How to Choose a Biological Indicator for Steam Sterilization

Choose a commercially prepared, FDA-cleared biological indicator (BI) containing Geobacillus stearothermophilus spores and labeled for the exact steam sterilizer and cycle you operate. Before buying, confirm the carrier or self-contained format, any process-challenge device (PCD) requirement, placement instructions, incubation temperature and time, reader compatibility, control procedure, and result criteria in the current BI instructions for use (IFU).

The BI is the part of a monitoring program that directly challenges microbial survival. Chemical indicators, mechanical records, and Bowie-Dick air-removal tests answer different questions; none replaces a BI. The practical choice is therefore not the product with the fastest advertised result, but the cleared BI that your team can process, incubate or read correctly, document, and act on under your sterilizer’s validated cycle.

What a steam-sterilization biological indicator does

A BI contains a known population of highly resistant bacterial spores. In steam sterilization, the standard test organism is Geobacillus stearothermophilus (often abbreviated G. stearothermophilus). If the cycle exposes the BI to adequate saturated steam, time, temperature, and pressure conditions, the spores should be inactivated. Growth after the cycle signals that the microbial challenge was not adequately killed or that the BI was mishandled.

CDC describes a typical steam-monitoring population of 105 G. stearothermophilus spores. CDC also cites D121°C values of approximately one to two minutes for this organism when used to characterize steam resistance. A D-value is a resistance measurement under specified test conditions, not a recommendation to set a sterilizer for a particular time or temperature. Your sterilizer’s validated cycle and the BI’s labeling control those settings.

CDC’s description of an ideal biological monitor emphasizes practical use, resistance to outside contamination, rapid positive results, and positive results when sterilization parameters are inadequate. Those criteria explain why product design matters as much as the organism: a technically suitable spore strip is not useful if staff cannot process, incubate, read, or document it reliably.

Step 1: Identify the sterilizer and the exact cycle

Start with the equipment, not the BI catalog. Record the sterilizer manufacturer and model, chamber type, cycle name, temperature, exposure time, load configuration, and whether the cycle uses gravity displacement or dynamic air removal. Also note any specialty cycle for porous loads, wrapped instruments, liquids, or other materials.

Gravity-displacement cycles

Gravity cycles remove air by allowing steam to displace it. The BI must be labeled for the temperature, exposure time, load type, and sterilizer configuration you use. A product marked only “steam” is not automatically validated for every gravity cycle.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Dynamic-air-removal cycles

Pre-vacuum and other dynamic-air-removal cycles actively remove air before steam exposure. They may require a PCD or test pack that creates a defined challenge at the hardest-to-sterilize location. Confirm whether the BI is intended to be placed in a specific PCD, and whether the PCD is supplied with the BI or must be obtained separately.

Why a generic steam label is insufficient

Steam sterilization is a family of processes, not one universal condition. A BI can be suitable for steam in general yet lack labeling for your cycle parameters or challenge configuration. Compare the sterilizer IFU and the BI IFU line by line. If either document excludes your cycle, obtain clarification from the manufacturer or select a different cleared product.

Step 2: Verify the organism, population, and BI format

Spore organism

For healthcare steam monitoring, select G. stearothermophilus. Do not substitute an organism intended for ethylene oxide, dry heat, or another process. The organism’s resistance profile must match the sterilization method being evaluated.

Stated spore population

Check the labeled population per carrier or device. CDC uses 105 spores as a typical steam-monitoring reference, but the product label and applicable standard determine the accepted population and performance criteria. Record the lot number and expiration date so an unexpected result can be traced.

Carrier and self-contained designs

A spore strip places spores on a carrier that is processed and then transferred to growth medium. A self-contained BI combines the carrier, growth medium, and a sealed container; after processing, the ampoule or activator is crushed according to the IFU and the unit is incubated. Self-contained designs can reduce transfer contamination and simplify handling, but they still require the correct incubator and activation procedure.

Process-challenge devices and test packs

A PCD or test pack creates a reproducible challenge that represents difficult air removal or steam penetration. Use the configuration specified for the sterilizer and cycle. Do not improvise a pack from gauze, instruments, or packaging unless your validated procedure explicitly permits it. The challenge device, BI, and placement location function as one system.

