No badge, reputation score, or clean scan can guarantee a file is safe.
Start with the source and the exact file
Open the mod through the game’s official mod channel or an established host, then confirm that the creator, description, supported game version, and download match what you intended to get. Follow the creator’s actual page rather than an unsolicited link or a repost. Steam warns that malware can be disguised as gaming-related utilities and recommends downloading custom content only from trusted sources: Steam’s account security recommendations and Steam’s guidance on VAC bans.

Read the installation instructions, recent comments, and announcements. Compare the listed files with the files the author says the mod needs. A separate login request, an unexpected “required tool,” or an unrelated download link is a reason to stop and verify the request through the creator’s trusted page.

Check the file-level scan status
A mod page can contain several downloads, and their scan status may differ. Check the status beside the specific file you intend to use rather than assuming that a badge for another file applies to it. On Nexus Mods, the indicators have platform-specific meanings:

These labels describe Nexus Mods’ system, not a universal standard for other hosts. Nexus explains its indicators and file checks in its file scanner guide. Open the linked VirusTotal report when one is available, and distinguish a file that could not be scanned from one that was scanned and returned no detections.
Inspect what the archive contains and what the instructions ask you to run
Compare the downloaded file types and contents with the mod’s stated purpose. An unexpected self-extracting installer, executable, nested archive, or program that connects to the internet deserves extra scrutiny—especially if the description does not explain why it is needed. This is a reason to investigate, not proof of malware: some legitimate mods or utilities may need executable components.

Nexus says potentially harmful file types may be blocked or reviewed, rejects nested archives so files can be scanned, and generally prohibits executables that connect to the internet unless that behavior is crucial to the mod or utility. Its file submission guidelines and quarantine system information explain those host policies.
Pause if the author asks you to disable protection, run an unrelated program, or grant broad privileges without a clear explanation. If the file list or requested actions do not make sense for the mod, ask the creator or host rather than guessing.

Scan the saved download before running or copying it
Save the file, update your security software, and scan the download before running an installer or copying files into the game. Treat a detection seriously while you establish whether it is a false positive. Steam advises users who are unsure about a detection to report it and contact the security vendor; it also says not to create an antivirus exception unless you are confident the file is not malware. If a security tool flags the file, leave it quarantined or remove it while you investigate. Report the file to the host and ask the security vendor to interpret the detection. Do not restore it or add an exception just to make the mod work.

Weigh the signals together; none proves safety
A clean scan, a platform badge, a long upload history, or positive community feedback can inform your decision, but none certifies a download. Steam warns that targeted malware can be tailored to evade common antivirus software. Nexus also notes that mod files can trigger false positives and recommends considering scan status alongside factors such as downloads, endorsements, and how long the file has been available.
There is no dependable universal cutoff such as “one detection is always harmless,” and a file extension alone does not settle whether a mod is malicious. Nexus describes a significant number of VirusTotal detections as a reason for review, but does not set a numerical threshold for users. If signals conflict, the file’s behavior is unexplained, or you cannot verify the source, skip the download and ask the host or security vendor.

Use Steam’s integrity check only for game-file repair
Steam’s “Verify integrity of game files” feature checks whether the game’s installed files are correct. Steam’s support instructions do not describe it as a malware scan or safety certification for third-party mods. Use it to repair game files if needed, not to decide whether a mod download is safe: Steam’s game-file integrity instructions.
