Chandel’s 2017 review discusses how ROS may support tumor signaling while also creating the potential for oxidative damage. Cheung and Vousden’s 2022 review likewise emphasizes that effects vary with ROS type, level, location, persistence, tumor origin, environment, and stage. A general statement about ROS is not a prediction for every cancer or every patient.

How cancer cells manage their redox balance

To persist in the presence of ROS, cancer cells can strengthen antioxidant defenses and other systems that support redox balance. These defenses do not necessarily eliminate every ROS molecule. Rather, they help keep ROS within a range that can allow signaling while limiting damage severe enough to threaten the cell.

Reczek and Chandel describe NRF2-driven antioxidant defenses and systems involving superoxide dismutases, peroxiredoxins, glutathione peroxidases, and catalase. They also describe pathways that support NADPH and glutathione, which contribute to a cell’s ability to manage oxidative conditions. Taken together, these mechanisms help explain why a cancer cell may tolerate a ROS burden that could otherwise become harmful.

This balancing act creates a research question: if a tumor relies on antioxidant defenses to cope with ROS, could treatment push ROS higher or interfere with those defenses enough to expose a vulnerability? That possibility is a rationale for investigation, not proof that every tumor has the same weakness. A strategy that affects one tumor’s redox balance may not have the same effect in another.

How ROS relate to cancer treatment research

Some cancer treatments can raise ROS or oxidative stress, and ionizing radiation produces high levels of free radicals as part of its cell-killing mechanism. Researchers are also investigating ways to increase ROS directly or to weaken the antioxidant systems that help cancer cells manage it. The hoped-for effect is to shift the tumor’s redox balance toward damaging stress.

A 2024 review by Li and colleagues surveys ROS-related approaches across several areas of cancer treatment. It includes direct ROS-inducing agents, chemotherapy, targeted therapies, DNA-damage-response therapies, radiotherapy, immunotherapy, and emerging nanosensitizers. These are broad research categories, not a list of interchangeable treatments or evidence that each approach is established care.

The review characterizes many ROS-inducing strategies as promising but still early in development. It also notes challenges in selecting strategies and combinations and identifying biomarkers that can predict which tumors might respond. Mechanistic plausibility—an explanation for how a treatment might work—does not establish that it improves outcomes for a particular patient.

Research approach What it aims to change What the reviewed evidence establishes
Direct ROS-inducing agents Increase ROS or oxidative stress in cancer cells. Included in the 2024 review; many ROS-inducing strategies remain early in development.
Interference with antioxidant defenses Reduce a tumor cell’s capacity to manage ROS and maintain redox balance. A research rationale described in the 2017 review; the material does not establish a universal patient treatment.
Chemotherapy, targeted therapy, and DNA-damage-response therapy Investigate or exploit treatment effects involving ROS, oxidative stress, or cellular damage responses. Discussed as treatment areas in the 2024 review; no individual regimen or ranking is established here.
Radiotherapy Use ionizing radiation, which produces high levels of free radicals as part of its cell-killing mechanism. The radiation-related mechanism is described by the National Cancer Institute; it does not mean every cancer responds identically.
Immunotherapy and emerging nanosensitizers Explore additional ways to use or alter ROS-related effects in treatment. Included in the 2024 review; the evidence described does not establish these as general ROS-based treatment recommendations.

The table separates the research rationale from what can be concluded for care. It does not compare effectiveness, provide a treatment ranking, or show that deliberately raising ROS will selectively destroy cancer while sparing healthy cells. The effect of a ROS-modulating intervention depends on the tumor and its setting, and the reviewed material does not support choosing an individual treatment from ROS biology alone.

Why a ROS measurement cannot answer the whole question

A single ROS reading is not a universal measure of how dangerous a tumor is or how it will respond to treatment. The meaning of a measurement depends on which reactive species it reflects, where and when it was measured, and the tumor’s capacity to respond. A snapshot cannot by itself capture persistence, location, antioxidant defenses, or the biological context that shapes the effects of ROS.

For the same reason, one cannot infer that a tumor with more ROS is necessarily more vulnerable, or that a treatment that raises ROS will necessarily work. Cheung and Vousden’s 2022 review highlights this variability. The reviewed material does not establish a single ROS value that predicts prognosis or treatment response across cancers.

