Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsProtecting privacy in brain-imaging research starts before the first scan: decide who may receive the data, what future uses are acceptable, and what safeguards those uses require. Obtain consent that addresses sharing, remove or transform identifiers from the complete data package, evaluate what recipients could infer by linking files with outside information, and select open or controlled access according to the remaining risk. De-identification is necessary in many projects, but it does not automatically make unrestricted publication appropriate.
Why brain images remain sensitive after names are removed
A brain scan is not merely a picture. It can contain anatomical detail, dates, acquisition settings, device information, clinical context, segmentation files, annotations, and a subject key that connects it to other records. Even when a name and medical-record number are deleted, a recipient may be able to recognize a person indirectly or infer sensitive facts about that person or a group.

The risk depends on the whole information environment: the pixels, headers, free-text fields, linked files, study description, rarity of a condition, timing of a scan, and information already available elsewhere. A dataset that appears anonymous in isolation may become identifying when combined with a public paper, an unusual diagnosis, a geographic event, or another dataset.
NIH privacy guidance therefore calls for a privacy assessment even when data satisfy a technical or legal definition of “de-identified.” The guidance says that researchers and institutions should proactively determine whether sharing needs controlled access regardless of whether data can legally be shared without additional protections. The NIH BRAIN Initiative neuroethics guidance similarly treats neural data as private, sensitive information whose collection, transmission, and storage should follow strong security and encryption practices.

Decide the sharing pathway before collecting data
Privacy choices made at the repository-upload stage are often too late. During protocol design, write down what will be collected, which elements may be shared, who the expected recipients are, what future research is contemplated, and what restrictions will apply. Include privacy review, secure acquisition, transfer, storage, and distribution in the study plan and budget.
Define the intended future uses
- List the scientific questions that the original study and foreseeable secondary studies may address.
- Identify whether recipients may use the data for methods development, validation, teaching, commercial research, or population-level analysis.
- State whether data may be combined with other datasets, redistributed, or used to contact participants.
- Separate uses that are reasonably foreseeable from uses that would require a new consent process or institutional determination.
Map recipients and governance
Identify the repository, collaborating institutions, contractors, analysts, and eventual downstream users. Decide who will approve requests, verify a requester’s identity and qualifications, monitor compliance, respond to incidents, and retire or amend access when conditions change. A governance plan should cover both public release and controlled-access alternatives rather than assuming that one route will fit every data element.
Build security into the acquisition process
Use approved devices, authenticated accounts, encrypted transfers, least-privilege permissions, and audit logs from the point of collection. Keep the linkage key in a separately governed location when a key is needed for follow-up. Do not place identifiable exports in ordinary email, personal cloud storage, removable media without encryption, or an analysis folder shared more broadly than the study team.
Recommended Free Tools
Make consent specific enough to guide later sharing
Consent should explain future sharing in plain language, not merely say that data may be used for “research.” Participants should understand what kinds of information will be released, which parties may receive it, whether access will be public or reviewed, what security and use restrictions are planned, and whether data may be linked with other sources.
Include meaningful limits
Explain limits that matter to a participant’s decision. Examples include a prohibition on re-identification, a ban on recontact, a requirement that recipients use the data only for approved research, and the possibility that complete withdrawal may not be feasible after an authorized copy has been distributed. Describe whether broad future research is requested or whether the consent is limited to named topics, diseases, populations, or methods.
Check consent against the proposed release
Before sharing, compare the planned dataset and recipient conditions with the consent language, protocol, and institutional review. A participant’s agreement to one type of collaboration does not automatically authorize every later use. If the proposed release is outside the permitted scope, obtain a new determination, narrow the dataset, change the access model, or do not release it.

Consent establishes sharing conditions; it does not erase the possibility of identification or inference. Participants may agree to broad research while the institution still determines that sensitive files need controlled access.

Perform a documented review of every component that travels with a scan. The review should address direct identifiers, indirect identifiers, technical metadata, image content, overlays, text, and associated files linked through subject keys.
DICOM headers and related metadata
For DICOM and similar medical-image formats, inspect patient name, medical-record number, accession number, birth date, study and series dates, institution and facility fields, operator information, device identifiers, referring physician, and private vendor tags. Replace or remove values according to a documented profile. Preserve only fields needed for the scientific purpose, and record what was changed so another team can reproduce the process.
Do not assume that a standard export removed every local or private field. Validate the output with a viewer or parser, inspect representative files from each scanner and protocol, and check that no unexpected text remains in tags or filenames.
Pixels, overlays, and burned-in text
Search the image itself for burned-in names, dates, accession numbers, or annotations. Remove or mask such content without damaging measurements that the approved analysis requires. Check derived images, screenshots, montages, quality-control exports, and PDF reports as well as the original scan.

