A Windows Defender alert that says Unable to remove, Remediation incomplete or asks for repeated restarts does not automatically prove that a Trojan is still running. Defender may have quarantined the file, removed one component but not another, detected a copy inside an archive, or left only a historical Protection History entry. Treat a recurring alert as potentially active until a fresh Full scan and, when needed, Microsoft Defender Offline scan find no additional malware.
Do not choose Allow or Restore unless you have verified the file and publisher. Follow this order: contain the computer, inspect the exact alert, update Windows and Defender, run a Full scan, escalate to Defender Offline, then use Microsoft Safety Scanner or the Malicious Software Removal Tool if necessary.

1. Contain the computer before troubleshooting
Disconnect Wi-Fi or unplug Ethernet immediately if the alert is severe or high, the computer is behaving unusually, ransomware or remote-access software is involved, or you entered passwords after the suspected infection. Do not use the affected PC for banking, email, work, cloud storage, social media or password-manager logins while active malware is possible.

- Do not open the suspected file again, add its path to Defender exclusions, or disable real-time protection to make the alert disappear.
- Photograph or copy the alert, including the complete threat name, status, timestamp and file path.
- Do not delete random Registry entries, System32 files, Defender quarantine folders or other system components based on an online post.
Removing a file does not undo passwords, cookies or access tokens that an information-stealing Trojan may already have copied.

2. Read what Defender actually did
Open Windows Security → Virus & threat protection → Protection history. On older Windows 10 builds the route may appear as Settings → Update & Security → Windows Security → Virus & threat protection → Threat history. Open the event and record the threat name, severity, date, status and affected path.
| Defender wording | What it generally means | What to do |
|---|---|---|
| Detected or action needed | A suspicious file or behavior was found; cleanup may not have completed. | Update protection intelligence and run a Full scan. |
| Quarantined | The item was moved to a restricted location and blocked from running. | Do not restore it. Delete the original download, archive or installer if it is still present. |
| Removed | Defender deleted the detected file. | Run a fresh scan to check for related components. |
| Partially removed | Some malware files were cleaned, while others may remain. | Escalate to Defender Offline and an additional on-demand scan. |
| Remediation incomplete or action failed | Defender could not finish an attempted cleanup, often because a file was locked, regenerated or otherwise unavailable. | Restart when requested, update Windows, then use Defender Offline. |
| Restart required | Cleanup needs to occur before Windows fully loads or after a locked file is released. | Save work and restart; do not assume the restart alone proves removal. |
| Protection History entry only | A record of an earlier event, not proof that the file is still on disk. | Check the current path and verify with a new scan. |
Microsoft explains quarantine, restore and partial removal in its Defender antivirus and antimalware FAQ. Restoring returns the file to its original location and can make it runnable again.
3. Update Windows and Defender
- Save work and close unnecessary applications.
- Install all available updates through Settings → Windows Update (or Settings → Update & Security → Windows Update on older Windows 10).
- Allow Microsoft Defender security-intelligence updates to install and restart if requested.
- In Windows Security, keep Cloud-delivered protection, Automatic sample submission and Real-time protection enabled where available.
- Recheck Protection history before scanning again.
Microsoft recommends updating Windows and Defender when repeated detection or removal errors occur. Defender also needs free disk space to quarantine or remove files; free space if the system drive is nearly full.

4. Run a Full scan
- Open Windows Security → Virus & threat protection → Scan options.
- Select Full scan and choose Scan now.
- Keep the PC plugged into power and leave it idle until the scan finishes.
- Restart and repeat the scan if Defender reports an error.
A Full scan can take a long time on a large disk or one containing many ZIP, ISO and other archives. Slowness alone does not show that Defender is broken. A result saying that no threats were found means only that this scan detected no additional malware.
5. Use Microsoft Defender Offline when the alert returns
If the same detection comes back after reboot, or cleanup fails while Windows is running, use the offline scanner:
- Open Windows Security → Virus & threat protection → Scan options.
- Select Microsoft Defender Offline scan → Scan now.
- Save all work. Windows will restart and scan from the Windows Recovery Environment.
Running outside the normal Windows session makes it harder for a rootkit, startup process, locked file or active malware component to interfere. Microsoft documents the feature in Virus and threat protection in the Windows Security app.

