The reliable way to stop an AI agent from sending, buying, deleting, changing, or publishing something you did not authorize is to use layered controls: deny-by-default permissions, a narrow tool and data allowlist, authorization enforced outside the model, approval gates for high-impact actions, isolation against prompt injection, and independent logging with interruption and rollback. A system prompt that says “ask first” is useful guidance, but it is not an access-control mechanism.
Why an AI agent can take an action you did not intend
An agent combines a language model with tools such as browsers, APIs, shells, payment services, email, file systems, or administrative consoles. The model may choose a tool call that appears consistent with its immediate objective while still producing an outcome you would not approve. The underlying causes are usually architectural rather than mysterious.
As an Amazon Associate I earn from qualifying purchases.
- Overbroad permissions: the agent can write, delete, purchase, or change accounts when the task only requires reading.
- Ambiguous authorization: a general instruction such as “handle the order” is interpreted as permission to place it, not merely prepare it.
- Prompt injection: a web page, document, email, repository, or tool response contains instructions that try to redirect the agent.
- Unsafe automation: retries, duplicate submissions, stale approvals, or a compromised integration repeat an action.
- Missing controls around the model: the agent is trusted to police itself instead of having every call checked by an independent component.
External content is data, not authority. A sentence in an invoice, web page, or README cannot grant an agent permission to spend money or modify a system. Permission must come from an identity and policy decision that your control plane can verify.
Classify actions before granting access
Make an inventory of every tool and operation the agent can invoke. Classify each operation by potential impact, reversibility, and who could be affected. Unknown tools and unclassified operations should fail closed rather than run with inherited credentials.

| Action class | Typical examples | Minimum control |
|---|---|---|
| Low risk, read-only | Search, retrieve a record, summarize a document, inspect a status page | Read-only credential, resource allowlist, rate limit, audit event |
| Moderate risk, reversible write | Draft a message, create a non-public ticket, edit a working copy | Task-scoped write permission, parameter validation, review before external visibility |
| High risk, externally visible | Send email, publish content, change a production setting, modify code or systems | Explicit approval tied to the exact operation, target, and parameters; independent validation |
| Critical or irreversible | Purchase, payment initiation, transfer, privilege change, deletion, production deployment | Human approval, step-up authentication, replay protection, transaction limits, cancellation or recovery plan |
Risk-based approval is consistent with the OWASP AI Agent Security Cheat Sheet: the agent may propose an action, but a policy or execution component should independently validate scope, privilege, and approval state before execution. Reclassify an action upward when it affects more users, money, production systems, regulated data, or irreversible state.
Start with deny-by-default, least-privilege access
Give the agent only the tools needed for one task
Create a separate agent identity for each workflow. If the task is to reconcile invoices, provide invoice-read access and perhaps a staging workspace; do not attach a general administrator token, shell access, or a payment credential. Prefer a small allowlist of named tools over a generic “HTTP request” or “run command” capability.
Scope resources, operations, and time
A permission should answer four questions: which identity may perform which operation on which resource during which time window. Restrict an agent to specific repositories, folders, accounts, projects, vendors, or environments. Issue short-lived grants that expire when the task ends or is cancelled. Do not leave a broad token active for the next run.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Scan for outdated or missing drivers - takes under a minute3Repair Windows errors before they cause bigger problemsSeparate read and write credentials
Use different credentials and policy paths for reading and modifying data. A read-only token should not be convertible into a write token by changing a parameter. Keep high-impact credentials out of the orchestrator and provide them only to a narrowly controlled execution service after approval.
Make unknown operations fail closed
When a new tool, endpoint, parameter, or resource appears, reject it until a policy owner classifies and allows it. “Best effort” behavior is dangerous at an authorization boundary: an unavailable policy decision should deny the call, not fall back to the model’s judgment.

Enforce authorization outside the model
Prompts can describe safe behavior, but they cannot reliably enforce it. Put a policy decision point, API gateway, or execution proxy between the model and every consequential tool. The model submits a structured proposal; the enforcement layer authenticates the caller, validates the operation, checks the resource and parameters, verifies approval, and only then invokes the downstream service.
What the policy check should validate
- Agent and human identity, including the tenant or project boundary.
- Tool name and operation, not merely a free-form description.
- Target resource, environment, recipient, merchant, or account.
- Normalized parameters, including amount, currency, quantity, destination, and timing.
- Current grant, expiration time, rate and spending limits, and policy version.
- Approval state and the identity of the approver.
- Whether the request is a duplicate, replay, escalation attempt, or out-of-sequence step.
Log the proposed action and the final policy decision separately. This lets you distinguish a model that proposed an unsafe operation from a gateway that incorrectly permitted one.

