AI-written code should be treated as an untrusted draft. Before it is merged, a qualified human must understand the intended behavior, inspect the complete change, and verify it with tests and analysis appropriate to the risk. There is no defensible universal rule such as “review 20% of AI code” or “one reviewer is always enough.” Review depth should increase with the change’s impact, exploitability, novelty, size, uncertainty, test coverage, and difficulty of reversal.
A comment-only edit may need ordinary peer approval and automated checks. Code that handles authentication, payments, personal data, secrets, cryptography, infrastructure, concurrency, or safety needs domain expertise, security analysis, and a controlled rollout. The sections below show how to choose the right level without slowing every small change to the same degree.

Why AI-generated code cannot be merged on trust
Code-generation systems produce plausible text, not a guarantee that the implementation matches your requirements. A generated change can be functionally wrong, incomplete at the edges, unsafe under adversarial input, incompatible with a dependency version, or based on an assumption that was never true in your system. It may also omit operational details such as logging, rate limits, authorization checks, migration safety, or a usable rollback path.
GitHub’s guidance for Copilot agents says generated code “may not always be secure” and should be reviewed and tested before merging. It also says to review and verify Copilot’s feedback and supplement it with careful human review so the result meets your requirements: GitHub Copilot Agents documentation. That advice applies to any coding assistant, regardless of the model or editor.

NIST’s secure-development guidance reaches the same practical conclusion. Its 2024 publication, Secure Software Development Practices for Generative AI and Dual-Use Foundation Models, says that code-review and analysis policies should include code for AI models and related components. In other words, using an AI assistant does not remove your normal secure-development controls; it makes it important to apply them consciously.
Does AI-written code need the same review as hand-written code?
It needs at least the normal review required for an equivalent human-written change, and sometimes more. “AI-generated” is not itself a risk category that tells you how many people must approve a pull request. The behavior, privileges, affected data, novelty, and ability to undo the change are better predictors.
For a small, isolated change, the author can usually perform a careful self-review, automated checks can provide strong coverage, and the usual qualified peer approval can be enough. That is still real human review: someone must read the resulting code and understand what it does, rather than merely accepting a green check mark.
Quick wins for a faster PC:
Scan for outdated or missing drivers - takes under a minuteDriver Scan →Repair Windows errors before they cause bigger problemsFix Now →Fix the driver behind crashes, sound loss and screen glitchesFind Drivers →For new business logic or an API contract, the reviewer should understand the surrounding system and inspect normal and failure paths. For a security-sensitive or hard-to-reverse change, involve the domain owner and security staff where appropriate, require deeper analysis, and deploy in stages. A generated implementation that looks idiomatic can still violate a local invariant that only an experienced maintainer knows.
A practical risk matrix for deciding review depth
Use the highest applicable row when a change spans categories. “Minimum human review” describes the floor, not a substitute for your organization’s policy or regulatory obligations.
| Change profile | Minimum human review | Additional controls |
|---|---|---|
| Formatting, comments, or an isolated low-risk refactor with strong existing tests | Author self-review plus the normal qualified peer approval | Continuous-integration tests, linting, and static analysis |
| New business logic or changed API behavior | A qualified peer who understands the system and its contract | Unit and integration tests, dependency inspection, and explicit review of error paths and compatibility |
| Authentication, authorization, payments, personal data, secrets, cryptography, infrastructure, concurrency, or safety behavior | The responsible domain owner plus security review when applicable | Threat modeling, negative and abuse-case tests, least-privilege checks, staged rollout, monitoring, and a tested rollback path |
| Large, novel, or difficult-to-reverse migration | Multiple reviewers with relevant domain expertise | Design review, rehearsal, canary or staged deployment, data-integrity checks, explicit rollback or forward-fix plan, and heightened monitoring |
If two rows seem applicable, follow the stricter one. A “small” patch that changes authorization is still a security-sensitive patch; a large comment-only reformat may be operationally low risk if it cannot alter behavior and is mechanically verified.
The human-review workflow, step by step
1. Record AI assistance when policy requires it
Follow your organization’s rules for declaring AI use, retaining prompts or generated patches, and handling confidential input. The record should identify the affected component and the nature of the assistance, not expose secrets or proprietary data in an uncontrolled system. This gives maintainers a useful provenance signal and supports later investigation when a defect is found.

