Scan for outdated or missing drivers - takes under a minuteDriver Scan →Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →If you received a notice that your medical information was exposed, first verify it through a phone number or website you already know belongs to the provider or health plan. Exposure does not by itself mean someone used your information. Then find out what data was involved and check for signs of misuse; the right response depends on whether medical, insurance, Social Security, account, or payment information was affected.
1. Save and verify the breach notice
Keep the original notice and note when you received it. Do not use an unexpected caller’s number or click a link in an unsolicited message to verify the incident. Instead, contact the organization using a number on an insurance card, a prior bill, or its independently located official website.

Ask what information was involved, the dates of the incident, what the organization is doing in response, and whether it offers support. Do not assume that every incident includes credit monitoring or that there is one universal deadline for patients to act. HIPAA breach-reporting deadlines apply to regulated organizations, not to affected people: for breaches involving 500 or more people, covered entities must report without unreasonable delay and no later than 60 calendar days after discovery; for fewer than 500, reporting is due within 60 days after the end of the calendar year of discovery. HHS explains those reporting requirements.
2. Find out exactly which information was exposed
Ask whether the incident involved medical details, insurance or Medicare information, your Social Security number, contact details, account credentials, or payment data. The distinction matters: a password exposure calls for securing the affected account, while exposure of insurance identifiers makes careful claim review especially important. Take additional steps only when they fit the data the organization says was affected.

3. Review insurer claims and explanation-of-benefits statements
Check recent and older explanation-of-benefits (EOB) statements and claims for unfamiliar providers, services, dates, or charges. An EOB is not necessarily a bill, but it can show that a service was submitted to your plan. Contact the insurer’s fraud department about anything you do not recognize, and keep a copy of the statement and your report. The FTC medical identity theft factsheet recommends contacting the health insurer and checking EOBs.
4. Request records if you find signs of medical misuse
If a claim, bill, or notice suggests someone may have received care using your identity, request copies of relevant records from the doctors, clinics, hospitals, pharmacies, laboratories, and health plans involved. Keep the written request and the response. If a record contains care you did not receive, the FTC advises reporting the error to the provider and to providers who treated the other person, and including a copy of the record that shows the mistake. The same FTC factsheet says you can complain to HHS OCR if a provider does not provide requested records or an explanation within 30 days of a written request; this is a records-access escalation point, not a general deadline for resolving corrections.

If you believe someone used your personal information to obtain medical care or benefits, report it at IdentityTheft.gov. The FTC site creates a personalized recovery plan and lets you track steps and prepare forms or letters. A breach notice alone is not proof of identity theft, so report suspected misuse rather than treating exposure as confirmed fraud.

7. Consider a credit freeze or fraud alert when identity data was exposed
If the exposed information could be used to open new credit accounts, consider either or both options. Both are free under FTC guidance. A freeze restricts access to your credit file and must be placed separately with Equifax, Experian, and TransUnion. A fraud alert asks lenders to take additional steps to verify your identity; you can generally place one with a single bureau, which notifies the others.
| Option | What it does | Where to place it |
|---|---|---|
| Credit freeze | Restricts access to your credit file, making it harder to open new credit accounts. | Contact each of Equifax, Experian, and TransUnion separately. |
| Fraud alert | Asks lenders to take additional steps to verify your identity. | Generally contact one bureau; it notifies the others. |
A freeze does not affect your credit score, correct medical records, or itself stop someone from using health benefits. The FTC says, “There’s no cost to place or lift a credit freeze, and it doesn’t affect your credit score.”

8. Use the right complaint channel
For a suspected HIPAA or Part 2 violation by an entity covered by those rules, you can file a written complaint with HHS’s Office for Civil Rights (OCR). Identify the entity and describe the alleged conduct. Complaints generally must be filed within 180 days of when you knew about the act or omission, though OCR may extend the period for good cause. HHS describes the process and scope. OCR does not oversee every organization that handles health-related information, so a health app or data holder is not automatically covered by HIPAA or Part 2.
9. Keep a recovery record and watch for warning signs
Save notices, letters, claim statements, record requests, and replies. For each call, write down the date, organization, person or department, and what was agreed or requested. This makes it easier to follow up and show what you have already reported.

Investigate if you receive a medical bill for services you did not get, a legitimate claim is rejected because your records show benefits are exhausted, or your plan cites a condition you do not have. These are warning signs, not proof of what happened or who caused it. The FTC lists them among its identity-theft warning signs.
What a breach notice does—and does not—tell you
A notice means an organization says information may have been exposed; it does not establish that anyone used it. Separate organization reporting obligations from your own response: HHS reporting timelines describe what covered entities owe the government, while your practical next steps are to verify the notice, identify the data involved, and investigate claims or records if misuse is suspected. Federal complaint guidance here applies to U.S. processes and does not determine every state privacy right or every health-data app’s obligations.
