Local long-term memory can help a coding agent carry project decisions across runs, but saving notes is only the first step. Memory can disappear, go stale, lose details during extraction, or fail to change what the agent does. Filesystem access adds separate risks: unwanted edits, data loss, and secret exposure.
The eight failure modes below are documented patterns across agent-memory designs and filesystem-safety research—not a verified account of eight incidents from one implementation. Treat them as design checks for a memory layer, not as proof that every system exhibits every failure.

1. The memory is written, but the next run cannot find it
A memory file is useful only if later runs can access the same file or a persisted copy of it. In the OpenAI Python SDK’s documented workflow, generated memory is stored in a configured directory. A fresh, empty sandbox that does not receive that directory or persisted state has no access to the previous run’s memory.
Make persistence an explicit part of the run lifecycle:
#1 Best Overall
- Choose the directory or snapshot that holds durable memory.
- Mount, restore, or otherwise provide it when starting a later run.
- Test a genuinely fresh run, rather than relying on a continuing process or workspace.
- Fail visibly when expected memory is absent; do not let the agent silently act as though it has recalled project history.
This is a deployment and state-transfer problem, not evidence that every local filesystem store loses data.
2. Old notes override the current project
A memory entry can accurately describe an earlier state and still be wrong for today’s checkout. Dependencies change, decisions are reversed, and a once-fragile file may be repaired. OpenAI SDK guidance treats memory as guidance rather than authority: the agent should trust current environmental evidence when it conflicts with a note, then correct the memory where appropriate.
Give memory entries enough provenance to judge their freshness. Record when a decision was made and, when useful, the relevant branch, version, or issue. Separate durable constraints from temporary observations. When a test, configuration file, or current user instruction contradicts a note, the present evidence should win; the system should then update or retire the stale entry instead of repeating it on every run.

3. Extraction drops details from a long conversation
Memory generation is not guaranteed to preserve every detail in accumulated conversation files. The SDK documentation says long input may be truncated to fit the model’s context window, preserving the beginning and end. Important information in the middle can therefore be omitted from the resulting memory.
Do not treat a generated summary as a complete transcript. Keep the summary small, and retain detailed run notes separately when their exact contents may matter. For work that depends on a specific command, error, or decision, retrieve the relevant detailed note rather than assuming the summary captured it. A long conversation’s size alone does not establish that its middle details survived extraction.
4. Memory grows into a conflicting, stale archive
Accumulation is a maintenance problem, not just a storage-capacity problem. Research on filesystem-based agent memory frames organization as vulnerable to clutter, conflicts, and staleness. A separate agent-memory data paper identifies unregulated growth, lack of semantic revision, capacity-driven forgetting, and read-only retrieval as recurring concerns in its taxonomy. These are research framings, not proof that every memory implementation suffers each problem.

Build maintenance into the design:
- Prefer concise, actionable entries over dumping entire conversations into a permanent file.
- Revise or remove superseded claims instead of adding a contradictory note indefinitely.
- Keep detailed history available for lookup without injecting all of it into every run.
- Make retrieval capable of finding relevant older material, not only the newest notes.
For a practical distinction, the OpenAI Cookbook separates context that helps the current run, memory intended to help future runs, and reviewed artifacts containing facts people rely on. Project instructions, accepted design decisions, and other durable commitments may belong in reviewed artifacts rather than being inferred automatically from an agent’s conversation history.

5. Memory access becomes a route to filesystem damage or secret exposure
Memory quality and filesystem safety are different concerns. An agent that can read and write local files may damage project data or expose secrets regardless of whether its memory is accurate. Microsoft Research’s 2026 publication page describes a systematic analysis of 290 public reports across 13 frameworks involving filesystem corruption, deletion, and secret leaks. That scope concerns filesystem misuse generally; it does not show that memory layers caused those incidents.
Limit the consequences of a bad action. Give the agent only the filesystem access its task requires, keep secrets out of ordinary memory files, and make consequential changes reviewable. A remembered instruction should not grant broader permission than the current task or environment allows.

