Your fee is set by the treating dentist or orthodontist, mainly according to case complexity and the number of aligners required. A reliable quote should show the complete treatment lifecycle—not just the first trays—including examination, scans or impressions, attachments, monitoring visits, refinements, retainers, replacement trays and post-treatment checks. Insurance, HSA/FSA funds and financing may reduce what you pay, but coverage and package rules vary by plan and provider.
Why clear-aligner prices vary so widely
Aligner fees are individualized rather than fixed at a national rate. Invisalign explains that the treating doctor determines the fee based on the complexity of the case and the quantity of aligners needed. A minor spacing correction may require fewer stages than a treatment involving significant crowding, bite changes or movement of many teeth.

The practice you choose and its location also affect the quote. Each treating practice sets its own professional and laboratory fees, so an online “average” may not describe what a clinic in your area charges. Two patients with similar-looking concerns can still receive different quotes if their examinations show different bone, gum, bite or tooth-position issues.

Package definitions create another major difference. One provider may include scans, all monitoring visits, unlimited refinements and the first retainer set. Another may charge separately for some or all of those items. Compare the written scope of treatment, not only the headline price.

What a complete package may include
There is no standardized package across providers. Ask the practice to mark every item as included, limited, optional or charged separately.
| Package element | What to confirm in writing |
|---|---|
| Initial examination | Whether the dentist or orthodontist performs an in-person examination and assesses teeth, gums, bite and jaw function before prescribing aligners. |
| Diagnostic records | Whether digital scans, photographs, impressions and any required radiographs are included, and whether a new scan is billed if the plan changes. |
| Aligner series | How many stages or trays the quoted fee covers, whether the quote is for one arch or both, and what happens if additional stages are needed. |
| Attachments and accessories | Whether tooth-colored attachments, elastics, buttons, interproximal reduction and other clinical procedures are included. |
| Visits and monitoring | How often in-person checks occur, whether remote monitoring is used, and whether urgent visits carry a fee. |
| Refinements | Whether additional trays after the initial series are included, how many refinement rounds are allowed, and the deadline for requesting them. |
| Retainers | Whether the first set is included, which type is supplied, how many are provided and what replacement retainers cost later. |
| Lost or damaged trays | The replacement-tray price, turnaround time and whether a temporary tray or previous stage can be worn while waiting. |
| Post-treatment care | Whether the final check, bite review and future retention visits are included and for how long. |
| Optional services | Whether whitening, enamel shaping or other cosmetic services are included, optional or excluded from the orthodontic fee. |
A quote that simply says “complete aligner treatment” leaves too many cost questions unanswered. Request the actual terms before paying a deposit.
Costs that can arise during the treatment lifecycle
Consultation and records
Some practices include the consultation and diagnostic records in the treatment fee; others charge a consultation or records fee that is credited only if you proceed. Ask whether radiographs are needed and who pays for them. An in-person examination is also a safety checkpoint: aligners should not be prescribed without evaluating the condition of the teeth, gums and supporting bone.

Attachments and planned procedures
Many treatment plans use small bonded attachments or other techniques to help move teeth. Confirm that placing and removing them are covered. If the plan includes enamel reduction, elastics or buttons, ask whether those procedures and supplies are part of the quoted amount.
Monitoring and changes
Regular reviews allow the clinician to check tracking, gum health and the bite. A quote should state how appointments are scheduled and whether remote photo checks replace or supplement office visits. If a tooth does not track, a tray is lost or the plan must be changed, find out whether a new scan and additional visit are included.
Refinements
Refinement trays are additional aligners made after the initial series when teeth need further movement or the result is not yet complete. Current consumer-market reporting indicates that refinements may cost extra unless the plan expressly includes them. The phrase “unlimited aligners” is not enough by itself; ask whether it means unlimited refinement rounds, a defined period, or only trays in the first series.

Retainers and replacement trays
Retention is a separate phase of care. The American Dental Association says aligner treatment is followed by wearing a retainer to hold teeth in their new positions. Ask whether the first retainer set is included, when it is delivered, and the price and process for replacements. Retainers wear out, are lost and can stop fitting if they are not worn as directed, so future replacement costs belong in your budget.
Do these 3 things before closing this tab:
1Clear out junk files and repair common Windows errors2Fix the driver behind crashes, sound loss and screen glitches3Repair Windows errors before they cause bigger problemsHow to compare two written quotes
- Confirm the clinical scope. Check whether both quotes cover one or both arches, the expected number of stages and the same type of correction.
- Separate professional and product fees. Identify examination, records, laboratory aligners, attachments, visits and retainers as separate line items.
- Read the refinement rule. Record how many refinement rounds are included, the eligibility period and the charge after that period.
- Price the failure scenarios. Ask what happens financially if a tray is lost, a tooth stops tracking, treatment is extended or you need a new scan.
- Check the retention commitment. Compare the first retainer set, follow-up checks and replacement policy rather than treating retention as an afterthought.
- Get cancellation and refund terms. The agreement should explain what happens if you stop treatment, transfer to another provider or cannot continue for medical reasons.
Use the same checklist for every provider. A lower initial quote can become more expensive if scans, refinements, retainers or additional visits are excluded.
Insurance, HSA/FSA funds and financing
Some dental plans contribute toward orthodontic treatment, but benefits differ by contract. Invisalign describes monthly financing, low-to-no-down-payment offers from some providers and the potential use of HSA or FSA funds. The provider—not the aligner manufacturer—sets financing terms, so review the interest rate, term, down payment, deferred-interest language and consequences of missed payments.

