Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsDeploying a 3D brain tumor segmentation model for clinical use is a lifecycle project, not just a matter of running inference on MRI scans. First define the intended use and obtain jurisdiction-specific regulatory review; then validate the model on representative local cases, integrate it into the imaging workflow with qualified human review, and govern its security and changes over time. A research model or a successful hospital study is not, by itself, an authorized clinical product or proof that the model will perform similarly at another institution.
1. Define the intended use before selecting an architecture
Write down who will use the model, which patients and MRI studies it supports, what it returns, and how the result may affect care. Specify whether the output is advisory, whether it can enter the clinical record, and where inference will run. These choices shape the technical workflow and the regulatory assessment; the right route cannot be determined without the jurisdiction and a precise intended-use statement.

In the United States, FDA’s Digital Health Policy Navigator, Step 6, says software intended to acquire, process, or analyze medical images may be a medical device. Its policy navigator includes MRI among medical imaging modalities. Whether a specific system is regulated depends on its intended use and applicable policy, so obtain institution- and jurisdiction-specific regulatory review before clinical use.

2. Establish exactly what the model accepts and produces
Document the model boundary so a clinical team can tell which inputs are supported and reproduce the path from image to mask. Keep the model, preprocessing, postprocessing, and output format under version control as one system.

The 2022 Yale implementation described by Aboian and colleagues analyzed FLAIR MRI for whole-glioma segmentation and used brain extraction, reorientation, resampling, and z-score normalization. Its use of BraTS 2021 and internal Yale data describes that study, not a default recipe for another model. Do not assume a model trained or configured for one sequence or population can safely accept a different one.
3. Validate performance in the population and workflow where it will be used
Evaluate the full deployed pipeline on representative local cases, not only the model in isolation. Use a reference standard created or adjudicated by qualified readers, and report what was measured in enough detail for clinicians to interpret the result.
- Describe patient, scanner, and protocol composition, the MRI sequences included, and any missing or rejected inputs.
- Define the tumor regions and segmentation metrics precisely; report subgroup results and quality or confidence flags where available.
- Review failure cases and assess the clinical consequences of both over-segmentation and under-segmentation.
- Test whether performance holds across the local scanners and protocols, and plan for additional evaluation if the model or imaging environment changes.
In the 2022 Aboian et al. study, the reference was manual segmentation by a board-certified neuroradiologist. The authors identified limited annotated data and lower performance on geographically distinct validation datasets as translation barriers. This is why a result from one hospital should not be treated as a performance guarantee elsewhere.

That study reported a median Dice similarity coefficient (DSC) of 0.86 for its automatically generated whole-tumor segmentation from FLAIR against its neuroradiologist reference. It also reported computation in under five seconds for its combined automatic glioma 3D segmentation and radiomic feature-extraction workflow. Both are study-specific measurements, not expected results or latency targets for a different model or institution.
4. Integrate with imaging systems without treating DICOM as a safety guarantee
Design a reliable path from study selection through input transfer, inference, output return, and visible status or error reporting. Decide how users open and edit the mask, how corrections are saved, and how the output is distinguished from a finalized clinical interpretation.
DICOM supports communication and management of medical imaging information, but it does not specify every implementation detail or provide a conformance test procedure. DICOM compatibility alone therefore cannot establish that the integrated workflow selects the correct study, associates the right mask with it, or handles failures safely.

The Yale paper describes one pattern: a PACS-connected inference service returned segmentation annotations to the study for review and editing in familiar radiology tools. The authors embedded their model using Docker and NVIDIA Triton. These are implementation choices in one reported workflow, not a universal architecture or product recommendation.

Before deployment, specify who reviews each result, what they can change, and what happens when the input is unsupported or inference fails. Make the model’s status clear in the user interface and in any saved output. The reported Yale workflow presented baseline segmentations for physician approval or modification; it does not establish that a segmentation model can replace specialist judgment.
- Set the review point and identify the responsible clinical role.
- Define how users correct a mask and how those edits are retained or communicated.
- Provide a clear fallback when no result is returned; do not silently treat a failed or unsupported case as a valid segmentation.
- State whether output is excluded from care until review, consistent with the intended use and applicable requirements.
6. Secure the system and manage the data lifecycle
Apply the institution’s controls for protected imaging data, access, logging, network boundaries, container and dependency updates, and incident response. The Yale paper reports anonymizing DICOM data moved from clinical PACS to research PACS and describes Docker practices including updates, restricted permissions, and limiting resource use. Those examples are not a complete compliance checklist.

FDA lists AAMI CR34971:2022 as a recognized standard. Its machine-learning risk-management material includes data management, feature extraction, training, evaluation, and cybersecurity or information-security risks. Use current local policies and applicable standards to assess the complete system, not just the trained weights.
7. Monitor performance and control changes after deployment
Assign versions to the model, preprocessing, deployment container, and interfaces together so an output can be tied to the system that produced it. Track input acceptance and failure rates, turnaround time, user corrections, performance drift, and subgroup signals in a privacy-appropriate way. Define who investigates alerts and how the system can be rolled back.
Set change-control triggers for updates to the model, data, scanner, imaging protocol, or surrounding software. Decide when each change requires investigation, rollback, or revalidation before the modified system is used clinically. FDA’s AI/ML materials address lifecycle oversight across development, deployment, use, and maintenance; the risk-management approach should likewise continue after launch.
What a defensible deployment decision requires
A deployment is ready to be considered for clinical use only when its intended use and regulatory status have been reviewed, its behavior has been validated for the intended population and local workflow, users can inspect and correct outputs, and the institution has defined security, monitoring, and change-control responsibilities. The evidence cited here supports that lifecycle approach, not a claim that any particular model is ready for a particular hospital.
