A game that improves performance on a computerized attention test has not necessarily been shown to reduce all ADHD symptoms or improve everyday functioning. For a product marketed as treatment, also examine study design, safety, privacy, cost, and how it fits with clinician-directed care.
First, identify what the app actually claims
Write down the claim in the product’s own words. “Supports focus,” “trains attention,” “improves ADHD symptoms,” “treats ADHD,” and “replaces medication” are materially different claims. Ask what outcome is promised, for whom, and over what period. Then compare that promise with the outcomes measured in studies and, where applicable, the product’s authorized indication.

The Federal Trade Commission says health-benefit claims require competent and reliable scientific evidence; it generally expects randomized, controlled human clinical testing for such claims. Search the FDA record for the specific product and version. Read the intended use, age range, limitations, and decision type rather than relying on a badge or phrase in an advertisement. A 510(k) decision that a device is “substantially equivalent” is not the same wording as FDA approval. Regulatory status is specific to a product and indication; it does not validate every marketing claim made about it.

Also distinguish a medical treatment claim from a wellness or organization feature. Not every app that may be useful to someone with ADHD is a regulated medical device. For a treatment claim, ask the person’s clinician whether the exact product and intended outcome belong in the care plan. Do not stop medication or other treatment because an app advertisement says to.
Read the evidence against the claim
For each study, note who participated and whether they match the intended user: age, ADHD presentation, inclusion criteria, and setting. Record the sample size, duration, comparator, adherence, dropouts, outcomes, adverse events, and funding or conflicts. Check whether the study was randomized and blinded, whether the comparison controlled for time spent gaming and expectations, and whether an independent group has replicated the result.

Pay particular attention to what was measured. A score on a computerized attention test is not interchangeable with broad symptom improvement or changes in school, social, or home functioning. A positive result on one measure does not establish benefits on outcomes the trial did not demonstrate.
What the EndeavorRx studies illustrate
EndeavorRx is a useful example of how narrow an evidence-based claim can be. The FDA’s 2020 review describes a four-week randomized study of 348 children ages 8–12, comparing the game with a digital control. The primary TOVA attention score changed by 0.93 points in the EndeavorRx group and 0.03 in the control group (p=0.006). The same review found no statistically meaningful difference on several secondary parent or clinical rating scales. Those results support a distinction between improvement on the specified attention measure and broader symptom claims. The FDA review is available at the pivotal-study summary.

A later FDA submission described a four-week, open-label study of 162 adolescents ages 13–17; 146 were included in the efficacy analysis. The submission reported a significant mean change on the TOVA measure, but the study had no sham control and the FDA noted that a placebo effect could have inflated the result. Understand what FDA status does—and does not—mean
On June 15, 2020, the FDA granted EndeavorRx De Novo classification and established a Class II device category for digital therapy software for ADHD. The FDA described it as prescription use, not stand-alone therapy, and as part of a program that may include clinician-directed therapy, medication, and/or educational programs. In 2023, the FDA found an updated EndeavorRx submission substantially equivalent through the 510(k) pathway. The submitted version expanded the age range from 8–12 to 8–17. Its indication is to improve attention function as measured by computer-based testing in children ages 8–17 with primarily inattentive or combined-type ADHD who have a demonstrated attention issue. The indication warns that patients may not benefit in typical behavioral symptoms such as hyperactivity. Read the FDA indications for use alongside the FDA 510(k) record.

Check safety, accessibility, and practical burden
Read the product’s warnings and eligibility restrictions. Consider whether the screen-based regimen, device, controls, and interface are usable for the person who would use it. For EndeavorRx, the vendor lists limitations including photosensitive epilepsy, color blindness, and physical limitations that affect device use. Its page also reports treatment-related adverse events in 17 of 342 participants in two supporting trials (4.97%), including decreased frustration tolerance and headache among events occurring in more than 1% of participants; the vendor says no serious adverse events were reported in those trials. These are vendor-presented trial details, not a guarantee about an individual’s experience. Consult the current vendor safety and product information and discuss product-specific concerns with a clinician.
Account for the time required, device compatibility, prescription access, subscription or cancellation terms, and whether the app adds a manageable task or a new source of frustration. For EndeavorRx, the vendor listed a price of $99 for a 30-day prescription on October 4, 2026; check the current vendor page because price and access terms can change.
Review privacy as carefully as the health claim
Read the privacy policy and app-store disclosures before entering health or child information. Look for what is collected, whether data are shared or used for advertising, how long they are retained, how deletion works, and whether account data can be exported. Do not assume HIPAA applies to every consumer health app.
The FTC explains that FDA oversight covers only a subset of medical-device apps, while FTC law applies to most app developers’ privacy and performance representations. COPPA applies in specified circumstances involving users under 13. See the FTC guidance on health apps and privacy.
Use a consistent comparison checklist
When comparing products, evaluate the same dimensions for each one. Keep treatment-oriented apps separate from ordinary wellness, reminder, or organization tools; the latter may be useful without claiming to treat ADHD.
