Lung cancer may cause a persistent cough, chest discomfort, breathlessness, wheezing, coughing blood, hoarseness, unexplained weight loss or fatigue—but it can also produce no symptoms and be found incidentally on a scan. Symptoms alone cannot diagnose cancer. The National Cancer Institute (NCI) advises seeing a doctor when symptoms last a couple of weeks; seek urgent care for severe breathing difficulty, substantial bleeding, fainting or rapidly worsening illness.
What are the first signs of lung cancer?
There is no single first symptom. Lung cancer can irritate or obstruct the airways, press on nearby structures or cause whole-body effects. The pattern depends on the tumor’s location, type and extent.

Respiratory and chest symptoms
- A cough that does not go away or is getting worse. A new cough, a change in a long-standing smoker’s cough, or a cough that returns after an apparent infection warrants assessment when it persists.
- Chest discomfort or pain. Pain can be persistent or may worsen with coughing or breathing.
- Shortness of breath. Breathlessness can result from airway narrowing, fluid around the lung or reduced lung function.
- Wheezing. A whistling sound, especially if it is new or affects one side, should be checked.
- Blood in sputum (hemoptysis). Even streaks of blood need medical advice; heavier bleeding is an emergency.
Systemic and other symptoms
- Hoarseness from irritation or pressure near nerves that control the vocal cords.
- Loss of appetite and unexplained weight loss.
- Fatigue or a general feeling of illness.
- Trouble swallowing.
- Swelling of the face or neck, or prominent neck veins. This can occur when a tumor affects major blood vessels.
These symptoms have many more common causes, including infections, asthma, chronic obstructive pulmonary disease, reflux, allergies and heart problems. NCI puts the distinction plainly: “Only a doctor can tell if your symptoms are caused by cancer.”

Use persistence as a reason to contact a clinician, not as a diagnostic cutoff. NCI’s general guidance is: “If you have symptoms that last for a couple of weeks, it is important to see a doctor.” A cough that is worsening, repeatedly returning, accompanied by blood, or associated with breathlessness, chest pain, hoarseness, weight loss or unusual fatigue deserves attention sooner.

People with a long-term smoking history or other lung-cancer risks should not wait for a symptom to become severe. A clinician may decide that an examination, chest imaging or another evaluation is appropriate even when the cough seems mild.
Get urgent help for red-flag changes
Call emergency services or go to an emergency department for severe or rapidly worsening difficulty breathing, substantial or ongoing coughing of blood, fainting, blue or gray lips, confusion, or a rapidly deteriorating condition. The amount of blood and the person’s breathing and circulation matter more than a fixed volume threshold.
Can lung cancer cause no symptoms?
Yes. Early tumors may be too small or positioned so they do not disturb breathing. Lung cancer is sometimes discovered incidentally when a chest scan is performed for another reason, or through screening in someone at high risk who feels well. Absence of symptoms therefore does not prove that the lungs are healthy.

How doctors diagnose lung cancer
Diagnosis is a process rather than a single appointment or test. NCI states: “There is no single test that can diagnose cancer.” Doctors combine the history, examination, imaging and tissue or cell evidence with the person’s overall health.

1. Medical history and physical examination
The clinician asks when symptoms began, whether they are changing, smoking and secondhand-smoke exposure, occupational or environmental exposures, prior lung disease, family history and previous cancers. The examination may include listening to the lungs, checking oxygenation, examining lymph-node areas and looking for signs of fluid retention or other illness.
2. Laboratory tests
Blood and other laboratory tests can assess general health, anemia, organ function and readiness for procedures or treatment. They can support the evaluation but do not, by themselves, establish lung cancer.
3. Chest imaging
A chest X-ray may be an initial test, while computed tomography (CT) provides more detailed information about a suspicious area, lymph nodes and nearby structures. Imaging can show a mass or other abnormality, but an image alone usually cannot identify the exact cancer type.
4. Bronchoscopy or another sample procedure
Depending on the location, a doctor may use bronchoscopy to view the airways and collect washings, brushings or a biopsy. A needle biopsy through the chest wall, a biopsy of a lymph node, or sampling of fluid around the lung may be preferable for a peripheral lesion or a particular clinical situation.
5. Pathology and molecular testing
A pathologist examines cells or tissue to determine whether cancer is present and whether it is non-small-cell or small-cell lung cancer. For many non-small-cell cancers, additional molecular tests look for alterations that can guide targeted treatment. The available tissue and the urgency of treatment can affect which tests are performed and in what order.

