How to Review and Test AI-Generated Code for Security Vulnerabilities

Review AI-generated code as an untrusted contribution from an unknown developer. Before merge or deployment, establish the intended security behavior, inspect every changed line, audit dependencies and generated configuration, run independent security scanners, add human-authored adversarial tests, and obtain qualified human approval. A passing test suite—especially one written by the same agent that produced the code—is not proof that the implementation is secure.

Why AI-generated code needs a security review

AI coding tools optimize for plausible, working output. They can produce code that compiles, passes ordinary tests and resembles familiar patterns while silently omitting an authorization check, accepting unsafe input, using obsolete cryptography or exposing sensitive data in logs. The missing control may only appear on an error path, a second tenant, a concurrent request or an unusual serialization payload.

Do not treat the model, prompt or generated test report as the owner of the security decision. Microsoft’s security guidance puts the principle plainly: “Test AI-generated code at least as thoroughly as hand-written code — models can generate plausible-looking code that is subtly wrong.” The same guidance says that AI tools change what you review, not whether you review it.

OWASP’s Secure Coding with AI Cheat Sheet makes the independence problem explicit: “A passing test suite generated by the same agent that produced the code provides no independent assurance.” Security confidence should come from adversarial testing and independent analysis, not from a green status badge alone.

1. Establish scope, ownership and sensitive-data boundaries

Start the pull request with a short record of what the agent actually touched. This makes the security review auditable and prevents generated files from hiding among routine changes.

  • List every generated or modified file, including lockfiles, CI workflows, infrastructure-as-code, database migrations and configuration templates.
  • Record the model or coding tool, its version when available, the prompt or task description, and the human owner responsible for the change.
  • Describe the security impact: new endpoints, permission changes, data flows, credentials, network calls, parsers, file operations or deployment settings.
  • Name the reviewer with authority to approve the risk. Security-critical changes should use separation of duties rather than self-approval.
  • Keep passwords, API keys, private customer records, production exports and other sensitive context out of prompts. Microsoft’s guidance specifically calls for careful handling of sensitive information during AI-assisted development.

If an agent used tools, retain an action log showing commands, files read or changed, network access and approvals. This record is useful when an apparently harmless generated change later turns out to have introduced a vulnerable dependency or a data disclosure.

2. Define the secure behavior before reading the implementation

The implementation must be judged against a security requirement, not the other way around. Write down the expected behavior using the application’s threat model, security requirements and controls derived from the OWASP Application Security Verification Standard (ASVS).

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Specify trust boundaries and assets

  • Identify unauthenticated users, ordinary users, administrators, service accounts and background jobs.
  • Mark tenant, account, object and geographic boundaries. State which principal may read, create, update or delete each resource.
  • List secrets, personal data, payment information, signing keys and operational systems that require protection.
  • Define what an attacker can submit, replay, delay, tamper with or cause the system to fetch.

Turn requirements into review oracles

For each endpoint or function, document the required authentication state, authorization rule, accepted input shape, output exposure, rate limit, audit event and failure behavior. For example, “a user may download only files belonging to the current tenant” is testable; “the download endpoint is secure” is not. Include negative requirements such as “a token for tenant A must never retrieve tenant B’s object.”

3. Read the diff manually and trace data to security-sensitive sinks

Automated tools find patterns; a human must understand the business logic. Read the complete diff, then follow untrusted values from each entry point to every operation that can interpret, store, transmit or execute them.

Authentication and session handling

  • Check that every protected route invokes the intended authentication middleware and that middleware ordering cannot bypass it.
  • Verify signature and algorithm validation for tokens. Reject expired, not-yet-valid, malformed and incorrectly scoped tokens.
  • Inspect session creation, rotation, revocation, cookie flags, CSRF defenses and logout behavior.
  • Look for fallback identities, default accounts, development bypasses and debug headers left in production paths.

Authorization and tenant isolation

  • Confirm authorization is enforced on the server for every object and action, not merely hidden in the user interface.
  • Check ownership or tenant predicates in reads, updates, deletes, exports and background jobs. An ID taken from a URL must not be trusted as proof of access.
  • Review bulk endpoints, search filters, GraphQL resolvers and alternate representations because they often bypass checks present in the main route.
  • Test privilege boundaries with a lower-privileged account and with an account from another tenant.

Input validation and output encoding

Validation should be allowlist-based where practical, applied at the trust boundary and appropriate to the data type. Check length, range, character set, structure and canonicalization. Reject ambiguous encodings and duplicate parameters when different layers could interpret them differently. Output must be encoded for its destination—HTML, JavaScript, SQL, a shell, a URL, a log or a template language. Escaping for one context does not make data safe in another.

Injection and dangerous interpreters

  • For SQL, verify parameterized queries or safe query builders; inspect dynamically assembled identifiers, sort clauses and migration scripts.
  • For operating-system commands, prefer direct library calls. If a command is unavoidable, use fixed argument arrays and strict validation rather than string concatenation.
  • For templates, ensure untrusted values cannot select templates, invoke helpers or execute expressions.
  • For LDAP, XPath, NoSQL, regular expressions and expression languages, use their parameterization or constrained APIs and test parser edge cases.

