Raman Spectroscopy: Key Facts on Cancer Testing Near the Bedside

Raman spectroscopy can reveal biochemical differences in tissue, but bedside use spans several developing workflows and does not yet amount to a universal cancer diagnosis.

Raman spectroscopy can detect biochemical differences in tissue that may help researchers distinguish cancerous from noncancerous tissue, but it is not a routine, universally available bedside cancer diagnosis. “Bedside” may mean testing a fresh biopsy near an operating room, touching tissue with a probe during surgery, or rapidly imaging tissue and analyzing it with software. These are distinct workflows, and their clinical readiness varies.

What does Raman spectroscopy measure?

Raman spectroscopy illuminates a sample and measures light scattered back at shifted frequencies. Those shifts correspond to molecular vibrations, creating a spectrum associated with the sample’s chemical composition. In tissue, the signal can reflect proteins, lipids, nucleic acids and carbohydrates. Researchers analyze spectral patterns, often with statistical or machine-learning methods, to look for differences between tissue states.

The spectrum is a biochemical fingerprint, not a direct picture of cancer cells. The method is described as label-free, but interpreting the signal still depends on how measurements are collected and analyzed. A review of Raman spectroscopy in cancer research and a review of clinical translation discuss these principles and challenges.

What can “bedside” Raman testing mean?

The term covers different ways of examining tissue. A fresh specimen measured outside the body is not the same as a probe used on a patient during surgery, and neither is identical to Raman-based microscopy that produces images. Results from one implementation should not be assumed to apply to another.

Workflow What is measured Potential role under study
Ex vivo biopsy analysis A fresh tissue sample, measured outside the body, potentially near the operating room. Rapid tissue characterization to inform a procedure; it does not by itself establish that a diagnosis can replace pathology.
In vivo probe A fiber-optic probe placed against accessible tissue during a procedure. Interrogating tissue in place, including possible characterization or surgical-margin guidance. Access and sampling are limited by where the probe can reach.
Stimulated Raman histology (SRH) Raman-based microscopy that creates images of tissue, with computational interpretation in some research workflows. Rapid assessment of brain tumor tissue during surgery. It is a distinct imaging approach, not simply a probe test.
Other developing platforms Research includes Raman microscopy, biopsy-needle guidance and endoscopic Raman approaches. Potential ways to examine tissue during neuro-oncology procedures; intended use and performance depend on the specific system and study.

Reviews describe both fresh-tissue measurements and in vivo fiber-optic probes, including probes intended for accessible surfaces or intraoperative use. A review of fiber-optic Raman probes and a 2026 review of Raman spectroscopy in neuro-oncology describe these as developing clinical workflows, not one standardized test.

Can it help diagnose cancer during surgery?

It is being investigated for that purpose, especially as a way to characterize tissue quickly or support decisions about surgical margins. The NCI described research combining SRH with artificial intelligence to assess brain tumor tissue during surgery. In that context, Daniel Orringer, M.D., called the technology encouraging for some patients undergoing repeat surgery, while Kareem Zaghloul, M.D., Ph.D., said it could help inform how aggressive or conservative surgery needs to be. These comments describe the promise of a research approach; they are not evidence that it has become routine or that it can independently direct every operation. The NCI report describes the work.

How accurate is Raman spectroscopy for cancer?

There is no single accuracy figure that can be applied to “Raman spectroscopy for cancer.” The evidence summarized in the clinical reviews does not establish a pooled, transferable figure for bedside diagnosis, performance across cancers, or universal effectiveness. A study result only makes sense in the context of the cancer and tissue examined, instrument, sampling method, classification approach, patient cohort and reference standard used to check the result.

When assessing a claim about a particular system, look for the following details:

  • Which cancer and tissue: performance in one tissue type does not establish performance in another.
  • How it was used: an ex vivo specimen, an in vivo point probe and an SRH image represent different tests.
  • What and how much was sampled: acquisition speed, sampled area, depth and tissue accessibility affect what the measurement can represent.
  • How results were classified: algorithms need robust training and validation; a model’s result should be tied to the data and reference standard used in its study.
  • What decision it is meant to support: research characterization or adjunctive guidance is not the same as a stand-alone diagnostic decision.

Reviews note practical obstacles including weak Raman signals, point-by-point measurement in conventional approaches, subtle spectral differences and interference from fluorescence. They also emphasize the need for algorithms trained and validated on large, independent measurement sets. A review of intraoperative vibrational spectroscopy and a clinical translation review discuss these limitations.

Can Raman spectroscopy replace a biopsy or pathology?

The evidence described here does not support treating Raman spectroscopy as a general replacement for biopsy or pathology. It is being studied as a way to characterize tissue and potentially add information during a procedure. Whether a specific Raman system can support a particular clinical decision depends on the exact device, intended use, cancer type and clinical evidence. A rapid signal or classification should not be mistaken for a universally validated cancer diagnosis.

Is Raman cancer testing available at the bedside?

Availability cannot be inferred from the phrase “bedside” or from promising research. The workflows described in reviews remain developing or investigational, and availability may depend on the institution, procedure and specific system. Regulatory status is also device- and indication-specific. The FDA’s list of authorized companion diagnostics is not a comprehensive database of Raman devices, so it cannot establish the status of every Raman system. For a named device, check its exact model, intended indication and authorization in the relevant country with the treating institution or regulator.
