India Air Pollution Linked to 1.67 Million Deaths in 2019

Short answer: Air pollution is linked to an exceptionally large mortality burden in India, but there is no single, year-independent percentage. The peer-reviewed Global Burden of Disease (GBD) analysis estimated 1.67 million deaths in 2019, or 17.8% of all deaths (95% uncertainty interval: 1.42–1.92 million deaths, or 15.8–19.5%). The Health Effects Institute’s State of Global Air 2024, using 2021 data and a different framework, estimated 2.1 million Indian deaths. A State of Global Air South Asia page describes almost 35% of deaths in India in 2021 as related to air pollution, but that percentage is not directly comparable with the GBD result or the 2.1-million count. These are modeled attributable-risk estimates, not death certificates that identify pollution as the sole cause of an individual death.

What the major estimates say

Several widely cited figures answer slightly different questions. The reference year, pollutants included, exposure categories, health outcomes and statistical model all affect the result. Comparing the numbers without those details creates the impression of a contradiction where there may simply be different definitions.

Estimate Reference year Result for India What it represents
India State-Level Disease Burden Initiative, published 2020–2021 2019 1.67 million deaths; 17.8% of all deaths (95% UI 1.42–1.92 million; 15.8–19.5%) Attributable deaths from ambient particulate pollution, household air pollution and ambient ozone, modeled for every state
India State-Level Disease Burden Initiative/IHME analysis 2017 1.24 million deaths; 12.5% of all deaths Earlier modeled estimate using the GBD approach
State of Global Air 2024 2021 2.1 million deaths Estimate using that report’s current air-pollution framework
State of Global Air South Asia summary 2021 Almost 35% of deaths in India described as related to air pollution Percentage presented on that regional page; its denominator and framework should not be assumed identical to the GBD estimate
State of Global Air 2024 health evidence 2019 980,000 deaths attributable to ambient PM2.5 (95% UI 0.77–1.19 million) Ambient fine-particle component only, rather than the full combined burden

The figures therefore support a clear conclusion: India has one of the world’s largest air-pollution mortality burdens in absolute terms. They do not establish one permanent percentage that applies to every year or every definition of pollution.

Why “almost a quarter” is an approximate headline

“Almost a quarter” is a shorthand for a burden that can be close to one-fifth, one-quarter or higher depending on the analysis. The 2019 GBD estimate of 17.8% is below one in five, although its uncertainty interval reaches 19.5%. The 2021 State of Global Air count of 2.1 million is a total number, not a percentage. The regional page’s almost-35% statement is substantially higher and should be reported with its own source, year and methodology rather than blended with the GBD percentage.

Differences arise for several reasons:

  • Reference year: Pollution concentrations, fuel use, population size and the total number of deaths change over time.
  • Pollutants: One estimate may combine ambient PM2.5, household smoke and ozone, while another may emphasize ambient PM2.5 or include nitrogen dioxide.
  • Exposure setting: Household air pollution from solid-fuel cooking is not the same exposure as outdoor urban pollution, even though both can be included in an overall burden.
  • Attribution rules: Models assign a share of disease risk to exposure above a counterfactual minimum-risk level. They are not counting bodies in which pollution was the only cause.
  • Health outcomes: The modeled outcome set can include cardiovascular disease, stroke, chronic respiratory disease, lung cancer, diabetes, pneumonia and other conditions.

A percentage from one release should therefore never be compared with a count from another without labeling these axes. A higher estimate does not automatically mean that pollution suddenly became more toxic; it may reflect updated exposure data, a revised risk relationship or a broader set of pollutants and diseases.

Which pollutants and exposures are included?

Ambient PM2.5

PM2.5 consists of particles no larger than 2.5 micrometres in aerodynamic diameter. They can penetrate deep into the lungs, enter the bloodstream and contribute to systemic inflammation. In the global estimates, PM2.5 is the dominant component of the air-pollution burden. The 980,000-death India estimate for 2019 covers ambient PM2.5 alone and therefore should not be presented as the country’s entire pollution burden.

Household air pollution

Smoke from cooking or heating with solid fuels creates high concentrations indoors. Its importance depends on which fuels households use, ventilation and time spent near the cooking area. Children are particularly affected by household exposure, which is associated with respiratory infections and other adverse outcomes. A national total that combines household and ambient exposure can be much larger than an estimate limited to outdoor PM2.5.

Ambient ozone and nitrogen dioxide

Ground-level ozone is a reactive gas formed through atmospheric chemical reactions. Nitrogen dioxide is closely associated with combustion from traffic, power generation and other sources. State of Global Air’s current framework tracks ambient PM2.5, ambient ozone, nitrogen dioxide and household air pollution linked to solid-fuel cooking. The contribution of each pollutant differs by location and by the health outcomes included.

Local mixtures and sources

Indian communities experience different combinations of exposure. Transport, energy production, industrial activity, windblown dust, seasonal agricultural burning and household fuels can all matter. A national average can conceal large differences between a city affected mainly by traffic and a rural household where solid-fuel cooking dominates exposure.

What illnesses are counted in the mortality burden?

