Nitrosamine contamination is a drug-quality problem in which compounds such as N-nitrosodimethylamine (NDMA), N-nitrosodiethylamine (NDEA) and N-nitroso-N-methyl-4-aminobutyric acid (NMBA) can appear as manufacturing or storage impurities. The modern scandal began when regulators found NDMA in valsartan in July 2018, then expanded to other blood-pressure drugs, heartburn medicines and several additional drug classes. Affected lots have been tested, held or recalled when concentrations exceeded the applicable acceptable intake.
For patients, the practical rule is straightforward: do not stop a prescription on your own. Check the exact product, strength, manufacturer and lot with a pharmacist or regulator, and arrange a clinically suitable replacement if that specific lot is affected. Nitrosamine limits are risk-management thresholds, not proof that every batch is contaminated or that exposure below a limit is absolutely harmless.

What the nitrosamine contamination scandal is
Nitrosamines are a family of chemical compounds that can occur in food, drinking water and medicines. Some, including NDMA and NDEA, are classified by the World Health Organization and the International Agency for Research on Cancer as probably carcinogenic to humans and part of the “cohort of concern” because they can damage DNA through mutagenic mechanisms.

The scandal concerns unintended impurities in medicines, not an ingredient deliberately added for therapeutic effect. A product can meet its specification for the active ingredient and still require investigation if a nitrosamine forms during synthesis, appears in a raw material, is introduced by a process change, or develops while the product is packaged or stored. The cause is product-specific; there is no single contamination pathway or universal test result for an entire drug class.

FDA explains that long-term exposure above a nitrosamine’s acceptable daily intake (AI) may increase cancer risk. Taking a medicine at or below its AI every day for 70 years is not expected to increase cancer risk, although an AI is a conservative risk-management threshold rather than a guarantee of zero risk.
How the issue unfolded
| Date | Development | Why it mattered |
|---|---|---|
| July 2018 | Regulators became aware of NDMA in products containing valsartan, according to the WHO information note. | The finding triggered testing of other angiotensin-receptor blockers (ARBs) and their manufacturing routes. |
| 2018–2020 | Investigations identified additional nitrosamines, including NDEA, NDIPA, NEIPA and NMBA, in some sartan products and expanded to ranitidine, nizatidine, metformin, rifampin, rifapentine and other medicines. | Authorities moved from a single-product investigation to class-wide risk assessments and supply-chain reviews. |
| September 2024 | FDA issued revised final guidance, “Control of Nitrosamine Impurities in Human Drugs.” | The guidance sets out expectations for root-cause analysis, risk assessment, validated testing, confirmatory testing, controls, mitigation and alternative bioequivalence approaches. |
| Current oversight | FDA continues to evaluate products, while EMA’s Article 5(3) framework requires marketing-authorisation holders to assess risk, test products at risk and report confirmed findings promptly. | Recall and mitigation decisions remain specific to a product, lot, country and measured concentration. |
Which medicines and impurities were involved?
The presence of a nitrosamine in one manufacturer’s process or one lot does not mean that every batch, manufacturer or medicine with the same active ingredient is contaminated. The relevant comparison is always the measured impurity against the AI for that impurity and medicine.
| Medicine or class investigated | Impurities reported in the investigations | Important qualification |
|---|---|---|
| Valsartan and other sartans (ARBs) | NDMA, NDEA, NDIPA, NEIPA and NMBA | Findings varied by active-ingredient source, manufacturing route and lot; not every ARB batch was affected. |
| Ranitidine and nizatidine | NDMA | FDA laboratory testing found NDMA in tested active pharmaceutical ingredients and finished products; results do not establish that every historical or current lot has the same level. |
| Metformin | NDMA in some investigations | Testing and disposition were product- and lot-specific. |
| Rifampin and rifapentine | Nitrosamine impurities identified during broader investigations | Patients should rely on current national notices for the exact product and lot. |
| Other medicines | Potential small-molecule nitrosamines or nitrosamine drug substance-related impurities (NDSRIs) | FDA’s ongoing overview and later testing can add products as new evidence emerges. |
What happened with ranitidine
FDA’s laboratory page reports that NDMA was present in all ranitidine samples tested in that laboratory evaluation. FDA’s stated ranitidine reference is an acceptable daily intake of 0.096 micrograms (96 nanograms) per day, corresponding to 0.32 parts per million (ppm), on the agency page current in 2026. That figure applies to the ranitidine assessment; acceptable intakes differ among nitrosamines and medicines.

Ranitidine illustrates why a finding needs context. Investigators considered both impurity present in supplied active ingredient and the possibility that NDMA could increase under certain storage conditions. A result for a tested sample cannot be generalized to every product in every country without lot-level evidence.
Why nitrosamines can form in medicines
FDA separates conventional, small-molecule nitrosamine impurities from NDSRIs. Small-molecule impurities are not structurally part of the active ingredient and may arise from reagents, solvents, catalysts, contaminated raw materials or recycled processing streams. NDSRIs share structural features with the active pharmaceutical ingredient and are often unique to that molecule.
Investigators examine the entire product life cycle:

- Manufacturing chemistry: Nitrite sources can react with amines under particular conditions, creating a nitrosamine if the process is not adequately controlled.
- Raw materials and intermediates: An upstream supplier may introduce a nitrosating agent or an already-formed impurity.
- Process changes: A change in solvent, catalyst, temperature, recovery step, equipment or supplier can alter impurity formation even when the finished formulation appears unchanged.
- Packaging and storage: Container materials, excipients, humidity, heat and time can affect whether an impurity forms or increases after manufacture.
- Drug-specific reactions: The structure of an API or its degradants can create an NDSRI pathway that does not exist for another medicine.
Because these pathways differ, a regulator may require a manufacturer to change a reaction, qualify a new raw-material source, alter packaging, set a tighter process control or add routine release testing rather than withdraw an entire class of medicines.
How acceptable-intake limits work
An acceptable intake converts toxicology into a daily exposure target, expressed as a mass per day, a concentration such as ppm, or both. Regulators compare the expected patient exposure with that target and consider dose, duration, maximum daily use and the medicine’s clinical importance. The calculation is specific to the nitrosamine and product; one compound’s limit cannot be transferred automatically to another.
When compound-specific carcinogenicity data are limited, EMA describes three principal approaches:

- Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft
- Carcinogenic potency categorisation approach (CPCA): Structural features place a nitrosamine in a potency category from which an AI can be derived.
- Read-across and structure–activity assessment: Data from chemically related compounds inform a scientifically justified limit.
- Enhanced Ames testing or in-vivo data: Additional genotoxicity or animal evidence can refine the estimate when standard information is insufficient.
These methods are deliberately conservative. Staying below an AI means the modeled lifetime cancer risk is considered acceptably low under the regulator’s assumptions; it does not certify a batch as “risk-free,” and it does not authorize a patient to exceed the labeled dose.
How regulators and manufacturers detect and control impurities
FDA’s 2024 guidance calls for a documented risk assessment, validated analytical methods, confirmatory testing, investigation of the root cause, preventive controls and mitigation. A typical sequence is:

- Test with a fit-for-purpose method: FDA’s methods work includes liquid chromatography–high-resolution mass spectrometry (LC-HRMS), liquid chromatography–tandem mass spectrometry (LC-MS/MS) and other methods for ARBs, ranitidine, metformin, varenicline, propranolol and bumetanide.
- Confirm unexpected results: Laboratories verify identity and concentration with an orthogonal or confirmatory procedure before a regulatory decision.
- Compare exposure with the applicable AI: The calculation uses the highest labeled daily dose and the measured impurity, not simply whether a signal was detected.
- Implement mitigation: Manufacturers may alter chemistry, suppliers, specifications, packaging, storage instructions or batch-release testing.
- Notify authorities and monitor: Confirmed findings are reported, affected distribution is held or recalled when warranted, and follow-up batches are tested to show that controls work.
EMA’s Article 5(3) framework similarly places the initial risk assessment and testing obligation on marketing-authorisation holders and requires prompt reporting of confirmed findings. This makes contamination control an ongoing quality-system responsibility rather than a one-time recall exercise.

What patients should do if a medicine may be affected
- Keep taking the medicine unless a clinician tells you otherwise. FDA’s patient advice is that people taking medicines with potential nitrosamine impurities should not stop treatment on their own. Abruptly stopping an antihypertensive, diabetes medicine or other chronic therapy can create a more immediate danger than the modeled impurity risk.
- Identify the exact product. Record the active ingredient, strength, dosage form, manufacturer, pharmacy, prescription number and lot or batch number. A recall may cover only selected lots, strengths or countries.
- Ask a pharmacist or prescriber to check current notices. They can compare your lot with the latest FDA, EMA or national-regulator communication and determine whether an alternative is clinically appropriate.
- Do not substitute by dose or appearance. Different products with the same active ingredient may have different release characteristics, excipients, manufacturing sources or impurity results. Use only the replacement and dose prescribed for you.
- Follow recall instructions. If a lot is recalled, the pharmacy or regulator will explain whether to return it, obtain a replacement, continue temporarily while supply is arranged, or dispose of it through an approved medicine take-back route.
- Report problems through the normal pharmacovigilance channel. Report suspected adverse effects and provide the product and lot details; this helps authorities connect quality signals with specific batches.
Seek urgent medical help for symptoms of an uncontrolled underlying condition, an overdose or an allergic reaction. Nitrosamine exposure itself usually does not produce a distinctive immediate symptom, so routine medical advice should focus on maintaining treatment and checking the product identity.
Are nitrosamines still found in medicines?
Yes, the possibility remains under active surveillance, but “still found” does not mean that all medicines or all batches contain them above a limit. New formulations, suppliers and manufacturing changes can create different risk profiles, while improved controls can reduce or eliminate a previously identified pathway. FDA continues to evaluate products, and the 2024 guidance gives manufacturers a framework for prospective prevention as well as retrospective investigation.
Regulatory status also differs by geography. A product held or recalled in one country may have a different manufacturer, formulation, lot distribution or legal status elsewhere. Current information must therefore be checked against the national regulator, product name, strength and lot rather than inferred from a headline about the same active ingredient.

How to read a recall or safety notice
A useful notice should answer six questions. If one is missing, ask the pharmacy or regulator for clarification:
- Which impurity? NDMA, NDEA, NMBA or another identified N-nitrosamine.
- Which medicine and source? The API, finished product, manufacturer and manufacturing site.
- Which lots and markets? Lot numbers, expiration dates, strengths, dosage forms and countries supplied.
- What level was measured? The result and the applicable AI, with units and testing conditions.
- What action was taken? Distribution hold, recall, process change, added testing or continued supply with controls.
- What should patients do? Continue, switch, return or dispose—and who will arrange replacement treatment.
This checklist prevents two common errors: assuming a positive result applies to every batch, and treating a recall notice as permission to stop an essential medicine without a replacement plan.
Bottom line for patients and clinicians
The nitrosamine saga exposed preventable weaknesses in pharmaceutical supply chains, but it also produced stronger testing and risk-management requirements. NDMA and related compounds are serious hazards at sustained exposures above their acceptable intakes; the response is lot-specific investigation, validated measurement and controlled substitution—not panic or unsupervised discontinuation. For any current concern, identify the exact product and lot and obtain advice from a pharmacist, prescriber and the regulator serving your country.
