How Targeted Therapy Compares With Chemotherapy and Immunotherapy

Chemotherapy, targeted therapy, and immunotherapy work in different ways, but none is universally best. Here’s how testing, side effects, and cancer-specific factors shape treatment choices.
Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Targeted therapy focuses on particular cancer-related targets or processes, chemotherapy acts on fast-growing cells, and immunotherapy helps the immune system fight cancer. They are not interchangeable treatments, and none is always the safest or most effective. A tumor’s features, cancer type and stage, the patient’s health, and the available treatment options all shape the choice. Some treatments also overlap or are used together.

How does targeted cancer therapy compare with chemotherapy and immunotherapy?

The main difference is what each approach acts on. Chemotherapy can affect cancer cells because they grow and divide quickly, but it can also affect healthy fast-growing cells. Targeted therapy acts on particular proteins or other processes that help cancer cells grow, divide, or spread. Immunotherapy works through the immune system, helping it recognize or attack cancer.

Those descriptions explain the broad mechanisms, not a ranking. A treatment’s usefulness depends on whether it fits the particular cancer and the patient’s situation. Targeted therapy often depends on finding a relevant tumor target; a biomarker match may help identify a treatment option but does not guarantee that it will work. Side effects differ by treatment and person, so the names of the categories alone cannot predict which option will be easiest to tolerate.

Approach What it acts on How selection may work Important limitation
Chemotherapy Fast-growing cells, including cancer cells; healthy fast-growing cells can also be affected. Depends on cancer type and extent, among other treatment-planning factors. Effects on healthy cells can cause side effects; the specific effects depend on the treatment.
Targeted therapy Particular proteins or processes that help cancer cells grow, divide, and spread. Tumor testing may be used to see whether a treatment-relevant target is present. A target result does not ensure response, and cancer cells may become resistant.
Immunotherapy The immune system’s ability to fight cancer. Biomarker testing can identify features that may support certain immunotherapies. Immune activity can affect healthy tissues and cause inflammation-related side effects.

The National Cancer Institute (NCI) describes these approaches as parts of a broader treatment landscape: chemotherapy may be used alone or with other treatments, and many monoclonal antibodies are targeted therapies, while some also count as immunotherapy. NCI’s overview of cancer treatment explains that options depend on the cancer and its circumstances.

In practical terms, the comparison has several separate parts. The mechanism tells you how a treatment is intended to work. Biomarker testing may help determine whether some targeted therapies or immunotherapies are options. Side-effect mechanisms can differ, but the treatment category does not establish the exact effects or their severity. Finally, the cancer’s type and extent, the patient’s circumstances, and the plan’s goals affect how clinicians use any of these approaches.

What chemotherapy does

Chemotherapy uses drugs to stop or slow the growth of cancer cells. Because many cancer cells grow and divide quickly, these drugs can affect them. But some healthy cells also grow quickly. The NCI identifies cells in the mouth and intestines and cells involved in hair growth among the healthy cells chemotherapy can harm, which helps explain why side effects can occur. See NCI’s patient guide, Chemotherapy and You.

The purpose of chemotherapy varies with the cancer type and how extensive it is. It may be used to cure cancer, control it, or ease symptoms. It can be given on its own, but may also be part of a plan that includes surgery, radiation, targeted therapy, or immunotherapy. That means “chemotherapy versus another treatment” is not always the right framing: a clinician may consider more than one approach in the same overall plan.

The fast-growing-cell mechanism is useful to understand, but it does not predict every person’s side effects or say whether chemotherapy is appropriate. The particular drug, cancer, treatment plan, and individual health matter. The NCI guide describes chemotherapy and its effects; a patient’s oncology team can explain which effects are relevant to the proposed regimen.

