Attack reconstruction turns scattered security evidence into a defensible account of an incident: how access was obtained, what happened next, which systems or data were affected, what remains uncertain, and what responders should do now. The method is not a single forensic tool or a perfectly linear recipe. It is a disciplined cycle of collecting and preserving relevant evidence, examining it, correlating observations into a timeline, testing alternative explanations, and reporting conclusions with confidence and limitations.
Use the workflow below to move from alerts and artifacts to decisions without confusing an investigator’s theory with something the evidence actually proves.

What attack reconstruction should establish
A useful reconstruction answers a set of operational questions in an order that helps people make safe decisions:
Recommended Free Tools
- What was the likely initial access vector?
- Which accounts, hosts, applications, cloud resources, or operational assets were involved?
- What sequence of actions can be supported by independent evidence?
- Was data viewed, changed, deleted, or transferred outside the environment?
- What persistence, privilege changes, or lateral movement occurred?
- What is the current scope, and which parts of the story remain unknown?
- Which containment, eradication, recovery, notification, or monitoring decisions follow from the evidence?
This is different from listing alerts. A firewall alert, a suspicious process name, and a login record become meaningful only when their relationships are examined. NIST describes forensic analysis as seeking an explanation of an incident’s root cause. The result should therefore distinguish observations (what an artifact directly shows) from interpretations (what those observations suggest) and from decisions (what the organization chooses to do).

The reconstruction is an explanation assembled from evidence, not a claim that every unanswered question has been solved. A defensible report says where the evidence is strong, where it is indirect, and where logging or preservation gaps prevent a conclusion.
Use a four-phase evidence lifecycle
NIST SP 800-86 describes a practical lifecycle of collection, examination, analysis, and reporting. Treating these as separate activities prevents an early suspicion from becoming a premature finding.
| Phase | Purpose | Typical output |
|---|---|---|
| Collection | Identify, acquire, and preserve relevant data while recording provenance and handling. | Evidence inventory, acquisition notes, integrity records, and chain-of-custody entries. |
| Examination | Use forensic techniques to identify and extract information from the collected material. | Parsed events, recovered files, process records, authentication entries, and other observations. |
| Analysis | Correlate observations, test explanations, determine scope, and assess confidence. | Timeline, supported findings, alternatives considered, gaps, and impact assessment. |
| Reporting | Explain what happened and connect evidence to containment, recovery, and follow-up decisions. | Decision-ready report with evidence references, limitations, actions, and open questions. |
SP 800-86 was published in August 2006, so use it as a foundational process guide rather than as a current catalog of tools or threats. NIST expressly warns: “This publication is not to be used as an all-inclusive step-by-step guide for executing a digital forensic investigation or construed as legal advice.” Consult management and legal counsel about applicable legal, regulatory, contractual, and privacy requirements.

Step 1: Define the questions and protect response priorities
Before collecting everything available, write down what the response team must establish. A focused question set prevents the investigation from becoming an unbounded data-gathering exercise.
Write an investigation brief
- Trigger: record the alert, report, or observation that started the response, including when it was received and by whom.
- Scope hypothesis: list currently suspected accounts, devices, applications, segments, cloud tenants, and data stores. Label each item as suspected rather than confirmed.
- Questions: specify whether the priority is initial access, privilege escalation, persistence, lateral movement, data access, exfiltration, destructive activity, or a combination.
- Decision owners: identify who can isolate systems, disable accounts, rotate credentials, take services offline, approve restoration, or make notifications.
- Constraints: note safety, availability, business-continuity, legal-hold, privacy, regulatory, and contractual requirements.
Set a decision clock as well as an evidence clock. If a compromised account is actively being used, preventing further harm may outrank collecting every volatile artifact. Record why an action was taken before collection and what evidence may have been lost as a result.
Protect the response itself
- Use a controlled case record with a unique incident identifier.
- Restrict access to evidence and record each handoff or examination.
- Preserve volatile data when it is safe and proportionate to do so, but do not endanger people, production, or operational processes for completeness.
- Coordinate with legal counsel before actions that could affect privilege, notification duties, privacy obligations, or evidence admissibility.
Step 2: Identify and preserve relevant evidence sources
Choose sources based on the incident, architecture, and questions—not on a fixed shopping list. NIST identifies files, operating-system artifacts, network traffic, and applications as potential evidence categories. Cloud control-plane logs, identity-provider records, endpoint telemetry, email systems, SaaS audit trails, and OT controllers may add essential context when they are part of the affected environment.

