Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteValidate every AI-generated mortgage QC finding against the loan file’s authoritative evidence and the exact rule the system says was met or violated. Treat the output as a review lead, not as proof. A qualified reviewer should reproduce the material check, confirm that the requirement applies to this loan and decision, classify the result, document the reasoning, and route any defect or uncertainty through the lender’s established process.
The workflow below is designed for mortgage quality-control, compliance, and model-risk teams. It separates file-level review from system-level monitoring and distinguishes requirements that apply to particular programs or institutions from general validation practices.

What validation means—and what it does not mean
An AI quality-control exception can be wrong in several ways. The source document may contain an error, the system may have extracted a value incorrectly, the reasoning may not follow the applicable underwriting rule, the rule may be outdated, or the model may have inferred a problem that the file does not support. Validation determines which of those situations exists.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Clear out junk files and repair common Windows errorsFree Scan →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Validation is not a second acceptance of the model’s explanation. The reviewer must inspect the underlying document or system-of-record field, identify the governing requirement, and independently check the amount, date, condition, or relationship at issue. The conclusion must be reproducible by another reviewer who has access to the same evidence.

- File-level validation: deciding whether one finding is correct for one loan.
- Process validation: checking that reviewers, sampling, independence, escalation, and retention operate as designed.
- System monitoring: measuring false positives, missed defects, and performance changes across files and after updates.
These layers should not be collapsed into a single “AI accuracy” number. A system can produce useful leads while still requiring substantial human review, and a low false-positive rate is not reassuring if the system misses material defects.
Step 1: Define the finding precisely
Start by freezing the exact output that triggered review. Record enough context that a later reviewer can identify the same event without relying on a mutable dashboard.
| Record | What to capture | Why it matters |
|---|---|---|
| Loan identity | Loan number or approved internal identifier, product and program, property, and file version or retrieval date | Prevents evidence from a different version or loan being attached to the decision |
| Finding text | Original wording, severity, confidence or priority indicator if supplied, and the document, field, or page named by the system | Preserves what the model actually asserted rather than a later paraphrase |
| Rule reference | The requirement, investor condition, policy control, or data-quality rule the finding claims was violated or satisfied | Allows the reviewer to test applicability and the precise pass/fail logic |
| System identity | Model or vendor name, model version, prompt or configuration version where relevant, workflow release, and run timestamp | Supports reproduction and change-impact analysis |
| Review metadata | Reviewer, review date, disposition, rationale, corrections, and escalation destination | Creates accountability and an auditable chain of decisions |
Reject labels that are too vague to test. An exception called “income issue” is not reviewable until it identifies the income source, document, amount, date, and rule. If the system cannot provide those details, classify the item as incomplete or unsupported rather than guessing what it meant.
Step 2: Trace the claim to authoritative evidence
Open the source document or system-of-record data named by the finding. For a document, verify the borrower or property identity, page sequence, dates, signatures where required, legibility, and completeness. For a data field, identify its originating system, entry history, and any transformations before it reached the AI workflow.
Check identity and scope
- Confirm that the document belongs to the subject loan and borrower.
- Confirm that the version is the one used for the underwriting or QC decision.
- Check whether the document covers the relevant period, property, transaction, or employment relationship.
- Determine whether a superseding document or corrected data exists elsewhere in the file.
Check consistency across the file
Compare the claimed value with reliable records such as the application, verification, closing documents, underwriting data, and required conditions. A discrepancy does not automatically prove that the AI is correct: it may reveal a stale record, a data-entry error, or a legitimate change. Record which source has priority under the lender’s procedure and why.
Fannie Mae’s post-closing QC guidance calls for checking the accuracy and integrity of information used for underwriting, the support for the underwriting decision, required approval conditions, and relevant loan-file data and documents. That principle is useful for any AI-assisted review, but its formal requirements apply within the guidance’s stated scope.