Step 3: Match incubation and readout to your workflow

Conventional growth readout

Conventional BIs are incubated and checked for visible growth over the period stated in the IFU. CDC describes an incubation range of 55–60°C for G. stearothermophilus. That range is general guidance; the product’s specified temperature, duration, medium, and interpretation rules take precedence.

Rapid-read systems

Rapid systems detect a metabolic or enzymatic signal before visible growth appears. They can shorten the time that a load remains on hold, which is valuable when implants or urgent instruments are involved. A rapid result is valid only when the BI and reader are expressly compatible. Do not place a rapid BI in a general incubator or use a reader approved for another product family unless the labeling authorizes that combination.

Controls and invalid results

Run the positive control required by the IFU, usually an unprocessed BI from the same lot, and process it with the test units when specified. The control must show the expected growth or signal. If the control fails, the run is invalid and a negative test BI cannot be interpreted as proof of sterilization. Document incubator temperature, start and stop times, reader identification, control results, and the person who released or held the load.

Step 4: Check regulatory status and standards documentation

For United States healthcare use, verify that the BI is cleared by the Food and Drug Administration (FDA) for the intended sterilizer-monitoring use. FDA identifies healthcare-sterilizer BIs as class II medical devices subject to 510(k) premarket notification. Clearance does not mean every cycle is covered; the intended-use statement and labeling still determine whether the product fits your equipment.

The relevant moist-heat BI standard is ANSI/AAMI/ISO 11138-3:2017/(R)2024, listed by AAMI. The complete standard, your jurisdiction’s regulations, and the product IFU establish acceptance criteria, population, resistance testing, storage, and interpretation details. Keep the current labeling and clearance documentation in your quality system rather than relying on a distributor description.

Comparison of common steam-BI choices

Choice What it contains or does Advantages Checks before purchase
Self-contained BI G. stearothermophilus spores, carrier, and growth medium in one unit Less transfer handling; simple activation and incubation Incubator temperature, activation method, cycle labeling, PCD requirement, control procedure
Spore strip BI Spores on a carrier that is transferred to culture medium Flexible carrier placement and established laboratory-style workflow Transfer asepsis, medium compatibility, incubation equipment, handling time, labeled population
Rapid-read BI BI designed for a proprietary or specified reader and accelerated signal detection Earlier result and potentially shorter load quarantine Reader/BI compatibility, validated read time, software or alarm interpretation, backup plan if the reader is unavailable
BI in a PCD or test pack BI enclosed in a defined challenge configuration Reproducible challenge for difficult air removal or penetration locations Correct PCD for the sterilizer and cycle, placement instructions, single-use or reuse limits, disposal requirements

Package quantity, shelf life, storage conditions, and total workflow cost vary by product and are not interchangeable assumptions. Compare the current label and supplier documentation for each candidate.

  • Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft

Step 5: Plan placement and processing

  1. Choose the challenge location. Use the location specified by the BI, PCD, sterilizer IFU, and facility procedure. Dynamic-air-removal cycles commonly use a defined challenge location rather than an arbitrary spot in the chamber.
  2. Prepare the BI or PCD. Check the package for damage, confirm the lot and expiration date, and assemble the PCD exactly as directed. Do not open a sealed self-contained BI before processing unless the IFU says to do so.
  3. Load the sterilizer correctly. Keep the BI from being shielded by dense metal, liquid containers, or packaging folds unless the validated procedure specifically places it there. Follow loading rules for spacing, orientation, and maximum weight.
  4. Run the selected cycle. Record the mechanical printout or electronic record showing time, temperature, and pressure. A BI does not replace these records.
  5. Recover and activate the BI. Allow the unit to cool as required, then crush, transfer, or activate it using the stated method. Avoid touching the carrier or opening the device in a way that can introduce contamination.
  6. Incubate or read immediately. Start the timer when the IFU specifies, place the unit in the approved incubator or reader, and include the required positive control.
  7. Interpret and document. Record a positive, negative, invalid, or no-result status exactly as defined by the manufacturer. Link the result to the sterilizer, cycle, load number, operator, BI lot, and release decision.

How often to run BIs

CDC recommends biological-indicator monitoring at least weekly for healthcare steam sterilizers. Use a BI in every load containing implantable items; when feasible, quarantine those items until the BI result is negative. Your state or national requirements, accreditation program, standard, and facility risk assessment may require more frequent monitoring.

