The right place to ask depends on which information you need. For a doctor, hospital, Medicare Advantage organization, prescription plan, or other provider’s records, contact the organization that holds the records and follow its access procedure. For claims information held by Medicare, sign in to a secure Medicare account and use its claims-download feature. A Medicare account download is claims data, not necessarily your complete clinical chart.
You or your legal representative generally may view or obtain copies of personal health information used to treat you, bill Medicare, enroll you, manage your care, or make decisions about you. Medicare’s guidance says a response may take up to 30 days. Electronic portal access is free in most cases; a mailed copy may involve a limited copying fee.

Choose the correct access route first
“My Medicare information” can mean several different record sets. Identifying the record holder before you submit a request prevents a common delay: asking Medicare for a doctor’s chart, or asking a doctor for Medicare’s claims file.

| What you need | Who holds it | How to get it | What you should expect |
|---|---|---|---|
| Clinical chart, test results, visit notes, or treatment records | The doctor, clinic, hospital, laboratory, or other provider that created or maintains them | Contact that organization and use its health-information access process | Paper or electronic copies may be available; the organization may require its own form |
| Plan-held enrollment, billing, case-management, or coverage records | Your Medicare health plan or prescription drug plan | Ask the plan that maintains the records for its current request procedure | The plan’s process, identity checks, and available formats apply |
| Claims information available through Medicare | Your secure Medicare account | Sign in and use the claims-download feature | Download, print, or share claims data; this is not a complete medical chart |
| Medicare-held information discussed with another person | Medicare | Use CMS-10106, “Authorization to Disclose Personal Health Information” | The authorization lets Medicare talk with the person you name; it is not a universal medical-record request form |
Medicare’s privacy notice also says beneficiaries can see and obtain a copy of information Medicare holds about them. Medicare’s “Your Rights” page states: “You have the right to have your personal and health information kept private.” If you are unsure which organization has a particular record, start with the office, hospital, or plan involved in the care or claim and ask where that record is maintained.

These routes are related, but they are not interchangeable. A provider or plan request is the route for records held by that organization; the Medicare account is a convenient route for claims data Medicare makes available online; and CMS-10106 addresses Medicare discussions with a person you choose. Decide which record holder and record type fit your goal before choosing a form or account feature.
What information can you request?
Medicare describes access to a broad range of personal health information held by providers that treat you and bill Medicare, and by health plans that pay for your care, including Medicare. Depending on the organization, the records may include:
- Claims and billing records
- Enrollment information
- Medical records and care-management records
- Information used to make decisions about your treatment, coverage, or care
The exact contents depend on the record holder. A hospital may have its own clinical chart and billing file, while a plan may have enrollment, utilization, authorization, and case-management information. A Medicare account’s claims download covers the claims data Medicare makes available through that account; it does not automatically include every note, image, medication list, or laboratory result in a provider’s electronic health record.
Think about the reason you need the information. If you are preparing for a medical appointment, you may want clinical notes or test results from the provider. If you are checking what Medicare processed, the account’s claims data may be useful. If you are trying to understand a coverage or care-management decision, ask the plan which records it maintains that relate to that decision. A request can cover different record categories, but each record holder only has the information it maintains.
Request records from a doctor, hospital, or health plan
1. Identify the organization that maintains the record
Use the provider or plan connected to the information you want. For a visit summary or imaging report, that is usually the treating provider or facility. For a coverage decision or plan billing record, it is generally the Medicare plan or drug plan. If several organizations were involved, you may need to submit a separate request to each one.
2. Ask for its current access instructions
Call the organization’s medical-records, health-information-management, privacy, or member-services department, or use its patient or member portal if one is offered. Medicare’s general guidance says a form may be required, but it does not provide one universal form for every doctor, hospital, and plan. Ask where to submit the request, whether electronic delivery is available, and what identity documents are accepted.
3. Describe the records precisely
A precise request is easier to process than “my entire file.” Include your full name, date of birth, contact information, and any member, medical-record, or account number the organization uses. Then state:
- The date range you need
- The specific facility, department, clinician, or plan involved
- The categories requested, such as progress notes, discharge summaries, laboratory reports, images, claims, invoices, enrollment records, or case-management records
- Your preferred format, such as a secure download, portal message, electronic file, or paper copy
- Where the records should be sent
If you want the organization to send records directly to another person or organization, say so in its designated authorization section. A request for your own copy and an authorization to disclose records to someone else may be handled as separate steps.

