A block usually means a configured guardrail classified the prompt or generated answer at or above a blocking threshold—or a separate control, such as Prompt Shields, intervened. To find the cause, inspect the response annotations and the filter configuration attached to the specific deployment.
Why can a legitimate medical question be blocked?
Foundry filters classify content by harm category and severity; they do not treat every mention of health or medicine as prohibited. The documented categories are hate, sexual, self-harm, and violence. Microsoft’s Guardrails and controls overview in Microsoft Foundry says safe-level content can appear in medical and other professional contexts. It also states: “Safe content is labeled in annotations but isn’t subject to filtering and isn’t configurable.”

A block therefore needs to be traced to the actual intervention. The topic alone cannot show whether a harm-category threshold was met, Prompt Shields acted, or filtering occurred on the input or output side. A user-facing error without the underlying annotations is not enough to identify the cause.

Which control might have intervened?
Harm-category content filtering
Foundry content filters can apply to user prompts and model completions, with configurable thresholds for harm categories. The selected threshold determines which severity levels and above are flagged; safe-level content is not a configurable blocking level in the overview. The configuration associated with the relevant deployment is what matters, not a setting viewed in isolation. Microsoft’s content-filter configuration documentation describes the separate input and output stages and deployment association.

- Read Before You Buy — No Video Output: These adapters support charging and USB 2.0 data transfer, but cannot transmit video signals. Except for standard USB webcams (which use USB data only), they are not compatible with HDMI/DisplayPort cables, video-capable USB-C hubs, or docking stations with video output.
- Convert USB-A Ports to USB-C: Designed to connect USB-C earphones, cables, flash drives, card readers, and other USB-C accessories to standard USB-A ports. Plug-and-play with no drivers or software required.
- Aluminum Alloy Housing: Built with a sturdy aluminum alloy shell that aids in heat dissipation and protects against daily wear and scratches. Designed to maintain a stable and secure connection.
- Compact & Travel-Friendly: The ultra-compact design allows the adapter to stay plugged into your device without blocking adjacent ports or adding bulk, reducing wear and tear on your original USB ports.
- 12-Month Warranty: Backed by a 12-month manufacturer warranty for peace of mind. Designed to meet strict quality control standards for reliable everyday performance.
Prompt Shields
Prompt Shields address attacks in user prompts and attacks embedded in documents. They can be configured to annotate detections or block them, and their response distinguishes whether an attack was detected from whether it was filtered. A detection alone does not prove that this control blocked the request. Microsoft’s Prompt Shields documentation recommends checking the detection subtype when investigating a suspected false positive.
How to diagnose a block without guessing
- Capture the full event. Save the exact prompt, response, model deployment, timestamp, and returned filter annotations. Preserve the API response rather than relying only on the message shown in the application.
- Identify the control and its result. Check whether the annotations point to a harm category or Prompt Shields. For Prompt Shields, distinguish
detectedfromfilteredand review the reported subtype. - Locate the intervention stage. Establish whether the input was blocked before generation or the generated output was filtered. Foundry can filter both stages, so check which stage the response identifies.
- Check the deployment’s attached configuration. Verify the settings actually associated with that model deployment, including the relevant category threshold and whether Prompt Shields scans user prompts, documents, or both.
- Compare severity with threshold. For a harm-category result, compare its reported severity with the configured threshold for that category. Do not assume a medical context overrides the classification.
- Run a controlled Prompt Shields diagnosis if indicated. Microsoft recommends considering annotate mode instead of block mode to record detections without filtering while investigating. Review document-attack scanning and, where appropriate, whether trusted document sources should be exempted.
- Re-test before changing policy. Test representative benign medical questions alongside unsafe cases, using the languages and application context your system serves. A diagnostic annotation records evidence; it does not itself resolve a policy or model concern.
What to consider before changing thresholds
Thresholds are a risk decision, not a universal fix for false positives. Microsoft’s Azure AI Content Safety FAQ says the appropriate moderation threshold depends on the dataset, community guidelines, and risk tolerance, and recommends starting at severity level 4 for new projects. That is general Content Safety guidance, not a universal setting for every Foundry model deployment.

Changing a threshold may reduce some blocks while allowing more content through. Test the effect against both benign medical prompts and unsafe cases before adopting a change. Do not assume adjustment is available in every configuration: Microsoft notes that approval constraints can apply to modified or disabled filters, depending on the service and setup.

Why results can vary across languages and applications
Microsoft reports the cited harm-category models as trained and tested on English, German, Japanese, Spanish, French, Italian, Portuguese, and Chinese. It warns that detection accuracy and false-positive rates may vary and advises thorough testing. That language list is not a guarantee of equal performance, nor does it establish a false-positive rate for legitimate medical questions.

Test the real prompts, documents, languages, and deployment settings used by your application. The available documentation does not establish that a particular phrase caused a particular block or that all legitimate medical questions should pass; those conclusions require the actual prompt, service and model, deployment configuration, and filter result.