MRI anatomy and facial features
Structural MRI may retain enough facial or other distinctive anatomy to make recognition or reconstruction possible in some circumstances. Defacing, cropping, or other transformations can reduce this risk, but each operation can remove anatomy needed for a legitimate analysis. Choose the method after considering the scientific utility, the population, the modality, and the intended access level; test that the output no longer contains avoidable facial detail and that the remaining data still support the approved analyses.

Free text and linked files
Review clinical notes, questionnaires, annotations, segmentation masks, electrophysiology files, genetic files, behavioral records, spreadsheets, and filenames. A subject code is still a link if a recipient receives a mapping file or can obtain the key from another source. Remove comments and hidden spreadsheet columns, and inspect compressed archives before delivery.
Document the method and its limits
Record the de-identification standard or expert assessment used, software and version, fields removed or transformed, image operations, quality checks, residual risks, and the utility trade-offs. Recognized regulatory methods may be appropriate for some datasets; other projects need a defensible expert assessment tailored to the data and release plan. The method should be understandable to the access committee and to downstream users.
Assess linkage and inference risk
Ask what a motivated recipient could learn by combining the proposed release with information already available. This is broader than asking whether a name can be recovered.
Look for uncommon combinations
- Rare diagnoses, unusual injuries, distinctive surgeries, or very small demographic groups.
- Exact or narrow dates that can be matched to public events, hospital announcements, or published case reports.
- Geographic, occupational, or family details that sharply reduce the number of possible participants.
- Study descriptions, trial registrations, papers, acknowledgments, or repository records that reveal who was scanned.
- Repeated scans whose timing or technical signatures allow a subject to be tracked across releases.
Consider sensitive inferences
Even without identifying a person, a dataset may support inferences about neurological disease, cognitive status, medication response, developmental traits, or membership in a stigmatized group. Evaluate possible harms to individuals, relatives, and communities. Consider whether a combination of brain images with genetics, behavior, or demographic variables could reveal more than any one file does.
Revisit the assessment
Risk is not fixed. Searchable databases, machine-learning methods, image-reconstruction techniques, and public records change over time. Reassess when adding a new modality, publishing a new description, transferring data to a different repository, or expanding the permitted uses. Keep the assessment collection-specific rather than relying on a generic statement that all data are anonymous.
Choose open or controlled access deliberately
Open access maximizes discoverability and reuse, but it gives up the ability to screen each requester or enforce a research-purpose limitation after download. Controlled access adds administrative work and may slow reuse, while allowing an institution to verify requesters, review proposed uses, require security commitments, and revoke or suspend access when appropriate.