6. The agent’s changes are hard to see or undo
Even when a command is authorized, an unexpected side effect can be difficult to diagnose if the system does not expose what changed. The YoloFS project describes a design that stages mutations for review, takes snapshots after commands, and gates sensitive accesses. Its 2026 project page reports that 40% of harm in its study was unrecoverable; it also reports that its agent undid hidden damage on 8 of 11 tasks while effects remained staged and reviewable. Those figures belong to the project’s study, not to local memory systems generally.
YoloFS also reports a project-specific evaluation in which user prompts fell from 0.9 to 0.4 per task. That result is not evidence that memory itself reduced prompts. The broader design lesson is to make filesystem effects visible and recovery practical: stage or snapshot risky changes, show the user what an action will affect, and ask before sensitive access or irreversible operations.

7. Process-local session state vanishes on restart
Some systems keep working state only in an application process. Redis documentation notes that this kind of in-process or app-server state disappears after a restart and is not shared across multi-agent or load-balanced deployments. That limitation applies to the described storage pattern; it should not be generalized to every local file store.

Decide what must survive a process restart, machine change, or handoff to another agent. Store that state in a persistence layer available to the next run, and test the handoff explicitly. If state is intentionally session-only, label it as such so neither an agent nor a user mistakes it for durable memory.

A system can successfully store a lesson and still fail to use it at the moment it matters. The projectmem abstract describes a contrasting design: an append-only typed event log, compact derived summaries, and a deterministic pre-action gate intended to warn about repeating failed fixes or editing known-fragile files. This is a design proposal, not independent evidence that it prevents every repeat mistake.
For a warning to shape behavior, retrieval must happen before the relevant action, the warning must be specific enough to evaluate, and the agent must have a defined response when a proposed edit conflicts with it. Test those conditions with a known failed fix or fragile-file example. A note that is only visible after the action is not an effective guardrail.
Choose the memory layer by its behavior, not its label
“Memory” can refer to quite different data and workflows. OpenAI SDK documentation describes generated cross-run notes, while Redis describes session and long-term memory in a combined memory-layer design; projectmem describes an event log with projected summaries and an action warning. These are not interchangeable just because each uses the word memory.
| Design question | What to establish |
|---|---|
| Persistence and portability | Is state a workspace directory, a persisted snapshot, process-local state, or a remote store? What exactly must be carried into the next run? |
| Retrieval and context cost | Does the agent start with a small summary and search detailed notes only when relevant, or load more history up front? The SDK sandbox guide documents progressive disclosure: inject a small summary, search MEMORY.md when prior work appears relevant, then open detailed rollout summaries selectively. |
| Correction and history | Can the system revise stale facts? Is history append-only? Can a recorded failure or fragile-file warning influence an action before it happens? |
| Safety and auditability | Are writes visible, permissioned, reviewable, and reversible? Are durable facts stored in reviewed artifacts separately from working memory? |
OpenAI’s SDK documentation also distinguishes reading memory from updating it: memory reads require shell access, while live updates require filesystem access. It offers read-only and generate-only configurations for runs that should consume memory without adding to it, or generate memory without also reading existing notes. Those switches make it possible to separate roles and permissions rather than giving every run unrestricted read-and-write access to the memory store.
Quick Recap
A practical design checklist
- Define which facts belong in current-run context, cross-run memory, or reviewed project artifacts.
- Specify how memory is persisted and restored, then verify behavior in a clean run.
- Use a compact starting summary with selective access to detailed notes.
- Preserve provenance and allow stale or contradictory entries to be corrected.
- Keep memory generation from silently standing in for a complete conversation record.
- Constrain filesystem permissions and protect secrets from being copied into durable notes.
- Make consequential changes inspectable and recoverable.
- Test that warnings are retrieved before relevant actions and have a defined effect on the agent’s choices.