When discussing chemotherapy, it can help to clarify its intended role in the overall plan. Ask whether the goal is to cure, control, or ease symptoms, and how the team will judge whether the treatment is helping. If chemotherapy is being considered alongside surgery, radiation, targeted therapy, or immunotherapy, ask what each component is intended to contribute. These questions apply to the specific plan; the broad label “chemotherapy” alone does not tell you its purpose or how it will be used.

What targeted therapy does—and why tumor testing may matter

Targeted therapy is designed to interfere with cancer-related proteins or other processes that help cancer cells grow, divide, and spread. Many such treatments interfere with particular proteins. Some block signals involved in growth or the formation of blood vessels that feed a tumor; other approaches deliver a cell-killing substance to cells carrying a specific target. The mechanism depends on the therapy. The NCI’s explanation is available in Targeted Therapy to Treat Cancer.

In many cases, a clinician needs information about the tumor to determine whether it has a target that a treatment can act on. Biomarker testing can look for features of a cancer that may help guide treatment selection. A test can therefore help answer an eligibility question: is there a feature that makes a particular targeted treatment—or, in some cases, an immunotherapy—worth considering? NCI explains the purpose and limits of this process in Biomarker Testing for Cancer Treatment.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

A result is not a promise of benefit. Not every tumor cell necessarily has the same biomarkers, other cancer features can affect response, and there may be no available treatment that matches a result. Testing can identify a possible route, but it does not by itself establish that a drug will work for an individual. That distinction matters when interpreting a report: a detected feature is a piece of treatment-planning information, not a guarantee.

Targeted therapy is also not automatically mild or free of serious effects. The NCI lists diarrhea and liver problems among common effects, with fatigue, high blood pressure, skin changes, mouth sores, and other effects possible depending on the drug. Cancer cells can also become resistant to targeted therapy. Which effects and monitoring apply cannot be determined from the category name alone; the treatment-specific plan is essential.

Testing is a decision aid, not a treatment in itself. A result may make a therapy worth discussing, but the care team still needs to consider whether that option fits the cancer and the patient’s circumstances. Conversely, if a test does not identify a relevant feature—or no treatment is available for the feature found—that result does not settle the whole treatment decision. It is one part of the broader comparison with other available approaches.

Resistance is another reason a match should not be treated as a guarantee of lasting benefit. NCI notes that cancer cells can become resistant to targeted therapy. Ask the oncology team how it will assess whether a treatment is working and what alternatives might be considered if it is not. The answer will depend on the specific cancer and plan; there is no category-level timeline or outcome that applies to every targeted therapy.

What immunotherapy does

Immunotherapy helps the immune system fight cancer. Different immunotherapies can work in different ways. Some release immune “brakes” or otherwise increase immune activity; some antibodies mark cancer cells or bring immune cells close to them. The key contrast with chemotherapy is that immunotherapy works through immune activity rather than simply acting on fast-growing cells.

That mechanism creates a different side-effect concern: an activated immune system can affect healthy organs and tissues, causing inflammation-related effects. The type and dose of immunotherapy and the patient’s health and cancer can influence what happens. Side effects may arise during treatment or after it. NCI describes this variability in Immunotherapy Side Effects. Patients should ask their care team which symptoms need prompt attention and how to report them; a general description cannot substitute for instructions tied to the specific treatment.

Immunotherapy should not be understood as a single drug or a treatment with one predictable side-effect profile. The immune system’s role helps explain why effects can involve healthy tissues, but the relevant risks depend on the treatment and patient. Nor does the label mean that the therapy is suitable for every cancer: biomarker information may support use of certain immunotherapies, and the cancer context remains central.

For a proposed immunotherapy, ask specifically which symptoms call for prompt contact with the care team, including symptoms that arise after a treatment visit. The possibility of later effects is a reason to understand the treatment-specific instructions, not a reason to assume that every patient will develop inflammation or that all immunotherapies have identical risks. The care team can explain what to watch for with the particular treatment.

