Do not wait to identify the substance or see whether they recover on their own. Check their breathing, follow the dispatcher’s instructions, and tell responders what they may have taken.
When to call emergency services
Call immediately if the person passes out and cannot be woken, has very shallow or stopped breathing, has blue or grey lips or skin, has a seizure, or has severe trouble breathing. These signs need urgent medical attention even if you are unsure whether ketamine is involved. The UK’s Ellergreen Medical Centre advises calling 999 without waiting to see if the person improves; in the United States, Poison Control advises emergency care for collapse, seizure, breathing trouble, or inability to wake.

Use the emergency number for the country where you are. In the UK, call 999 for these emergency signs. In the US, call 911. If you are elsewhere, use your local emergency number.
What to do while help is on the way
- Check whether they respond and are breathing. Follow the emergency dispatcher’s instructions and keep checking their breathing.
- If they are unconscious but breathing, place them in the recovery position. Keep monitoring them until help arrives.
- If they are unresponsive and not breathing, start CPR. Follow the dispatcher’s directions.
- If they are awake, keep talking to them. Check that they remain conscious and do not leave them alone.
- Do not induce vomiting or give them food or drink.
- Share what you know with responders. Tell the dispatcher and clinicians what may have been taken, when you think it happened if known, and whether alcohol or other drugs may be involved. If available, bring packaging or medicine information.
Do not let uncertainty, fear, or embarrassment delay a call. The NHS advises seeking medical advice immediately when poisoning is suspected.

What ketamine intoxication may look like
Ketamine can cause dissociation: a person may seem spaced out, detached from their surroundings, or caught in a dreamlike experience. Other possible effects include confusion, nausea, altered perception, hallucinations, difficulty moving, or difficulty making sense of what is happening. Ketamine may also raise heart rate and blood pressure.
These effects can suggest impairment, but they cannot confirm that ketamine is the cause. Similar signs may occur with other substances or medical problems. Act on dangerous symptoms rather than trying to diagnose the substance by appearance.

Why falls, mixtures, and other substances matter
Ketamine can reduce pain sensation and alter awareness, perception, and movement. A person may be injured in a fall or otherwise hurt without recognizing or reporting it clearly. Check for possible injuries, but do not delay emergency care to do so.
Mixing ketamine with alcohol, benzodiazepines, or opioids is dangerous. Do not assume the person took ketamine alone; tell emergency responders about any other substances that may be involved.

Even without the emergency signs listed above, suspected poisoning warrants prompt medical advice. In the UK, NHS guidance recommends 111 when you are unsure whether an exposure is harmful. In the US, Poison Control can advise on poison questions, but its guidance is to seek emergency care for collapse, seizure, breathing trouble, or inability to wake.

Repeated ketamine use is associated with urinary urgency, painful urination, incontinence, and bladder damage. These problems need medical care, but they are different from immediate emergency signs such as unresponsiveness, abnormal breathing, or seizures.
