A newborn can be carried in a wrap only when the carrier is appropriate for the baby’s size and is used exactly as its instructions specify. Keep the baby’s face visible, nose and mouth clear, chin away from the chest, and hips and knees bent with the thighs supported. Check often, manage clothing for the temperature, and take the baby out of the wrap to feed.
How to keep a newborn’s airway clear in a wrap
Airway safety comes first. A young infant may not be able to move out of a position that makes breathing difficult. The American Academy of Pediatrics (AAP) advises keeping the neck straight, the chin off the chest, and the face visible, with the nose and mouth unobstructed. Check the baby frequently rather than assuming the position will stay safe. The AAP carrier guidance was last updated April 11, 2025: HealthyChildren.org carrier safety guidance.

- Keep the face visible: You should be able to see the baby’s face without moving fabric.
- Keep the nose and mouth uncovered: Fabric and the caregiver’s body should not press against or cover the airway.
- Avoid a curled posture: The baby’s chin should not fall toward the chest. AAP warns that some slings can curl an infant into a C-shape, increasing breathing risk.
- Recheck often: Confirm the head and neck remain supported and the airway stays clear as the baby moves or the carrier settles.
The U.S. Consumer Product Safety Commission (CPSC) warns that sling carriers can suffocate young babies if fabric covers the airway or if a curled position bends the chin toward the chest. Its 2017 safety release urges particular caution with babies younger than four months. After nursing, reposition the baby with the head facing up and clear of both the sling and caregiver; NHS guidance says to take the baby out of the sling to feed. Read the CPSC infant sling-carrier safety guidance.
CPSC recorded 159 sling-carrier incident reports from January 2003 through September 2016: 17 were fatal and 142 nonfatal. Among those 142 nonfatal reports, 67 involved an infant injury during product use, and 10 injury reports involved hospitalization. These historical U.S. reports are not current incident rates or a measure of any individual baby’s chance of harm.

What a newborn’s hips and legs should look like
The International Hip Dysplasia Institute (IHDI) describes a hip-healthy carrying position as one in which the hips and knees are bent, the thighs are spread, and the thighs are supported toward the knees. This is often called the M-position, jockey position, straddle position, or spread-squat position. See IHDI’s baby-carrier guidance.
Use the wrap’s instructions to position and support the baby. The carrier must fit the child; IHDI emphasizes thigh support particularly for prolonged carrying and for infants under six months. Do not force the legs into a position the carrier does not allow, and do not let the legs dangle unsupported if the instructions call for thigh support.

Can a wrap cause or prevent hip dysplasia?
IHDI’s carrier consensus states: “Based on the best available evidence, IHDI maintains that no baby carrier can cause or prevent hip dysplasia.” The M-position is educational guidance supporting healthy hip development, not proof that a particular wrap prevents dysplasia. IHDI also says it is not a certifying body and has not safety-tested products; a partner listing is not a safety certification or endorsement. Read its carrier consensus statement.

This should not be confused with tightly swaddling an infant’s legs straight for many hours, nor should a wrap be treated as therapy for diagnosed hip dysplasia. IHDI’s educational statement says approximately one in ten newborn infants has mild hip instability and one in 100 may require treatment; the page does not state a publication year, so these figures should not be read as a current global prevalence estimate. How to manage clothing and temperature
There is no universal carrier-specific layer count. A wrap and caregiver can add warmth, so consider the baby’s clothing together with the ambient temperature and the warmth of the carrier and the person wearing it.
- Feel the baby’s chest or the back of the neck. If the skin feels hot or sweaty, remove a layer.
- When going indoors or into a warm vehicle, remove hats and extra clothing.
- As general context—not a babywearing-specific formula—the AAP says to dress infants for the environment, generally in no more than one layer beyond what a comfortable adult would wear. Its overheating guidance concerns infant sleep environments and general dressing, not wraps specifically. Look for sweating, flushed skin, or a hot chest.
For sling positioning, NHS summarizes its guidance as T.I.C.K.S.: Tight; In view; Close enough to kiss; Keep the chin off the chest; Support the back. Its temperature advice is to check the chest or back of the neck and remove a layer if the baby is hot or sweaty. See NHS guidance on baby slings and carriers and the AAP guidance on preventing overheating.

When to check the carrier instructions or ask a clinician
Carrier designs and approved positions differ. Before using a wrap, confirm its instructions expressly cover the baby’s age and size, and follow the specified tying, adjustment, and positioning steps. Inspect the material and fasteners as directed, and do not use a damaged carrier. A carrier’s label or instructions—not a general rule about wraps—determine whether that specific product is suitable for a newborn.

Ask the baby’s clinician for individualized advice before using an upright positioning device if the baby was premature or has breathing problems or other medical concerns. The AAP cautions that positioning may make breathing harder for premature infants and babies with respiratory problems.
