The Silent Hill: Townfall pharmacy locker code is 6539. Enter it on the rusted four-digit locker in the pharmacy’s back room after registering the CRTV broadcast clue sequence.
How to reach the pharmacy locker
- In the Clinic’s Z Ellis room, take the Scalpel from the green medical case beside the computer.
- Use the Scalpel on the covered hatch in the pharmacy’s rusted exterior door to enter.
- Find the rusted four-digit locker in the back room.
The Scalpel opens the pharmacy entrance; it is not part of the locker combination.

How the pharmacy locker code works
Tune the pharmacy CRTV until the signal is clear, then let its broadcast play. It shows four poster fragments in a specific order. Match them to the full posters around the pharmacy, and use the broadcast order—not the order in which you visit the posters—to enter the digits.
- 6: The hand-washing poster in the bathroom shows three fingers on each illustrated hand: six total.
- 5: The prescription safety poster near the phone and window has five steps.
- 3: The nurse consultation poster near the front window indicates a blood-pressure gauge value of three. Use the gauge, not the nearby clock or another number.
- 9: The antibiotic poster on the noticeboard by the lockers shows a blister pack with one pill missing, leaving nine.
Enter the digits left to right as 6539.
If 6539 does not open the locker
- Check that you are using the rusted four-digit locker in the back room, not a pharmacy cabinet or the entrance hatch.
- Tune and store the CRTV broadcast, then allow the clue sequence to play. Walkthroughs report that the clues may need to be registered before the code works, even if you already know it.
- Use the broadcast’s order for the four clues rather than the order you found the posters.
- After the door opens, interact with the locker again to collect the item inside.
Guides published or updated September 21–25, 2026, agree on this solution. They do not establish whether the code varies by save, difficulty, or platform, so follow the current in-game clues if a later patch changes the puzzle.

The locker contains the Prescription Dosage Error Note. It is one of three items for the Clinic scales and belongs in the middle position; the other items are Zoe’s Lanyard and the Broken Stethoscope.
