AI extraction should be treated as a draft record, not self-authenticating underwriting data. A defensible review keeps the original loan document immutable, records where every extracted value came from, tests the value against deterministic and cross-document rules, and sends high-impact or uncertain fields to a trained reviewer. When a material value changes, preserve both versions, record the evidence and reason, and re-run every downstream calculation, automated-underwriting input, disclosure, and delivery check affected by the correction.
This workflow is designed for mortgage and other loan files in which an OCR or document-AI system turns PDFs or images into structured data. It covers source control, triage, field-by-field verification, correction history, quality-control requirements, standards alignment, and the compliance consequences of an extraction error.

Why an AI extraction result is only a draft
Document AI can read text, tables, checkboxes, and handwriting, but a plausible string is not proof that the value is correct. An extraction can be wrong because a decimal shifted, a minus sign disappeared, two table columns were swapped, or a value was assigned to the wrong borrower. A high confidence score can still describe the wrong page, the wrong table row, or the wrong person.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsIn mortgage quality control, the practical standard is verification against the source and supporting evidence. Fannie Mae’s prefunding guidance requires procedures that identify analysis or calculation errors, inaccurate data, and inadequate documentation before closing or acquisition. Its post-closing guidance extends the review to application accuracy and completeness, underwriting support, third-party data-analysis output, automated-underwriting data, closing-document consistency, and prompt correction when errors are found.

Reverification has a specific purpose: confirming the authenticity of the loan documentation and the accuracy and integrity of information relied on in underwriting. A discrepancy must be reconciled; depending on its significance, the file may need re-underwriting rather than a silent edit.
1. Lock the source and capture provenance
Start by making the source document immutable. Keep the original PDF or image in its controlled repository and perform review on a derivative or a structured record. A reviewer must always be able to open the exact page and image region that produced a field.
Record provenance for every extraction run
- Loan or file identifier and document identifier.
- Document type, page number, and image or region coordinates when available.
- Extraction timestamp in UTC.
- OCR or model name and exact version.
- Preprocessing applied, such as deskewing, rotation, contrast changes, cropping, or table detection.
- Field-level confidence, exception flags, and any parser warnings.
- Transformation steps between the extracted text and the value stored in the loan-origination system.
NIST’s Generative AI Profile calls for documenting data origin and content lineage and evaluating output against known ground truth. In practice, lineage means a reviewer can move from a stored value to the extraction event, then to the exact source page, without relying on a mutable screenshot or an undocumented manual edit.

Do not review fields in page order. Route work according to the potential effect on eligibility, terms, disclosures, and delivery. Combine two signals: business impact and uncertainty. A high-impact field deserves verification even when the model is confident; a low-impact field with poor image quality can be queued for later or sampled, depending on policy.
High-impact fields to review first
- Identity and attribution: names, Social Security numbers or other identifiers, borrower-to-document assignment, and consistency across the file.
- Income and employment: employer, employment dates, pay frequency, base and variable income, calculation inputs, and verification identifiers.
- Assets, liabilities, and credit: account ownership, balances, debts, payment amounts, credit-history data, and funds needed to close or required reserves.
- Property, valuation, and occupancy: address, legal description, appraisal or property data, value, occupancy, insurance, and eligibility attributes.
- Transaction and closing terms: sales price, loan amount, interest rate, fees, credits, settlement figures, note, security instrument, and final disclosures.
- Conditions and decision outputs: approval conditions, automated-underwriting entries, red-flag messages, and third-party analysis results.
Uncertainty signals that trigger a human queue
- Low confidence, an exception flag, or an unreadable region.
- Conflicting values on different pages or in different documents.
- Unusual formats, handwritten amendments, stamps over text, or a cropped table.
- A value that violates a permitted format or a deterministic rule.
- A value that is plausible in isolation but belongs to another borrower, property, loan, or transaction.
3. Compare each value with the exact source evidence
Present the extracted value beside the source page and region. The reviewer should be able to highlight the characters that support the value, not merely view a full document and rely on memory. Never accept a number because it looks reasonable.

Common OCR and layout failures
- Decimal shifts, such as reading 10.50 as 1050 or 1,250 as 125.
- Comma and period confusion in amounts and dates.
- Lost negative signs that turn a liability or adjustment into a positive figure.
- Confusion between zero and the letter O, or one and the letter I.
- Truncated names, addresses, account numbers, or employer names.
- Table-column drift, where a value is assigned to the adjacent borrower, period, or line item.
- Dates read in the wrong order or with a transposed month and day.
- Checkboxes, strike-throughs, stamps, and handwritten changes interpreted as ordinary text.
When the source is ambiguous, do not resolve it by intuition. Obtain an independent document or verification report, escalate to the responsible reviewer, or mark the field as unresolved under the organization’s exception policy.
4. Run deterministic and cross-field checks
After visual comparison, test whether the value behaves correctly in the file. Deterministic rules catch errors that a human may miss when reviewing one field at a time.