| Source category | Examples of useful observations | Preservation concerns |
|---|---|---|
| Files and storage | Creation, modification, deletion, archive contents, scripts, configuration changes, and recovered remnants. | Preserve original media or approved forensic images; document mounted paths and acquisition method. |
| Operating system | Logons, services, scheduled tasks, process execution, accounts, drivers, registry or configuration changes, and local logs. | Record host identity, time zone, clock condition, collection time, and whether logs had rolled over. |
| Network traffic and controls | Connections, DNS lookups, proxy requests, firewall decisions, VPN activity, flow records, and packet captures where available. | Capture retention windows, sensor location, filtering, NAT or proxy effects, and gaps in coverage. |
| Applications and identity | Authentication, authorization, API calls, administrative actions, database queries, mailbox access, and audit events. | Export in a format that preserves event identifiers and original timestamps; retain tenant, account, and object context. |
| Endpoint and security telemetry | Process trees, command lines, hashes, detections, isolation actions, and analyst annotations. | Keep the original alert and sensor metadata; note rule changes or sensor outages. |
| Operational technology | Engineering-workstation activity, controller or historian events, network segmentation changes, and process alarms. | Coordinate with operators and safety owners; collection must not create unsafe states or unacceptable downtime. |
Record provenance at acquisition
For every item, capture the system or device, source location, collection date and time, time zone, collector, tool or method, case identifier, and any integrity check used. Record who accessed or transferred it, when, and for what purpose. If a hash or other integrity mechanism is used, store the value with the acquisition record and explain what was hashed. These details allow another investigator to assess whether a later observation is trustworthy.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Clear out junk files and repair common Windows errors3Scan for outdated or missing drivers - takes under a minutePreserve originals according to organizational procedure and perform analysis on controlled copies where appropriate. A chain of custody is not a decorative form: it is the history that connects an observation in a report to the source from which it was obtained.
Step 3: Examine artifacts before writing the story
Examination is the extraction stage. Parse logs, inspect file-system metadata, review process and authentication records, and identify relevant objects without yet declaring the incident’s cause. Maintain an observation log in which each entry contains:
- the exact artifact or event identifier;
- the literal value or content observed;
- the parser, query, or technique used;
- the source and acquisition reference;
- any transformation, filtering, or normalization applied; and
- an investigator note that is explicitly labeled as interpretation rather than observation.
For example, an event may show that an administrative account authenticated to a server from an unusual address. That does not, by itself, prove that an attacker controlled the account, that the address belonged to the attacker, or that the login led to a compromise. Those are hypotheses to test against endpoint, identity, network, and user-activity evidence.
Keep originals and derived data separate
Store raw exports, images, packet captures, and original log files separately from parsed tables, screenshots, timelines, and analyst notes. Document parsing errors, truncated fields, unavailable decryption keys, and records that could not be acquired. A derived timeline should always point back to the underlying evidence reference.
Free tools Windows power users keep installed

Step 4: Build a normalized, confidence-aware timeline
Correlation is where separate observations become a sequence. Normalize event times to a declared standard, usually UTC, while retaining the original timestamp and time zone. Document known clock drift, daylight-saving changes, ingestion delays, retention boundaries, and periods when a sensor or host was offline.

