UnitedHealthcare sells health-benefit coverage and collects premiums while taking responsibility for members’ medical costs. Optum sells care delivery, pharmacy-benefit services, data and technology, consulting, administrative services, and financial products. In fiscal 2025, the group reported $447.567 billion in consolidated revenue, but its segment figures cannot simply be added because internal transactions are eliminated.
The business in one view
UnitedHealth Group is neither only an insurance company nor only a health-care services company. Its operating model combines a large risk-bearing insurer with several businesses that provide services to health plans, employers, providers, governments, consumers and pharmaceutical customers.

The 2025 Form 10-K identifies four reportable segments: UnitedHealthcare, Optum Health, Optum Insight and Optum Rx. UnitedHealthcare is the insurance engine. Optum is the services platform, divided into care delivery, information and technology, pharmacy services and related financial offerings.
| Business or measure | Fiscal 2025 amount | What the figure represents |
|---|---|---|
| UnitedHealth Group consolidated revenue | $447.567 billion | Revenue after eliminating transactions between UnitedHealth Group businesses |
| UnitedHealthcare revenue | $344.903 billion | Health-benefit coverage across commercial, government and individual markets |
| Optum total revenue | $270.620 billion | Optum segment revenue before Optum-related eliminations |
| Optum Rx revenue | $154.726 billion | Pharmacy-benefit, pharmacy-care and related services |
| Optum Health revenue | $101.957 billion | Medical-care delivery and value-based health services |
| Optum Insight revenue | $19.417 billion | Data, analytics, consulting, technology and managed services |
UnitedHealthcare’s $344.903 billion and Optum’s $270.620 billion are not additive corporate revenue. A UnitedHealthcare plan may purchase services from an Optum business, for example, and the group removes that internal sale when preparing consolidated accounts. The correct company-wide figure is therefore $447.567 billion.

How UnitedHealthcare earns money from insurance
Premiums in exchange for taking medical-cost risk
UnitedHealthcare generally receives a fixed premium per person for a contract period. In return, it pays or arranges covered medical care and absorbs the economic risk that claims and administration cost more or less than expected. If medical costs are lower than the premium and operating expenses, the underwriting result is favorable; if costs rise faster than pricing, margins are pressured.
The premium is not simply a fee for processing paperwork. It finances an obligation to cover eligible care, so pricing, member health, utilization, provider rates, pharmacy costs and government rules all affect the result. The company also earns investment income on funds held before claims and other expenses are paid, although the insurance model is primarily driven by premiums and medical-cost management.
Markets that supply the premiums
- Employer and individual coverage: Employers and consumers pay for commercial health plans, with pricing influenced by expected medical use, benefits and regulatory requirements.
- Medicare Advantage and Medicare Part D: The federal government pays plans for serving eligible beneficiaries. Revenue is affected by enrollment, CMS risk adjustment and quality-related bonuses.
- Medicaid and other state programs: State and government contracts pay for coverage of eligible populations. Membership mix and the medical needs of enrollees influence profitability.
Why government-program details matter
Medicare Advantage payments are adjusted for members’ documented health conditions, so a population with more complex conditions can produce higher risk-adjusted payments but also higher expected care costs. Quality scores can add bonuses or affect future payment levels. Commercial and Medicaid products can also be subject to Affordable Care Act risk adjustment and minimum medical-loss-ratio requirements; when applicable, insurers may owe rebates if too little premium revenue is spent on medical care.
What changed in fiscal 2025
UnitedHealthcare revenue increased 16% in 2025 to $344.903 billion. UnitedHealth Group attributed consolidated growth mainly to serving more people through Medicare Advantage, higher-acuity Medicaid membership, growth in Optum Rx and pricing trends. Those factors show why enrollment alone is not enough to explain revenue: the type of member, the contract’s payment formula and the cost of care all matter.