Deserialization, file and network access

Search for deserializers that instantiate arbitrary classes, evaluate expressions or accept untrusted type metadata. Require an explicit schema and a restricted type set. For uploads and archive extraction, check size limits, content validation, filename normalization, path traversal defenses, decompression limits and storage permissions.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Any feature that fetches a URL needs an SSRF review: restrict schemes, resolve and validate the destination, block private and link-local ranges, re-check after redirects and set connection, response-size and time limits. For file access, canonicalize paths and enforce that the result remains inside the intended directory.

Error handling, logging and availability

  • Errors returned to clients should not reveal stack traces, SQL text, secrets, internal hostnames or authorization details.
  • Logs should support investigation without recording passwords, tokens, full payment data or unnecessary personal information. Check that log fields cannot inject fake entries or control characters.
  • Rate limits and quotas must cover expensive parsing, password attempts, token issuance, exports and fan-out operations.
  • Look for unbounded loops, uncontrolled concurrency, large allocations, recursive parsing and retry storms that can create denial-of-service conditions.

Cryptography and security configuration

Prefer well-maintained platform libraries and established protocols. Verify key generation, storage, rotation, random-number sources, nonce or initialization-vector handling, certificate validation and algorithm selection. Reject hard-coded keys, predictable randomness, custom encryption formats and silent fallback to insecure modes.

Inspect generated configuration as carefully as code: debug flags, permissive CORS, wildcard origins, disabled certificate checks, public storage, overly broad IAM roles, open network listeners, default passwords and secrets committed to repositories are all security changes.

4. Audit dependencies and generated configuration

Models may suggest package versions that are stale, incompatible with your supported runtime or already affected by a published vulnerability. Pin versions where your build policy requires it, review lockfile changes and verify transitive dependencies.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
Check Example command or data source Review purpose
JavaScript dependency audit npm audit Find known vulnerabilities in direct and transitive packages.
Python dependency audit pip-audit Match installed Python packages to published advisories.
Go dependency audit govulncheck Identify vulnerable Go packages and reachable symbols.
Rust dependency audit cargo-audit Check Cargo dependencies against the RustSec advisory data.
Advisory cross-check NVD, OSV and the GitHub Advisory Database Confirm severity, affected versions, fixes and exploitability context.

Do not dismiss a finding because the model selected the package or because the vulnerable function is not obvious. Confirm whether the vulnerable code is reachable, upgrade or replace the component, and document a compensating control when an upgrade is impossible. Re-run the audit when the model, prompt or dependency set changes.

5. Run independent automated security gates

Every pull request containing AI-generated code should pass the same build and test gates as hand-written code, with security checks added where the architecture supports them. Run these checks independently of the generating agent and review their configuration so that the new code cannot disable or narrow the scan.

Gate What it examines Typical evidence for approval
Compilation and ordinary tests Build correctness and documented functional behavior Reproducible build and test results from a clean environment.
SAST Source patterns such as injection, unsafe APIs and missing controls Results triaged by a human, with critical findings fixed or formally accepted.
SCA Direct and transitive dependency vulnerabilities Lockfile, audit output and documented treatment of advisories.
Secret scanning Keys, tokens, passwords and high-entropy credentials Clean scan plus verification that exposed credentials were revoked.
Infrastructure-as-code scanning Cloud, container, Kubernetes and CI configuration Findings addressed before deployment, including network and identity permissions.
DAST Running application behavior over HTTP or another exposed interface Authenticated and unauthenticated scans against a representative environment.
IAST Runtime data flow while tests exercise the application Findings correlated with the executed code paths and tests.

Automation is a gate, not a verdict. Static analysis can miss a broken business rule; dynamic scanning can miss an unexercised permission path; secret scanners can produce false positives; and a clean report says nothing about a requirement that was never encoded. OWASP’s DevSecOps secure-code-review guidance treats automation as support for, not a replacement for, human review.

6. Add tests the generating agent did not author

Create a separate test set from the threat model. Keep authorship and review independent from the agent that wrote the implementation, and inspect the test diff for deleted cases, weakened assertions, broad mocks or a switch from a real dependency to a harmless fake.

Adversarial input cases

  • Empty, oversized, truncated, duplicated, incorrectly typed and boundary-value fields.
  • Unicode confusables, mixed encodings, null bytes, delimiter characters and canonicalization variants.
  • Malformed JSON, XML, multipart data, compressed archives and serialization type metadata.
  • SQL, command, template, expression, path and URL payloads that verify rejection or safe handling rather than merely causing an error.

Authorization-negative cases

  • Unauthenticated access to every protected route.
  • A normal user attempting administrator actions.
  • A user reading, editing, deleting or exporting another user’s object.
  • A valid identity from tenant A presenting an object identifier from tenant B.
  • Bulk, batch, alternate-format and background-job paths that might skip the primary authorization check.

Token, session and replay cases

  • Expired, not-yet-valid, malformed, incorrectly signed and wrong-audience tokens.
  • Replayed one-time links, refresh tokens and password-reset tokens.
  • Sessions after logout, password change, role change and account disablement.
  • Concurrent requests that attempt to use the same nonce, token or authorization decision.

