Misophonia is usually a strong reaction to particular sounds, even when they are quiet; hyperacusis is reduced tolerance of sound itself, so ordinary sounds may seem too loud, uncomfortable, or painful. The distinction is useful, but the conditions can overlap, and neither anger nor pain alone establishes a diagnosis.
How are misophonia and hyperacusis different?
The clearest distinction is the pattern of sounds that causes difficulty. Misophonia is typically selective: particular sounds prompt an intense response. Hyperacusis is more closely related to sound tolerance or intensity, and can affect a broader range of sounds.

| What to compare | Misophonia | Hyperacusis |
|---|---|---|
| Sound pattern | Often particular trigger sounds, including mouth or nose sounds such as chewing or sniffing. | Often a broader range of sounds that may be experienced as excessively loud or uncomfortable. |
| Relationship to volume | The trigger can provoke a reaction even when it is quiet or ordinary in volume. | Ordinary sounds may seem too loud; some presentations involve physical discomfort or pain. |
| Commonly emphasized response | Intense emotional reactions are often emphasized, though responses can also be physical. | Loudness discomfort or pain may be emphasized; emotional distress can also occur. |
These are broad descriptions, not a diagnostic test. Experts continue to work toward reliable definitions and assessment tools. A 2022 expert consensus study defined misophonia through a Delphi process while noting unresolved diagnostic questions, including the need to distinguish it from hyperacusis. See the 2022 consensus definition of misophonia and the American Journal of Audiology tutorial on sound-tolerance conditions.
Why do certain sounds make me angry?
A strong reaction to a specific sound can fit the pattern often described as misophonia, particularly when the sound is not loud but reliably triggers distress. Chewing and sniffing are commonly cited examples. However, anger is not required, and an emotional response by itself cannot distinguish misophonia from other sound-tolerance difficulties.

Some descriptions of misophonia emphasize intense emotional responses to certain sounds that are not determined by how loud those sounds seem. People may also report bodily reactions or avoidance. What matters clinically is the full pattern: which sounds trigger a reaction, how loud they are, what the person experiences, and how much the issue affects daily life.
Why do everyday sounds hurt or seem too loud?
When a range of everyday sounds seems excessively loud or physically uncomfortable, hyperacusis may be one possibility to discuss with a clinician. Pain can occur, but it is not required: the American Tinnitus Association distinguishes loudness hyperacusis from pain hyperacusis, and a 2024 study likewise notes that pain is not universal or defining.

In its Spring 2025 article, audiologists James A. Henry, PhD, and Kelly N. Jahn, AuD, PhD, give a broad estimate that “As many as 17 percent of people have difficulty tolerating everyday sounds that do not bother others.” That figure concerns sound hypersensitivity broadly; it is not a separate prevalence estimate for misophonia or hyperacusis.

Yes. Misophonia may occur on its own or alongside hyperacusis and tinnitus. A person who reacts intensely to a specific trigger may also find other sounds uncomfortably loud or painful. One label does not automatically rule out another.

A 2024 study of 119 participants with misophonia found dissociable effects of hyperacusis and misophonia severity, supporting the idea that the conditions may involve different mechanisms. It does not provide a self-test or settle every diagnostic question. Read the study, “Dissociable effects of hyperacusis and misophonia severity imply different mechanisms of decreased sound tolerance”.
How is sound sensitivity assessed?
There is no simple self-test in the evidence available here. A qualified audiologist or clinician can consider the complete sound pattern, physical symptoms, emotional responses, and impact on everyday functioning, as well as other possible causes.

- Which sounds cause a problem and whether the triggers are specific or widespread.
- Whether the sounds seem too loud, feel uncomfortable, cause pain, or provoke a selective emotional reaction.
- How the response affects sleep, work, relationships, or activities you avoid.
Do not assume that a normal hearing test rules out sound intolerance; the relevant question is the person’s sound-tolerance pattern and its effects.

What can help, and what should you avoid assuming?
Management depends on the individual presentation. The American Tinnitus Association describes graded sound exposure and counseling among options for some people with loudness hyperacusis, but sound therapy does not help everyone and may worsen symptoms for some people with pain hyperacusis. Do not force exposure to painful sounds or treat constant earplug use as universal advice.
If sound sensitivity causes pain, substantial avoidance, distress, or interference with daily activities, seek an evaluation from an audiologist or qualified clinician. An assessment can help determine which approaches are appropriate rather than relying on a label or a one-size-fits-all remedy.
