Diagnose those layers in order and use the actual error or discovered-tool list to choose a fix; reconnecting services or broadening permissions without evidence can miss the cause.
1. Confirm the tool is configured and available to this agent
Before investigating model behavior, check whether the agent has been given the tool at all. In an API-based setup, inspect the request or agent configuration: OpenAI’s tool guide describes tools configured through the tools parameter, while its Agents API uses agent.tools. The application must also handle function calls and provide an execution environment when a tool needs one. In an SDK setup, check that the tool is attached to the correct agent or exposed to the specialist expected to use it.

For MCP, verify that the server is configured and registered before the agent runs. Microsoft Agent 365’s tooling documentation describes registering configured servers during initialization so their tools and authentication are available.

- Inspect the tools actually returned or exposed at runtime; do not rely only on the configuration file.
- If an allowlist is used, compare the discovered list with the list after filtering. Microsoft Foundry notes that misspelled or incorrectly cased
allowed_toolsnames can hide tools. - Check the platform’s naming convention before editing a call target. Foundry may prefix MCP tool names with the server label.
If the tool is absent, fix registration, attachment, naming, or filtering before examining the call itself. Foundry’s MCP troubleshooting guide distinguishes missing tools caused by filters or connection problems from other failure modes.

2. Diagnose MCP startup and network reachability
A configured server can still fail to initialize or be unreachable from the process that runs the agent. Start with the failed-turn event and, for a stdio server, its process logs. OpenAI’s remote MCP guide recommends checking the connection URL and origin, whether the execution environment can reach the network, whether the configured command and dependencies work there, and whether an inline stdio configuration uses an existing absolute working directory.
Match the connection type to where the server runs

A locally reachable URL is not proof that a hosted service can reach it. Conversely, an environment-origin connection depends on the executor’s own network access.
Use the symptom to narrow server-side causes
- On Microsoft Foundry, zero tools can indicate absent or invalid connection credentials.
- If OpenAPI tools are absent, check whether the specification is malformed.
- If the list is incomplete, inspect the allowed-tools filter for spelling and case.
- For a transient
tools/listserver error, Foundry advises retrying after a few seconds.
These are platform-specific findings, not interchangeable explanations for every empty tool list. Follow the instructions for the platform and connection type in use.

3. Separate authentication, consent, and authorization
Once the server is available, check whether the runtime identity can authenticate and whether that identity is authorized for the requested operation. For HTTP MCP, verify that the token or headers are present and intended for the configured server URL. Keep secrets out of reusable agent definitions and logs.

Read status codes and protocol errors as clues
- OpenAI’s Agents API error guide maps
401to unauthenticated and403to forbidden. A424 mcp_server_startup_failedpoints back to server startup, configuration, or credentials. - Microsoft Foundry documents
-32006as an OAuth consent requirement. Its troubleshooting guide also describes a401case involving an expired token or wrong scope. Check the current service documentation for the exact scope required; do not transfer a scope from another platform.
Authentication is not the same as access to the underlying data. OpenAI Help Center’s app troubleshooting guidance says to check the provider account or administrator-managed source permissions: connecting an app does not grant additional permissions in that source system. For a published agent, test access using the identity that actually runs the tool, which may differ from the person who configured it.
4. If the agent finds a file but cannot use it
A filename or file detail appearing in search does not prove that the agent retrieved usable contents. First determine whether the symptom is “file not found” or “file found, but contents cannot be used.” The first points toward source access, indexing, or scope; the second points toward retrieval, supported format, or feature capability.

Check the file path from source to usable content
- Confirm the connected provider account and its permissions in the source system.
- Check workspace configuration, sync or index status, and whether the file falls within the configured sync scope.
- Verify that the connected feature supports the file type and the requested operation.
- If appropriate, use a supported export or transcript, but only when you are authorized to share it.
OpenAI Help Center’s app troubleshooting guidance covers these source-access and file-availability checks. Reconnecting an app alone is not a dependable fix for an indexing, scope, permission, or format problem.

If the tool is available and the server responds, inspect the request and response structure rather than relying on the agent’s explanation of what happened.

Check that every tool call has a matching result
Anthropic’s tool-use troubleshooting guide documents errors caused by tool-use IDs without corresponding tool-result blocks. Return one result for every tool-use block and put results before text in the user message. When continuing a request, preserve protected thinking blocks unchanged and append the tool result in the documented format; altering those blocks can produce a 400 error.
Inspect platform-specific response errors
On Microsoft Foundry’s toolbox endpoint, inspect whether the MCP response contains an error field and check its code. The troubleshooting guidance distinguishes consent-required responses, expired or wrong-scope tokens, prefixed tool names, unsupported non-streaming calls, and transient server errors. Apply the fix that matches the observed response rather than retrying indiscriminately.
For OpenAI structured errors, inspect error.code, error.message, and error.param when supplied. The official error guide covers invalid input or configuration, authentication and permission failures, unavailable resources, MCP startup failures, and temporary service or overload errors. Correct invalid input before retrying; for temporary outages, follow the platform’s recovery guidance and check saved work where relevant.

A quick decision path
- The tool is absent: check agent attachment, server registration, discovered tools, allowlists, and platform-specific naming.
- The server fails to start or respond: inspect failed-turn events and process logs; verify endpoint origin, network access, command, dependencies, and working directory.
- The server responds with an access error: identify the runtime identity, then check its credentials, consent, token scope, and source-system permissions.
- The file appears but its contents are unusable: check retrieval, sync/index status, scope, supported format, and feature capability.
- The request or continuation fails: validate tool-use IDs, matching result blocks, message ordering, and the platform’s response schema.
Platform behavior varies across hosted versus environment execution, HTTP versus stdio MCP, agent versus delegated-user identity, and file search versus content retrieval. Use the relevant platform’s current documentation for the precise API version, authentication requirements, error mappings, and feature availability.
