How to Fix a Local Feedback Compiler That Misses Themes or Produces Misleading Summaries

Fix the diagnosis before changing the model, prompt, clustering algorithm, or storage layer. First determine whether the evaluator is reporting a real defect. If it is, replay one known failure through every stage—input preparation, grouping or code assignment, theme labeling, and summary generation—until you find where information disappears or gets distorted. Change only that stage, rerun the affected cases, and keep the failure as a regression test.

Start by separating the two symptoms

“Misses themes” and “produces misleading summaries” are related, but they do not identify the same failure. A theme can be absent because records were dropped before analysis, because related records were split across groups, or because a valid group received an uninformative label. A summary can be misleading even when grouping is correct: it may omit a material request, combine separate needs, or state a conclusion more strongly than the records support.

  • Coverage: Is every material issue represented by a record, code, cluster, or theme?
  • Faithfulness: Does each label and summary claim accurately reflect the records assigned to it?

Do not treat a fluent label or polished paragraph as evidence that the underlying analysis is correct. Inspect the artifacts that produced it.

Check the evaluation signal before changing the compiler

If reviewers consider an output acceptable while an automated test reports failure, investigate the evaluator first. Microsoft’s evaluation-triage guidance describes this situation as an evaluation problem: “Use this category when the agent behavior is acceptable, but the evaluation produces an incorrect or misleading signal.” The same guidance lists stale expected answers, a grader that is too strict or measuring the wrong property, ambiguous test cases, factual judge errors, and systematic bias as possible causes.

Validate the expected answer

  • Confirm that the reference answer reflects the current source records and current product or policy wording.
  • Check whether an acceptable paraphrase is being rejected by an exact keyword or string match.
  • Look for expected themes that no longer apply because the input set or question changed.

Validate the grader

  • Read the rubric and identify the behavior it actually rewards: exact wording, semantic coverage, factual support, or something else.
  • Test the grader on deliberately clear positive and negative examples.
  • Check whether the judge systematically favors short answers, certain vocabulary, or one interpretation of an ambiguous request.
  • Require concrete acceptable and unacceptable examples in the rubric instead of relying on abstract terms such as “good summary.”

If the output is acceptable and the evaluation is wrong, correct the evaluation setup and rerun it before changing the compiler. If reviewers can point to an unsupported claim, an omitted issue, or a missing theme in the records, treat it as an analysis defect and continue with stage-level tracing.

Map the pipeline into inspectable stages

Many local implementations combine several operations behind one command. For debugging, separate them conceptually even if the code remains in one process. The Qualitative Feedback Analysis API version 2.9.0 documents bulk analysis, per-record summaries, bulk summaries, and hierarchical code assignment as distinct operations. Those operations provide a useful inspection model; they do not imply that your compiler uses that API, a particular language, model, database, embedding system, or clustering method.

Stage Artifact to inspect Question
Input and preprocessing Original records, transformed text, inclusion counts Did the relevant evidence enter analysis unchanged?
Analysis or code assignment Codes, clusters, memberships, confidence or scores Were related records grouped and assigned consistently?
Theme characterization Labels, keywords, representative records Does the name communicate the actionable requirement?
Summary generation Draft summary and claim-to-record mapping Is every material claim supported, relevant, and appropriately qualified?
Evaluation Expected output, grader result, human review Is the reported pass or failure measuring the intended quality?

The API documentation describes faithfulness, coverage, and clarity as analysis quality dimensions. Use those dimensions as inspection questions rather than assuming that one aggregate score identifies the faulty stage.

Step 1: Capture a reproducible failure

Do not debug from a screenshot or a remembered prompt. Save a replayable case with enough context to compare runs.

  1. Preserve the original records. Store the exact text, identifiers, timestamps if relevant, and source metadata used by the failing run.
  2. Save the analyst question or task prompt. A change in wording can alter both grouping and summary scope.
  3. Record intermediate artifacts. Keep transformed text, filters, deduplication results, assigned codes or clusters, representative examples, labels, and the final summary whenever the implementation exposes them.
  4. Record configuration. Include software version, prompt or rubric version, model and parameter settings, thresholds, random seed where available, and data-selection rules.
  5. Write the expected behavior. State the theme that should appear, the evidence supporting it, and the exact summary claim that is wrong, omitted, or overstated.
  6. Build a small review set. Include known missed themes, summaries that overstate or distort records, and counterexamples that currently work. Counterexamples reveal when a broad label merges different requests.

This logging format is a practical debugging recommendation, not a universal standard. The essential requirement is that another run can use the same inputs and configuration and produce comparable artifacts.

Step 2: Find where a theme disappears

Input and preprocessing checks

Compare the source records with the records presented to analysis. Verify the expected input count and inspect transformed text rather than trusting counters alone.