Require approval for spending and irreversible actions
Purchases, payment initiation, transfers, privilege changes, deletion, production changes, and externally visible publication should be approval-gated. OpenAI’s Help Center states that purchases made with saved merchant cards require approval, and that approval may be given in advance only when it specifically covers the purchase. Treat that as a narrow authorization, not a blanket instruction to spend.
Bind approval to the exact action
An approval should identify the actor, tool, target, normalized parameters, timestamp, expiry, and limits. For a purchase, that means at least the merchant, item or service, quantity, total amount, currency, shipping or destination details, and the account charged. If any material field changes, require a new approval. Do not let an approval for a $50 order authorize a $500 order or a different recipient.
Use step-up authentication for critical operations
Require the approver to authenticate again for high-value or destructive actions. The approval interface should show the exact effect in plain language, not the model’s vague justification. Include a short expiry and prevent reuse after execution. For transfers and deletions, add a second-person approval where your risk model requires separation of duties.

Implement a safe approval flow
- The agent prepares a structured proposal without executing it.
- The policy service classifies the operation and checks limits, identity, scope, and current state.
- If approval is required, the service presents the normalized action and affected resources to an authorized human.
- The human approves or rejects through an authenticated channel. The approval token records the exact parameters and expiry.
- The execution proxy revalidates the token immediately before calling the tool. It rejects changed parameters, expired tokens, and duplicate transaction identifiers.
- The proxy records the result, including downstream confirmation or failure, and revokes the temporary grant.
For operations that cannot be undone, design compensation before enabling the action. A deletion may require a recoverable archive; a payment may require a documented refund path. If no safe recovery exists, use a stricter approval threshold and a smaller transaction limit.

Prompt injection occurs when untrusted content tries to make the agent ignore its task, reveal data, or invoke a tool. OpenAI notes that its protections reduce the risk of malicious instructions causing an unwanted action but do not eliminate that risk. Assume that any page, email, file, code comment, search result, or tool output can contain adversarial instructions.
Keep data and instructions separate
Label external content as untrusted data in the agent’s context. Parse structured fields instead of feeding whole pages into an instruction channel. Do not allow text returned by a tool to redefine the system’s policy, identity, or approval requirements.
Constrain what injected content can reach
- Run browsing, document parsing, and code execution in a sandbox with no default access to production credentials.
- Restrict network egress to the domains and methods required for the task.
- Strip or quarantine active content and suspicious instructions before a high-impact decision.
- Require an independent policy check even when the model claims that a page instructed it to act.
- Keep high-impact credentials in the execution proxy rather than in the browsing or orchestration context.
Test with adversarial pages that ask the agent to buy an item, upload secrets, change a recipient, or disable safeguards. A successful test should show the agent declining or producing a proposal that remains blocked by policy.
Use sandboxes and segmented environments
Contain risky work so a model error cannot cross directly into production. Give development agents disposable workspaces, synthetic data, and non-production endpoints. Separate networks and credentials for browsing, code execution, and business transactions. A sandbox is not a substitute for authorization: it limits the blast radius while the gateway still decides whether an operation is allowed.

