Do not start by installing the patch. Remove the Ivanti Connect Secure gateway from production, isolate connected identity and enterprise systems, preserve evidence and configuration, then factory-reset the hardware appliance or deploy a clean virtual build. Upgrade that clean instance to the currently supported release for its exact platform, rotate every credential and trust object that may have been exposed, run the latest internal and external Integrity Checker Tool (ICT), and restore service only with heightened monitoring.
A patch-only response is not sufficient when compromise is possible. Ivanti’s 2024 emergency guidance and its January 2025 notice about limited exploitation of CVE-2025-0282 both point to a rebuild-and-rotate process rather than assuming that a successful update proves the appliance is clean.

Why a zero-day requires more than a software update
A zero-day incident has two separate problems: closing the vulnerability and determining whether an attacker already used it. Installing a fixed image addresses only the first. An intruder may have created accounts, changed web resources, stolen configuration secrets, obtained certificates or keys, or moved through systems that trusted the VPN.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesFor that reason, treat an affected Connect Secure appliance as an incident-response case. The recovery sequence is:

CISA’s February 2024 supplemental direction required federal agencies to disconnect affected instances, isolate connected systems as far as practical, audit privileged access and complete rebuild and credential actions on its stated deadlines. Those controls are a useful baseline for any organization, even when the organization is not a federal agency.
Contain the gateway before touching its software
Remove production access
Use your change and incident procedures to take the VPN endpoint out of the access path. Disable or withdraw the external listener, block client and administrative traffic at upstream firewalls, and prevent the appliance from reaching internal resources except where a forensic or recovery task explicitly requires it. Do not rely on a graceful shutdown as your only control; network isolation prevents additional sessions and outbound activity while the incident team works.
Separate management and identity dependencies
Restrict connections to directory servers, RADIUS or other authentication services, certificate authorities, SIEM collectors, orchestration systems, backup stores and cloud identity services. Preserve the network rules and timestamps that show when each connection was blocked. If the appliance shared privileged credentials with another system, assume those credentials are exposed and begin a coordinated reset.
Start threat hunting immediately
Hunt on endpoints, servers, identity infrastructure and network devices that were reachable from the gateway or used it for authentication. Look for unusual administrative logins, newly created local or domain accounts, unexpected policy changes, web-resource modifications, outbound connections and authentication activity at unusual times. A clean result is not proof that the appliance was never accessed; it is one input to the rebuild decision.
Preserve evidence and export configuration safely
Capture evidence before reset
Follow your incident-response retention process for appliance logs, system images, firewall records, authentication logs, packet captures and relevant endpoint data. Record the appliance model, serial or virtual-machine identity, software release, time zone, clock status and the exact time production access was removed. Preserve chain-of-custody information if legal, regulatory or insurance requirements apply.
Export configuration as reference data
Export the Connect Secure configuration before a reset so that required settings can be reviewed and rebuilt. The export is not evidence that the appliance is trustworthy. Treat every object as untrusted input until an administrator validates it.

Inventory the export rather than importing it blindly. At minimum, identify:
- Authentication servers, realms, directory groups and authorization mappings.
- Local users, administrator roles and service accounts.
- API integrations, automation credentials and stored tokens.
- Server and client certificates, private keys and trust stores.
- Routes, VPN policies, split-tunnel rules and access-control objects.
- Logging destinations, alert integrations and time sources.
- Web resources, scripts, custom pages and administrative objects.
Keep an untouched copy for investigators and create a separate working copy for review. Hash or otherwise identify both copies so that later changes are detectable.
Choose the correct rebuild path
The safe choice depends on whether you use a hardware appliance or a virtual appliance and on the evidence available. The following matrix avoids treating every deployment as identical.
| Situation | Preferred action | Reason | What must happen next |
|---|---|---|---|
| Hardware appliance with possible compromise | Vendor-directed factory reset | Removes the appliance state before reinstallation | Install the supported release, rotate trust material, reimport reviewed settings and run current ICT |
| Virtual appliance with possible compromise | Deploy a new clean virtual build | A new instance provides a known-good software and storage baseline | Use approved media, patch before exposure, rotate credentials and selectively restore configuration |
| No evidence of compromise, but the appliance was exposed to the zero-day | Use the vendor’s current incident guidance; do not assume patch-only is adequate | Absence of findings does not establish absence of persistence | Complete current ICT and threat hunting, and rebuild if compromise cannot be ruled out |
| Unsupported model or unclear upgrade path | Pause restoration and obtain the supported path from the authenticated vendor portal | A historical build number may no longer be secure or supported | Document the target release and compatibility before deployment |
Hardware factory reset
Use the factory-reset procedure supplied for the exact hardware model. Confirm that you have console or out-of-band access, licensed installation media, a documented management address and a tested way to reach the appliance without reconnecting it to production. A reset destroys local state, so preserve the export and evidence first.
Clean virtual redeployment
For a virtual appliance, create a new instance from vendor-approved media rather than cloning the potentially affected disk. Keep the new virtual machine on an isolated network while you apply the supported release and harden management access. Do not attach old virtual disks or snapshots unless investigators have approved them for analysis.
Obtain and install the currently supported release
Do not copy a version number from an older runbook into a current recovery. Sign in to Ivanti’s authenticated download portal and read the release notes for the exact Connect Secure model, hardware or virtual form, license and supported upgrade path. Confirm the image checksum or other vendor integrity information before installation.