- Recalculate income totals, qualifying income components, liabilities, cash to close, reserves, fees, credits, and settlement figures.
- Check required-field presence, permitted formats, sign conventions, decimal precision, and valid date ranges.
- Verify date order, including employment periods, document dates, closing dates, and expiration dates.
- Confirm that ratios and other derived values use the corrected source inputs rather than stale cached values.
Identity and attribution checks
- Match names and identifiers across the application, income evidence, asset statements, credit data, disclosures, note, and security instrument.
- Confirm that each account, debt, income source, and document is assigned to the correct borrower or co-borrower.
- Check property address, legal description, and transaction identifiers across appraisal, title, insurance, and closing records.
Independent evidence checks
Use the evidence appropriate to the field: verification reports, tax or employment documents, credit data, appraisal or property data, and closing documents. Fannie Mae guidance requires lenders to investigate inconsistent or contradictory information even when a validation service was used. A validation result does not remove the obligation to reconcile a conflict.
5. Review the loan file in risk order
The following sequence keeps the highest-consequence decisions ahead of routine metadata.
- Identity and borrower assignment. Confirm names, identifiers, and document ownership before using any income, asset, or liability value.
- Income and employment. Verify employer, dates, pay frequency, base and variable components, calculation methodology, and verification identifiers. Reperform totals from the source evidence.
- Assets, liabilities, and credit. Confirm account ownership, balances, debts, payment amounts, credit-history data, funds to close, and reserves. Check that liabilities were not duplicated or omitted.
- Property, valuation, and occupancy. Match address and legal description, then verify value, occupancy, insurance, and eligibility attributes against the applicable property records and appraisal.
- Transaction and closing terms. Compare sales price, loan amount, rate, fees, credits, settlement figures, note, security instrument, and final disclosures. Look for a value that changed in one document but not another.
- Conditions and decision outputs. Review approval conditions, automated-underwriting entries, red-flag messages, and third-party analysis results. Confirm that each output used the current, verified inputs.
6. Correct values without destroying the original record
Do not overwrite an extraction in place. Create a correction event that preserves the original value and explains the decision. The correction should be understandable to a later quality-control reviewer who has not seen the original discussion.

Minimum correction-record fields
| Field | What to retain |
|---|---|
| File identity | Loan or file ID and document ID |
| Location | Page number and evidence region or source reference |
| Data element | Field name and the business object it belongs to |
| Before and after | Original extracted value and corrected value |
| Evidence | Source document, independent verification, or reconciliation record supporting the change |
| Reviewer | Reviewer ID and role |
| Timing | Date and time in UTC |
| Reason | Reason code and a concise explanation of the discrepancy |
| Model lineage | OCR or model version and preprocessing information |
| Downstream action | Systems recalculated, decisions refreshed, documents corrected, re-underwriting performed, or delivery held |
Use append-only event history or another immutable audit mechanism. If the reviewer changes the corrected value again, create another event rather than editing the first correction. For a material discrepancy, preserve the decision state before and after correction and document whether corrected documents were issued, the file was re-underwritten, or delivery was stopped.
Use reason codes consistently
Reason codes should distinguish an OCR character error, a table-column assignment error, a source-document amendment, a conflicting document, a calculation defect, and a manual attribution correction. A controlled vocabulary makes recurring problems measurable and prevents vague explanations such as “fixed” or “looks right.”
7. Re-run every affected downstream decision
A corrected source field is not the end of the task. Determine the dependency graph for that field and refresh every affected output.
- Recalculate income, debt, cash-to-close, reserves, ratios, and other derived values.
- Refresh automated-underwriting inputs and retain the new submission or response where applicable.
- Recheck approval conditions, exceptions, and red-flag messages.
- Compare note, security instrument, settlement figures, and final disclosures after the correction.
- Determine whether the change affects eligibility, pricing, required documentation, or closing terms.
- Assess whether re-underwriting, corrected documents, a new disclosure, or a delivery hold is required.
Fannie Mae’s post-closing quality-control guidance calls for prompt correction when errors are found. Its reverification guidance recognizes that discrepancies may require reconciliation and re-underwriting. A workflow that updates only the database field while leaving a stale underwriting response or disclosure is incomplete.
8. Use confidence as a routing aid, not as proof
Confidence scores are useful for prioritizing queues, setting sampling levels, and identifying pages that need better capture. They are not evidence that a value is correct. A high score does not establish that the characters were read correctly, that the value belongs to the right borrower, or that the model selected the right table column.