Before treatment, ask your insurer to verify the benefit in writing. Confirm the lifetime orthodontic maximum, age limits, waiting period, in-network requirements, claim rules and whether the plan covers adult treatment. Also ask whether refinements, replacement trays and retainers are eligible expenses. A benefit estimate is not a guarantee of payment until the claim is processed.
Align reports company-collected historical figures from its OrthoFi verification tool: among 112,243 U.S. patients with dental insurance in calendar year 2021, average reported coverage was $1,772; 92% qualified for up to $3,000 and 77% qualified for up to $2,000. These are company-reported, historical averages—not a promise that your plan will pay those amounts.
| Payment source | Questions to ask |
|---|---|
| Dental insurance | What is the remaining lifetime orthodontic maximum, and are the provider, age and waiting-period rules satisfied? |
| HSA or FSA | Which treatment and retention expenses qualify under your account rules, and what documentation is required? |
| Practice financing | What is the total repayment, annual percentage rate, deposit, term and policy if treatment is stopped or transferred? |
| Outside financing | Who is the lender, and are promotional or deferred-interest terms triggered by late payment? |
Supervised treatment versus direct-to-consumer aligners
The largest difference between supervised treatment and an unsupervised mail-order model is clinical responsibility. The ADA strongly discourages direct-to-consumer dental-laboratory services because they remove the dentist’s role in diagnosis, treatment planning and management. Clear aligners are prescription devices, and an FDA warning reported by ADA News says unauthorized unsupervised products may increase complications.

- The tooth retainer remover can keep your hands away from your mouth.
- Non-slip design and offer a comfortable grip for effective and intuitive clear aligner removal.

In a 2023 statement, the ADA warned: “Moving teeth without knowing all aspects of a patient’s oral condition has the potential to cause bone loss, lost teeth, receding gums, bite problems, jaw pain, and other damaging and permanent issues.”
Compare any online option against these requirements:
- An in-person examination and appropriate radiographs before treatment.
- A licensed dentist or orthodontist who accepts responsibility for the prescription and treatment plan.
- A defined schedule for in-person follow-up, plus clear rules for remote monitoring.
- A written policy covering refinements, lost trays, breakage, refunds and retainers.
- A realistic total-cost estimate if treatment must be extended or corrected.
The FDA warning described reports of new or worsening dental pain, jaw pain, bite problems, worsening misalignment and new gaps associated with unauthorized direct-to-consumer aligners. If you develop new or worsening symptoms, contact a dentist or orthodontist promptly rather than continuing to change trays on your own.
Questions to ask at the consultation
- What diagnosis is being treated, and are there gum, bone, cavity or bite issues that must be addressed first?
- How many aligner stages are expected, and is the quote for one arch or both?
- Which scans, photographs, impressions and radiographs are included?
- Are attachments, elastics, enamel reduction and their removal included?
- How many in-person visits are expected, and what remote monitoring is available?
- Are refinements included? If so, for how long and under what conditions?
- What does the first retainer set include, and what is the replacement price?
- What happens if a tray is lost, damaged or no longer fits?
- What is excluded from the quoted fee, including whitening or other cosmetic services?
- What are the refund, cancellation and transfer terms?
- How will insurance claims be submitted, and what amount is the practice estimating you will owe?
Planning for the cost after the last aligner
Finishing the final tray does not end the financial or clinical commitment. Teeth can shift without retention, and the retainer schedule should be explained by your treating clinician. Keep the written retainer policy, follow the prescribed wear instructions and ask what to do if a retainer cracks, is lost or begins to feel tight. A replacement orthodontic retainer is clinician-prescribed and fit-dependent; it should not be ordered as a generic substitute when the fit or bite has changed.
When comparing treatment plans, calculate the cost through the retention phase rather than stopping at the first set of aligners. That approach exposes differences that a promotional starting price can hide.
Bottom line for choosing a quote
Choose the quote that clearly defines the clinical service and the exceptions, not necessarily the one with the lowest starting number. The essential comparison is total expected cost for diagnosis, aligners, supervision, refinements, retainers and foreseeable replacement or extension scenarios. Verify insurance and financing terms independently, and do not trade away examination and follow-up for a lower advertised price.