Use a working table such as this one:
| Time and confidence | System or account | Observed event | Source artifact | Interpretation |
|---|---|---|---|---|
| 2026-10-12 09:14 UTC (high) | Identity service / user account | Successful authentication from an address not previously associated with the account. | Identity audit event ID | Unusual login; investigate whether it was legitimate, proxied, or compromised. |
| 2026-10-12 09:18 UTC (medium) | Endpoint-17 | New command interpreter process appears under the account. | Endpoint process telemetry | Possible post-authentication execution; requires command-line and parent-process context. |
| 2026-10-12 09:26 UTC (high) | File server | Large outbound transfer to an external destination. | Proxy and flow records | Potential data movement; identify the process, destination ownership, and transferred objects. |
The rows above are an illustrative format, not evidence of a real incident. In your case, include an explicit gap row whenever telemetry is missing or contradictory. If two sources disagree, retain both observations and explain plausible causes—such as clock skew, delayed ingestion, NAT, incomplete logging, or a parser error—instead of silently forcing one chronology.
Use confidence consistently
- High: a directly recorded event with reliable provenance and corroboration, or a record whose meaning is unambiguous in context.
- Medium: a credible observation that depends on one source, uncertain clock accuracy, or an interpretation that still has a plausible alternative.
- Low: an indirect indicator, incomplete record, or hypothesis that has not yet been independently confirmed.
State the basis for every rating. Confidence belongs to the specific claim—not to the incident as a whole. A timeline can contain a high-confidence login, a medium-confidence process interpretation, and a low-confidence attribution hypothesis at the same time.
Step 5: Test competing explanations
Do not stop when the first plausible narrative appears. Turn it into a hypothesis and actively look for evidence that should exist if it were true.
Use a hypothesis matrix
| Question | What to check | How to record the result |
|---|---|---|
| What would this hypothesis predict? | Expected logons, process ancestry, file changes, network connections, or account actions. | List each prediction before searching, so absent evidence is not ignored. |
| Is the predicted evidence present? | Search independent sources and verify the time window and retention coverage. | Mark confirmed, not found, unavailable, or contradictory. |
| Could benign administration explain it? | Compare approved changes, maintenance windows, automation, help-desk activity, and known scanners. | Record the specific benign explanation and whether it fits all observations. |
| What would refute it? | Look for an impossible sequence, a different user, an incompatible process parent, or an alternate source location. | Keep disconfirming evidence visible rather than burying it in notes. |
A tool name, malware label, indicator, or technique can support an investigative lead; none alone proves attacker identity or intent. Keep attribution claims narrow and separately qualified. The evidence may establish that an account was used, without establishing who operated it or why.
Step 6: Determine scope and impact without overclaiming
Translate the timeline into an asset-and-data scope. For each potentially affected item, record the evidence that places it in scope, the activity observed, and the status of containment.
| Scope question | Evidence to correlate | Careful conclusion |
|---|---|---|
| Which accounts were affected? | Authentication, token, password-reset, privilege, and identity-provider events. | State whether use, compromise, or only exposure is supported; these are not interchangeable. |
| Which systems were accessed? | Remote sessions, endpoint process records, administrative logs, network paths, and service activity. | Separate confirmed access from systems merely scanned or contacted. |
| Which data was viewed or changed? | Application audit logs, file access records, database queries, object-storage events, and transfer records. | Distinguish attempted access, successful access, modification, and confirmed transfer. |
| What persistence remains? | New accounts, scheduled tasks, services, startup entries, tokens, keys, and configuration changes. | Identify artifacts still present versus mechanisms already removed. |
When logs cannot prove whether a record was viewed, say so. “No evidence found” means the reviewed sources contained no supporting event; it does not mean the event was impossible. Include retention limits, disabled auditing, encrypted content, and systems that were never acquired.

Step 7: Report findings so decision-makers can act
Reporting is a distinct forensic phase. A decision-ready report should let a technical reviewer reproduce the reasoning and let a leader understand the consequences without reading every raw log.
Quick wins for a faster PC:
Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Clear out junk files and repair common Windows errorsFree Scan →Recommended report structure
- Executive finding: a short statement of the supported sequence, current scope, and immediate risk.
- Incident questions and boundaries: what the investigation was intended to establish and what was outside scope.
- Environment and sources: systems examined, collection times and time zones, methods, provenance, and integrity records.
- Chronology: the normalized timeline, confidence for each material event, and visible gaps or conflicts.
- Findings: observations, interpretations, affected assets, data impact, persistence, and alternative explanations considered.
- Limitations: missing logs, clock uncertainty, retention boundaries, unavailable systems, parser constraints, and assumptions.
- Actions and decisions: containment, credential changes, eradication, recovery, monitoring, legal or regulatory review, and owners.
- Open questions: unresolved issues, the evidence needed to answer them, and a responsible person and due date.
Link each material conclusion to an evidence reference such as an event ID, image identifier, hash, file path, or acquisition record. Explain why the evidence supports the conclusion and identify any assumption that connects them.
Connect reconstruction to recovery
Use the reconstruction to decide what must be isolated, rebuilt, credentialed, monitored, or validated. NIST recovery guidance emphasizes planning, playbooks, testing, improvement, and learning from past events; its Respond guidance is available at https://www.nist.gov/cyberframework/respond. Preserve the timeline and decisions as lessons for detection and recovery improvements, not only as an incident archive.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. When the environment is operational technology
Do not apply an IT-first evidence plan mechanically to industrial control, building-management, medical, transportation, or other operational technology (OT). A production controller or engineering workstation may affect physical safety and availability. In those environments, the immediate goal may be enough forensic understanding to contain and recover safely, even when that means collecting less detail before action than an IT investigation would.
NIST IR 8428, the Digital Forensics and Incident Response (DFIR) Framework for Operational Technology (OT), published June 22, 2022, discusses OT preparation, incident handling, and forensic techniques. Its framework should be used with operators, control engineers, safety personnel, and asset owners.
Adapt the workflow for safety and continuity
- Have an OT operator and a safety owner approve collection and isolation actions.
- Prefer passive or vendor-supported collection where active tooling could alter timing, load, or controller state.
- Record process conditions, maintenance windows, and operator actions alongside cyber events.
- Sequence evidence capture around safe containment and controlled recovery, not around a theoretical requirement to image every device first.
- Test restoration and monitoring procedures in a manner that does not introduce a new process hazard.
NIST’s OT framework also notes that training and practice are needed for proficiency. Teams that may respond to OT incidents should rehearse roles, evidence handling, and safe decision points before an emergency.