2. Make the author explain the change
Before approval, the author should describe the intended behavior, assumptions, data and permission boundaries, known limitations, and why the chosen approach fits the existing design. Ask for this explanation in the author’s own words. If the author cannot explain a function, dependency, or security decision, the change is not ready to merge, even if the generated code compiles.
3. Read the complete diff, not only the suggested lines
Inspect every added, deleted, and modified line, along with configuration, generated files, lockfiles, database migrations, and deployment manifests. Look for changes outside the stated scope, silently broadened permissions, altered defaults, disabled validation, swallowed exceptions, unsafe logging, and behavior that differs between success and failure paths. Review the final tree as it will build, not just the assistant’s conversational snippet.
4. Trace dependencies, permissions, and data flows
Check every new package, version constraint, transitive dependency, model endpoint, and system call. Confirm that licenses and supply-chain controls are satisfied. Follow untrusted input from entry point to storage, command execution, templates, network calls, and output. Verify authentication and authorization at the point of use, not only at an upstream route. Check that service accounts and tokens have the least privilege needed and that sensitive values never enter logs, prompts, test fixtures, or error messages.
5. Verify tests—and assume generated tests can share the same blind spot
Run the project’s existing checks and add tests for the new behavior. Read the tests rather than treating their presence as proof: an AI assistant can generate assertions that merely restate its implementation or omit the edge case that matters. Cover valid inputs, malformed inputs, boundary values, retries, timeouts, partial failures, permission denials, concurrent access, and compatibility with existing clients where those cases apply.

The approver must be able to challenge the design and evaluate the evidence, not just confirm that the author followed a checklist. Add a security or domain approval for sensitive areas. Record unresolved risks and the decision to accept, mitigate, or defer them. Do not merge first and rely on a later review to repair a production defect.
How to scale review by risk
Six questions help determine whether ordinary peer review is sufficient or whether to escalate.
What is the impact if it is wrong?
A typo in a non-critical comment and an incorrect payment calculation do not belong in the same review queue. Consider financial loss, privacy harm, safety consequences, service availability, data integrity, contractual commitments, and the number of users affected. A low-probability defect with severe consequences deserves stronger controls than a frequent but harmless formatting issue.

Can an attacker exploit it?
Public endpoints, parsers, authentication flows, authorization checks, file handling, serialization, command execution, and secret management are exposed to adversarial input or misuse. Escalate when a change creates a new trust boundary, handles untrusted data, changes identity decisions, or expands a credential’s privileges. Security review should include abuse cases and negative tests, not only happy-path functionality.

How large or novel is it?
A one-line change in a well-tested utility is easier to reason about than a generated subsystem that introduces a new framework, protocol, storage engine, or concurrency model. Novel designs need an architectural review because the main risk may be an invalid assumption rather than a visible coding error. Large diffs also make it harder for one person to maintain a reliable mental model; split them into reviewable increments when possible.
How strong is the test coverage?
Automated tests reduce uncertainty but do not establish correctness by themselves. Coverage may miss undocumented requirements, production configuration, race conditions, data-shape changes, or an attack path. Treat low coverage, flaky tests, weak assertions, and untested integration boundaries as reasons to increase human scrutiny and add targeted checks.
Does the reviewer have the right expertise?
Review quality depends on understanding the component. A generalist may review style and maintainability while missing a cryptographic misuse or a subtle database isolation problem. Route changes to reviewers who know the relevant domain, and involve security specialists for security-sensitive behavior. A larger reviewer count cannot compensate for the absence of the person who understands the risk.
How reversible is the change?
A feature behind a flag can often be disabled quickly. A destructive migration, irreversible data transformation, firmware update, or permission change may not be recoverable by reverting the source commit. The less reversible the operation, the more you should require rehearsal, backups or verified recovery, staged deployment, monitoring, and an explicit rollback or forward-fix plan.
When security review is mandatory in practice
Ask for security involvement when the change touches any of these areas:
- Authentication, session handling, identity proofing, or authorization policy.
- Payments, billing, financial calculations, or transaction integrity.
- Personally identifiable information, health information, credentials, API keys, or other secrets.
- Cryptographic algorithms, key storage, certificate validation, token signing, or randomness.
- Infrastructure-as-code, network policy, container privileges, deployment systems, or production access.
- Concurrency, distributed locks, queues, or safety-critical control paths.
- File uploads, template rendering, deserialization, command execution, or any boundary that accepts untrusted input.
For these changes, review the threat model and trust boundaries, test denial and abuse cases, verify least privilege, and examine logging and alerting for both attacks and accidental misuse. A security reviewer should be able to see the complete data flow and deployment context; reviewing an isolated function without its configuration can miss the actual vulnerability.

- Please Note: To reach 10Gbps speed, keep the cable under 3.3 ft. For USB A Male to USB C adapters, try flipping the USB C connector. USB C Male to USB A adapters support bidirectional 10Gbps transfer within 3.3 ft
What testing should accompany the review?
Choose tests according to the failure modes of the change:
- Unit tests: Validate deterministic rules, boundary values, validation, and error handling close to the code.
- Integration tests: Exercise databases, queues, external services, authentication middleware, configuration, and serialization as they operate together.
- Security tests: Try unauthorized access, malformed input, replay, privilege escalation, injection, secret exposure, and rate-limit bypass where relevant.
- Static analysis and linting: Catch unsafe patterns, type errors, dependency issues, dead code, and policy violations before deployment.
- Performance and concurrency tests: Measure contention, timeouts, memory growth, throughput, and behavior under retry or partial-failure conditions when the change can affect them.
Run the checks in the same build and deployment configuration used in production whenever possible. If a check cannot run locally, document where it runs and what evidence the reviewer should inspect. A passing test suite supports a review; it does not replace reading the implementation and judging whether the tests express the actual requirement.

