Stop Trusting Autonomous AI Agents Blindly: Why We Need Deterministic Firewalls

AI agents with tool access need an independent policy boundary that checks proposed actions before execution. Here’s what deterministic firewalls can—and can’t—protect against.

AI agents that can use tools need a security boundary outside the model: before a proposed action reaches an email account, database, browser, or other system, an independent enforcement point should check it against explicit policy. A system prompt or a model’s explanation is not authorization. This deterministic boundary can limit what an agent is allowed to do, but it cannot guarantee that every permitted action is safe or solve prompt injection on its own.

Why can an agent turn untrusted content into a security risk?

Instructions can arrive inside ordinary task data

An agent may read an email, document, or webpage and then act through connected tools. If that content contains malicious instructions, it can try to redirect the agent—for example, from summarizing a message to searching for sensitive information and forwarding it. NIST’s Center for AI Standards and Innovation (CAISI) calls this agent hijacking: indirect prompt injection in which malicious instructions are placed in data an agent may ingest, leading it to take unintended harmful actions.

The underlying problem is that the agent processes both trusted instructions and untrusted language. Untrusted tool output must therefore be treated as data, not as authority to change the agent’s permissions or goals. In its January 17, 2025 technical blog, CAISI described agent hijacking as a vulnerability affecting many agents; that warning identifies a risk, not a claim that every agent will be compromised in every deployment.

The threat is broader than an attacker’s prompt

Security problems can arise from indirect prompt injection, insecure models, or harmful actions that occur without adversarial input. NIST/CAISI’s January 12, 2026 request for information on securing AI agent systems asked for input on threats and mitigations, cybersecurity approaches, measurement, and ways to constrain and monitor agent access. Its comment period ended March 9, 2026. The broader lesson is that security cannot depend on detecting a malicious prompt: an agent may make a risky decision even when no attacker has supplied one.

What does a deterministic firewall do?

Here, a deterministic firewall means a logically separate enforcement point that intercepts a proposed action and checks it against explicit rules before execution. It might sit in a gateway or execution component between the model and its tools. The model can propose an action; the enforcement point decides whether that exact action is authorized under the applicable policy.

A useful policy check can consider the requested tool or function, the resource being accessed, normalized action parameters, the privilege scope, and whether any required human approval is present. For example, a rule might allow an agent to read a particular folder but block it from sending mail, or require a person to approve a transfer to a named recipient for a specific amount. Approval should apply to the actual action—including its target and parameters—not serve as a generic permission slip for later actions.

OWASP’s 2025 Excessive Agency guidance puts the principle plainly: “Implement authorization in downstream systems rather than relying on an LLM to decide if an action is allowed or not.” The enforcement decision belongs outside the model because the model is the component making the proposal, not a trustworthy authority for granting itself permission.

How should teams put the boundary into practice?

  1. Restrict access before relying on a filter

Give each agent only the tools and permissions its task requires. Prefer read-only access when it is sufficient, and remove capabilities—such as sending email—when they are not needed. This limits the damage available to an agent even if it is manipulated or behaves unexpectedly.

  2. Check every action at the execution boundary

Route tool calls and downstream requests through a separate policy service or execution component. Validate the tool, resource, parameters, and privilege scope before execution. Do not treat a system prompt, the agent’s reasoning, or its statement that an action is safe as proof of authorization. Ensure every connector and execution path is covered; an unguarded path can bypass the policy.

  3. Require specific approval for high-impact actions

Use human approval for actions that are externally visible, irreversible, financial, or administrative when the risk warrants it. Show the reviewer what will happen and bind the approval to the specific tool, target, and parameters. For email, a safer pattern may be to let the agent draft a message and have the user review and send it.

  4. Monitor and constrain activity over time

Keep audit records of proposals, policy decisions, approvals, and executed actions. Monitoring can help detect suspicious patterns; rate limits can reduce how quickly unwanted actions occur; replay protection can prevent a previously approved request from being reused improperly. These controls support enforcement but do not replace checking authorization for each action.