6. Tests that determine spread
Once cancer is suspected or confirmed, further imaging and procedures assess the lungs, lymph nodes, brain, bones, liver and other areas when clinically indicated. These staging tests show whether disease is confined to the lung, involves nearby structures or nodes, or has spread to distant organs.
Screening when you have no symptoms
For people at high risk, NCI’s clinician guidance identifies low-dose helical CT as the only screening method shown to reduce lung-cancer mortality. Chest radiography and sputum cytology have not demonstrated that mortality benefit. Screening still has possible harms, including false-positive findings, incidental abnormalities, radiation exposure and invasive follow-up procedures, so the decision should be shared with a clinician who can verify eligibility and explain local program requirements.
A screening scan is not a substitute for evaluating symptoms. Someone with a persistent cough, coughing blood or new breathlessness needs diagnostic assessment even if they have never had a screening scan.
What the stages of lung cancer mean
Stage describes how far cancer has spread when it is assessed. Doctors use the TNM system: T describes the primary tumor’s size and invasion, N describes involvement of nearby lymph nodes, and M records distant metastasis. TNM findings are grouped into broad stages, but the exact substage depends on the cancer type and the detailed measurements.

| Stage 0 | Abnormal cells are confined to the airway lining. They may become invasive cancer but have not invaded deeper tissue. |
| Stages I–III | Increasing local tumor extent and/or spread to regional lymph nodes or nearby structures. Exact substages come from the TNM findings. |
| Stage IV | Distant metastatic spread is present. In non-small-cell lung cancer, stage IVA and IVB distinctions depend on the number and location of distant sites and other TNM details. |
What stage IV means—and what it does not
Stage IV means that cancer has spread beyond the original lung region to distant sites. It does not identify one uniform treatment or outcome. Doctors still consider whether the cancer is non-small-cell or small-cell, its molecular findings, the locations and burden of spread, symptoms, previous treatment and the person’s overall health. A stage label assigned at diagnosis remains the original stage even if the disease later changes; new scans and examinations are recorded as progression, recurrence or response rather than silently replacing the original label.

The two main types of lung cancer
Non-small-cell lung cancer (NSCLC)
NSCLC is the more common broad category and includes several subtypes. Treatment planning uses the subtype, TNM stage, molecular findings, whether the tumor can be removed safely and the patient’s health and preferences.
Small-cell lung cancer (SCLC)
SCLC is biologically different and often grows or spreads more quickly. Doctors use its extent, response to treatment and overall health to choose therapy. Because the two major types behave differently, a pathology result is essential before making a treatment plan.
How stage and type guide treatment
NCI lists surgery, radiation therapy, chemotherapy, targeted therapy and immunotherapy among lung-cancer treatments. The combination depends on the pathology, stage, molecular results, resectability and fitness for treatment.
Early-stage NSCLC
Selected patients may be treated with surgery to remove the tumor and nearby lymph nodes. Focused radiation can be an alternative when surgery is unsafe or unsuitable. Additional treatment may be recommended according to the pathology and recurrence risk.
Stage II and III disease
Care commonly combines modalities—such as surgery, chemotherapy, radiation, immunotherapy or targeted therapy—in an order chosen by a multidisciplinary team. Whether a tumor can be removed, which lymph nodes are involved and the molecular profile are decisive.
Stage IV disease
Systemic treatment is usually central because cancer is present beyond one local surgical field. Options can include chemotherapy, immunotherapy or a targeted drug when a treatable molecular alteration is found. Radiation or procedures may be used to relieve pain, airway obstruction, bleeding or other site-specific problems. Symptom control, function and quality of life are considered alongside tumor control.
No stage or symptom list can predict an individual prognosis. A cancer team needs the pathology report, TNM details, molecular results and health history to discuss realistic options.
What to bring to a medical appointment
- A timeline of symptoms, including when they began, what makes them better or worse, and whether they are progressing.
- A medication and supplement list, allergies and major medical conditions.
- Smoking history, including years smoked, average amount and date of quitting, if applicable.
- Prior scans, pathology reports and information about previous cancers or lung disease.
- Questions about which test is being recommended, what it can and cannot show, when results are expected, and what happens if it is abnormal.
Ask who will explain the pathology and stage, whether molecular testing is appropriate, and which symptoms should trigger an urgent call while evaluation is under way.

Bottom line for anyone worried about lung-cancer symptoms
A cough or chest symptom is usually caused by something other than cancer, but a persistent or worsening change should not be ignored. Arrange a medical assessment when symptoms last a couple of weeks—or sooner for blood in sputum, increasing breathlessness, chest pain, hoarseness, unexplained weight loss or rapid decline. Diagnosis requires a coordinated process, often including imaging and biopsy; stage then describes local, regional or distant spread and helps determine treatment.