Random freezes, missing sound and display glitches usually trace back to one bad driver. Find and replace yours safely.Free scan · under a minuteWhen lighter review is reasonable
Lightweight review is appropriate only when the consequences are limited, the change is easy to understand and undo, and automated checks provide meaningful coverage. Examples include a comment correction, formatting-only change, or an isolated refactor whose behavior is protected by strong tests and whose diff contains no configuration or dependency surprises.
Even then, the author should inspect the final diff, confirm that no behavior changed unintentionally, and obtain the normal peer approval. Do not use “AI-generated” as a reason to skip review, and do not use “low risk” as a reason to skip verification. If the patch unexpectedly touches a sensitive file, broadens its scope, or fails a check, move it to the stricter review path.

Approval is not the end of risk management. For a high-impact or difficult-to-reverse change, deploy to a limited audience or environment first. Use a feature flag, canary, staged percentage, or maintenance window that lets you observe behavior before full release. Define success and failure signals in advance, including error rates, latency, authorization failures, data-integrity checks, business metrics, and security alerts.
Keep rollback available for as long as the change can cause harm. Test the rollback or forward-fix procedure before release, and verify that it works with the data and schema state created by the new version. Assign an owner to watch the deployment and a clear threshold for pausing or reversing it. Monitoring cannot make unsafe code safe, but it can limit exposure when a defect escaped pre-merge review.

NIST’s AI Risk Management Framework Generative AI Profile describes situations in which generative AI warrants different levels of oversight, including additional human review, tracking, documentation, and management attention. Translate that principle into a policy that tells engineers when to disclose AI assistance, which data may be sent to a model, which reviewers must approve sensitive changes, and what evidence must accompany a merge.

A useful record includes the affected component, risk classification, author explanation, reviewers and their roles, tests and analysis run, known limitations, accepted residual risks, deployment plan, and rollback owner. Store enough information for a later maintainer to understand why the change was accepted without retaining confidential prompts or secrets. Update the policy as your systems, model capabilities, and threat profile change.
How many reviewers are enough?
No authoritative number or percentage establishes how much AI-written code must be human-reviewed, and no fixed reviewer count fits every project. One qualified peer may be sufficient for a reversible, low-risk patch; a security-sensitive migration may need the domain owner, security review, and several people involved in design and deployment. Set approval requirements by impact and reversibility, then document exceptions rather than inventing a universal quota.
Examples of applying the framework
Comment cleanup in a well-tested library
The assistant changes comments and formatting in files with no executable or configuration changes. The author checks the complete diff, runs lint and the normal test suite, and obtains ordinary peer approval. If the diff includes an unexpected import, generated file, or behavior change, it no longer qualifies for this lightweight path.
What’s actually slowing this PC down?
Pick the symptom - the matching free tool is one click away.
New API endpoint for account data
The endpoint changes business behavior and handles user information. A reviewer who knows the service checks the contract, authentication and authorization path, validation, pagination, error responses, logging, and backward compatibility. Unit and integration tests cover valid, malformed, unauthorized, and boundary requests. Because personal data is involved, security review and a monitored rollout may be appropriate even if the implementation is short.
Generated authentication or payment code
The change affects a high-impact trust boundary. The domain owner and security reviewer inspect the full data flow, dependencies, secrets, cryptographic or transaction assumptions, and failure behavior. Threat modeling, negative tests, static analysis, staged deployment, monitoring, and a verified rollback plan are required before broad release. A green test suite alone is not evidence that the design is safe.
Large database migration
A generated migration can be hard to reverse and may affect every customer. Reviewers rehearse it on representative data, check locking and performance, validate backups and recovery, plan compatibility between old and new application versions, and define a canary or staged rollout. The approval record states whether rollback is possible or whether a forward-fix is the only recovery route.
A concise pre-merge checklist
- Can the author explain the intended behavior, assumptions, and limitations without relying on the model?
- Has a qualified human read the complete final diff, including dependencies, configuration, migrations, and generated files?
- Are inputs, permissions, data flows, error paths, logging, and resource limits correct?
- Do tests cover normal behavior, boundaries, failures, abuse cases, and integration points appropriate to the risk?
- Have static, security, performance, or concurrency checks been run where relevant?
- Are domain and security approvals present for sensitive behavior?
- Is the deployment staged and monitored, with a tested rollback or forward-fix plan when reversal is difficult?
- Are AI assistance and accepted residual risks documented according to policy?
If any answer is “no,” pause the merge or reduce the scope until the missing evidence is available. Human review is not a ceremonial signature: it is the control that connects generated text to a real requirement, a real system, and an accountable release decision.