Offline scanning is not an absolute guarantee. It may fail or behave differently if the Windows Recovery Environment is disabled or damaged; see Microsoft’s Defender Offline documentation. Have a BitLocker recovery key available because a security-related restart can trigger a recovery prompt.
6. Add Microsoft’s on-demand scanners
Microsoft Safety Scanner
Download a fresh copy from Microsoft and run it after updating Defender. It is an on-demand second check, not a replacement for real-time protection. Do not rely on an old installer or an unofficial mirror.

7. Use the file path to find the source
- Downloads or Desktop: Usually an installer, crack, document or archive. Delete the entire untrusted package.
- Browser cache: May be a blocked web download rather than an installed Trojan.
- Email attachment: Remove the attachment and its downloaded copy.
- ZIP, ISO or another archive: A malicious member may never have executed; delete the complete archive unless independently verified.
- AppData or Temp: Can indicate a script, updater or program recreating the file.
- Startup folder, scheduled task, service or Run key: More concerning when the file returns after every reboot. Investigate only with the exact path as a lead.
- Defender quarantine: The item is already contained; never browse into protected quarantine storage.
- Restore point or backup: A historical copy can be detected without being active, but do not restore it blindly.
8. If the same Trojan keeps returning
- Record the exact path and timestamp for each new event; the same name can refer to a newly created file.
- Disconnect USB drives, network shares and cloud-sync folders temporarily.
- Remove the original installer, archive, suspicious application or browser extension that could be supplying the file.
- Run Defender Offline, then Microsoft Safety Scanner or MSRT.
- Review startup entries and scheduled tasks only when the detection path gives you a concrete, identifiable lead.
- If detections continue, preserve logs and seek qualified incident-response or IT help instead of deleting system files at random.
Microsoft warns that another undetected component can silently reinstall a detected threat and recommends Offline scanning for recurring malware. Its troubleshooting guidance is at Troubleshoot problems with detecting and removing malware.

9. Handling a possible false positive
Never click Allow merely to clear an alert. Verify that the file came from the official publisher, check its digital signature and compare the download source with the publisher’s site. If it is legitimate, report the suspected false positive to Microsoft or the software vendor. Uploading a proprietary document or business file to a public scanner can expose confidential data.

Permanent exclusions and temporarily disabling Defender increase risk and should not be the default solution. A broad detection name such as Trojan:Win32/... is a classification, not proof of one unique malware family; the complete name, path and behavior matter.
10. Protect accounts separately
From a different clean device, change the email password first, then passwords for banking, work, cloud storage, password managers and social accounts. Revoke active sessions or refresh tokens where each service allows it, enable multifactor authentication, and contact your bank or employer if sensitive credentials may have been exposed. Preserve suspicious messages, filenames, timestamps and alerts for investigation.
11. When resetting or reinstalling Windows is safer
Choose a reset or clean reinstall when Offline scanning and independent checks continue to find persistent malware; Defender has been disabled or tampered with; there are signs of a rootkit, bootkit, ransomware or remote-access compromise; system settings or accounts were materially altered; or you need a high-confidence clean state for financial, administrative or business work.
- Back up irreplaceable documents and photos, not cracked software, executables, scripts, unknown installers or entire application folders.
- Scan the backup from a clean computer.
- Gather activation, BitLocker, application and account-recovery information.
- After reinstalling, change passwords from a clean device and restore only verified personal files.
Microsoft’s removal guidance covers backing up and reinstalling Windows at its malware-removal troubleshooting page.
12. When to get professional help
Stop experimenting and contact your organization’s IT team or a qualified incident-response professional if the PC handles business or regulated data, ransomware or remote access is suspected, security tools are being disabled, accounts show unauthorized activity, or detections persist after Offline scanning and additional Microsoft tools. A business machine may need isolation, evidence preservation and centralized investigation rather than ad hoc cleanup.
The Bottom Line
A Defender “unable to remove” alert is a warning to verify, not automatic proof of an active Trojan. Do not allow or restore the item; inspect Protection History, update Defender, run a Full scan, escalate to Defender Offline when the alert returns, and protect your accounts from a separate clean device. Persistent or high-impact compromise justifies a carefully prepared Windows reset or reinstall.
