Treat the repository as the shared source of code and write a concise handoff note for everything the next tool needs to know: the goal, current state, decisions, changed files, checks, constraints, and next step. You can separately resume a saved conversation in the same tool when its history is useful.
What transfers—and what does not
Both tools can work with a local repository, so the files, branch, and committed project artifacts are available to the receiving tool when it opens the same working copy. Conversation history is different: Claude Code and Codex document their own session-resumption features, but the official documentation cited here does not describe a supported, lossless conversion between their conversations. Plan to reconstruct task context from the repository and a written note, not from a shared transcript.

Claude Code’s session documentation describes its JSONL transcript format as internal and subject to change. If you need a readable record, use its documented /export feature or script interfaces rather than relying on parsing transcript internals. See Anthropic’s session documentation. Before switching, make the repository state clear
First establish exactly what the next tool will see. From the repository, inspect the branch, working tree, and relevant changes. For example:

git status --short
git diff --
git diff --staged --
These commands show the current branch, a concise working-tree status, unstaged changes, and staged changes. They are checks to perform, not proof that the work is correct. Review the relevant files as well, and record whether changes are uncommitted, committed, or stashed. If you create a commit or stash, include its identifier in the handoff note. Make clear which edits are intentional and whether any checks have actually been run.
Write a portable handoff note
Create a short task note in the repository, such as HANDOFF.md. Keep it focused on this task’s current state; recurring project conventions belong in the instruction files each tool recognizes.

- Goal and acceptance criteria: the requested result and how to tell it is done.
- Current state: what is complete, in progress, or blocked.
- Decisions: important choices, constraints, and rejected approaches the next tool should not revisit.
- Files and locations: changed files and the most relevant implementation or documentation.
- Branch and worktree: branch name, relevant worktree, and commit or stash reference if applicable.
- Checks: exact commands run and their actual results. Mark checks that were not run as “not run.”
- Risks and boundaries: compatibility, security, scope, permissions, or files that must not change.
- Next action: the single most useful thing for the receiving tool to do next.
A handoff note is workflow advice, not a required format from either vendor. Its purpose is to make the task’s transient context explicit and easy to verify against the files.

Keep project instructions separate from task progress
Project instruction files provide durable guidance; they do not replace a task handoff. Codex documents instruction discovery through AGENTS.md files at global, project, and nested scopes. Anthropic documents CLAUDE.md as project context Claude Code reads at session start for the applicable directory. Their documented conventions are separate by default: do not assume one tool automatically reads the other tool’s instruction file.

See the official guidance for Codex instruction discovery and Claude Code context and prompting. If both files are needed, review their shared rules together so they do not conflict; keep tool-specific instructions in the relevant file. Codex can be configured to use fallback instruction filenames, but that alone does not mean Claude Code recognizes those filenames.
Hand off from Claude Code to Codex
- Inspect the working copy. Record the repository, branch, changed files, staged and unstaged diffs, and any commit or stash reference. Note any checks run and their real results.
- Update
HANDOFF.md. State the goal, acceptance criteria, decisions, current status, constraints, and next action. Identify uncommitted work plainly. - Open the same repository in Codex. Point Codex to the handoff note and relevant files. Its CLI documentation describes work against a local repository and resuming saved Codex chats.
- Ask for an initial read before edits. For example:
Continue the task in
HANDOFF.md. First inspect the current branch, working tree, and relevant diff. Summarize the goal, decisions already made, checks actually run, and proposed next step. Do not repeat completed work. If the repository state conflicts with the handoff note, report the conflict and ask before overwriting changes. - Verify its understanding. Correct any mismatch between the note and the repository before authorizing more changes. Keep the acceptance criteria and boundaries explicit.
Hand work back from Codex to Claude Code
Use the same repository-first method in reverse. Before leaving Codex, update the note with its changes, actual verification results, outstanding work, and any decisions or assumptions that changed. Then open the relevant Claude Code conversation if its history is useful, or start a fresh one in the repository and direct it to the note. Ask Claude Code to inspect the current branch and diff rather than assuming that the previous conversation still describes the files accurately.
Resume a conversation only within its own tool
Same-tool resumption can restore useful history, but it is separate from cross-tool handoff. According to Anthropic’s session documentation, Claude Code offers these options:

Codex documents codex resume for returning to a saved chat, including finding older chats. Consult the Codex CLI documentation for its current behavior. These are product-specific continuation features, not a way to import the other tool’s conversation. After resuming either tool, compare its understanding with the current repository state.