Air-pollution attribution is not limited to deaths labeled “respiratory.” The models include cardiovascular disease, stroke, chronic respiratory disease, lung cancer, diabetes, pneumonia and other outcomes for which long-term exposure raises population risk. State of Global Air 2024 reports that noncommunicable diseases account for nearly 90% of the global air-pollution disease burden. Respiratory infections remain important, especially for children and for people exposed to household smoke.

Fine particles can trigger inflammation, oxidative stress, changes in blood clotting and deterioration of lung function. These pathways can worsen existing heart and lung disease and increase the probability of a fatal event. Ozone irritates airways and can reduce lung function, while nitrogen dioxide contributes to airway inflammation and often signals exposure to combustion-related pollution. The models combine evidence from epidemiological studies with exposure estimates; they do not prove that pollution caused every particular heart attack, stroke or pneumonia death.

How the studies turn exposure into an estimate

  1. Measure or model exposure. Researchers combine monitoring, satellite observations, emissions information and other data to estimate population exposure to each pollutant and to household smoke.
  2. Apply exposure–response relationships. Epidemiological studies provide risk relationships describing how disease risk changes as exposure rises. These relationships are applied to the Indian population and to specific diseases.
  3. Choose a counterfactual level. The model asks how many deaths might not occur if exposure were reduced to a theoretical minimum-risk level. This is a statistical comparison, not a claim that a pollution-free society can be created instantly.
  4. Calculate attributable fractions. For each disease and location, the model estimates the share of deaths statistically attributable to the exposure above that counterfactual, then sums across diseases and pollutants.
  5. Quantify uncertainty. Exposure data, risk relationships and population statistics all have uncertainty. The GBD 2019 result reports a 95% uncertainty interval of 1.42–1.92 million deaths, which is why the central estimate should not be treated as an exact count.

This process is standard in population health burden assessment. It is useful for comparing risks and prioritizing interventions, but it cannot identify which pollutant caused an individual person’s death when that person had several risk factors.

Where the burden is highest within India

The 2019 India analysis estimated deaths, disease burden and economic effects for every state, rather than treating the country as one uniform exposure zone. Concentrations, household fuel use, age structure, healthcare access and dominant pollution sources vary widely.

The earlier 2017 analysis estimated that the largest potential gains in average life expectancy under a minimum-risk pollution counterfactual—more than two years—were in Rajasthan, Uttar Pradesh and Haryana. This does not mean every resident of those states loses two years, nor that the same intervention would work everywhere; it indicates a larger modeled population-level gain where baseline exposure and related risks were especially high.

Economic effects also differ. The 2019 indexed abstract identified Delhi as having the highest per-capita economic loss, with a 5.4-fold spread between states. Such estimates generally combine the economic value of premature mortality and disability. They are not household bills or a direct measure of what an individual will spend.

Are these proven individual causes of death?

No. “Attributable deaths” means that, across a population, statistical evidence indicates fewer deaths would be expected if the modeled exposure were lower. Most people who die after long-term exposure also have other risks, such as age, smoking, infection, hypertension or diabetes. A death certificate may record a proximate disease—such as myocardial infarction, stroke or pneumonia—without naming air pollution.

The estimates are nevertheless meaningful. Public-health decisions concern changes in risk across millions of people, not only cases in which a doctor can isolate one cause. If exposure falls, the model predicts fewer pollution-attributable events even though no single patient’s counterfactual outcome can be observed.

What the numbers imply for public health

The consistent signal across the studies is that reducing exposure could prevent a substantial number of premature deaths and years lived with disability. Because source mixtures differ, effective action must be tailored: cleaner household cooking, controls on combustion and industry, transport and fuel policies, management of agricultural burning, and reduction of dust and other local sources address different parts of the exposure profile.

Monitoring and policy evaluation should track both ambient pollutants and household conditions. A city can improve outdoor PM2.5 while households continue to experience high indoor smoke, or reduce household exposure while seasonal regional pollution remains severe. The burden estimates support measuring these pathways separately rather than assuming one national intervention will have the same effect everywhere.

UNICEF summarized the global stakes in June 2024: “Air pollution is having an increasing impact on human health, becoming the second leading global risk factor for death.” Its accompanying global estimate was 8.1 million air-pollution deaths in 2021, including 7.8 million attributed to PM2.5. India’s figures should be read within that global context while retaining their own year and method labels.

How to read an India air-pollution statistic responsibly

  • Check the reference year before comparing two numbers.
  • Identify whether the estimate covers ambient pollution, household pollution or both.
  • Look for the pollutant list: PM2.5, ozone, nitrogen dioxide and other categories are not interchangeable.
  • Read the uncertainty interval, not just the central estimate.
  • Confirm whether the figure is a death count, percentage, life-expectancy counterfactual or economic estimate.
  • Use “attributable to” or “associated with modeled exposure,” not “pollution was the sole cause,” unless discussing a different type of evidence.

On that basis, saying that air pollution may contribute to almost a quarter of deaths in India is a defensible high-level warning, provided the article identifies the year and method. Saying that exactly one in four Indian deaths is caused by pollution, regardless of year or location, overstates what the studies establish.
