Catch a security control that is not working by testing three things: whether it is implemented correctly, whether it operates as intended, and whether it achieves its security objective. A written policy or enabled setting proves only that something exists; it does not show that the control works under real conditions. Plan the assessment around the risk, gather operational evidence, correct deficiencies, then retest.
What does it mean for a security control to work?
A control is effective only when it is correctly implemented, operates as intended, and produces its intended security outcome. NIST defines a security control assessment as an examination of controls to determine the extent to which they are implemented correctly, operating as intended, and producing the desired outcome. See NIST’s glossary definition.

Those dimensions are related but distinct. A setting may be configured correctly while a process around it fails; a process may run regularly but not address the threat it was meant to reduce. Assess the result, not just the presence of a policy, procedure, or product.

Use a documented plan and procedures tailored to the control, system, and risk. NIST SP 800-53A Revision 5 is its final guide for assessing security and privacy controls, and its procedures can be tailored to an organization’s risk-management needs. It is a framework for assessment, not a single universal test.

- State the objective and scope. Describe what the control is meant to prevent, detect, or enable. Identify the systems, processes, relevant conditions, and risk tolerance included in the assessment.
- Decide what evidence would demonstrate success. Select evidence suited to the control: for example, relevant configurations, records, procedures, or observed results. A document can establish what should happen; it does not by itself establish that the activity happens consistently.
- Test implementation, operation, and outcome. Check whether the control is set up correctly, observe whether it functions as intended under relevant conditions, and determine whether it achieves its stated objective. Choose procedures that fit the control and environment rather than applying a checklist mechanically.
- Record the basis for the conclusion. Note scope, evidence examined, method, limitations, and results so another assessor can understand how the conclusion was reached.
- Investigate gaps and exceptions. Compare observed operation with the objective and expected evidence. Missing evidence, inconsistent performance, or an unmet outcome warrants investigation; a checklist pass is not persuasive if the evidence points the other way.
- Prioritize and correct deficiencies. Document weaknesses, evaluate their risk, assign corrective actions, and communicate the security state to the people responsible for decisions.
- Retest and monitor. After remediation, assess again to verify that the control’s operation or outcome has changed. Continue monitoring selected controls and relevant system or environmental changes between formal assessments.

What evidence can reveal a failing control?
Match the evidence to what the control is supposed to do. For a technical control, configuration evidence may show implementation, while records or observed results may show whether it operates. For management or operational controls, procedures and records can be relevant, but assess whether the described activity actually occurs and whether it produces the required result.
Rank #2
- Implementation gap: the control is absent, misconfigured, or applied to only part of the stated scope.
- Operational gap: the control exists but is inconsistently performed, does not work under relevant conditions, or lacks evidence of execution.
- Outcome gap: the control operates, but the risk it was intended to address remains insufficiently managed.
- Evidence gap: available records or observations cannot support a reliable conclusion. Treat this as uncertainty to resolve, not as proof of effectiveness.
How should you choose an assessment method?
Compare candidate methods against the control’s risk and the strength of evidence they can produce. These are practical decision factors, not a NIST scoring scale.

- Coverage: Does the method include the relevant controls, environments, and operating conditions?
- Evidence quality: Can it show operation and outcomes, rather than only policy language or configuration?
- Repeatability: Could another assessor follow the method and understand the same conclusion?
- Independence: Is a self-assessment adequate, or is an independent review also needed?
- Cadence and change response: How often is evidence refreshed, and which system or environmental changes should trigger reassessment?
- Risk fit: Does the assessment scope reflect the organization’s risk tolerance and the potential impact of control failure?
How do you catch problems between formal assessments?
Formal assessments provide a structured view at a point in time. Monitoring selected controls and tracking relevant changes can help reveal degradation sooner. NIST SP 800-137 describes an information security continuous monitoring program that provides visibility into assets, threats, vulnerabilities, and the effectiveness of deployed controls, supporting timely responses when controls appear inadequate.

Use findings to make corrective work specific and verifiable: identify the deficiency, assess its risk, assign responsibility, and define what evidence will demonstrate that the fix worked. Then reassess the control against its original objective. NIST emphasizes that assessment is not merely paperwork for an inspection; its January 25, 2022 announcement states: “Control assessments are not about checklists, simple pass/fail results, or generating paperwork to pass inspections or audits.” See NIST’s announcement on updated security and privacy controls.
