How Robotic Surgery Works—and What the Surgeon Controls

Robotic-assisted surgery is directed by a surgeon at a console. Learn how the system moves instruments, what it may change, and how to discuss options with your care team.

In robotic-assisted surgery, a surgeon—not an autonomous machine—controls the operation. From a console in the operating room, the surgeon views the surgical area and directs a camera and instruments that enter the patient through small incisions. The robot translates those commands into instrument movements; the operating-room team remains with the patient.

What makes surgery “robotic”?

Robotic-assisted surgery uses a computer-assisted system that lets a surgeon manipulate instruments inside the body from a console. It is still surgery performed by a clinician. The U.S. Food and Drug Administration (FDA) puts it plainly in its Computer-Assisted Surgical Systems guidance: “The device is not actually a robot because it cannot perform surgery without direct human control.”

The system does not choose where to operate, identify a treatment on its own, or carry out the procedure independently. The surgeon supplies the decisions and movements; the system provides an interface for directing instruments.

How the system works in the operating room

A typical robotic-assisted surgical system has several connected parts: a surgeon console, a bedside cart with mechanical arms, an endoscope and surgical instruments, and supporting hardware and software that may be housed on another cart. The FDA describes these components in its guidance on computer-assisted surgical systems.

  1. The surgeon views the surgical field. The surgeon sits at the console and looks through the system’s endoscope view. For da Vinci systems, Intuitive describes this as a magnified 3D view of the surgical area.
  2. The surgeon directs the camera and instruments. Hand controls at the console let the surgeon position the camera and move the instruments. The system translates the surgeon’s hand movements into movements of instruments inside the patient.
  3. Instruments work through small incisions. The instruments are held by mechanical arms on a cart beside the patient and pass into the body through small incisions. Depending on the instrument and procedure, they can grasp, cut, dissect, cauterize, retract, or perform other surgical tasks.
  4. The bedside team supports the patient and procedure. The surgeon operates from the console in the operating room; the rest of the care team stays with the patient. The console does not replace the people providing care at the bedside.

For a patient-facing explanation of da Vinci systems, see How Does Da Vinci Surgery Work?; MedlinePlus also explains the surgeon-controlled mechanism in its Robotic surgery overview.

What the surgeon controls—and what the robot does not

The surgeon controls the camera and instruments, directing where they move and how they act. The instruments may carry out tasks such as grasping, cutting, dissecting, cauterizing, or retracting tissue, depending on the procedure and equipment. The system translates the surgeon’s commands into movements; it does not make the surgical decisions.

How robotic-assisted surgery differs from laparoscopy

Both robotic-assisted surgery and conventional laparoscopy are minimally invasive approaches that use a camera and instruments introduced through small incisions. The difference is principally in how the surgeon operates the instruments. In conventional laparoscopy, the surgeon generally stands beside the patient and uses long-handled instruments while watching a screen. In da Vinci surgery, the surgeon sits at a console, views a magnified 3D image, and controls miniaturized instruments.

Approach Where the surgeon works How the instruments are controlled What is established here
Conventional laparoscopy Generally standing next to the patient Long-handled instruments, while watching a screen These are the distinctions described in the da Vinci patient Q&A.
Da Vinci robotic-assisted surgery At a console in the operating room Console controls direct miniaturized instruments; the surgeon views a magnified 3D image These are the distinctions described in the da Vinci patient Q&A.

Open surgery is another possible approach. Which method is appropriate depends on the specific operation, the patient’s circumstances, and the clinician’s assessment—not simply on whether a robotic system is available.

What robotic assistance may change—and what it cannot promise

The FDA says robotic-assisted surgical devices may facilitate minimally invasive surgery and complex work in confined areas. MedlinePlus describes possible benefits including small, precise movements and improved visualization. These are potential advantages of the approach, not a guarantee of better results for a particular patient.

Robotic surgery can also involve additional setup, and MedlinePlus notes that an operation may take longer for that reason. Access varies among hospitals, and robotic surgery is not always the best approach. Recovery time and outcomes depend on the procedure, the patient, and the clinical circumstances. The presence of a robot alone does not establish that an operation will be safer, faster, or more effective than an alternative.

Which procedures use robotic-assisted systems?

The FDA says systems have been cleared for trained physicians to use in operating-room procedures across several specialties, including general, cardiac, colorectal, gynecologic, head and neck, thoracic, and urologic surgery. Examples include gallbladder removal, hysterectomy, and prostatectomy. These categories and examples do not mean every operation in those specialties—or every patient—is suitable for a robotic approach.

For cancer care in the United States, the FDA makes an important distinction: it has not granted marketing authorization for a robotic-assisted surgical system specifically to prevent or treat cancer. A device may be cleared for certain procedures that can be performed in patients with cancer, but that does not establish cancer-related outcomes such as survival or recurrence.

Risks and questions to discuss with your surgeon

All surgery carries risks. Intuitive’s patient information lists serious complications that can include injury to tissues or organs and the possibility that the surgeon may need to convert to another surgical technique. Outcomes can depend on patient characteristics, the disease, and the surgeon’s experience. The FDA advises patients to discuss risks, benefits, alternatives, and the surgeon’s training and experience.

Useful questions for a consultation include:

  • Am I a candidate for this approach for my condition?
  • What are the risks and benefits compared with open surgery, conventional laparoscopy, nonsurgical treatment, or other appropriate options?
  • How much experience do you have with this specific procedure and approach?
  • What recovery time, hospital stay, restrictions, and home support should I expect?
  • What will my insurance cover for this procedure?

These questions can help structure a conversation with your care team; they are not a substitute for advice tailored to your health and circumstances.