Ivanti’s later security documentation records fixes for CVE-2025-0282 and CVE-2025-0283 in the 22.7R2 stream. Its January 2025 advisory said CVE-2025-0282 had been exploited on a limited number of Connect Secure appliances at disclosure and that a fix was available. Those facts do not make 22.7R2 a universal target today; the supported target must come from the current portal and platform-specific release notes.
Installation controls
- Verify that the image is intended for your hardware or virtual form factor.
- Review prerequisites, intermediate releases and rollback limits before starting.
- Install while the appliance remains isolated from production users and identity systems.
- Record the image name, release, checksum, installation time and operator.
- Apply all required post-install updates before allowing any external or internal client traffic.
Handle temporary mitigations correctly
Some emergency responses used an XML mitigation or another temporary control. Do not remove such a file merely because the appliance has been reset or appears to be patched. Review the current Ivanti knowledge-base instructions and remove the mitigation only when those instructions say it is appropriate for the installed release. Record who removed it, when and from which build.
Rotate every credential and trust relationship
Credential rotation is a separate workstream from software installation. Complete it even when scans show no obvious persistence.

- Reset the administrator enable password and every local user password.
- Replace stored API keys, automation tokens and secrets used by monitoring or orchestration.
- Change service-account passwords used for authentication-server configurations.
- Revoke and reissue certificates and private keys that were stored on, generated by or trusted by the affected appliance.
- Review certificate-authority trust stores and remove unapproved entries.
Directory and cloud identities
When the appliance was affected, treat associated domain accounts as potentially compromised. Coordinate resets with the identity team, revoke active Kerberos tickets, invalidate cloud access and refresh tokens, and disable cloud-joined or registered devices when your incident policy requires it. Review privileged groups and recent changes rather than resetting only the account used by the VPN administrator.
Sequence resets so that the rebuilt appliance receives new secrets only after the old credentials have been revoked. Otherwise, an attacker who obtained an old password or token may retain access during the transition.
Reimport configuration selectively
After the clean build is patched, reimport only the configuration that has passed review. CISA’s recovery sequence places reimport after factory reset or clean deployment and patching; that order prevents an old export from becoming the first state on an unpatched appliance.

