For everyday stress, breathing exercises are the simplest place to start; heart-rate variability (HRV) biofeedback adds a heart-rate feedback signal that can guide practice. Vagus nerve stimulation (VNS) is a medical intervention, not another name for slow breathing, and noninvasive transcutaneous VNS (tVNS) remains experimental for mental health treatment according to the National Institute of Mental Health (NIMH). The evidence summarized here is indirect: no head-to-head trial comparing all three approaches for general stress was identified.
How the three approaches differ
Breathing exercises
Breathing exercises are behavioral practices that can be done without a stimulation device. They vary in pace, duration, guidance, and frequency, so findings across studies do not establish one universally effective routine.

HRV biofeedback
Heart-rate variability biofeedback combines breathing practice with heart-rate information, usually supplied by a sensor or other device. The feedback can help guide practice, but the evidence concerns biofeedback training as an intervention; it does not establish that a particular consumer wearable works or that a device is required to practice paced breathing. The National Center for Complementary and Integrative Health (NCCIH) describes biofeedback as using device-provided information to help a person learn to control certain body functions.

VNS means stimulating the vagus nerve. It can refer to an implanted device placed through surgery or to noninvasive stimulation through the skin, often called transcutaneous VNS (tVNS). Neither should be conflated with breathing exercises or generic wellness claims about “resetting” the vagus nerve. NIMH describes tVNS research for mental health as ongoing and experimental, with questions remaining about stimulation sites, parameters, and protocols.

What the evidence says about stress and anxiety
Breathing exercises
A systematic review by Morgan, Lengacher, and Seo included 19 studies of breathing interventions in adults. Twelve studies reported significant anxiety improvements and nine reported significant stress improvements. No adverse events were reported in the included studies, but the authors noted a shortage of large randomized controlled trials. The article appeared online August 16, 2024, and in the journal’s December 2025 issue; its findings cover varied interventions rather than a single prescribed technique. Read the review abstract on PubMed.
A separate 2024 systematic review examined 58 studies and classified 54 of 72 interventions as effective within its framework. The authors associated effective practices with avoiding fast-only pacing and sessions shorter than five minutes, and with human-guided training, multiple sessions, and continued practice. These are patterns identified across the reviewed literature, not a guarantee or universal prescription for an individual. Read the review on PubMed Central.

HRV biofeedback
A 2017 meta-analysis by Goessl, Curtiss, and Hofmann included 24 studies and 484 participants. It reported a Hedges’ g of 0.81 for within-group change from before to after training and 0.83 compared with a control condition. The authors described HRV biofeedback as promising while calling for more well-controlled studies. These pooled effects summarize study samples and should not be read as a prediction of an individual’s results. Read the meta-analysis on PubMed.
VNS and tVNS
The evidence cited here does not establish VNS or tVNS as a routine treatment for everyday stress. A review of auricular stimulation—stimulation delivered at the ear—focused on preoperative anxiety, not ordinary life stress. It included 15 studies and 1,603 patients; in eight sham-controlled trials with 701 patients, anxiety scores were lower with stimulation (standardized mean difference −0.72, 95% confidence interval −1.09 to −0.36; moderate certainty). The authors still called for further research. This clinical setting and outcome do not prove that an at-home tVNS product treats everyday stress. Read the review on PubMed.

Which option makes sense as a starting point?
| Approach | What it involves | Evidence relevant to stress | Equipment or medical involvement |
|---|---|---|---|
| Breathing exercises | Practicing a breathing pattern; methods and study routines vary. | Adult review found reported improvements in stress and anxiety, with limited large randomized trials. | No stimulation device is required. |
| HRV biofeedback | Breathing practice guided by heart-rate information. | Meta-analysis found promising pooled effects; more well-controlled studies are needed. | A sensor or other feedback device may be used; the cited evidence does not validate a specific consumer product. |
| VNS/tVNS | Nerve stimulation, either through an implanted device or noninvasively through the skin. | The cited evidence does not establish it as routine care for everyday stress; NIMH describes tVNS for mental health treatment as experimental. | Implanted VNS involves surgery. Noninvasive stimulation still requires attention to device, protocol, and medical context. |
For a low-equipment first step, breathing practice has the most direct fit with everyday stress among these options. HRV biofeedback may be useful if seeing heart-rate feedback helps you practice, but a wearable is optional rather than a proven prerequisite. VNS is a different category: consider it in the context of a specific medical indication and clinician guidance, not as a wellness substitute for breathing.
What VNS is—and is not—approved to treat
NIMH says implanted VNS has an FDA-approved use for certain adults with severe or recurrent depression lasting at least two years after at least four other treatments. NIMH also notes that findings on effectiveness are mixed. This is not an approval for ordinary stress. NIMH states that tVNS is not currently FDA-authorized as a depression treatment and describes research as ongoing; regulatory status may change. See NIMH’s overview of brain stimulation therapies for its current explanation.

Safety and when to stop
Relaxation techniques are generally considered safe for healthy people, but NCCIH notes that some people experience increased anxiety, intrusive thoughts, or fear of losing control. Rarely, relaxation techniques may worsen symptoms in people with epilepsy, certain psychiatric conditions, or a history of trauma. If a breathing or relaxation practice causes distress, stop and seek appropriate support. NCCIH’s safety overview provides further context.
NIMH lists possible complications of implanted VNS including infection, device movement or malfunction, voice changes, cough, breathing problems, difficulty swallowing, nausea, and other effects. Reported mild tVNS effects include tingling, pain or itching at the stimulation site, nausea or vomiting, and dizziness. Long-term side effects of VNS forms are unknown. These are general risks, not a personalized assessment; discuss a device or treatment with a qualified clinician.