| Decision factor | Questions for open access | Questions for controlled access |
|---|---|---|
| Consent and permitted uses | Did participants clearly agree to broad public release and foreseeable future uses? | Does consent limit recipients, purposes, countries, topics, or onward sharing? |
| Residual identification or inference risk | After transformation, is the remaining risk demonstrably very low for this collection? | Could linkage, facial detail, rare traits, or sensitive inferences remain? |
| Scientific utility | Can the full-resolution or richly annotated files be released without exposing disproportionate risk? | Would tiered files, delayed release, or review of each request preserve more useful detail? |
| Potential harm | Would misuse create little foreseeable harm to people or groups? | Could disclosure cause discrimination, stigma, distress, security problems, or group harm? |
| Enforcement feasibility | Is it acceptable that downloaded copies cannot be reliably recalled? | Can the institution verify identity, approve a protocol, audit activity, and enforce terms? |
Open access may be reasonable when participants explicitly consented or a qualified institutional review concludes that residual risk is very low. Controlled access is generally more defensible when data are highly sensitive, identity-related inferences remain possible, consent is narrower, or new technology increases the chance of misuse. A dataset can be split into tiers: documentation and heavily transformed derivatives may be public, while full-resolution images, keys, or rare clinical variables remain restricted.
Set enforceable conditions for recipients
Use a data-sharing or data-use agreement for controlled releases and for collaborations that need obligations beyond a repository’s standard terms. The agreement should identify the dataset and approved project, name responsible people and institutions, and specify how access is granted and ended.
Core clauses
- Use the data only for the approved purposes and populations.
- Do not attempt to re-identify participants or contact them unless the agreement expressly authorizes it and the responsible institution approves the process.
- Do not link the files with other datasets in a way that creates an unapproved identification or inference risk.
- Do not share the data onward except under written permission and equivalent protections.
- Apply access controls, encryption, secure backups, and incident-response procedures appropriate to the sensitivity of the files.
- Notify the provider promptly about suspected disclosure, loss, unauthorized access, or a material change in the project.
- Return, destroy, or archive copies as specified when the project ends or authorization is withdrawn.
- Report publications and describe the de-identification method and access conditions accurately.
Communicate the de-identification method and residual-risk assessment to downstream users. Recipients need enough information to avoid treating a coded dataset as risk-free and to understand which uses are prohibited.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone.Secure transfer, storage, and day-to-day use
Before transfer
- Create the release from a clean, access-controlled workspace rather than from a shared analysis directory.
- Run automated checks for prohibited header values, filenames, hidden columns, and unexpected archives.
- Have a second qualified person review a sample from every modality, scanner, site, and file type.
- Generate a manifest with file counts, checksums, transformations, and the approved data dictionary.
- Encrypt the archive and transfer it through an institution-approved channel; send credentials through a separate channel.
After transfer
- Keep an access log showing who received which version, under which agreement, and when access expires.
- Use named accounts and multi-factor authentication where available; prohibit shared credentials.
- Limit downloads, exports, and administrator privileges to the minimum needed.
- Monitor for unusual bulk access and investigate failed authentication or unexpected transfers.
- Maintain an incident plan that identifies whom to notify, how to preserve evidence, and how to suspend access.
Encryption protects data in transit and at rest, but it does not solve an overly broad consent, an unsafe release design, or a recipient who is permitted to download unrestricted copies. Technical controls and governance must work together.
Apply the rules that actually govern the project
Obligations may arise from federal, Tribal, state, and local law, institutional policy, contracts, funder terms, repository rules, and the consent document. Which requirements apply depends on the research, participants, organization, data type, and recipient. Do not assume that a particular health-privacy or research rule automatically covers every brain-imaging dataset, and do not treat NIH supplemental guidance as a replacement for binding legal advice.
Ask the responsible privacy, legal, information-security, and research-ethics offices to determine applicability before release. Preserve their determination with the protocol and data-management record. If collaborators are in different jurisdictions, identify the stricter or additional requirements that affect transfer, storage, access, and participant rights.

A practical release review
Use this checklist at protocol approval, before the first external transfer, and whenever the dataset or recipient changes:
- Future uses, recipients, access model, and withdrawal limits are described in the consent and protocol.
- The institutional review confirms that the proposed release is within the approved purpose.
- Every direct and indirect identifier in headers, pixels, overlays, text, filenames, and linked files has been assessed.
- MRI-specific facial or distinctive anatomical detail has been considered, with transformations documented where appropriate.
- Linkage and inference risks have been evaluated against current public information and likely recipient capabilities.
- The scientific utility lost through masking, cropping, aggregation, or removal is recorded and justified.
- Open, controlled, delayed, tiered, or no-release access has been selected for a stated reason.
- Recipient identity, proposed use, security capability, onward-sharing rules, and expiration are documented.
- Agreements prohibit unauthorized re-identification and recontact and define incident reporting.
- Transfer, storage, logging, backup, and destruction procedures have named owners.
- The legal and policy requirements for every relevant jurisdiction and institution have been checked.
- A reassessment date or trigger is set for new technology, new linkage sources, or a changed research purpose.
Common mistakes and how to correct them
“We removed names, so the files are anonymous.”
Names are only direct identifiers. Correct the mistake by reviewing headers, pixels, dates, rare traits, linked records, and what an outside party could infer from the study context. Record residual risk instead of using “anonymous” as a blanket label.
“The repository is public, so open access is required.”
A repository’s technical capability does not override consent or institutional review. Use a controlled collection, a restricted tier, or a transformed derivative when unrestricted release is not justified.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
“Defacing solved the privacy problem.”
Defacing addresses one source of anatomical detail. It does not remove identifying dates, rare diagnoses, metadata, linked files, or inference risk. Review the entire package and explain what the transformation does and does not protect.
“A data-use agreement is enough by itself.”
An agreement cannot undo an unsafe release or prevent every breach. Pair contractual restrictions with minimization, technical controls, requester review, monitoring, and an incident plan.
“One assessment remains valid forever.”
External datasets and analytical tools evolve. Revisit the risk when the collection, description, access route, or intended use changes, and document the reason for continuing, narrowing, or suspending access.
What responsible sharing looks like
A well-designed release lets legitimate researchers answer useful questions without asking participants to accept avoidable exposure. The study team explains future uses before collection, obtains consent that matches those uses, removes identifiers from every relevant component, evaluates what can still be linked or inferred, and chooses the least permissive access model that preserves the approved scientific value. Recipients receive clear rules, security expectations, and a prohibition on re-identification attempts unless a specific authorization says otherwise.