  • Look for records dropped by date, language, permissions, null handling, or an accidental filter.
  • Check deduplication: near-duplicate removal can erase the only example containing a distinguishing requirement.
  • Check truncation limits. A request at the end of a long record may be removed before embedding or classification.
  • Inspect normalization that strips negation, product names, error messages, quantities, or other words that distinguish one need from another.
  • Compare an original record and its transformed form side by side for the failing examples.

If the theme is absent from the analyzed input, fix ingestion or preprocessing before touching clustering or prompts.

Analysis and code-assignment checks

Locate each failing record in the assigned codes, clusters, or intermediate analysis. A theme that exists in the records but has no coherent group indicates a coverage or assignment problem.

  • Check whether similar records were split across several small groups.
  • Check whether unrelated records were merged into a broad group that hides the requirement.
  • Inspect thresholds, minimum group sizes, outlier handling, and hierarchical parent-child assignments if those controls exist.
  • Compare membership before and after any sampling, balancing, or ranking step.

Do not infer a clustering defect solely from a weak label. First establish whether the relevant records are together and whether the group contains enough evidence to justify a distinct theme.

Theme-characterization checks

Read representative records inside each group. Ask whether the label describes the requirement that a decision-maker needs to act on, not merely the general subject. A label such as “login problem” may be topically accurate while concealing the actionable issue—expired one-time codes, missing account recovery, or a request for single sign-on.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Mark a label as defective when it is unsupported by its examples, indistinguishable from neighboring labels, or too general to guide a product or policy decision. The group may be sound even when its name is not.

Summary-generation checks

Map every material sentence in the summary to the records it claims to represent. Mark omissions, unsupported claims, irrelevant material, and excessive certainty separately. This prevents a better-sounding label from being mistaken for an improved summary.

Distinguish missing clusters from weak labels

Observed pattern Likely problem Next diagnostic action
No record expressing the issue reaches any relevant group Input, preprocessing, filtering, or assignment failure Compare source and transformed records; trace membership and exclusions.
Records are scattered across several groups Grouping or code-assignment coverage Inspect similarity, thresholds, outlier rules, and hierarchical assignments.
Records are together but the name says only “problem” or “feedback” Theme characterization Rewrite the label from representative evidence and test distinction from neighboring groups.
Label is accurate but summary omits a material request Summary coverage or compression Build a claim-to-record checklist and identify the omitted evidence.
Summary combines separate needs or asserts unsupported causes Summary faithfulness Split claims, remove unsupported inferences, and qualify prevalence or certainty.

Research on software user-feedback clusters distinguishes general meaning from actionable detail and compares unigram, bigram, trigram, and sentence characterizations. It supports evaluating labels for three properties: descriptive accuracy, distinction from other clusters, and focus on requirement-relevant information. It does not establish one universally correct label length or algorithm.

Choose label length as a local trade-off

  • Short phrase: fast to scan, but may omit the condition or requested action.
  • Multiword phrase: often balances scanability with specificity.
  • Sentence: can preserve context and constraints, but may be harder to compare in a long list.

Compare alternatives on your review set. Have reviewers identify the actionable point, distinguish the label from adjacent groups, and verify it against the examples. Treat sentence labels as a possible context advantage, not a guaranteed improvement.

Audit a misleading summary claim by claim

Use a table or annotation sheet with one row per material claim:

Claim Supporting records Audit result Repair
What the summary says Record IDs and quoted evidence Supported, omitted, irrelevant, merged, or overstated Retain, add evidence, split, remove, or qualify

Look for five distinct errors

  • Omission: an important issue appears in the records but not in the summary.
  • Unsupported attribution: the summary assigns a motive, cause, or preference that the records do not state.
  • Conflation: different needs are presented as one request.
  • Irrelevance: material unrelated to the analyst’s question consumes the summary.
  • Overconfidence: words such as “all,” “most,” “caused by,” or “users demand” exceed the reviewed evidence.

A COLING 2025 study by Kirstein, Ruas, and Gipp describes a two-stage approach: identify mistakes, then refine the summary using actionable feedback. Its QMSum Mistake dataset contains 200 automatically generated meeting summaries annotated by humans across nine error types, including structural, omission, and irrelevance errors. That dataset concerns meeting summaries; it is not a failure rate or performance benchmark for a local feedback compiler.

Separate evidence from interpretation

Rewrite a claim so that the reader can tell what the records say and what the analyst infers. For example, “Several records request recovery by email” is an evidence-level statement when the records support it. “Users cannot access their accounts because email recovery is unreliable” adds a causal conclusion that requires direct support. If the records do not establish cause, state the request without inventing one.