Where the categories overlap

The three labels are useful for explaining mechanisms, but they are not perfectly separate boxes. NCI notes that many monoclonal antibodies are targeted cancer therapies, and some also qualify as immunotherapy because they help turn the immune system against cancer. In other words, one treatment can fit more than one mechanism-based description.

Combinations are also possible. Chemotherapy may be used with targeted therapy or immunotherapy, among other treatments. Whether to combine treatments is a clinical decision based on the cancer and the expected role of each treatment; the fact that a combination is possible does not mean it is appropriate for every patient. Ask the oncology team which part of a proposed plan is intended to do what, and whether the treatments are given together or in sequence.

This overlap is why a simple three-column comparison is a starting point rather than a complete treatment map. A drug may fit a mechanism-based category, and the overall plan may contain more than one treatment approach. When reviewing a recommendation, ask the clinician to name the specific treatment, explain its intended role, and describe how it relates to the other parts of the plan.

How clinicians decide which approach fits

There is no universal winner among chemotherapy, targeted therapy, and immunotherapy. Treatment decisions depend on the cancer type and how advanced it is, whether relevant biomarkers are present, the expected benefit in that situation, the possible side effects and monitoring, and the patient’s circumstances. A clinician may also consider whether treatments can be combined, what alternatives exist, or whether a clinical trial is relevant. NCI advises patients to discuss treatment choices with their care team.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For a useful conversation, work through the questions in context rather than asking which category is best in general:

  • What cancer are we treating, and what is its stage or extent? The treatment purpose and available options can vary with these facts.
  • Is biomarker testing relevant? Ask whether testing could identify a treatment-relevant feature, what a result would change, and whether any candidate treatment is actually available for that result.
  • What benefit is expected for this situation? Ask what the proposed treatment is intended to do—cure, control, or ease symptoms, where applicable—and how the team will assess whether it is helping.
  • What side effects and monitoring apply to this specific treatment? Ask which effects are expected, which symptoms require prompt attention, and whom to contact if they occur.
  • Could treatments be combined, or are there alternatives? Ask why a single approach, a combination, a sequence, or another option is being considered, and whether a clinical trial is appropriate to discuss.

These questions do not replace a treatment recommendation. They help clarify how the clinician connects the cancer’s features and the patient’s needs to the proposed plan. For general treatment-choice information, see NCI’s Cancer Treatment overview.

A practical way to use the discussion is to compare the actual options on the same points: what each treatment is meant to do, what information supports considering it, what effects and monitoring are relevant, and whether it would be used alone or as part of a combination. If a biomarker test is proposed, ask what kinds of treatment decisions its result could inform and whether any possible result would change the options being discussed. The answer may be that testing is useful, or that it does not change the plan; either way, ask the clinician to explain the reasoning for this cancer.

Patient circumstances matter alongside the tumor information. The broad comparison cannot account for an individual’s health, treatment goals, or tolerance of possible effects. Tell the care team what matters most to you and ask how those concerns weigh against the expected benefit and monitoring needs of each specific option. A clear explanation should connect the proposed treatment to both the cancer and the person receiving it.

How to interpret a biomarker result

A biomarker report is one input to a decision, not a stand-alone answer to “which therapy will work?” Its practical meaning depends on the feature found, the cancer in which it was found, and whether there is a treatment option relevant to it. Even when a result points to a possible match, the tumor may not respond; tumor cells may differ from one another, and other features can influence the outcome.

When reviewing testing with the oncology team, ask what the result means for the current cancer and whether it changes the options under consideration. If no matching treatment is available, or if the result does not establish likely benefit, the team will need to weigh other choices using the full clinical picture. NCI’s biomarker testing guide explains how testing can inform treatment without guaranteeing a response.

It is useful to separate three questions when reading a result: Was a feature detected? Is there a treatment that may act on that feature in this cancer context? Does the evidence and the patient’s situation make that treatment a reasonable option? The first answer does not automatically answer the second or third. Bring questions about the report to the oncology team rather than interpreting a biomarker name as a treatment instruction on its own.