4. Complete identity and representative checks
The record holder may ask you to verify your identity. Follow its instructions rather than sending sensitive documents to an unverified email address. If a legal representative is making the request, the organization may require documentation establishing that authority. Medicare’s access guidance recognizes that you or your legal representative may view or obtain copies, but the specific documentation process is set by the organization holding the records.
5. Submit the request and keep proof
Use the submission method the provider or plan specifies. Save a copy of the completed form, the date you submitted it, confirmation numbers, and any delivery receipt. If the organization offers a secure portal, that is generally safer than ordinary email for sensitive records.
6. Allow the stated response period
Medicare says a records response may take up to 30 days. That is the possible response time described in Medicare’s current rights guidance, not a promise that every request will take that long. If the organization asks you to clarify the date range, identity, or format, respond promptly because processing may not begin until the request is complete.

Download claims from a secure Medicare account
The online account route is useful when you need Medicare claims information that Medicare has made available for download. It is different from requesting a provider’s chart.
What you need before signing in
Medicare currently lists ID.me, CLEAR, and Login.gov as identity-verification options for account access. Use an email address that belongs to you rather than one shared with another person. If you cannot use a smartphone or computer, Medicare’s login help directs you to call 1-800-MEDICARE (1-800-633-4227).
Steps to retrieve the available claims data
- Create or sign in to your secure Medicare account.
- Complete the account’s identity-verification and security prompts.
- Open the claims area and choose the claims-download feature.
- Select the available dates or records you need, following the options shown in the account.
- Download the file to a device you control.
- Print it or share it only with people or services you trust.
Labels and screen layouts can change, so follow the current instructions displayed in your account. If the claims you expect are missing, check whether the service has finished processing and whether the claim belongs to a different insurer or plan. The account download cannot substitute for records that only a provider or plan maintains.
Important Medicare Advantage limitation
Medicare says that, for a Medicare Advantage plan, the Medicare account data will include only drug claims. If you need medical-service claims, clinical notes, authorizations, or other plan records, contact the Medicare Advantage organization directly and use its records-request process.
Sharing through Blue Button and connected apps
Medicare describes Blue Button as an optional way to connect apps and share your claims information. Review an app’s privacy practices, permissions, retention policies, and security before importing data. You choose whether to connect an app, and Medicare says you can stop sharing with an app through your account. An app is not required to obtain a copy; downloading or printing the file yourself remains an option.

Medicare’s app library lists record-management options such as HealthEx and Flexpa. These are examples to consider, not requirements or endorsements. Check the app’s current features and privacy practices, and confirm that it supports the records you want before sharing anything. The fact that an app appears in a library does not mean it is necessary to access Medicare claims.
When another person needs to help
Use CMS-10106 for Medicare conversations
CMS-10106 is titled “Authorization to Disclose Personal Health Information.” Its stated purpose is to authorize Medicare to talk with a person you choose about your claims or records. Complete it when you want Medicare to discuss your information with a family member, caregiver, advocate, or another helper.
This authorization should not be described as the general request form for a hospital, doctor, or health plan’s records. Those organizations may have their own forms and representative requirements. If your helper needs to obtain records directly from a provider or plan, ask that organization what authorization or legal-representative documentation it requires.

- Conversation only: authorize Medicare to discuss claims or records with the named person.
- A copy sent to the helper: confirm that the relevant record holder accepts the authorization for that delivery and identify the permitted format.
- Legal representation: provide the documentation requested by the provider, plan, or Medicare office to establish that authority.
Limit the authorization to the people, information, and period you actually want to share. Store a copy of anything you sign and ask how to end or change the authorization.
It helps to distinguish “talk to my helper” from “send my records to my helper.” CMS-10106 is described as authorization for Medicare to talk with the person you name. If you want a provider or plan to provide records to that person, that provider or plan’s own release process applies. If you are not sure whether your helper needs to receive a copy or simply help you understand the information, clarify that before completing paperwork.
Fees, formats, and delivery choices
Medicare says a copy fee may be charged, but a mailed copy cannot cost more than the total cost of copying labor, supplies, and postage. Viewing, searching, downloading, or sending information through an electronic portal is free in most cases. The provider or plan should tell you the applicable charge and delivery options before completing the request.
Electronic delivery is often faster and easier to search, but it creates a security responsibility once the file reaches your device or email. Paper can be useful for a permanent binder or for someone who cannot use a portal, but mailed copies should be stored securely. If a requested format is unavailable, ask what equivalent electronic or paper format the organization can provide.
Free tools Windows power users keep installed
One-click scans. No signup required.
Consider the trade-off before choosing: a portal download can avoid mailing and is free in most cases, while a paper copy may be easier to review away from a device but could involve a copying fee within Medicare’s stated limit. A claims download may be convenient for reviewing Medicare claims, but it should not be treated as a substitute for the clinical chart or all plan-held information.

