There is no evidence-based percentage of AI-generated code that is safe to merge without human review. The safe boundary is a control, not a number: every AI-assisted change needs a named human owner who understands the code and approves it. Authentication, authorization, cryptography, data handling, infrastructure, CI/CD, and safety-critical code require qualified review, independent testing, security scanning, and controlled deployment.
AI can draft substantial amounts of code, including boilerplate and routine tests. That can increase throughput, but it does not prove correctness, security, maintainability, or compliance. Treat generated code as an untrusted contribution until a responsible developer has verified the behavior and the risks.

What the evidence actually tells us
Higher throughput is not a safety threshold
A June 2025 Microsoft Research report combined randomized field experiments at Microsoft, Accenture, and an anonymous Fortune 100 company. Across 4,867 developers, users of the studied AI tools completed 26.08% more tasks, with a standard error of 10.3%. That is a productivity result: it counts completed work in those organizations and under those conditions. It does not measure vulnerability rates, defect escape, permission boundaries, or whether reviewers can be skipped.
Do these 3 things before closing this tab:
1Scan for outdated or missing drivers - takes under a minute2Repair Windows errors before they cause bigger problems3Fix the driver behind crashes, sound loss and screen glitchesA team can therefore produce more code while increasing the amount that must be inspected. Throughput is useful for planning capacity; it is not a percentage of code that can safely bypass approval.

Generated code can miss and introduce vulnerabilities
A 2025 DevGPT study by Belozerov, Barclay, and Sami examined 1,586 C, C++, and C# snippets. Static scanning identified 124 potentially problematic files. Manual analysis confirmed 32 vulnerabilities in 26 files. ChatGPT detected 18 of those 32 vulnerabilities and resolved 17, yet it introduced 22 vulnerabilities in the snippets studied. The authors concluded that generated code was not sufficiently reliable for secure deployment without static scanning and manual review.
Those counts should not be treated as a universal failure rate. The work is a preprint, covers C-family snippets, and evaluates particular prompts and model behavior. Its practical lesson is broader and more defensible: an assistant can both repair a flaw and create one, so the person approving the change must check both the requested behavior and the new attack surface.
Developers are not uniformly confident
A 2025 ACM/SIGAPP survey found that only about one quarter of developers expressed confidence in AI-generated code. Experienced developers also perceived reduced secure-coding proficiency when using generative-AI tools. Perception is not a defect measurement, but it is a warning against treating fluent output as self-validating.
Standards put accountability on people
NIST SP 800-218A, finalized in July 2024, augments the Secure Software Development Framework with generative-AI-specific practices throughout the software life cycle. OWASP’s Secure Coding with AI Cheat Sheet states: "AI-generated code must have a human owner. Every AI-assisted change should be reviewed, approved, and attributable to a developer who is responsible for its security and maintainability." It also states: "Do not deploy AI-generated code that no human has reviewed and approved."

The UK’s Code of Practice for the Cyber Security of AI calls for training developers in secure coding and system design specific to AI development, plus documentation of AI security risks and mitigations. Together, these frameworks point to traceable ownership, not an allowed percentage.
Why a single percentage fails
“Allow up to 30% AI code” sounds measurable, but it hides the variables that determine harm. Ten lines changing a button label and ten lines changing an authorization check are not equivalent. A small change in a shared library can affect every service, while a large generated fixture may never reach production.
| Decision factor | What to ask | Why it changes review depth |
|---|---|---|
| Blast radius | How many users, services, or environments can this change affect? | A shared package, deployment script, or platform component needs more scrutiny than an isolated tool. |
| Security impact | Does it handle identity, permissions, secrets, cryptography, input validation, or untrusted content? | Security failures can permit compromise even when ordinary tests pass. |
| Data sensitivity | Will the code process personal, financial, health, confidential, or regulated data? | Privacy, retention, and disclosure obligations add controls beyond functional correctness. |
| Change reversibility | Can the deployment be rolled back quickly without losing data or violating an obligation? | Irreversible migrations and safety actions require staged release and deeper approval. |
| Dependency exposure | Does the output add packages, native code, build steps, permissions, or network access? | Every new component expands the supply-chain and maintenance surface. |
| Reviewer capability | Does the approver understand the language, framework, threat model, and operational context? | A nominal approval is not meaningful if the reviewer cannot challenge the implementation. |
These factors also explain why counting generated lines is a poor governance metric. A 5% change can be high risk; a 90% generated internal script can be low risk only after its owner has verified its boundaries and consequences.
A defensible policy for AI-assisted changes
Use the following workflow for every pull request that contains AI-assisted code. It creates an auditable decision without pretending that all changes have identical risk.

