Track demands and pace, hours and shifts, control, role clarity, support, staffing, task distribution and relevant well-being indicators. Use findings to change work conditions and check whether those changes help; a questionnaire alone cannot diagnose burnout or explain why a team is struggling.
What should an assessment measure?
Burnout risk is connected to psychosocial conditions at work—not just individual resilience or a self-reported score. The International Labour Organization (ILO) identifies factors such as workload and work pace, working time, limited control, weak support and unclear roles. Its guidance on psychosocial risks and stress at work frames hazards in the design and management of work.

- Demands and pace: Work volume, intensity, deadlines, time pressure, long or unsocial hours, shift patterns and peaks in demand.
- Control and resources: Whether employees can influence tasks and workload, and whether staffing, equipment and supervisor or coworker support are adequate.
- Work organization: Role clarity, changes in duties, task design, communication, participation in decisions and fairness of processes.
- Distribution: Who performs each task, when it happens and whether duties are unevenly distributed across a team.
- Worker feedback: Relevant perceptions of work and well-being, gathered with a method suited to the decision and workforce.
Consider ordinary operating conditions as well as busy periods, emergencies and unusual tasks. Workload can be excessive or insufficient: the UK Health and Safety Executive (HSE) notes that performance can be affected when workload is too high or too low. Its workload guidance recommends looking across the team, including task redistribution, shift balance and duties that accumulate gradually. Reassess when responsibilities, equipment or systems change.

Before choosing a survey or staffing method, identify the question the assessment must answer. Different tools serve different purposes; a staffing calculation, a psychosocial hazard assessment and a broad well-being questionnaire are not interchangeable.

- Staffing or capacity: Determine whether available staff can meet the work required and where capacity is insufficient.
- Unsustainable work conditions: Identify demands, limited control, weak support or other work-design hazards that should be addressed.
- Work-design change: Establish how work is experienced before a change, then assess the changed conditions and their effects.
- Worker well-being: Understand broader dimensions of well-being using a questionnaire designed for that purpose.
Choose a method whose population, setting, language and administration suit the workforce. Check the instrument owner’s current version, validation evidence, access terms and licensing before adopting it. The WHO/ILO resource listing questionnaires describes instruments with differing language availability; it does not identify one tool as best for every employer.
How do the available methods differ?
| Method | What it is designed to assess | Best fit and limits |
|---|---|---|
| NIOSH Worker Well-Being Questionnaire (WellBQ) | An integrated assessment of worker well-being, including quality of working life, circumstances outside work, and physical and mental health. | Useful for a broad view of well-being; it is not a workload staffing calculator. The NIOSH page identifies a revision dated June 2026. NIOSH WellBQ |
| NIOSH Quality of Worklife Questionnaire | Relationships between job or organizational characteristics and worker health and safety. Listed subjects include hours, workload, autonomy, job security, stress and well-being. | Relevant when examining how work characteristics relate to health and safety; it does not by itself calculate staffing needs. NIOSH Quality of Worklife Questionnaire |
| WHO Workload Indicators of Staffing Need (WISN) | A workload-based staffing method that applies activity-time standards to workload components. | Intended for health workforce staffing needs, not a universal burnout score or general worker well-being survey. WHO links a second-edition user manual dated November 23, 2023. WHO WISN overview |
| COPSOQ and CBI | A WHO/ILO resource lists COPSOQ among psychosocial-factor questionnaires and CBI among measures related to burnout fatigue and exhaustion. | Potential options to investigate, but the listing does not establish current licensing, validation for a particular workforce or which is best for a given employer. WHO/ILO occupational health tools |
These methods answer different questions, and the available sources do not establish a single best instrument or head-to-head winner. A questionnaire can contribute evidence about worker experience; it does not establish a clinical diagnosis of burnout or substitute for investigating how work is organized.

How to carry out the assessment
- Define the scope. Specify the team or roles, the decision to be made and the work conditions to examine. Include routine work, demand peaks, emergencies and unusual tasks.
- Map the work. Review workload volume and pace, working hours and shifts, staffing and equipment, task allocation, role changes, control, communication and available support.
- Gather worker feedback. Select a questionnaire or other feedback method that matches the purpose and workforce. Explain how responses will be used, protect confidentiality appropriately and avoid using scores to label individual employees.
- Look for patterns with employees. Compare feedback with work-system evidence: which tasks or periods create pressure, where duties accumulate, and whether employees have enough resources, support and influence over their work.
- Prioritize work-level controls. Record important risks and decide what can change in workload, task distribution, schedules, communication, participation, support or resources.
- Communicate and review. Tell affected employees what actions will be taken and revisit relevant measures after changes to see whether conditions improved. There is no universal survey interval established by these sources; review timing should suit the risks and changes being assessed.
Turn results into changes in work
Assessment only helps when it leads to prevention. The ILO recommends comprehensive risk assessment and collective as well as individual preventive measures, including increasing workers’ control and participation. HSE likewise advises acting on stress-risk assessment findings. Its guidance on assessing and acting on work-related stress risks is specific to Great Britain.

Where findings point to excessive demands, limited control or weak support, consider changes to how work is designed and delivered. Depending on the problem, that may mean reducing or redistributing workload, adjusting schedules, improving communication, involving workers in decisions, strengthening teamwork or providing adequate resources. WHO’s guidance on mental health in the workplace gives workload reduction, schedule changes, communication and teamwork as organizational approaches for health workers; use these as sector-specific examples, not as a claim that every measure applies identically to every occupation.

What the evidence does—and does not—establish
There is no universal score or cutoff in these sources that definitively identifies a team as being “at burnout risk,” and they do not prescribe one survey frequency for every workplace. Treat assessment results as evidence to investigate and guide prevention, not as a label for individuals. Keep conclusions tied to the workforce and conditions actually assessed.
The ILO’s 2026 report estimates that psychosocial risk factors are responsible for more than 840,000 deaths annually, nearly 45 million disability-adjusted life years (DALYs) lost each year, and annual losses equivalent to 1.37 per cent of global GDP. These are estimates reported by the ILO, not a separate measurement of risk at a particular employer. ILO report summary