Routine monitoring is only one part of the record. Mechanical monitoring documents the cycle’s time, temperature, and pressure. Chemical indicators placed inside and outside packs show exposure to one or more process conditions and help identify problems with individual packages. Keep all three information streams with the load record.

BI, chemical indicator, mechanical record, and Bowie-Dick test: different questions

Monitor Question answered What it cannot prove
Biological indicator Were resistant spores in the test configuration inactivated? It does not replace mechanical records or prove that every item in a poorly loaded chamber was sterile.
Chemical indicator Was the indicator exposed to one or more specified process conditions? Exposure response is not direct evidence that all microorganisms were killed.
Mechanical monitoring Did the sterilizer record the programmed time, temperature, and pressure? A normal printout cannot detect every problem with air removal, steam quality, packaging, or placement.
Bowie-Dick test Did an empty dynamic-air-removal sterilizer remove air and allow steam penetration in the test device? It does not challenge the process with spores and is not a substitute for a BI.

CDC recommends using chemical indicators together with BIs. The tests are complementary: a BI measures microbial lethality, while chemical and mechanical monitors provide process and exposure information.

What to do after a positive steam BI

Treat a positive BI as a potential sterilization failure until the investigation is complete. Follow the current facility protocol and local reporting and recall rules.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.
  1. Take the sterilizer out of service. Notify the supervisor and the infection-prevention or quality contact designated by your facility.
  2. Secure the records and load. Identify all loads processed since the last acceptable BI, preserve mechanical and chemical records, and quarantine affected items when possible.
  3. Verify the test itself. Check the BI lot, expiration date, storage, PCD assembly, placement, activation, incubation temperature, control result, and operator technique. Confirm that the BI was labeled for the cycle used.
  4. Repeat BI testing for three consecutive cycles. Use the procedure specified by your facility and the BI manufacturer. Do not release items solely because one repeat is negative if the investigation identifies additional concerns.
  5. Check the sterilizer and steam supply. Maintenance should inspect chamber performance, sensors, vacuum or air-removal function, valves, door seals, drain, and steam quality or supply conditions.
  6. Escalate persistent positives. CDC’s suggested protocol includes considering a BI from a different manufacturer if positives continue. If additional tests remain positive, treat implicated items as nonsterile and recall or reprocess them according to facility policy.
  7. Document the disposition. Record findings, corrective actions, repeat results, maintenance work, affected loads, and the authorization to return the sterilizer to service.

Using the wrong incubator or reader

Temperature, optics, timing, and activation chemistry are product-specific. Use only equipment named or authorized in the BI IFU, and maintain a documented backup procedure.

Replacing the BI with a chemical indicator

A chemical endpoint demonstrates exposure to selected conditions, not spore inactivation. Continue BI monitoring at the required frequency, including every implant load.

Ignoring the PCD or placement instruction

A BI processed in an easier location than the validated challenge location can produce false reassurance. Reassemble and place the device exactly as directed.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Releasing an implant load before the result is available

CDC recommends quarantine when possible until a negative BI result is obtained. If clinical necessity requires an alternative decision, follow the documented facility policy and authorization process.

Accepting a negative result when the control failed

A failed positive control invalidates the run. Repeat the test with a valid control and investigate incubator, media, handling, and lot conditions.

A practical decision sequence

For most facilities, the selection decision can be made in this order:

  1. List every steam sterilizer and cycle that must be monitored.
  2. Remove products whose labeling does not cover those cycles.
  3. Among the remaining products, select the G. stearothermophilus format and PCD that staff can place correctly.
  4. Choose conventional or rapid readout only after confirming compatible incubation or reader equipment, controls, and result timing.
  5. Verify FDA clearance, current IFU revision, storage, shelf life, lot traceability, and documentation requirements.
  6. Run a controlled implementation: train operators, process a known load, verify control behavior, and audit records before routine use.

The best BI is therefore the cleared, cycle-specific system that produces interpretable results in your actual workflow. Organism, challenge configuration, incubation, equipment compatibility, monitoring frequency, and response procedures all have to align; selecting on any one feature alone is unsafe.
