Organizations running Exchange Server on premises—including in Hybrid deployments—should inventory every server, verify its supported build and security updates, and check for required follow-up actions. As of October 9, 2026, Exchange Server 2016 and 2019 are past end of support; only customers enrolled in the Extended Security Update program are eligible for their December 2025 and later security updates. If a server may have been compromised, patching is necessary but does not by itself remove an attacker.
Who is at risk?
The immediate concern is an on-premises Exchange server that is missing an applicable security update or still needs an administrator action required by that update. Risk depends on the vulnerability, installed build, configuration, accessibility and evidence of exploitation; an old vulnerability report cannot establish whether a server is exposed to a different, current issue.

Lifecycle status matters too. Microsoft says Exchange Server 2016 and Exchange Server 2019 support ended on October 14, 2025. Organizations running either product without eligible Extended Security Updates should treat migration to a supported path as an urgent lifecycle task, not assume that installing an older cumulative update restores support.

Include management-tools-only computers in the update inventory as well.

What does the current support status mean?
Microsoft’s Exchange Server supportability matrix lists Exchange Server Subscription Edition RTM as a supported build. Microsoft’s build-history guidance says customers enrolled in the Extended Security Update (ESU) program are eligible for security updates for Exchange Server 2016 and 2019 released from December 2025 onward. Organizations not enrolled are directed to migrate to Exchange Server Subscription Edition to continue receiving the latest security updates.
Microsoft identifies Exchange Server Subscription Edition and Microsoft 365 or Office 365 as paths to consider in its end-of-support roadmap. The right choice depends on whether the organization needs to retain on-premises Exchange and on its architecture and migration requirements; there is no universally preferable path.

Microsoft’s Exchange update FAQ was last updated May 9, 2025. It remains useful for general update-process guidance, but its examples of particular cumulative updates predate the end of support for Exchange 2016 and 2019. Do not use those sample update numbers as current recommendations; check Microsoft’s live supportability matrix, build history and release guidance for version-specific instructions.

- Inventory all installations. Record each Exchange server and management-tools-only computer, its product version and build, its role, and whether it is on premises or part of a Hybrid deployment. Use Microsoft’s supportability matrix and build-history pages to verify support and applicable updates.
- Resolve lifecycle coverage. For Exchange 2016 or 2019, verify ESU enrollment and applicable updates with Microsoft. If the organization is not covered, plan migration to a supported path rather than treating an old update as a substitute for support.
- Apply the supported update path. Microsoft’s general guidance is to install the latest applicable cumulative update (CU) and security update (SU). Install SUs on Exchange servers and Exchange Management Tools-only computers, and update Windows as well: operating-system vulnerabilities can contribute to an attack chain. Follow current release instructions, not outdated CU examples.
- Check for follow-up actions. Run Exchange Server Health Checker before updating to identify missing CUs, SUs and manual steps. Review the relevant SU instructions for actions that must be completed by an administrator, then run Health Checker again after installation to confirm the resulting state.
- Use emergency mitigations only as a bridge. The Exchange Emergency Mitigation (EM) service can apply temporary protections to Exchange Mailbox servers and checks for mitigations hourly, according to Microsoft. A mitigation may disable vulnerable functionality and requires administrator visibility and follow-through. It is not a replacement for an SU; install the code fix when it is available.
What is the difference between an update and a mitigation?
A security update fixes vulnerable code. An EM service mitigation temporarily blocks or disables an attack path; it does not make the underlying code fix unnecessary. Microsoft warns that mitigations can be bypassed, so use them to reduce immediate exposure while completing the applicable update and any required follow-up work.

What if the server may already have been compromised?
Investigate in parallel with updating when exposure or compromise is possible. Microsoft’s incident guidance for the vulnerabilities exploited in March 2021 explains that applying the March 2021 Exchange Server security updates was critical to prevent reinfection, but would not evict an adversary who had already compromised a server. That guidance concerns those historical vulnerabilities; it is not an announcement about current active threats. The principle remains important: a patched server is not automatically a clean server.
- Follow the organization’s incident-response plan and examine indicators of exploitation and persistence.
- Remediate any confirmed attacker foothold, and assess whether the attacker moved laterally or compromised other systems.
- Until the privacy of normal infrastructure communications is established, use isolated identities and communications for incident coordination, as Microsoft’s 2021 resource center advised.
- If evidence indicates compromise, escalate to the incident-response team or other qualified responders.
Microsoft’s 2021 resource center said Exchange Online was unaffected by the specific vulnerabilities addressed there. That historical statement applies only to those vulnerabilities; it should not be used to infer that Exchange Online is unaffected by a later issue. Check the advisory for the specific vulnerability and deployment at hand.

Does “fully up to date” mean there is nothing else to do?
No. Confirm the server is on a supported build or has applicable ESU coverage, that Health Checker reports no missing updates or required manual actions, and that any update instructions have been completed. If the server was exposed before patching or there are indicators of compromise, investigate separately; update status alone does not establish that an attacker was never present or has been removed.
