Ultra-accurate sequencing now lets researchers see DNA changes present in only a tiny fraction of cells. The results show that somatic mutations accumulate as tissues divide, and that age-related expansion of blood-cell clones can amplify otherwise hidden mitochondrial variants. These are important findings about mutation and clonal biology—not a consumer aging score or a stand-alone medical diagnosis.
Why mutations in only a few cells are difficult to find
A somatic mutation is a DNA change acquired after conception by an individual cell or one of its descendants. It can arise from a replication mistake, chemical exposure, radiation or another form of DNA damage. Unlike a germline variant, it is not copied into every cell and is not automatically inherited by a person’s children.

Most somatic variants begin at extremely low abundance. If one altered cell is mixed with thousands or millions of unaltered cells, ordinary bulk sequencing can mistake a genuine low-frequency signal for a sequencing error. The signal becomes easier to see when the altered cell produces a large clone. In blood, this process is called clonal hematopoiesis: a blood-forming stem or progenitor cell and its descendants become unusually abundant, carrying any mutations that occurred in the original cell.

Mitochondrial variants become more visible around age 60
A 2026 Nature analysis of approximately 736,038 people from the All of Us and UK Biobank resources examined single-nucleotide variants in mitochondrial DNA (mtDNA). The variants accumulated sharply around age 60, were generally present at low levels and were likely neutral rather than directly harmful.

The study’s proposed sequence of events is straightforward: random mtDNA variants arise first at low abundance; later, age-related expansion of blood-cell clones increases the number of cells carrying some of those variants; bulk blood sequencing can then detect them. A higher mtDNA-variant burden was linked to clonal-hematopoiesis-related genes and to variants near TERT, TCL1A and SMC4. Those genetic links describe a biological association, not proof that every listed variant causes disease.
Mutation patterns differ by age, tissue and exposure
BotSeqS uses molecular barcodes to label original DNA molecules before sequencing. Researchers can then group reads that came from the same molecule, suppress many technical errors and quantify rare point mutations across both nuclear and mitochondrial genomes. The reported mutation prevalence and mutation spectra varied with age, tissue type, DNA-repair capacity and carcinogen exposure.
Work on adult stem-cell-derived organoids, reported in Nature in 2016, found approximately 40 novel mutations per year across the tissues assessed. That figure is an average for the study’s organoid system and tissues; it is not a universal yearly rate for every cell in a human body.

How sequencing methods separate true variants from errors
Molecular barcodes
A barcode is attached to an original DNA fragment before amplification. Reads with the same barcode are treated as observations of one starting molecule. A variant supported consistently within that family is more credible than a change seen in just one read. BotSeqS applies this principle to very rare point mutations in bulk DNA.
Duplex sequencing and NanoSeq
Duplex methods read both complementary strands of the same DNA molecule. A base change is accepted only when the two strands, or repeated observations of the original molecule, agree in the expected way. This sharply reduces false positives caused by polymerase mistakes, damaged bases and instrument noise.
NanoSeq, described in Nature in 2025, reported an error rate lower than five errors per billion base pairs. Such accuracy is valuable when the true mutation frequency is close to the instrument’s ordinary error floor, although it requires carefully prepared libraries, deep sequencing and stringent computational filtering.

Long-read sequencing
Short-read data are excellent for many single-nucleotide variants but can struggle with long repeats, rearrangements and other changes whose meaning depends on DNA sequence extending across a larger region. Long-read sequencing follows much longer molecules, making it better suited to some structural or complex variants. The trade-off is that sample quality, platform performance and analysis requirements differ from short-read workflows.
Single-cell sequencing and SMaHT
Single-cell methods assign variants to individual cells rather than averaging across a bulk sample. That gives direct information about which cells carry a mutation and how clones are related, but resolution, throughput and cost vary substantially by protocol.
The SMaHT program combines experimental calibration, sequencing and computational analysis across many normal tissues. Its purpose is to make somatic variation measurable outside tumors, where the signal is often sparse and technical artifacts can dominate. As SMaHT Network researchers were quoted by Baylor College of Medicine in 2026: “Every cell in the human body carries its own genome, shaped by mutations acquired over a lifetime, yet this vast somatic variation has been nearly impossible to measure.”

Method comparison
| Approach | Main strength | Variant or resolution focus | Important requirement or trade-off |
|---|---|---|---|
| Single-cell sequencing | Shows which individual cells carry a change | Cell-level SNVs and indels, depending on protocol | Requires suitable single-cell preparation; scale and cost vary by platform |
| Molecular-barcoded sequencing (BotSeqS) | Groups reads by original DNA molecule to reduce false positives | Rare point mutations in nuclear and mitochondrial DNA | Needs barcode-aware library preparation and consensus analysis |
| Duplex sequencing (including NanoSeq) | Requires agreement from complementary strands or repeated molecule observations | Extremely low-frequency base substitutions | Deep, high-quality sequencing and strict filtering are essential; NanoSeq reported fewer than five errors per billion base pairs in 2025 |
| Long-read sequencing | Reads larger pieces of DNA in one stretch | Structural and complex changes that short reads may fragment | Needs high-molecular-weight DNA and a platform-specific analysis workflow |
| SMaHT-style integrated studies | Combines calibration, sequencing and computation across normal tissues | Broad somatic-variation maps rather than one single variant class | Large multi-step studies require coordinated experimental and computational controls |
What clonal hematopoiesis means for health
In a New England Journal of Medicine cohort of 17,182 people, detectable clonal mutations were found in 9.5% of participants aged 70–79, 11.7% of those aged 80–89 and 18.4% of those aged 90–108. The same study associated clonal hematopoiesis with higher risks of hematologic cancer, death, coronary heart disease and ischemic stroke.
Those percentages describe groups, not an individual’s destiny. An association does not establish that a particular mutation will cause cancer or cardiovascular disease, and a research sequencing result is not itself a diagnosis. Clinical decisions require validated assays, medical history and professional interpretation.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesCan a blood test measure age-related mutations?
Yes, research laboratories can detect some age-related variants in blood, especially when clonal expansion has raised their abundance above the detection threshold. A negative or low-level result does not prove that a tissue contains no somatic mutations: variants may be confined to another organ, present in too few cells, or missed by a method that targets a different mutation class.
Best Value
The current evidence does not establish a general consumer screening test, a personal “biological age” score based on these variants or a treatment that reverses ordinary somatic mutation accumulation. Blood is useful because it is accessible and contains renewing cell populations, but it is not a complete survey of every tissue.
How a rare-variant study is performed
- Collect and preserve a sample. Investigators obtain blood or another tissue and extract DNA under conditions that limit degradation and contamination.
- Mark or duplicate original molecules. A barcode library, duplex design or another consensus strategy records the identity of starting molecules before amplification.
- Sequence deeply. High read depth increases the chance of observing a variant present in only a small fraction of molecules.
- Build consensus reads. Software combines observations from one original molecule or both DNA strands and rejects discordant evidence.
- Call and quantify variants. Researchers calculate variant abundance, distinguish nuclear from mitochondrial changes and inspect the mutation spectrum.
- Compare biological variables. Age, tissue, DNA-repair function, exposure history and clonal-hematopoiesis genes are evaluated to identify patterns while controlling for technical artifacts.
Does mutation accumulation explain aging?
A 2024 review concluded that somatic mutations do accumulate with age, but current evidence does not support the claim that random accumulation alone explains most gradual aging phenotypes. Aging reflects interacting processes, including changes in cell regulation, tissue structure, immune function and metabolism. Rare-variant sequencing clarifies one part of that biology: how individual lineages change and sometimes expand over a lifetime.