For material fields, require both source comparison and business-rule validation. NIST recommends comparing AI output with known ground truth and combining human oversight with automated evaluation. The ground truth in a loan file may be the original signed document, an independently verified report, or a reconciled set of authoritative records; document which one was used.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.9. Close the loop with error monitoring
Every correction is also a signal about the capture process, document quality, validation rules, or model version. Maintain an error log that can be grouped by document type, page quality, field, vendor or model version, and reviewer override.
Useful monitoring dimensions
- Error category, such as character recognition, table layout, attribution, calculation, or source conflict.
- Document type and page characteristics, including scan quality, rotation, handwriting, and stamps.
- Field and business impact.
- Model, OCR version, preprocessing configuration, and release date.
- Whether the error was caught by confidence routing, a deterministic rule, cross-document reconciliation, or a human reviewer.
- Correction frequency, override frequency, unresolved exceptions, and time to resolution.
Use the results to improve capture instructions, validation rules, reviewer sampling, exception thresholds, and model evaluation sets. NIST’s AI Risk Management Framework emphasizes defined human oversight, ongoing monitoring, incident tracking, and correction or recalibration. Do not claim an industry-wide accuracy rate: no broadly applicable authoritative statistic establishes one for AI extraction of loan documents. Measure performance on a representative, labeled sample of your own document mix instead.
10. Align the data vocabulary with mortgage standards
Use the applicable versions of URLA and ULAD for application data, UCD for closing-disclosure data, and ULDD for delivery data. The Uniform Mortgage Data Program is a joint Fannie Mae and Freddie Mac effort directed by the Federal Housing Finance Agency to improve quality and standardization across appraisal, application, closing-disclosure, and delivery data.
Quick wins for a faster PC:
Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →ULDD is the common set of data elements required for single-family loan deliveries. Fannie Mae lists Phase 5.1.0 specification guidance as released on May 20, 2025. Record the exact standard version and the geography or program context in the audit trail because edits and mandates can change. A value that is valid in one implementation or release may not satisfy another.
Best Value
Standards checks to include in a workflow
- Map each extracted field to the current data element and permitted value set.
- Store the implementation or specification version used for validation.
- Check that the same borrower, property, and transaction identifiers persist across application, closing, and delivery representations.
- Run delivery edits after any material correction rather than assuming an earlier pass remains valid.
11. Check adverse-action and fair-lending consequences
An extraction error can become a credit-decision, explanation, or disclosure defect. If corrected data contributed to a denial or a change in credit terms, perform a final adverse-action reason check. CFPB guidance states that creditors must provide accurate and specific reasons reflecting the actual basis for the decision; generic model labels are not sufficient. As CFPB Director Rohit Chopra put it in September 2023, creditors cannot simply use sample adverse-action forms and checklists when they do not reflect the actual reason for the denial or change in credit conditions.
Review whether the corrected field changed underwriting, disclosures, appraisal treatment, or HMDA data. CFPB examination materials identify underwriting, disclosures, appraisals, and HMDA data accuracy as review areas. Preserve the before-and-after decision state and the evidence used to determine the final reason.
12. Choosing a review workflow or tool
Whether you build the process into a loan-origination system or select mortgage document data-quality software, compare capabilities rather than headline extraction confidence.

| Capability | Questions to ask |
|---|---|
| Evidence and page highlighting | Can a reviewer open the exact page and region supporting each field? |
| Confidence and exception routing | Can thresholds be set by field impact, and are low-confidence or conflicting values queued automatically? |
| Cross-document reconciliation | Can the workflow compare borrower, property, income, asset, liability, and transaction values across documents? |
| Standards support | Does validation use the applicable URLA, ULAD, UCD, and ULDD versions? |
| Immutable audit logs | Are original values, corrections, evidence, reviewers, timestamps, and downstream actions retained? |
| Roles and approvals | Can responsibilities, segregation of duties, and required approvals be enforced? |
| Reprocessing | After a correction, can dependent calculations, underwriting inputs, disclosures, and delivery edits be rerun? |

| Privacy and retention | Are access, retention, deletion, and sensitive-data controls appropriate for loan files? |
| Integration | Does the workflow synchronize safely with the loan-origination system and prevent stale values? |
| Reporting | Can teams report error categories, overrides, unresolved exceptions, and trends by document type and model version? |
Operational checklist for each file
- Freeze and retain the original PDF or image.
- Confirm document, page, model, preprocessing, timestamp, and confidence provenance.
- Prioritize identity, income, assets, liabilities, credit, property, terms, and conditions.
- Compare each material value with its exact source region.
- Run arithmetic, date, format, attribution, and cross-document rules.
- Obtain independent evidence for contradictions and document the reconciliation.
- Record every correction as an append-only event with old value, new value, evidence, reviewer, UTC timestamp, reason, and model version.
- Recalculate ratios and refresh automated-underwriting, conditions, disclosures, and delivery checks.
- Assess re-underwriting, corrected documents, adverse-action explanations, or a delivery hold.
- Log the error for trend analysis and feed recurring patterns into rules, sampling, and model evaluation.
Bottom line: make the record explainable after the fact
A reliable AI-assisted loan review is a controlled verification process, not a confidence-score approval. Preserve the source, show the evidence, validate high-impact fields with rules and independent records, and route uncertainty to trained reviewers. Keep an immutable correction history and refresh every downstream decision after a material change. That combination lets a lender demonstrate what the system extracted, what a human changed, why the change was justified, and how the final underwriting and delivery record was verified.