How Optum Health turns care into revenue
Services and care it provides
Optum Health combines physician practices and other clinical operations with contracts that reward managing a patient population’s total care. Its activities include primary, specialty and surgical care; chronic, behavioral and complex-care programs; post-acute planning; telehealth; remote monitoring; and health-financial services. It served approximately 95 million people as of December 31, 2025 and reported $101.957 billion of revenue for the year.
Three payment models
| Arrangement | How Optum Health is paid | Who carries the main cost risk |
|---|---|---|
| Fully accountable value-based | A monthly premium or population payment | Optum assumes responsibility for the covered population’s health-care costs |
| Administrative-fee | A monthly fee to manage or administer products and services | The contracting health plan or other customer generally retains medical-cost risk |
| Fee-for-service | A contracted fee for each medical or health-related service delivered | The payer bears the broader population risk; Optum is paid for delivered services |
These arrangements give Optum Health a different economic profile from a pure insurer. In a fully accountable contract, better coordination, prevention and lower avoidable utilization can improve the result, but unexpectedly severe illness can reduce it. Administrative-fee and fee-for-service contracts are more directly tied to managing activity or delivering care and generally expose Optum to less population-level claims risk.
Why scale and coordination can affect revenue
A patient may use several services over time: primary care, specialty treatment, behavioral support, remote monitoring and post-acute coordination. Optum Health’s value-based model seeks to manage that continuum rather than treating every encounter as an isolated transaction. Revenue can therefore reflect the number of people under management, the type of contract, medical intensity, rates and the mix between risk-bearing and fee-based work.

How Optum Rx earns money from pharmacy services
What the pharmacy business sells
Optum Rx provides pharmacy-benefit and pharmacy-care services to health plans, employers, unions, purchasing coalitions and public-sector customers. Its network and operations include about 64,000 retail pharmacies, home delivery, specialty and community pharmacies, infusion services, and support for rare-disease and gene-therapy treatments.

- [Dont Hesite to Order] 24 hours customer services and professional technology team are standing by
Customers pay Optum Rx to administer prescription benefits, negotiate and manage pharmacy access, operate dispensing channels and support complex therapies. Depending on the contract, economics can involve administrative fees, pharmacy service revenue and the management of prescription spending. The business is not limited to dispensing pills: specialty pharmacy, infusion and high-cost therapies require clinical support, distribution and coordination.
2025 scale indicators
- Optum Rx reported $154.726 billion of fiscal 2025 revenue.
- It managed $188 billion in pharmaceutical spending, including nearly $87 billion in specialty pharmaceutical spending, during 2025.
- Adjusted scripts increased to 1,659 million in 2025 from 1,623 million in 2024.
The company said revenue growth reflected higher prescription volumes from new and existing clients and growth in pharmacy services. Script counts indicate transaction volume, while managed-spending figures indicate the value of medicines administered for customers; neither number by itself equals Optum Rx revenue.
How Optum Insight earns money from information and technology
Customers and offerings
Optum Insight sells data, analytics, research, consulting, technology and managed-services solutions. Its customers include health plans, hospitals and other providers, employers, governments and life-sciences organizations. A health plan might buy analytics or administrative technology; a provider might buy revenue-cycle or clinical information services; a government or life-sciences customer might buy research, data or consulting.
Unlike UnitedHealthcare’s premium model, Insight is principally a business-to-business services operation. Contracts can be recurring managed services, technology arrangements, consulting engagements or other project and subscription structures. Revenue depends on signed customers, implementation activity, service volumes, renewals and the timing of large contracts.