Current NIST guidance and how to use it
For current general incident-response recommendations, use NIST’s incident-response publications index. It lists SP 800-61 Rev. 3 as final, released April 3, 2025. The SP 800-61 Rev. 2 page records that Rev. 2 was withdrawn on April 3, 2025 and superseded by Rev. 3.
Use Rev. 3 for the current incident-response context and SP 800-86 for its enduring forensic lifecycle and evidence-handling concepts. Neither publication removes the need to adapt procedures to your systems, authority model, safety constraints, or applicable law.
Common reconstruction failures and corrections
| Failure | Why it damages the investigation | Correction |
|---|---|---|
| Starting with a favorite attacker narrative | Confirming evidence is selected while contradictory evidence is ignored. | Write predictions for at least one alternative explanation and test both against independent sources. |
| Treating timestamps as absolute | Clock skew, time zones, ingestion delay, and retention gaps can create a false order. | Normalize times, retain originals, document drift and delay, and assign confidence. |
| Collecting without provenance | Later reviewers cannot determine where an observation came from or whether it changed. | Record source, collector, method, time zone, integrity check, and every handling step at acquisition. |
| Using one log source as the whole story | A single source may be incomplete, forged, delayed, or missing context. | Correlate identity, endpoint, application, network, and file evidence where relevant. |
| Equating an indicator with attribution | Tools, hashes, addresses, and techniques can be reused or spoofed. | State only what the evidence establishes and separate identity or intent as an unresolved question. |
| Erasing evidence during containment | Reimaging, account deletion, or aggressive cleanup can destroy useful artifacts. | Capture proportionate evidence first when safe; document any action that necessarily sacrifices data. |
| Applying IT procedures to OT unchanged | Collection or isolation can interrupt a physical process or create a safety hazard. | Use OT operators and safety owners to choose a safe evidence-and-recovery sequence. |
| Reporting certainty without limitations | Decision-makers may treat an inference as a proven fact and make the wrong response choice. | Attach evidence references, confidence, assumptions, gaps, and open questions to each material finding. |
Illustrative reconstruction example
The following is a hypothetical example showing how to write the reasoning, not a claim about a real incident.
PC Slower Than It Used to Be?
A free scan shows the junk files, broken settings and background clutter dragging Windows down - then fixes them in one click.Free scan · Windows 10 & 11Outdated Drivers Are Slowing You Down
One free scan finds every outdated or missing driver and matches the right update for your exact hardware.Free scan · exact hardware matchAn identity log records a successful sign-in to a finance account from an address the account had not used before. Eighteen minutes later, endpoint telemetry shows a command interpreter launched by the same account, and proxy records show a large outbound transfer from a file server. The three observations are placed in one normalized timeline, but the first conclusion is deliberately narrow: the account authenticated, a process ran under that account, and an outbound transfer occurred.
Investigators then check approved remote work, VPN records, the process parent and command line, file-access auditing, destination ownership, and the account holder’s activity. If a scheduled finance export explains the transfer and the process ancestry matches an approved automation service, the intrusion hypothesis weakens. If the command line enumerates files, the account has no approved session, and the destination is unrelated to the organization, the hypothesis gains support. Even then, the report should distinguish confirmed transfer from the unresolved question of who operated the account.
This structure keeps the chronology useful without turning correlation into proof. Every step points back to an artifact, an alternative explanation is considered, and uncertainty remains visible.
Field checklist for an evidence-led reconstruction
- Have the investigation questions, decision owners, and safety or continuity constraints been written down?
- Are suspected assets and accounts labeled as suspected until evidence supports inclusion?
- For each acquisition, are source, time zone, collector, method, location, integrity information, and handling recorded?
- Are originals protected and derived data traceable to them?
- Does the timeline preserve original timestamps, normalized times, clock limitations, and confidence?
- Are missing, conflicting, and unavailable records shown rather than silently omitted?
- Has at least one plausible benign or alternative explanation been tested?
- Are access, viewing, modification, and transfer distinguished in the impact assessment?
- Does every material finding have an evidence reference and a stated limitation?
- Have containment and recovery actions been linked to the supported scope and residual risk?
- For OT, did operators and safety owners approve the sequence of evidence collection and recovery?
- Have lessons been converted into tested logging, playbook, monitoring, and recovery improvements?
Bottom line for investigators
Attack reconstruction is disciplined reasoning over preserved evidence. Define the questions, collect only what is relevant and safe, keep provenance intact, examine artifacts before interpreting them, correlate independent sources into a time-aware timeline, test competing explanations, and report findings with confidence and limits. The strongest reconstruction is not the most dramatic story; it is the one another competent investigator can trace from decision back to observation and from observation back to preserved evidence.
