Normal post-workout soreness usually develops gradually after unfamiliar or harder exercise, affects the muscles you worked, and eases over a few days. Pain that begins suddenly during exercise, is sharp or strong, worsens, or limits movement is more concerning for an injury. These patterns can guide what to do next, but they cannot diagnose the cause.
How normal soreness and injury tend to differ
| Clue | More consistent with normal delayed-onset muscle soreness (DOMS) | More concerning for injury |
|---|---|---|
| Onset | Builds over hours after exercise and is often noticed the next day or within one to three days. | Starts suddenly during activity, particularly if the pain is sharp or strong. |
| Location and sensation | Dull soreness, tenderness, or stiffness in the muscles that were exercised. | Localized or persistent pain, especially when it restricts movement or prevents normal use. |
| Course | Usually improves over a few days. | Worsens, stays severe, does not improve with rest, or continues beyond a short recovery period. |
| Other signs | Mild, temporary stiffness or tenderness may occur. | Substantial swelling or bruising, deformity, difficulty bearing weight or using the area, cold or blue-grey skin, or dark or bloody urine after intense exercise. |
DOMS commonly follows a new or more demanding workout. Its delayed timing and improvement over a few days are useful clues, not guarantees that an injury is absent. Pain intensity alone is not a reliable diagnostic cutoff; how it began, whether it is changing, and whether you can move and use the area matter too. Cleveland Clinic’s DOMS guidance describes the usual timing and course.

What to do when you are sore or in pain
If soreness is mild and improving
Give the sore muscles a break from strenuous training while symptoms are prominent. Gentle everyday movement may be tolerable, but do not work those muscles hard just to test them. Rest is generally the best treatment for DOMS. Gradually increasing the demands of a new exercise program can reduce its severity, and soreness is not required for fitness gains. ACSM’s guidance on delayed-onset muscle soreness notes that measures such as an ice pack may ease symptoms, but symptom relief does not establish that recovery is complete.
If pain starts during exercise
Stop the activity that causes pain rather than pushing through sharp or acute pain. The American College of Sports Medicine advises stopping an exercise that causes pain. Feeling better after a symptom-relief measure is not clearance to return to strenuous exercise; resume only when the area is functioning comfortably, and seek advice if symptoms persist or interfere with use.

When to get medical help
- Contact a healthcare professional if pain is severe, persistent, worsening, or limits movement or normal use.
- Seek medical advice for heavy swelling or significant bruising, difficulty bearing weight, or a body part you cannot use. A visibly deformed area also needs urgent assessment.
- Cold or blue-grey skin around an injured area is a warning sign that needs urgent assessment; see the NHS guidance on sprains and strains.
- Dark or bloody urine after intense exercise can indicate serious muscle breakdown and needs urgent medical assessment.
Do not use a soreness checklist to rule out injury. A clinician can assess symptoms that are severe, persistent, worsening, or function-limiting. Cleveland Clinic’s overview of sports injury symptoms includes swelling, bruising, deformity, and difficulty moving or bearing weight. In a 2025 article, Cleveland Clinic sports medicine physician Dominic King, DO, put the exercise distinction this way: “A certain low level of soreness is acceptable, but you should not push through pain while exercising.” Read Cleveland Clinic’s discussion of post-workout soreness.