Apply the smallest fix that matches the evidence

Once the faulty stage is identified, change that component rather than broadly retuning the entire system.

If input or preprocessing is at fault

  • Correct the filter, deduplication rule, truncation limit, or text transformation.
  • Preserve negation, quantities, error messages, and requirement-specific terms.
  • Add an input-count and transformed-text check to the run output.

If grouping or code assignment is at fault

  • Adjust the diagnosed threshold or outlier treatment, not every parameter at once.
  • Review split groups for a shared requirement and review merged groups for incompatible requests.
  • Use hierarchical codes when a broad subject contains distinct actionable subthemes.

If characterization is at fault

  • Generate candidate labels from representative records, then reject labels that lack actionable detail.
  • Require a label to be distinct from neighboring labels and supported by more than one example when the data permits.
  • Keep the label’s scope aligned with the group; do not add a cause, audience, or prevalence claim absent from the records.

If summary generation is at fault

  • Provide the summarizer with the records or evidence snippets that justify each theme.
  • Require a claim-level support check before publication.
  • Ask for omissions, unsupported inferences, and irrelevant content to be flagged separately.
  • Use qualified language when the reviewed sample cannot establish prevalence or causation.

Verify the repair and protect it with regression cases

  1. Rerun the original failing records with the corrected component.
  2. Confirm that the missing theme is now represented or that the label communicates its actionable requirement.
  3. Check every repaired summary claim against source records.
  4. Rerun previously correct counterexamples to detect regressions.
  5. If the rubric or grader changed broadly, rerun the affected evaluation set rather than judging success from one example.
  6. Store the original failure, the repaired output, and the reason the change was accepted.

Microsoft’s remediation guidance recommends rerunning after a fix and moving to another remediation only when the failure persists. A single successful example is not enough when the defect arose from a broad rule, threshold, or evaluator change.

Use a compact review rubric

Microsoft’s rubric reference recommends selecting three to five relevant themes, defining them for the domain, and using those definitions for automated grading and human review. Adapt the dimensions to feedback analysis:

Dimension Pass condition Failure example
Accuracy The label or claim matches what the records state. A request is described as a confirmed defect or cause without evidence.
Completeness All selected material themes and requirements are represented. A secondary but actionable request disappears from the summary.
Groundedness or faithfulness Each material statement can be traced to source records. The summary attributes a preference that no record expresses.
Clarity A reviewer can understand the actionable point and distinguish it from neighboring themes. A label such as “general issue” covers several unrelated needs.
Hallucination-free behavior No invented facts, causes, users, frequencies, or outcomes appear. The summary claims a majority preference from a few uncounted examples.

Common traps that waste debugging time

  • Changing the model first: A dropped record or invalid grader cannot be repaired by a more capable model.
  • Judging labels without examples: A label must be evaluated against the records in its group.
  • Equating fluency with faithfulness: Smooth prose can still omit or invent information.
  • Using one aggregate score: Coverage, clarity, and faithfulness can move in different directions.
  • Overfitting to one failure: A local prompt patch may fix one record while merging or omitting themes elsewhere.
  • Assuming a universal label format: Phrase and sentence characterizations involve a scanability-versus-context trade-off that must be tested locally.
  • Reporting external figures as local performance: The 200-summary QMSum Mistake dataset describes an annotated study resource, not your compiler’s error rate.

A practical run checklist

  • Can the exact input, prompt, configuration, and software version be replayed?
  • Does the evaluator use current expected answers and accept valid paraphrases?
  • Did every relevant record survive loading, filtering, deduplication, and transformation?
  • Where is each known theme located in codes or clusters?
  • Do labels state an actionable requirement and differ from neighboring labels?
  • Can every summary claim be mapped to supporting records?
  • Are omissions, conflations, irrelevance, and overconfidence tracked separately?
  • Was only the implicated stage changed?
  • Did the original failure and counterexamples pass after the change?
  • Was the failure preserved as a regression case?

What the available evidence can and cannot establish

The documented API operations and quality dimensions are useful for designing stage inspections, but they do not prescribe your implementation. The software-feedback cluster study supports testing labels for descriptive, distinct, and requirement-focused qualities; it does not prove that a particular number of words or a particular clustering method is optimal. The COLING 2025 summary study supports mistake identification followed by refinement, while its meeting-summary dataset should not be presented as a benchmark for feedback analysis. No general failure rate, guaranteed accuracy improvement, or universally best cluster-label size has been established for local feedback compilers.

The Bottom Line

Trace the evidence, not just the final prose: validate the evaluator, replay one failure through input, grouping, labeling, and summarization, then make the smallest stage-specific fix and lock it in with regression cases.