Preserve the evidence reference
Store a stable reference to the document and location: document type, page or section, field path, capture date, and file hash or equivalent control if your institution uses one. Do not rely on a screenshot of a transient interface when the underlying record can be retained under normal access controls.
Step 3: Independently reperform the material check
Reperform the check that would determine whether the finding changes a QC conclusion. The model’s explanation is a hypothesis; it is not evidence that its extraction or arithmetic is right.
- Re-read the source. Transcribe the relevant values and qualifiers directly from the authoritative document or field.
- Recalculate when applicable. Independently recompute the amount, ratio, date interval, tolerance, or condition using the lender’s approved method.
- Compare inputs. Check that the values sent to the automated underwriting system or other decision workflow match the source information.
- Apply the rule. Use the version of the underwriting, investor, or compliance requirement that was in force for the loan and decision date.
- Test materiality. Determine whether the discrepancy changes eligibility, approval conditions, pricing, collateral treatment, reporting, or a consumer-facing reason.
For calculations, retain the inputs, formula or rule section, rounding convention, and result. For qualitative requirements, quote or precisely describe the condition that must be present and identify where it is or is not satisfied. A reviewer should be able to reproduce the outcome without access to the AI system.
Step 4: Confirm that the cited requirement applies
An apparently valid exception can be a false positive when the rule is for a different product, investor, decision stage, date, or jurisdiction. Before deciding, establish the governing context.
- Loan and program: conventional, government, portfolio, correspondent, or another product; applicable investor or insurer.
- Decision stage: prefunding, underwriting, closing, servicing, post-closing, complaint response, or adverse action.
- Effective date: rule and policy version in force when the relevant action occurred.
- Data purpose: underwriting eligibility, collateral valuation, fraud control, servicing, reporting, or quality assurance.
- Geography and institution: state, federal, investor, and regulator requirements that actually govern the transaction.
Fannie Mae requirements are not universal mortgage law. The same is true of the Federal Reserve’s model-risk principles, CFPB adverse-action guidance, and the interagency automated-valuation-model rule. Use each authority only for the population and function it covers.

Step 5: Classify the result with a controlled disposition
Use a finite set of dispositions so aggregate reporting is meaningful. A free-text outcome such as “looks okay” cannot distinguish a verified pass from an unresolved question.

| Disposition | Use it when | Required explanation |
|---|---|---|
| Confirmed defect | The source evidence and applicable rule establish a material error, omission, or unmet condition | Identify the controlling evidence, rule version, impact, and corrective action |
| Valid exception | The finding is correct, but an approved exception, waiver, tolerance, or documented compensating factor applies | Reference the authorization, scope, date, and conditions of the exception |
| False positive | The evidence does not support the asserted issue, or the cited requirement does not apply | Explain the correct value or rule and why the model’s inference fails |
| Incomplete or unsupported | Evidence is missing, illegible, contradictory, or the system did not identify a testable rule | List what is missing and the action needed to resolve it |
| Escalation required | The issue is material, unresolved, potentially adverse to the borrower, or outside the reviewer’s authority | Name the qualified reviewer or function, urgency, and interim control |
Separate the cause from the disposition. For example, a confirmed defect may result from a borrower document error, an extraction error, an outdated rule, or a reviewer data-entry error. Tracking those causes is essential for remediation and model monitoring.
Step 6: Correct the authoritative record and escalate appropriately
When a defect is confirmed, correct the authoritative loan file, data field, condition tracker, or decision record through the lender’s established change process. Editing the AI finding alone does not repair a wrong underwriting input or missing document.
Escalate when the reviewer cannot safely resolve the issue
- The rule’s applicability is uncertain or conflicting authorities exist.
- The issue could change approval, pricing, eligibility, collateral treatment, servicing, or a borrower communication.
- Required evidence is unavailable or appears altered.
- The finding suggests a systemic failure, repeated extraction error, or model behavior outside its approved use.
- The reviewer lacks authority to waive, cure, or change the affected decision.
Use a named queue or role, not an informal message. Record the handoff, response, decision, and any temporary hold. If a finding may affect an adverse action, involve the responsible compliance or credit decision function before a notice is issued.
Step 7: Retain an auditable review packet
Retention should allow an independent person to reconstruct what the system saw, what the reviewer checked, and why the final disposition was reached. At minimum, retain:
- The unmodified AI output and run timestamp.
- Model, prompt, configuration, workflow, and vendor-version identifiers applicable to the run.
- References or controlled copies of source documents and system-of-record fields.
- Reviewer calculations, rule versions, notes, and disposition.
- Corrections to the loan file or downstream data, including who approved them.
- Escalation records, responses, and final resolution.
- Access history and retention classification under institutional privacy and security controls.
Protect borrower information according to the lender’s access, retention, and information-security policies. A complete audit trail is not a license to copy sensitive documents into uncontrolled model prompts or personal workspaces.
How prefunding and post-closing reviews shape the workflow
Fannie Mae’s prefunding QC guidance describes a written plan, documented timing and loan selection, verification procedures, and reviewers who were not involved in originating, processing, or underwriting the reviewed loan. It also describes full-file and component reviews and risk-informed sampling across production channels. Those are program requirements in their stated scope; elsewhere they are useful design patterns, not automatic obligations.

- Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft
Apply the same separation of duties to AI-assisted review. The person who configured a workflow or approved the loan should not be the sole person deciding whether the AI exception is valid. Define when a review occurs, which loans or components are selected, what evidence is checked, and how unresolved findings are reported. Risk-informed selection can include new channels, unusual document types, high-severity outputs, and areas where prior monitoring found misses.
Adverse-action decisions require a separate reason check
When an AI-generated finding influences a credit denial, withdrawal, or change in terms, validate the reason that reaches the borrower against the actual decision logic and evidence. CFPB guidance says adverse-action reasons must be specific and accurate and must reflect the actual basis for the decision; generic sample reasons that do not match the decision are insufficient.
- Identify the final decision factors actually used, including any human overrides.
- Link each communicated reason to the verified file evidence and applicable rule.
- Remove a reason that was generated by the AI but did not affect the decision.
- Ensure the explanation is specific enough for the consumer to understand the relevant deficiency or factor.
- Have the responsible compliance or credit function approve the notice under its normal controls.
Do not assume that a plausible narrative is an accurate reason. A model can summarize a document persuasively while citing a factor that was not part of the decision.
Governance boundaries and applicable authorities
| Authority or guidance | What it addresses | How to use it when validating AI findings |
|---|---|---|
| Fannie Mae post-closing QC guidance | Accuracy and integrity of underwriting information, support for the decision, approval conditions, and file data and documents | Use as the file-evidence standard for loans within its scope; do not generalize it to every lender or investor |
| Fannie Mae prefunding QC guidance | Written plans, timing, selection, verification, reviewer independence, and full-file or component reviews | Use its controls where applicable and as a model for designing independent review elsewhere |
| Fannie Mae Lender Letter LL-2026-04, April 8, 2026 | AI/ML governance for seller/servicers, including development, implementation, use, maintenance, risk management, vendor and subcontractor oversight, and disclosure of use and safeguards upon request; effective 120 days after publication | Confirm the current Selling Guide and contract before implementation and apply the letter only to institutions and practices within its scope |
| Federal Reserve model-risk guidance | Understanding vendor models, design and development data, performance, validation, ongoing monitoring, and outcome analysis; the cited principles describe traditional quantitative models and non-generative, non-agentic AI | Use it to structure model-risk controls, while identifying additional requirements for generative or agentic workflows |
| CFPB adverse-action guidance | Specific and accurate reasons that reflect the actual basis for a credit decision | Use it whenever an AI finding contributes to a borrower-facing adverse action |
| Interagency AVM final rule | Quality-control controls for covered automated valuation models used in certain mortgage collateral decisions, including confidence, data manipulation, conflicts, testing, and nondiscrimination; FHFA lists October 1, 2025 as the effective date | Treat it as an AVM-specific requirement, not a universal rule for mortgage AI QC |
| NIST AI Risk Management Framework and bias-testing resources | General practices for identifying and testing AI risks | Use as an evaluation aid; the cited resources are not mortgage-specific regulations |
Test the system across loans, not just one exception
Individual validation tells you whether a finding is right. Monitoring tells you whether the workflow remains dependable after a model, vendor, prompt, document template, policy, or channel changes.
Build a representative review sample
Stratify samples by finding type, source-document type, loan program, origination channel, geography where relevant, severity, and whether the item was accepted or overridden. Include files with no AI exception so missed defects can be found. Preserve the selection method and review period; otherwise rates cannot be interpreted.
Measure both false positives and misses
- False-positive rate: the share of raised findings classified as false positives after qualified review.
- Miss rate: material defects discovered by reviewers that the system did not flag.
- Unsupported or incomplete rate: findings that cannot be tested because evidence or rule context is inadequate.
- Severity-weighted results: separate minor documentation issues from findings that affect eligibility, terms, collateral, or compliance.
- Time-to-resolution: how long material findings remain unresolved and where escalation queues bottleneck.
No universal accuracy, savings, or error-reduction percentage should be assumed. Report results with the sample definition, review method, date range, population, and confidence limitations.

