I built a handover board for a friend who had become the person everyone relied on to remember what was happening. The point was simple: make the shared, practical details visible so that care tasks and updates did not have to live in one person’s head.
Why I made a shared handover board
When one person keeps track of appointments, visits and the latest update, the rest of the group can depend on that person without noticing how much coordination they are carrying. A board cannot make every decision or conversation happen, but it can give everyone a common place to check the basics and add changes.

That problem is familiar in home care. A 2024 qualitative study found that people receiving care and their relatives described limited collaboration between professionals and took on substantial organizational coordination and information exchange themselves. The study does not establish how common that experience is across all households, but it helps explain why a simple shared tool can matter.

What to put on a caregiver handover board
Keep the board focused on details that help the next person know what is happening and what needs attention. The exact layout should fit the people using it; the available account of my board does not establish its materials or a particular template.

- Task ownership: identify who has agreed to handle a practical task, if that has been decided.
- Useful updates: add brief, dated information the next person needs to know.
- Open questions: flag something that needs follow-up, rather than leaving it to be remembered.
A 2024 home-care study describes households using a weekly calendar or board to track therapy visits. Its participant account is an example of a household coordination aid, not evidence that every board should hold clinical information or replace professional communication.
How to share care tasks without creating another job
- Choose a place or format the group will actually check. A visible paper board can work for people who share a home; a shared digital calendar or document may fit a group that coordinates remotely. No single format is right for every household.
- Agree what belongs there. Keep entries practical and brief. Decide together whether the board is for schedules, task handoffs, or both.
- Make updates easy to spot. Add dates to changing notes and remove or cross out details that are no longer current. A handover tool is only useful if people can tell what is up to date.
- Share responsibility for maintaining it. Agree who will enter changes and how others should report them. Otherwise, the board can become one more task for the person who already remembers everything.
- Keep sensitive details appropriately private. Consider who can see a physical board or digital document before putting personal information on it.
Keep the board separate from official care records
A household handover board is a practical reminder and coordination aid, not a substitute for a care provider’s records, a clinical handover, or direct communication with professionals. Use it for the purpose the household has agreed on, and pass important changes to the appropriate people through their usual channels.

A qualitative action-research study of palliative-care handovers found that out-of-hours forms could be out of date, making decisions harder when other records were not readily available. That is a useful warning for any informal handover system: stale notes can mislead. Treat dates and clear updating habits as part of the tool, not optional decoration.

The research describes household calendars and boards, handover books and therapy sheets, but it does not establish that one format works best. A small dry-erase board is one possible choice if a household wants a visible, reusable surface; a notebook or care log may suit people who need a written running record. A shared digital tool can make remote updates easier, provided everyone can access and use it. The best option is the one the group will keep current and check.