Settings to revalidate manually
- Authentication-server addresses, certificates and group-to-role mappings.
- Administrative roles and local accounts.
- VPN policies, routes, split-tunnel behavior and access restrictions.
- Logging destinations, time synchronization and alerting.
- API integrations and least-privilege permissions.
- Certificate names, expiration dates, key usage and trust chains.
Do not restore unknown scripts, web resources or administrative objects just because they existed in the old export. Compare each item with a known change record or an approved baseline. If a setting cannot be explained, leave it out until an investigator clears it.
Run current integrity checks, not just an old scan
Run the latest available Ivanti internal and external ICT after the rebuild and patch. Use the versions and procedures currently published by the vendor, not a copy retained from the original incident.
CISA and allied agencies warned that threat actors could deceive earlier ICT results. Therefore, a successful scan performed before the rebuild, or with an older checker, is not conclusive evidence of a clean appliance. Keep the output, version, timestamp, appliance identifier and operator in the incident record.
What a clean result can and cannot prove
- It can show: the tested indicators were not detected by that tool against that instance at that time.
- It cannot show: that no credentials were stolen, no connected host was accessed, or no attacker activity occurred outside the tool’s coverage.
- It should trigger: continued network, endpoint, identity and gateway monitoring, plus documented threat hunting.
Ivanti advises customers who already completed a successful factory reset or clean virtual build and patched to continue updated ICT and continuous monitoring. Apply that same discipline after a new rebuild.
Restore service in controlled stages
- Validate the isolated appliance. Confirm the release, hostname, management controls, time source, certificates, routes, logging and authentication behavior without exposing it to general users.
- Test with restricted identities. Use dedicated test accounts and a limited policy set. Verify authorization boundaries, session termination, multifactor behavior and log delivery.
- Reconnect monitoring first. Ensure SIEM, network sensors, endpoint telemetry and identity alerts are receiving events before users return.
- Open access gradually. Start with a small, approved user group or a narrow policy scope, then expand after reviewing logs.
- Keep the incident watch active. Do not close enhanced monitoring when the first users connect; retain it for the period defined by your response team and risk assessment.
Watch for these post-restore signals
- Authentication failures or successes that do not match user schedules or locations.
- Unexpected administrative changes, new local accounts or role assignments.
- Unplanned certificate issuance, replacement or trust-store changes.
- Outbound connections from the gateway to unfamiliar destinations.
- Changes to VPN routes, policies, web resources or API integrations.
- Suspicious activity in directory, Kerberos, cloud identity and endpoint systems connected to the VPN.
Document the recovery so it can be audited
Record the containment time, evidence locations, configuration-export hashes, reset method, clean-build source, image checksum, installed release, mitigation removal, credential-rotation scope, ICT versions and results, configuration items reimported, test accounts used, restore time and approving personnel. Keep a list of every system hunted and the time range examined.

Common mistakes and how to avoid them
“The patch installed, so we are done.”
A successful update does not remove persistence or invalidate stolen credentials. Complete the rebuild assessment, rotation and current ICT steps.
“The configuration export is trusted because it came from our appliance.”
An export can contain malicious or unauthorized objects. Review it as untrusted data and reimport only approved settings.
“The old ICT report was clean.”
Earlier checks may have been deceived or may not cover later attacker actions. Run the current internal and external tools after rebuilding.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
“We can reuse the same certificates and service passwords.”
Assume secrets stored on an affected appliance were exposed. Revoke and reissue certificates and keys, and reset local, administrator and service-account credentials.
“A virtual snapshot is a clean rebuild.”
A snapshot may preserve the compromised state. Deploy a new virtual instance from approved media unless incident responders specifically clear the snapshot.
“We can remove the emergency mitigation immediately.”
Follow the current Ivanti instructions for the installed release. Removing a mitigation at the wrong point can reopen exposure or leave the appliance in an unsupported state.
When is patch-only remediation defensible?
Patch-only treatment is the exception, not the default, when a zero-day affected an internet-facing VPN. It may be considered only when incident responders can establish that the appliance was not exposed to the vulnerable condition, compromise is credibly ruled out, the vendor explicitly supports that path and current ICT and threat hunting are complete. If any of those conditions is uncertain, use the factory-reset or clean-deployment sequence.

Quick Recap
Decision checklist
- Has the gateway been removed from production and isolated from connected systems?
- Are logs, forensic evidence and a configuration export preserved?
- Has threat hunting begun on identity, endpoint, network and gateway-adjacent systems?
- Was hardware factory reset or a new virtual build selected according to the evidence?
- Was the current supported release confirmed for the exact platform?
- Were temporary mitigations handled according to current vendor instructions?
- Were administrator, local, API, service-account and directory credentials rotated?
- Were exposed certificates, keys, Kerberos tickets and cloud tokens revoked or reissued?
- Was only reviewed configuration reimported?
- Were the latest internal and external ICT checks run after patching and rebuilding?
- Are monitoring, logging and post-restore threat hunting active?
- Is the complete recovery timeline documented and approved?