Monitor changes and drift
Trigger a focused review after model or vendor updates, prompt or configuration changes, document-template changes, new loan programs, new channels, or policy revisions. Compare pre-change and post-change distributions of findings, dispositions, severity, and missing-evidence rates. Investigate sudden shifts rather than treating them as automatic improvement.
Check performance across relevant populations
Where legally and operationally appropriate, analyze whether extraction, escalation, false positives, or misses vary across channels, document formats, borrower or property characteristics, or other relevant populations. Protect sensitive information and use qualified compliance and legal guidance for any fairness analysis. A stable overall rate can conceal concentrated errors in a subgroup or document type.
Evaluation criteria for an internal workflow or external system
Whether the capability is built internally or obtained from a vendor, evaluate the control surface rather than a marketing accuracy claim.
Recommended Free Tools
- Traceability: Can each assertion be tied to a page, field, or source record?
- Rule reproducibility: Is the requirement and version visible, and can a reviewer rerun the check?
- Error measurement: Are false positives and missed defects measured by type and severity?
- Coverage: Does performance hold across file types, programs, and origination channels in the approved use?
- Auditability: Are outputs, versions, changes, overrides, and escalations logged immutably enough for review?
- Human control: Can a qualified reviewer override, defer, or escalate without losing the original output?
- Change monitoring: Are model, vendor, prompt, and document updates identified and tested?
- Privacy and security: Are borrower records protected in transit, at rest, in prompts, and in retention stores?
- Regulatory fit: Can the workflow support the investor, regulator, and consumer-notice requirements that apply to the lender?
Common failure modes and corrective actions
| Failure mode | Why it fails | Correction |
|---|---|---|
| Accepting the confidence score as proof | Confidence does not establish that the source was read correctly or that the rule applies | Require source evidence and an independent rule or calculation check |
| Reviewing only flagged loans | Missed defects remain invisible | Include unflagged files and component samples in monitoring |
| Using a current rule for an older decision | The reviewer evaluates the wrong obligation | Lock the effective-date and version context at review time |
| Fixing the narrative but not the file | The authoritative data or condition remains wrong | Correct the source record through controlled procedures |
| Allowing the originator to be the sole validator | Conflicts of interest can suppress defects | Use independent review for the applicable QC stage |
| Sending a generic adverse-action reason | The reason may not reflect the actual decision basis | Reconcile every communicated reason to verified evidence and logic |
| Ignoring version changes | Performance shifts can be mistaken for random variation | Record versions and run change-triggered validation samples |
A practical reviewer worksheet
A controlled worksheet can make the process consistent without replacing judgment. Require these fields before closure:
- Loan identifier, program, decision stage, and file version.
- Original finding, severity, model and workflow versions, and run date.
- Exact source document or data field, page or path, and evidence reference.
- Requirement name, jurisdiction or investor, effective date, and version.
- Independent extraction, calculation, or comparison performed.
- Disposition: confirmed defect, valid exception, false positive, incomplete or unsupported, or escalation required.
- Plain-language rationale that another reviewer can reproduce.
- Correction, owner, due date, and escalation record where applicable.
- Impact on underwriting, QC reporting, servicing, consumer communication, and system monitoring.
Supervisors should sample completed worksheets for evidence quality and reasoning, not merely for whether the reviewer agreed with the AI. Trends in unsupported findings, repeated causes, and overrides should feed policy, training, and model-management decisions.
Bottom-line decision rule
Close an AI-generated mortgage QC finding only when the evidence is authoritative and complete, the applicable requirement is identified for the correct loan and date, an independent check supports the conclusion, and the disposition and rationale are recorded. If any of those conditions is missing, mark the item incomplete or escalate it. Then monitor both flagged and unflagged loans so the institution can detect false positives, missed defects, and performance changes after system updates.
