How to Choose a Cognitive Training App for Older Adults

A practical guide to choosing a cognitive training app for an older adult, with clear limits on what training research can establish.

Choose a cognitive training app for a modest goal—such as enjoyable, structured practice—and judge it by the evidence for its specific exercises, how well it works for the person on their own device, its full cost and data practices, and whether they want to use it. Some structured training can improve practiced skills, but that does not show that consumer apps prevent dementia or deliver the same results as clinical-trial programs.

What cognitive training can—and cannot—show

Cognitive training is structured practice aimed at particular skills, such as memory, reasoning, or processing speed. It is not simply any game marketed as “brain training.” The National Academies’ review of the ACTIVE study found evidence of improvement in the domain people trained, while transfer to other domains was rare. Its strength rating was moderate at two years and low at five and ten years. The trial enrolled 2,802 older adults and used a specific, structured intervention; those results cannot be assumed to apply to any app.

The National Academies stated in 2017: “At present, there is no evidence to support the notion that the beneficial long-term cognitive effects suggested by the ACTIVE trial apply to computer-based ‘brain training’ applications.” The National Institute on Aging likewise says there is not enough evidence that commercially available computer-based brain-training apps have the same impact on cognitive abilities as ACTIVE. The Academies also noted that most computer-based app trials it identified were excluded from its systematic review because their follow-up was too short to assess long-term outcomes. A 2026 NIH release adds a notable but narrow finding: in a study following adults aged 65 and older for 20 years, one specific rapid object-detection speed-training regimen was associated with a 25% lower rate of dementia diagnosis, based on Medicare claims data. Two other training types were not associated with that finding. This is an association for a particular regimen—not proof that brain-training apps generally, or any commercial app in particular, prevent dementia. NIH, February 10, 2026.

Start with the person’s goal

Before comparing products, decide what the app is for. Enjoyable mental activity, practice on a particular skill, rehabilitation recommended by a clinician, and evaluation of memory symptoms are different purposes. A general consumer app may offer exercises, but it is not a diagnostic tool. If memory or thinking changes are new, worsening, or concerning, discuss them with a healthcare professional rather than relying on an app score.

For a training goal, ask what the app actually practices and whether that matches the person’s interest. Claims about a specific exercise, improvement on that exercise, transfer to other skills, everyday functioning, and dementia diagnosis are distinct outcomes; evidence for one does not establish the others.

Use a practical comparison checklist

  • Evidence: Does the company identify the exact training program and point to peer-reviewed studies in a relevant older-adult population? Check who conducted the study and what it measured. Vendor summaries can help locate claims, but they are not independent proof.
  • Fit and accessibility: Try the app on the older adult’s own phone or tablet. Check text size, contrast, audio or captions, tap-target size, instructions, language, dexterity demands, and internet requirements. Consider whether someone can help with setup. These checks are practical, not a tested ranking of current apps.
  • Engagement: Is each exercise understandable and enjoyable enough that the person wants to return? Do not assume there is one evidence-based daily duration that applies to every app or person.
  • Total cost: Check the price after any introductory offer, how often payment renews, and how to cancel before subscribing. Ask a local library or health plan whether it provides access; availability depends on the specific library or plan. The National Academies notes that commercial apps generally involve a purchase or subscription cost.
  • Privacy: Read the current privacy policy and app-store disclosures. Look for what account, performance, or health-related data are collected, whether they are shared, and how to delete an account. A company’s security statements do not by themselves answer whether its data practices suit a particular user.
  • Claims: Be skeptical of promises to prevent Alzheimer’s disease, reverse aging, or diagnose impairment. The current evidence does not establish that commercial apps have the same cognitive impact as the ACTIVE training.

Examples to evaluate, not rankings

BrainHQ

Posit Science describes BrainHQ as offering exercises for memory, attention, processing speed, social cognition, decision-making, and navigation, with adaptive levels, progress tracking, and short exercise levels. It also says some libraries and some Medicare Advantage plans provide access; confirm availability with the specific provider. These are company descriptions, not independent recommendations. The company’s BrainHQ site also markets a connection to ACTIVE, but the National Academies cautions that ACTIVE’s complex, group-based program does not establish equivalent long-term benefits for commercial apps.

At the time of the cited App Store listing, the developer had not indicated which accessibility features the app supports. Listing details can change, so check the current BrainHQ App Store page and, more importantly, try it on the intended device. Posit Science says it uses technical safeguards and annual SOC 2 audits; that is the company’s account of its practices, not a complete assessment of every user’s privacy needs. CogniFit describes training and assessment tools and an offering for healthy aging. That gives a reader features and claims to inspect, not evidence that it is better than another app. CogniFit’s healthy-aging page.

Do not confuse training with clinical screening

The FDA classification for a computerized cognitive assessment aid describes a clinician-used screening aid for adults aged 55 and older, with referral for further testing where warranted. That classification is about a clinical assessment aid; it does not mean consumer training apps generally are FDA-cleared diagnostic devices. FDA Product Classification: Computerized Cognitive Assessment Aid.

Make the choice in a low-risk way

  1. Write down the goal. Name the skill or kind of activity the person wants, without treating the app as a medical test or promise of disease prevention.
  2. Shortlist by evidence and fit. Look for a clearly described training approach and relevant evidence, then confirm that the exercises and interface suit the person.
  3. Test the real experience. Use the person’s own device to check instructions, readability, controls, audio, connectivity, and enjoyment before committing to regular use.
  4. Review terms and privacy first. Confirm total and recurring cost, renewal and cancellation steps, data collection and sharing, and any library or health-plan access.
  5. Reassess based on experience. If the app is frustrating, inaccessible, or no longer enjoyable, it may not be a good fit. Do not interpret a score or a change in score as a diagnosis.

There is no established universal “best” app for older adults, and current evidence does not provide an independent head-to-head ranking of the named products. Choose based on the individual’s goal, the exact training and outcome evidence, accessibility, willingness to use it, cost, and privacy—not on broad dementia-prevention promises.