Why side effects cannot be ranked by treatment category

The mechanisms suggest different ways healthy tissue can be affected: chemotherapy can harm healthy fast-growing cells, targeted therapies can produce effects such as diarrhea or liver problems depending on the drug, and immunotherapy can cause inflammation when immune activity affects healthy tissue. But that comparison does not establish that one type is generally safer. The possible effects, their seriousness, and the monitoring needed depend on the particular treatment and patient.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

For that reason, compare actual proposed treatments rather than broad labels. Ask for the treatment-specific side-effect information and a clear plan for what to do if symptoms arise. For immunotherapy in particular, effects can occur during treatment or later, so ask which symptoms require prompt attention and how to reach the care team. Do not assume that a treatment is low-risk simply because it is targeted, immune-based, or described as more precise.

Monitoring is also treatment-specific. The category tells you something about the intended mechanism, but not which checks, symptoms, or follow-up instructions apply to an individual regimen. Ask what the team will monitor and whom to contact with concerns. The goal is not to predict a person’s experience from a general list, but to know what applies to the treatment actually being considered.

Common misconceptions to avoid

  • “Targeted” does not mean guaranteed to work. A target can make a treatment a possibility; it cannot promise benefit.
  • “Targeted” does not mean side-effect free. Targeted therapies can have significant effects, and the profile varies by drug.
  • “Immunotherapy” does not mean only cancer cells are affected. Immune activity can affect healthy tissues.
  • “Chemotherapy versus targeted therapy versus immunotherapy” is not always an either-or choice. Treatments can be combined, and some therapies fit overlapping categories.
  • A category-level comparison cannot determine the best plan. Cancer type, stage, biomarkers, patient factors, expected benefit, and treatment-specific effects all matter.
  • A biomarker match is not a guarantee of benefit. Tumor cells may not all share the same biomarkers, other features can influence response, and a matching treatment may not be available.
  • A possible combination is not automatically the right combination. Whether to combine or sequence treatments depends on the cancer and the role expected of each option.

Frequently asked questions

Is targeted therapy better than chemotherapy?

Not in every case. Targeted therapy acts on particular cancer-related targets or processes and often requires testing to see whether a relevant target is present. Chemotherapy acts on fast-growing cells and can be used to cure or control cancer or ease symptoms. Which is appropriate depends on the cancer, its extent, expected benefit, patient factors, and the specific treatment plan.

Does targeted therapy require biomarker testing?

In many cases, tumor testing helps determine whether a treatment-relevant target is present. Testing may also identify features that support considering certain immunotherapies. A result can point to a possible option, but it does not guarantee that the treatment will work or that a matching treatment is available.

Special offer. See more information about Outbyte and uninstall instructions. Please review EULA and Privacy policy.

Can targeted therapy and chemotherapy be used together?

They can be part of the same treatment plan. Chemotherapy may be combined with targeted therapy, immunotherapy, or other treatments, depending on the cancer and the plan’s goals. A clinician can explain why a combination or sequence is being considered for an individual case.

Is immunotherapy a type of targeted therapy?

Some treatments overlap categories. Many monoclonal antibodies are targeted cancer therapies, and some are also immunotherapy because they help turn the immune system against cancer. The labels describe mechanisms and are not always mutually exclusive.

Which treatment has fewer side effects?

There is no general answer based on category alone. Chemotherapy can affect healthy fast-growing cells; targeted therapy can cause effects such as diarrhea or liver problems depending on the drug; and immunotherapy can cause inflammation when immune activity affects healthy tissue. The specific treatment and patient determine the relevant risks and monitoring.

Can immunotherapy side effects happen after treatment?

Yes. NCI notes that immunotherapy side effects can occur during treatment or after it. Ask the care team which symptoms require prompt attention and how to report them for the specific therapy.