- Record AI assistance. Mark the pull request or change record as AI-assisted. Identify the tool or model, the contributor, and the files or components affected. Do not place confidential source, credentials, production data, or regulated information into a model unless your approved controls explicitly allow it.
- Name one accountable owner. Assign a developer who can explain the intended behavior, assumptions, dependencies, and failure modes. The owner remains responsible even when another person operates the tool.
- Classify the change before review. Rate blast radius, security impact, data sensitivity, reversibility, dependency changes, and operational consequences. Escalate to the highest applicable category rather than averaging scores.
- Constrain the generation task. Give the assistant a narrow objective, explicit interfaces, coding standards, and forbidden files. Keep the resulting diff small. Ask it to explain assumptions and identify uncertainty, but treat those explanations as claims to verify.
- Inspect the complete diff. Review every changed file, including generated tests, lockfiles, build scripts, configuration, comments, and deleted checks. Look for out-of-scope edits, weakened validation, new network calls, permission changes, hard-coded secrets, and error paths that silently continue.
- Run independent verification. Use automated tests, type checks, linting, static analysis, dependency and license checks, and relevant integration tests. Tests authored by the same agent are useful coverage, not independent assurance; add cases designed by a human or another independent process.
- Perform security and adversarial checks. For exposed inputs, try malformed, oversized, unauthorized, replayed, and boundary-value requests. Verify authentication and authorization failures, logging, rate limits, secret handling, and safe error messages. Threat-model high-impact changes.
- Obtain explicit approval. A qualified reviewer signs off on the actual diff and the evidence, not merely on the fact that CI is green. Record unresolved assumptions and required follow-up work.
- Deploy with controls. Use staged rollout, feature flags, migration backups, least-privilege credentials, monitoring, and a tested rollback path when the risk warrants them. Observe behavior after release rather than treating approval as the end of assurance.
- Keep provenance and outcomes. Retain the owner, reviewer, risk category, scans, test results, deployment decision, incidents, rollbacks, and later fixes. This record lets the organization improve its policy using actual outcomes.
How much review each risk level needs
A practical policy can define minimum controls while allowing teams to add stricter ones. The categories below describe the floor, not permission to ignore an unusual hazard.
| Risk level | Typical examples | Minimum review and release controls |
|---|---|---|
| Low | Formatting, isolated boilerplate, documentation tooling, or a non-sensitive internal utility with no new dependency or privilege. | Named owner; normal peer review; inspect the full diff; automated tests, linting, and type checks; confirm no secrets, sensitive data, or out-of-scope files. |
| Medium | Business logic, API handlers, database queries, shared library code, or a change that adds a package or modifies error handling. | Qualified peer review; focused threat and data-flow review; independent tests for negative and boundary cases; static analysis; dependency and license audit; staged deployment where practical. |
| High | Authentication, authorization, payment or personal-data processing, cryptography, input parsing at a trust boundary, CI/CD, infrastructure, or code used by many services. | Reviewer with relevant security and domain expertise; documented threat model; static and dynamic testing; dependency provenance review; adversarial tests; least-privilege checks; staged release, monitoring, and rollback. |
| Critical | Safety-critical control, irreversible migration, core identity plane, production credential path, or a change whose failure could cause severe physical, legal, or systemic harm. | Multiple qualified reviewers or an independent approval path; formal requirements and evidence; specialized testing; separation of duties; controlled environment; explicit release authority and post-deployment observation. |
If a change crosses categories, apply the stricter category to the whole change or split it into smaller, separately reviewable changes. Do not use a low-risk label merely because the assistant produced familiar-looking code.
What a human reviewer should examine
Intent, invariants, and assumptions
Start with the requirement, not the generated implementation. Write down what must always be true: who may call the function, which states are valid, what happens when a dependency is unavailable, and what data may be stored or returned. Then check whether the code enforces those conditions on every path. AI output often handles the happy path while leaving authorization, retries, time zones, encoding, concurrency, or partial failure implicit.

