Radiologists do not accept an AI alert as a diagnosis. They treat it as a task-specific aid—such as a suspected finding, measurement, or image annotation—then judge it against the original images, the examination’s purpose, and relevant clinical information. The radiologist remains responsible for the final interpretation, while the medical practice evaluates the tool before deployment and monitors it afterward.
The exact sequence varies with the imaging modality, clinical indication, software, and local protocol. There is no single universal checklist that applies to every radiology AI system. The reliable principle is independent image interpretation informed—but not replaced—by the algorithm’s output.

What an AI finding actually means
An imaging algorithm is built for a defined intended use. It may flag a suspected abnormality, calculate a measurement, mark a region of interest, or prioritize a study for review. That output answers a narrow software question, not the full clinical question posed by the examination.

Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →Repair Windows errors before they cause bigger problemsFix Now →Before relying on a result, a radiologist needs to know what the tool was designed to detect or measure, the clinical setting in which it was evaluated, and how its output is generated and displayed. A model intended to identify a particular finding on a specific type of scan should not be treated as a general-purpose reader for every scan, protocol, scanner, or patient group.

| AI output | What the radiologist must establish | What it cannot establish by itself |
|---|---|---|
| Flag or alert | Which finding the alert represents, the tool’s intended population, and the evidence shown with it | That the suspected abnormality is present in this patient |
| Measurement | What was measured, how the software defined the boundaries, and whether the image quality supports the value | That the measurement has clinical meaning without context |
| Annotation or marked region | Whether the marked area corresponds to the intended anatomy or lesion on the source images | Diagnosis, cause, severity, or treatment recommendation |
| Worklist prioritization | What rule moved the case and whether the result arrived on time | That a higher-priority case is more dangerous than every unflagged case |
The 2026 RSNA primer on interacting with AI results emphasizes that output generation, presentation, delays, storage, workflow alignment, and medicolegal questions all affect how an alert can be used. A technically valid output that arrives late, disappears, or is displayed away from the images may be clinically unhelpful.
How a radiologist checks an AI finding
The following sequence describes the general reasoning process. Local instructions and the software’s labeling determine the operational details; a radiologist should not assume that every system supplies a heat map, confidence score, or a particular viewing mode.
1. Confirm the task and the case
The radiologist first identifies what the algorithm was asked to do and whether that purpose fits the current examination. They consider the modality, body part, acquisition protocol, indication, and patient circumstances. A tool evaluated for one clinical setting may perform differently when images come from another protocol or population.
Do these 3 things before closing this tab:
1Fix the driver behind crashes, sound loss and screen glitches2Repair Windows errors before they cause bigger problems3Scan for outdated or missing drivers - takes under a minuteThis step also exposes a common category error: treating a narrow alert as a complete interpretation. A flag for a suspected finding does not tell the reader whether other abnormalities are present, whether the finding explains the symptoms, or what diagnosis best fits the whole case.
2. Review the source images and clinical context
The original images remain the primary evidence. The radiologist examines the relevant anatomy and the rest of the study in the normal clinical context, using the history, comparison examinations, laboratory information, and other data available under local practice. The point is not to follow a software-generated mark blindly, but to decide whether the underlying image supports it.

Clinical context can change the significance of an identical appearance. An algorithm may identify a visual pattern while lacking the patient history needed to distinguish an acute process from a chronic or incidental one. Conversely, an unflagged abnormality can still matter when the radiologist sees it during independent review.
3. Examine how the result was produced and presented
The reader checks where the result came from and how it appears in the viewing workflow. Depending on the product, the output may be an overlay, a separate series, a measurement, a text notification, or a worklist status. The radiologist verifies that the result belongs to the correct patient and study and that it has not been truncated, delayed, or attached to the wrong examination.
Presentation affects attention. A conspicuous alert can create anchoring, while a result that is buried in another application can be missed. The RSNA primer identifies delays, missing results, storage, and workflow misalignment as practical end-user issues. These are reasons to understand the local interface rather than assume that a displayed mark has the same meaning in every system.
4. Test the finding against the images
The radiologist asks whether the suspected abnormality is visible on the source images and whether its location, appearance, and extent are plausible. They look for explanations such as motion, noise, reconstruction artifacts, normal anatomy, postsurgical change, or an unrelated lesion that could have triggered the algorithm.
This is a clinical judgment, not a requirement to perform a particular viewing maneuver. Some tools provide localization or explanatory information; others do not. A radiologist should use the evidence the approved tool supplies, while following the manufacturer’s instructions and the facility’s protocol.

5. Consider alternatives and the rest of the examination
Even when a mark corresponds to a real image feature, the feature may not represent the condition named by the software. The radiologist considers competing explanations and checks for additional findings that the algorithm was not designed to detect. This guards against “automation bias,” in which a reader accepts a computer suggestion without sufficient independent assessment.

Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteInterpretability can be limited: some systems provide little insight into how they reached a conclusion. The 2024 multi-society statement from the ACR, CAR, ESR, RANZCR, and RSNA notes that transparency, representative data, and suitability for the intended use all affect safe adoption.
6. Reconcile the result in the final interpretation
The radiologist decides whether the AI output supports, contradicts, or adds nothing to the image interpretation. A confirmed finding is described according to normal reporting practice, with its clinical significance and limitations. A rejected alert is not automatically reported as disease; the radiologist documents the actual image finding and conclusion required by local policy.
When a result is missing, technically implausible, or discordant with the images, the radiologist follows the facility’s escalation or incident process. That may involve notifying the clinical team, recording a software issue, or sending the case to a quality-review pathway. The precise action depends on the tool and local governance.
Why a plausible AI alert can still be wrong
Regulatory clearance means that a product met the applicable review requirements for its stated use. It does not guarantee a correct output for every patient, scanner, protocol, or workflow. Training and test data may not represent the population in which a practice uses the tool, and performance can change when acquisition conditions differ.

A case described in an RSNA report on the 2024 paper FDA Review of Radiologic AI Algorithms: Process and Challenges illustrates the failure mode. An FDA-cleared algorithm labeled a finding as intracranial hemorrhage, but the patient was ultimately diagnosed with ischemic stroke. This is an example of a possible misclassification, not an error rate for radiology AI generally. It shows why a radiologist must check the underlying images and the clinical picture rather than treating a plausible label as self-validating.
Independent reader supportYour contribution helps us test, update, and keep practical guides available for everyone. How a radiology practice evaluates AI before routine use
Verification happens at two levels: the individual case and the healthcare organization. A practice should decide whether a tool is suitable before making its output part of routine care.
| Evaluation area | Questions for the practice |
|---|---|
| Intended task and setting | What exactly does the tool detect or measure, for which modality, indication, and patient population? |
| Training and test data | Are the data representative of the scanners, protocols, demographics, and disease spectrum seen locally? |
| Measured performance | Was performance assessed on appropriate independent test data, and are the reported measures relevant to the intended use? |
| Clinical usefulness | Will the output improve care, prioritization, or efficiency rather than add distracting alerts or delays? |
| Interpretability | What information accompanies an alert, and can users understand its scope and limitations? |
| Workflow fit | Where is the result generated, displayed, stored, and documented, and what happens if it is late or absent? |
| Privacy and governance | How are images and outputs protected, who can access them, and who is accountable for oversight? |
| Monitoring plan | How will the practice detect changes in stability, safety, utility, or disagreement after launch? |
The 2023 RSNA guide to evaluating publications about clinical AI recommends examining the study question and context, data, performance, usefulness, interpretability, and reproducibility rather than relying on one headline accuracy number. The multi-society 2024 statement likewise places responsibility on purchasers and users to evaluate a tool for their own setting and to monitor it over time.
A local pilot can reveal problems that a published evaluation cannot: unexpected scanner behavior, alert volume, interface confusion, or a mismatch between the algorithm’s intended population and the practice’s patients. The pilot should have a defined owner, a way to report discordant cases, and criteria for pausing or changing use when safety concerns emerge.
What monitoring looks like after deployment
Track stability and safety
Performance is not a one-time procurement fact. Practices monitor whether outputs remain available, timely, and clinically sensible as scanners, protocols, software versions, and patient populations change. A drift in alert frequency or a new pattern of disagreement warrants investigation rather than silent acceptance.
Best Value
Review discordant and unexpected cases
Cases in which the radiologist and algorithm disagree are valuable quality-improvement material. Reviewers can determine whether the disagreement reflects an image artifact, an out-of-scope case, a user-interface problem, or a genuine model error. The ACR’s 2026 announcement describes ACR Forensics for local investigation of discordant cases and Assess-AI for monitoring. These programs sit within broader ACR-SIIM practice-parameter work covering selection, evaluation, monitoring, privacy, and continuous quality improvement.
Maintain governance and privacy controls
Governance defines who approves updates, reviews incidents, manages access, and communicates limitations to clinicians. Privacy controls cover image transfer, storage, retention, and use of outputs for quality review. A change to the model or its interface should be treated as a change that may require re-evaluation, not as a purely technical update.
Measure workflow consequences
Useful monitoring includes operational behavior: whether results arrive when needed, whether users can locate them, whether alerts interrupt the intended worklist, and whether documentation remains accurate. A tool that is accurate in isolation can still create risk if it encourages delays, duplicate work, or misplaced confidence.
Limits that should shape a reader’s expectations
- No universal verification script exists. The appropriate checks depend on modality, indication, software instructions, and local policy.
- AI may lack clinical context. The algorithm can analyze supplied image data but may not know the full history or competing diagnoses.
- Bias can enter through data. Nonrepresentative training or test populations can reduce suitability in another population.
- Transparency varies. Some systems offer limited information about how a conclusion was reached.
- Missing or late output is itself a workflow risk. A radiologist must be able to interpret the study when the AI result is unavailable.
- Published performance is not a local guarantee. Scanner, protocol, patient mix, and workflow differences can change real-world behavior.
What patients and referring clinicians should take from this
An AI label is one input to a radiologist-led interpretation. A radiologist can agree with it, reject it, or decide that it is irrelevant after reviewing the images and context. Patients should therefore ask about the clinician’s interpretation of the scan, not whether a computer “confirmed” a diagnosis. Referring clinicians should provide the indication and relevant history so that both the radiologist and any supporting software are used within their intended scope.
The most dependable safety model is layered: software performs a defined analysis, a radiologist independently interprets the images, and the practice evaluates and monitors the technology as part of its quality system. That arrangement preserves the speed and consistency AI may provide without confusing an alert with clinical judgment.