  5. Test the agent and policy as they change

Run adversarial and regression tests when models, prompts, tools, connectors, or policies change. Include task-specific checks as well as aggregate measures: a good overall score can hide failure on a particular workflow. NIST/CAISI recommends adapting evaluations as systems and attacks evolve, rather than assuming one test remains representative.

What do the evaluations show—and what do they not show?

In a 2025 CAISI evaluation, model-specific red-team attacks raised the measured attack success rate from 11% for the strongest baseline attack to 81% for the strongest new attack. These figures came from a held-out set of user tasks in AgentDojo’s Workspace environment, using agents powered by the upgraded Claude 3.5 Sonnet described in CAISI’s article. They are results from that evaluation, not estimates of real-world attack prevalence and not a rate that can be applied to every agent.

CAISI also added evaluation tasks involving remote code execution, database exfiltration, and automated phishing, and reported frequently inducing the agent to follow malicious instructions in these risk areas. That finding makes a case for testing the actual tasks and tools an agent uses; it does not establish that all agents are vulnerable in all deployments.

Why isn’t a deterministic firewall enough?

Explicit rules are well suited to clear authorization questions: may this agent call this function, against this resource, with these parameters, under this privilege scope? They are less suited to deciding every semantic question about whether an otherwise permitted action is wise or safe. A rule that allows access to a mailbox, for instance, does not by itself establish that a particular message should be sent.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A firewall also only protects actions that pass through it, and its rules can be incomplete or incorrectly configured. It does not ensure that the model reasoned correctly, remove weaknesses in the connected software, or make untrusted content trustworthy. Authentication and ordinary software security—including protection against memory-management bugs—remain relevant alongside prompt-injection defenses.

Control What it contributes What it does not replace
Deterministic policy enforcement Checks proposed actions against explicit permissions and required approvals before execution. Semantic judgment about every task or protection for paths that bypass the enforcement point.
Least privilege and read-only access Reduces the actions and data available to an agent, limiting potential impact. Action-by-action policy checks where access is still permitted.
Human review Lets a person assess selected high-impact actions before they occur. Automated enforcement for every action, or informed approval if the reviewer cannot see the exact action.
Monitoring, audit, and rate limits Help identify activity, support investigation, and constrain the pace of actions. Authorization decisions before each action executes.
Adversarial and regression testing Finds failures in specific tasks and helps reveal regressions as systems change. A guarantee that future attacks or changed deployments will be safe.

These controls work best as layers: identity and authorization define who or what may act; a policy boundary checks proposed actions; sandboxing can constrain execution; human approval can gate consequential actions; monitoring and audit make behavior visible; and repeated testing probes for failures. Meta’s LlamaFirewall illustrates a layered guardrail approach that combines prompt-attack detection, experimental reasoning checks, and code analysis. Its existence is an example of combining defenses, not proof that any one layer is sufficient.

Is a deterministic firewall now a NIST requirement?

No universal mandatory requirement is established by the cited NIST material. NIST’s National Cybersecurity Center of Excellence (NCCoE) summarized public comments on a concept paper that supported deterministic policy and enforcement, sometimes with probabilistic capabilities layered on for context. Commenters commonly proposed a separate governance component or gateway, while the summary also recorded open questions about metadata and architecture. This describes positions in public comments, not a finalized NIST mandate.

NIST’s AI Agent Standards Initiative, updated August 14, 2026, describes ongoing work on voluntary guidelines, interoperability, and research into agent authentication, identity, and security evaluation. It is active standards work, not a settled universal specification for deterministic firewalls. For now, teams should assess a design by where enforcement occurs, which policy facts it checks, whether all tool paths are covered, how it handles least privilege and approval, the quality of monitoring and audit, how it is tested against adaptive attacks, and its operational effects such as latency, false blocks, and policy maintenance.