Antioxidant supplements and cancer: what the evidence does—and does not—say

Antioxidants occur naturally in the body and in foods, and some are sold as supplements. They can neutralize free radicals, but that general function does not show that taking a supplement prevents cancer or treats an existing cancer. A cell’s ROS biology is more complex than simply removing all reactive molecules: ROS also have signaling roles, and cancer cells can depend on antioxidant defenses to tolerate their own oxidative conditions.

The National Cancer Institute’s page on antioxidants and cancer prevention reports mixed results from observational studies of antioxidant supplements and cancer risk. Observational results do not establish that a supplement causes a particular outcome. Li and colleagues’ 2024 review says the evidence is insufficient to recommend routine antioxidant chemoprevention—that is, taking antioxidants routinely to prevent cancer.

These findings do not amount to advice to start or stop a supplement, and they do not resolve how a particular product might interact with an individual’s cancer treatment. Anyone receiving cancer care should discuss supplement use with their oncology team, which can consider the person’s treatment and circumstances.

How to interpret claims about ROS and cancer

When evaluating a claim that an intervention “targets ROS,” first identify what the claim actually means. It might refer to increasing ROS, interfering with antioxidant defenses, or using a treatment whose effects involve ROS. Those are different strategies. Then consider whether the evidence concerns a biological mechanism, early experimental work, or a treatment shown to benefit patients. The 2024 review’s characterization of many approaches as early-stage is an important distinction.

  • Ask what is being changed: Is the approach intended to increase ROS, weaken defenses, or alter a particular ROS-related process?
  • Ask which setting is discussed: Tumor type, stage, local conditions, and the surrounding environment can affect the result.
  • Check the kind of evidence: A plausible mechanism is not the same as demonstrated clinical benefit.
  • Be wary of universal claims: ROS biology does not establish that one supplement or treatment works for every cancer.
  • Keep personal decisions clinical: Questions about treatment or supplements belong with the treating oncology team.

Comparisons between approaches are most useful when they separate five questions: whether the strategy raises ROS or weakens antioxidant defenses; which ROS species and cellular location are involved; what tumor type, stage, and microenvironment are being studied; whether the evidence is mechanistic, preclinical, or clinical; and whether the approach is established care or an early-stage investigation. The reviewed material does not support ranking individual treatments for patients.

ROS matter in cancer because they sit at the intersection of signaling, cellular defense, and damage. Their presence is neither automatically harmful nor automatically useful to a tumor. Understanding which role dominates requires context, and research into manipulating that balance has not become a blanket treatment rule.

Frequently Asked Questions

Are reactive oxygen species always harmful?

No. ROS occur in normal cells and can participate in signaling. They become a potential source of injury when they are excessive relative to the cell’s defenses, causing oxidative stress that can damage DNA, proteins, and membranes.

Why might ROS help a cancer cell instead of killing it?

At moderate or localized levels, ROS can alter signaling proteins and support pathways associated with cancer-cell growth, survival, and adaptation. Cancer cells may also strengthen antioxidant systems to keep ROS within a tolerable range.

Can raising ROS treat cancer?

That is an area of research, not a general treatment recommendation. Some approaches aim to raise ROS or weaken antioxidant defenses, but many ROS-inducing strategies remain early in development. Effects depend on tumor biology and the treatment context.

Does a high ROS level mean a tumor will respond better to treatment?

Not by itself. A reading depends on the species measured, its location and timing, how persistent it is, and the tumor’s antioxidant defenses and environment. The reviewed sources do not establish a universal ROS value that predicts prognosis or response.

Do antioxidant supplements prevent or treat cancer?

The sources do not establish that they do. The NCI reports mixed observational results for supplements and cancer risk, and a 2024 review says evidence is insufficient to recommend routine antioxidant chemoprevention. Discuss supplements with an oncology team during cancer care.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Should I stop an antioxidant supplement during cancer treatment?

This research does not support a blanket instruction to start or stop a supplement, and it does not resolve individual treatment interactions. Tell your oncology team what you take so they can consider your specific treatment and circumstances.

The Bottom Line

ROS are normal parts of cell biology, but excessive ROS can cause oxidative stress and cellular damage. In cancer, ROS can also support signaling and tumor-cell survival, while antioxidant defenses help cancer cells manage that balance. Research is exploring ways to alter it, but ROS-targeting strategies are not a blanket treatment rule, and antioxidant supplements should not be treated as cancer prevention or therapy.