Availability and race cases

Exercise simultaneous updates, duplicate submissions, lock contention, retries, timeouts and partial failures. Verify that rate limits, idempotency controls and transaction boundaries hold under concurrency. Add differential fuzzing or property-based tests for security-critical parsers and policy functions: the same invariant should hold across many generated inputs, not only the examples in a hand-written fixture.

Test the failure path

Assert the response, side effects and logs when validation fails, authorization is denied, a dependency times out or storage is unavailable. A secure failure should not commit partial state, leak secrets, bypass auditing or turn a recoverable error into an unlimited retry loop.

7. Secure the AI development loop against indirect prompt injection

Anything an agent can read may influence its next action. Treat issue descriptions, pull-request comments, README files, generated documentation, dependency metadata, tool output and repository fixtures as untrusted content. An instruction embedded in one of those sources can attempt to make the agent reveal secrets, weaken a test, disable a scanner or modify an unrelated file.

  • Run agents in a sandbox with least-privilege filesystem and network access.
  • Keep command execution, network access, package installation and write operations behind explicit approvals.
  • Use scoped, short-lived credentials; never expose production keys to a coding session.
  • Allowlist tools and MCP servers, pin their versions and review changes before enabling them.
  • Separate planning, implementation and approval identities so one compromised context cannot approve its own changes.
  • Log prompts, tool calls, command output, approvals and resulting file changes for later investigation.

Do not allow an agent to edit the security pipeline that evaluates its own output without a separate review. A generated change that removes a test, lowers a severity threshold or excludes a directory is itself a security-sensitive change.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

8. Apply merge and release gates

Require a qualified human to approve the implementation, test evidence and scanner results before merge. For security-critical changes, use a second reviewer or a security owner under a separation-of-duties policy. The approval record should link the code revision, reviewer, model and tool version, dependency audit, security tests, exceptions and deployment decision.

Define blocking thresholds in advance. OWASP AISVS Appendix C gives CVSS 9.0 or higher as an example threshold for critical findings that must block progress. Your policy may be stricter, but an exception should always name the finding, affected asset, compensating control, owner and expiration date. “The model probably meant well” is not a compensating control.

Release gates should also confirm that production configuration matches the reviewed configuration, migrations preserve tenant isolation, observability is enabled and rollback is available. If a scanner cannot inspect a component, record the blind spot and use a targeted manual or dynamic test instead of treating the missing result as a pass.

9. Re-test after deployment and when the code changes

Security review ends only when the feature is retired. Monitor new dependency advisories, scanner findings, authentication failures, authorization denials, unusual data access, rate-limit events and error spikes. Investigate behavior that differs from the test environment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Repeat targeted security tests whenever the model, prompt, generated files, dependency set, build configuration or deployment environment changes. A regenerated function can alter validation or authorization even when its public interface stays the same. Keep the original threat model and approval trail so a later reviewer can identify what changed and why.

A practical pull-request checklist

  • Scope lists all generated files, model and tool versions, security impact and human owner.
  • No secrets or sensitive customer data were placed in prompts or committed files.
  • Expected security behavior and trust boundaries are documented before implementation review.
  • Every changed entry point was traced to database, command, template, file, network, log and serialization sinks.
  • Authentication, authorization, tenant isolation, validation, encoding, cryptography, rate limits and error handling were reviewed.
  • Dependency versions and generated configuration were audited; advisories were checked against current data.
  • Compilation, ordinary tests, SAST, SCA, secret scanning and infrastructure-as-code scanning ran independently.
  • DAST or IAST was used where the architecture supports it.
  • Human-authored negative, malformed-input, replay, concurrency, SSRF, path-traversal and deserialization tests passed.
  • Test changes were inspected for deleted cases, weakened assertions and unrealistic mocks.
  • Agent permissions, tool allowlists, credentials and action logs were reviewed for prompt-injection exposure.
  • Qualified human approval, exception records and deployment or rollback evidence are attached to the change.

How to judge the strength of a review approach

When comparing review processes, score them on the dimensions that determine whether a plausible-looking implementation receives real scrutiny:

Dimension Stronger approach Weak signal
Independence Human-authored tests and a reviewer who did not generate the code. The same agent writes code, tests and the approval explanation.
Coverage Ordinary tests plus SAST, SCA, secret, infrastructure-as-code and applicable DAST or IAST checks. Only compilation or a unit-test count is reported.
Business-logic depth Explicit review of authorization, tenant isolation, workflows and failure states. Reliance on pattern matching without a threat model.
Adversarial strength Negative cases, fuzzing, replay, race, SSRF, traversal and deserialization probes. Only happy-path examples supplied by the generating prompt.
Governance Qualified approval, separation of duties, documented exceptions and an audit trail. Unattributed “looks good” approval or an automatic merge.
Freshness Current dependency and advisory data rerun after changes. Trust in package versions suggested by the model.

The result you want is not proof that an AI system is trustworthy. It is evidence that this particular change satisfies its security requirements under independent scrutiny, with a named human accepting responsibility for the decision.