For code-changing agents, begin with read-only repository access. Anthropic describes this pattern: the agent can analyze and review information in its initialized directory without asking for approval, but must ask before actions that modify code or systems. Apply the same split to configuration, infrastructure, customer records, and content publishing.
Monitor, interrupt, and recover
Keep an audit trail outside the agent
Record every proposal, policy decision, approval, tool call, response, credential scope, and cancellation in storage the agent cannot edit. Use tamper-evident controls and synchronized timestamps. Include a correlation ID that follows a proposal from the model through approval and downstream execution.
Alert on behavior that signals trouble
- Repeated denied calls or attempts to access a new resource.
- Rapid increases in call volume, spend, or privilege requests.
- Changes to recipients, destinations, or parameters after approval.
- Attempts to disable logging, bypass the proxy, or obtain credentials from tool output.
- Unexpected network destinations or activity outside the task’s time window.
Provide a real stop control
Implement an operator kill switch that revokes active grants, blocks new tool calls, and cancels queued work. It must operate outside the model and remain usable when the agent is looping. For long-running tasks, add checkpoints so an operator can pause before the next consequential step.
Make retries safe
Use idempotency keys or explicit duplicate confirmation for payments, messages, deployments, and other operations that may be retried after a timeout. Treat an unknown result as an unresolved transaction that needs reconciliation, not as permission to submit again. Store the downstream receipt or transaction ID in the audit record.
Recommended Free Tools
A practical implementation sequence
- Inventory: list every agent, tool, credential, resource, and operation it can reach.
- Classify: assign risk based on impact and reversibility; mark unknown operations as denied.
- Reduce: remove unused tools, replace administrator credentials, and create read-only identities.
- Define policy: express allowed tools, resources, parameters, time windows, rate limits, and spending limits in an enforcement service.
- Add the proxy: route every consequential call through a gateway that authenticates and validates the request.
- Design approvals: specify which actions need a human, what exact fields are displayed, expiry, step-up authentication, and separation of duties.
- Isolate: sandbox untrusted browsing and code, segment networks, and keep high-impact secrets out of the model context.
- Instrument: create tamper-evident logs, alerts, correlation IDs, and an operator kill switch.
- Test abuse cases: inject instructions through pages and documents, alter approved parameters, replay requests, expire grants, and simulate gateway failure.
- Roll out gradually: begin in a read-only or staging mode, review denied and approved calls, then enable narrowly bounded writes.
How to evaluate an agent-control implementation
When comparing platforms or internal designs, evaluate the enforcement details rather than the agent’s marketing description.
Best Value
| Evaluation area | Questions to ask |
|---|---|
| Permissions | Are defaults read-only? Can access be scoped per tool, resource, operation, task, and time window? |
| Authorization | Is policy enforced by a gateway or execution service outside the model? Does failure deny the call? |
| Approvals | Is approval bound to normalized parameters, target, actor, expiry, and a one-time transaction ID? |
| Spending controls | Are there per-transaction, per-task, and cumulative limits, with step-up authentication for critical actions? |
| Isolation | Are browsing, code execution, and transactions sandboxed and segmented? Is network egress restricted? |
| Identity | Are agent, user, approver, and execution identities distinct and auditable? |
| Observability | Are proposals, denials, approvals, calls, responses, and cancellations stored outside agent memory with tamper evidence? |
| Recovery | Can operators interrupt immediately? Are actions idempotent, reversible, or paired with a tested compensation process? |
Common failure modes and fixes
“The system prompt says never buy anything without asking.”
Why it fails: a prompt does not revoke a payment credential, and injected content or model error can still produce a tool call. Fix: deny the purchase at the gateway unless an exact, unexpired approval token is present.
“We gave the agent one broad service account for convenience.”
Why it fails: one compromised workflow can read or change everything that account can reach. Fix: create task-specific identities, separate read and write credentials, and scope resources and expiry.
“The human approved the task once, so all later actions are covered.”
Why it fails: the target, amount, recipient, or effect can change during execution. Fix: bind approval to normalized parameters and require reapproval for material changes.
“A timeout means the payment did not happen.”
Why it fails: the downstream service may have accepted the request before the timeout. Fix: reconcile by transaction ID and use idempotency keys before retrying.
“The browser is isolated, so prompt injection is harmless.”
Why it fails: injected text can still influence proposals, exfiltrate data available in the sandbox, or exploit an overly permissive egress rule. Fix: treat all external content as untrusted, restrict network access, and enforce policy on every consequential call.
What major guidance says
OpenAI’s Help Center documents approval requirements for purchases made with saved merchant cards and warns that protective measures reduce but do not eliminate malicious-instruction risk. Anthropic describes read-only permissions by default, with human approval required before actions that modify code or systems. OWASP’s AI Exchange emphasizes least model privilege, while its AI Agent Security Cheat Sheet places independent validation of scope, privilege, and approval state in a policy or execution component rather than relying on the model. These principles converge on the same design: the model proposes, an external control plane decides, and a constrained executor performs only the approved operation.
Quick Recap
Minimum launch checklist
- Every tool and operation is inventoried and risk-classified.
- Unknown tools, resources, and policy-service failures deny by default.
- Agents start with read-only, task-scoped, expiring permissions.
- All writes, spending, deletion, privilege changes, and external publication pass through an independent gateway.
- Approvals show and bind the exact target and normalized parameters.
- Critical actions use step-up authentication and replay protection.
- Untrusted content is isolated, labeled as data, and unable to grant permission.
- High-impact credentials are withheld from the model and browsing context.
- Logs are external, tamper-evident, and correlated across proposal, approval, and execution.
- Operators can revoke grants and stop queued or running work.
- Retries, unknown outcomes, and compensation paths are tested before production access.