Fiscal 2025 performance
Optum Insight revenue was $19.417 billion in 2025, up 4% from 2024. UnitedHealth Group attributed the increase to technology services and reduced effects from the Change Healthcare cyberattack, partly offset by lower business-services volumes. That combination illustrates why a services segment can grow even when some transaction-based or project volumes decline.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Where Optum Financial fits
Optum Financial serves consumers through nearly 26 million accounts and had more than $27 billion in assets under management at December 31, 2025. Its products include health-related financial accounts and services. It earns fees and also receives investment and interest income on managed funds and loans.
The reporting presentation changes on January 1, 2026: UnitedHealth Group said Optum Financial, including Optum Bank, would be presented within Optum Insight for segment reporting, with prior periods recast in 2026 filings. That accounting change does not mean the underlying customer accounts suddenly became a different product; it changes where the activity appears in segment disclosures.
UnitedHealthcare and Optum compared
| Comparison point | UnitedHealthcare | Optum |
|---|---|---|
| What it sells | Health-benefit coverage and the assumption of insurance risk | Care delivery, pharmacy services, information, technology, administration and financial services |
| Typical payers or customers | Employers, individuals, Medicare and Medicaid programs, and other government purchasers | Health plans, employers, providers, governments, unions, public-sector customers, consumers and life-sciences organizations |
| Main contract economics | Premium per member for a coverage period, with medical-cost risk | Value-based monthly payment, administrative fee, fee-for-service payment, technology or managed-service contract, and financial-service fees or investment income |
| Useful scale measure | People enrolled and the mix of commercial, Medicare and Medicaid membership | People served by Health, adjusted scripts and pharmaceutical spending in Rx, and customers and service volumes in Insight |
| Fiscal 2025 reported revenue | $344.903 billion | $270.620 billion before Optum eliminations |
The businesses are complementary. UnitedHealthcare can supply covered members and contracts, while Optum can provide clinical care, pharmacy administration or technology. The relationship can create operating scale, but it also makes the segment numbers unsuitable for simple addition.

What drives UnitedHealth Group’s total revenue
Enrollment and membership mix
More covered people generally create more premium and service activity, but the effect depends on where they are enrolled. Medicare Advantage, Medicaid and commercial members have different payment formulas, medical needs and provider arrangements. A shift toward higher-acuity members can increase revenue and claims at the same time.
Pricing and medical-cost trends
Insurers set or negotiate premiums before all future care costs are known. Pricing updates attempt to reflect provider rates, utilization, drug costs, member health and regulation. If prices lag medical-cost inflation, premium growth may not translate into stronger profitability.
Prescription volume and specialty complexity
Optum Rx benefits from additional prescriptions, new customers and pharmacy services. Specialty drugs, infusions and gene therapies carry high dollar values and require more clinical and distribution support. They can expand managed spending and service activity while also increasing the cost and complexity of care for the payer.
Quick wins for a faster PC:

Optum Insight grows when customers purchase technology, analytics and managed services, renew contracts or expand implementations. Revenue can be affected by project timing and business-service volumes, so a modest annual increase may conceal strong technology demand alongside weakness in another offering.
Capital and account balances
Optum Financial’s account base and assets under management support fee revenue and investment or interest income. Interest-rate conditions, balances and lending activity can therefore influence this part of the business differently from premiums or clinical encounters.
How to read the numbers without common mistakes
- Start with the fiscal-year label. The figures above are for fiscal 2025. The 2025 Form 10-K was filed in 2026, so the filing date and the financial year are not the same thing.
- Use consolidated revenue for the group total. The company-wide amount is $447.567 billion after eliminations.
- Use segment figures to understand engines, not to build a larger total. UnitedHealthcare’s $344.903 billion and Optum’s pre-elimination $270.620 billion include activity that can occur between group businesses.
- Separate revenue from operating indicators. Optum Health’s 95 million people served, Optum Rx’s 1,659 million adjusted scripts and $188 billion of managed pharmaceutical spending are scale measures, not additional revenue lines.
- Track reporting changes. From January 1, 2026, Optum Financial is presented within Optum Insight, and the company said prior periods would be recast in 2026 filings.
The bottom line
UnitedHealth Group makes most of its money from health-benefit premiums through UnitedHealthcare, while Optum adds several large service businesses. UnitedHealthcare monetizes enrollment and insurance risk; Optum Health monetizes coordinated care under risk-bearing and fee-based contracts; Optum Rx monetizes pharmacy administration and specialty services; and Optum Insight monetizes data, technology and managed services. Fiscal 2025 revenue of $447.567 billion reflects all of those activities after internal transactions are removed, which is why the segment totals must be read as overlapping views of the business rather than added together.