Protect the information after you receive it
Claims and medical records contain sensitive information. Medicare advises treating downloaded files like banking information or other private data.
- Keep your Medicare username, password, and verification codes private.
- Download files to a device protected by a screen lock and current security updates.
- Do not leave files in a public or shared computer’s downloads folder.
- Use secure transfer methods rather than ordinary email when possible.
- Before connecting an app, read its privacy policy and check what data it will import, retain, or share.
- Store paper copies in a secure location and shred unwanted duplicates.
- Sign out of your Medicare account on shared devices.
Only share the minimum information needed for the task. A claims file may contain identifiers and details unrelated to the person who is helping you.
Troubleshoot common problems
“The Medicare download does not contain my doctor’s notes.”
That is expected in many cases. The account route provides claims data, not necessarily a complete clinical chart. Request notes, reports, images, and other clinical material from the provider or facility that maintains them.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitches“I have Medicare Advantage and cannot find medical-service claims.”
Medicare states that Medicare Advantage account data will include only drug claims. Contact the Medicare Advantage organization for its medical-service claims and plan records.
“The provider says there is no single Medicare form.”
Medicare does not supply one universal request form for every record holder. Ask the provider, hospital, or plan for its current process and submit the form or portal request it specifies.
“I want Medicare to speak with my caregiver.”
Use CMS-10106 to authorize Medicare to talk with the person you name. If the caregiver also needs records held by a doctor, hospital, or plan, ask each organization about its separate authorization requirements.
“I cannot sign in to my account.”
Review the identity-verification instructions for ID.me, CLEAR, or Login.gov. If you do not have access to a smartphone or computer, call 1-800-MEDICARE (1-800-633-4227) for login help.

A practical checklist before you submit
- Have I identified whether the record is held by Medicare, a provider, or a plan?
- Am I asking for claims data, a clinical chart, enrollment information, billing records, or case-management records?
- Did I specify the date range and facility or plan?
- Do I want an electronic file, a paper copy, or both?
- Will another person receive or discuss the information, and do I need CMS-10106 or a separate provider authorization?
- Do I know the organization’s submission method, expected timing, and any permitted fee?
- Do I have a secure place to store the file when it arrives?
Frequently Asked Questions
Who should I contact to get my Medicare medical records?
Contact the doctor, hospital, or other provider that maintains the clinical record, or the health plan that holds plan, enrollment, billing, or care-management information. Medicare says you or your legal representative generally may view or obtain copies of personal health information from providers that treat you and bill Medicare and plans that pay for your care. Each organization sets its own request process.
Is a Medicare account claims download the same as my medical chart?
No. The Medicare account download provides claims data made available through the account; it is not necessarily a complete clinical chart with visit notes, images, or laboratory results. Request clinical records from the provider that maintains them. Medicare says Medicare Advantage account data will include only drug claims.
How long can a records request take, and can I be charged?
Medicare’s current rights guidance says a response may take up to 30 days. A copy fee may be charged; for a mailed copy, it cannot exceed the total cost of copying labor, supplies, and postage. Viewing, searching, downloading, or sending information through an electronic portal is free in most cases.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →What form lets Medicare talk to my caregiver?
CMS-10106, “Authorization to Disclose Personal Health Information,” authorizes Medicare to talk with the person you name about your claims or records. It is not a universal request form for medical records held by a doctor, hospital, or plan. Ask those organizations about their own authorization and representative requirements.
Can I download or share Medicare claims electronically?
Yes. Sign in to a secure Medicare account and use its claims-download feature. You can print or share the file, and Blue Button is an optional way to connect apps. Review an app’s privacy practices before sharing, and remember that Medicare says you can stop sharing with an app through your account.
What if I cannot use a smartphone or computer to sign in?
Medicare’s login help directs people who cannot access a smartphone or computer to call 1-800-MEDICARE (1-800-633-4227). Medicare currently lists ID.me, CLEAR, and Login.gov as identity-verification options for account access.
The Bottom Line
Start with the organization that holds the information: providers and plans for their records, or a secure Medicare account for available claims data. Keep in mind that account claims are not a full medical chart, and Medicare Advantage account data includes only drug claims. CMS-10106 lets Medicare discuss your information with someone you choose; it does not replace a provider’s or plan’s own records process. Medicare says requests may take up to 30 days, electronic portal access is free in most cases, and mailed-copy fees are limited to copying labor, supplies, and postage.
