Build the workflow as a sequence: accept a validated PDF on a dedicated Express upload route, rotate pages when their orientation is known, run OCR when scanned pages need searchable text, then inspect the generated file before returning it. Multer handles the upload; pdf-lib can apply explicit page rotations; OCRmyPDF can add an OCR layer and optionally detect cardinal orientation. These components solve different problems, so the right order depends on whether the document has existing text, which pages need correction, and whether the output must remain searchable.
How the PDF rotation and OCR pipeline fits together
Treat the endpoint as an orchestration layer rather than a single PDF operation. Express receives the request, upload middleware manages the incoming file, PDF tools modify or process it, and the application verifies and delivers the result. Multer is designed for multipart/form-data uploads in Express; its official documentation describes route-scoped middleware, storage options, and upload limits.

- Receive and validate: accept the file only on the route that processes PDFs, enforce limits, and check that its contents are a valid PDF rather than trusting the filename or client-supplied MIME type.
- Choose a rotation strategy: apply explicit rotations when the affected pages are known, or use automatic orientation detection as part of OCR processing when they are not.
- Run OCR when needed: process scanned or image-only pages to add searchable text, with the appropriate OCR language configured.
- Verify and deliver: confirm the result opens, has the expected page count and orientation, and retains the text and visual quality the application requires.
The cited tools provide the underlying operations, but they do not define one universally correct order for every PDF. In particular, decide whether to preserve an existing text layer and whether orientation must be corrected before OCR.
Accept uploads safely with route-specific Multer middleware
Apply Multer only to the endpoint that accepts the document. The Express Multer documentation warns, “Never add multer as a global middleware since a malicious user could upload files to a route that you didn’t anticipate.” See the official Multer documentation for its .single() route pattern and configurable storage and limits.

- Set a deliberate maximum file size and upload count. Choose limits appropriate to the service rather than leaving upload volume unrestricted.
- Choose memory or disk storage based on expected workload. Memory storage holds the complete upload in a Buffer; temporary disk storage can avoid keeping the entire input in process memory. Actual memory use and throughput depend on the implementation and workload.
- Validate file contents as a PDF. A
.pdfsuffix or client-provided MIME type is not proof of file format. - Clean up temporary files or buffers on success, failure, timeout, and cancellation paths.
For larger or CPU-intensive OCR jobs, consider moving processing to a worker or queue instead of tying up a latency-sensitive HTTP request. Bound concurrency and execution time, and treat isolation and cleanup as application responsibilities; they are not automatic Express or OCRmyPDF features.
Choose explicit rotation or automatic orientation detection
Use explicit rotation when you know which pages need it
pdf-lib supports changing a page’s rotation through PDFPage.setRotation(), with angles in 90-degree increments such as 90, 180, and 270 degrees. The library runs in Node.js and other JavaScript environments. Consult the PDFPage API and project overview.

This approach gives the application control over specific pages, but the cited API documentation establishes rotation capability—not OCR or automatic orientation detection. Check how rotation interacts with existing page rotation and page boxes in the PDFs your service accepts, and validate the resulting output.
Use OCRmyPDF rotation when orientation needs detection
OCRmyPDF can add an OCR text layer and offers --rotate-pages to correct pages whose cardinal orientation is wrong, such as a page turned sideways or upside down. It is a command-line tool, so an Express application must manage process invocation, isolation, timeouts, concurrency, temporary files, and cleanup. OCRmyPDF’s rotation threshold is conservative by default. Lowering it can cause more pages to be rotated, but also raises the risk of false positives. Test the setting against representative documents before applying it broadly.
Rotation is not deskewing
Orientation correction and deskew address different defects. --rotate-pages corrects a page that is oriented incorrectly in cardinal directions; --deskew corrects a scan that is slightly crooked. A sideways page needs orientation correction, while text lines tilted by a few degrees need deskewing. A document may need either operation or both.

Neither operation guarantees a better result on every input. OCRmyPDF cautions that image processing can rasterize pages or introduce artifacts, so inspect processed pages rather than assuming the output preserves every detail.

OCR is relevant when a page contains an image of text without a usable text layer and the output needs searchable or selectable text. OCRmyPDF adds an OCR layer to PDFs. Configure the language data to match the document: OCRmyPDF assumes English unless told otherwise, and using the wrong language can harm OCR quality. Its Cookbook documents OCR, language selection, rotation, and deskew options.

For documents with existing text, decide whether processing should preserve that layer and how the output should behave. The available documentation describes the operations, but does not establish one processing sequence that is correct for all mixtures of scanned pages, text pages, and existing page rotations.

For any option, assess page fidelity and searchability, support for image-only scans, language configuration, false-positive rotation risk, request latency and workload, runtime dependencies, file cleanup, and the degree of control needed over individual pages.
Validate the output before returning it
Successful processing does not by itself show that the document is usable. Check the generated PDF against the requirements of the endpoint:

- Image-only pages have the expected searchable text when OCR was required.
- Processed scans remain visually acceptable and do not show unwanted artifacts.
- Temporary inputs and outputs are removed after both successful and failed jobs.
Include mixed-orientation PDFs and image-only scans in validation. Review pages after OCRmyPDF image processing because its documentation warns that processing can change page representation or introduce artifacts.