Trust boundaries and input handling
Trace values from network requests, files, queues, environment variables, and user-controlled templates to interpreters, database queries, shell commands, deserializers, and logs. Confirm canonicalization occurs before validation, limits exist for size and recursion, and errors fail closed where appropriate. Check that authorization is performed on the server and on the specific object or action, not inferred from a client-supplied role.

Whole-diff and repository context
Review generated tests and configuration with the same care as application code. An agent may alter a build rule, disable a check, change a container base image, or add a dependency while appearing to implement a small feature. Search for hallucinated packages, unused imports that conceal a wrong approach, and documentation that promises behavior the code does not provide.
Independent tests
Use tests that challenge the implementation rather than reproduce its assumptions. Include unauthorized users, malformed input, empty and maximum values, duplicate requests, timeouts, dependency failures, concurrent access, rollback behavior, and compatibility with existing data. For security-sensitive paths, have a person who did not write the prompt design at least some adversarial cases.
Supply-chain and operational review
Verify each new dependency’s exact name, version, source, license, maintenance status, and required permissions. Review lockfile changes and transitive packages. Confirm that build scripts cannot download arbitrary code or exfiltrate credentials. Check runtime permissions, outbound network access, secret injection, logging redaction, alerting, and resource limits. A correct function can still create an unsafe system when its packaging or deployment is wrong.

- “The model wrote the test, so the test proves the code.” Shared assumptions can reproduce the same mistake. Independent cases and human-designed negative tests are needed.
- “It is only boilerplate.” Boilerplate can embed insecure defaults, permissive headers, weak random-number generation, or an incorrect framework pattern. Verify the resulting behavior and version-specific API.
- “The diff is small.” Small changes in a shared authorization helper, parser, or deployment file can have a large blast radius.
- “CI passed.” CI demonstrates only the checks it runs. It may not detect a logic flaw, an insecure dependency, a missing permission check, or a dangerous production configuration.
- “A senior developer approved it.” Seniority does not replace relevant expertise or time spent reading the complete diff. Approval should be attributable and evidence-based.
- “We can fix it after release.” Post-release fixes do not undo data disclosure, credential exposure, unsafe actions, or a compromised dependency. Use deployment controls before exposure.
- “The percentage target is our safeguard.” A quota can encourage teams to maximize generated lines or hide high-risk changes inside a permitted allowance. Govern risk and outcomes instead.
How to measure whether your policy is working
Organizations should tune review depth with local evidence rather than adopt a universal authorship percentage. Segment measurements by risk category, component, language, and team so a safe internal utility does not obscure failures in an identity service.
Best Value
| Measure | What it reveals | Useful interpretation |
|---|---|---|
| Escaped defects | Functional problems found after approval or release. | Compare AI-assisted and non-assisted changes with similar risk, not raw totals. |
| Security findings | Vulnerabilities found in review, scanning, testing, or incidents. | Track severity, root cause, and whether the control should move earlier. |
| Rework and rollback rate | How often a change needs correction or reversal. | High rates may indicate inadequate scoping, review time, or testing. |
| Review time and changed-file count | Whether reviewers can realistically understand the contribution. | Set diff-size limits or split work when comprehension is poor. |
| Dependency and configuration changes | Supply-chain and operational exposure. | Require additional approval when generated work adds privileges or packages. |
| Incident and near-miss data | Failures that ordinary test metrics miss. | Feed lessons into prompts, checklists, training, and risk classification. |
Do not use productivity alone as the success criterion. A responsible program seeks faster delivery without increasing escaped defects, severe findings, recovery time, or unowned risk.
Answers to the questions developers ask
How much AI-generated code can I trust?
Trust the behavior you have verified, not the fraction produced by a model. For routine, isolated code, normal peer review and automated checks may be proportionate. For code that controls access, handles sensitive data, changes infrastructure, or affects safety, require the stronger controls in the high or critical category regardless of how little code the model generated.
Can I merge assistant-written code without reviewing it?
No. The OWASP position is explicit that every AI-assisted change needs human review, approval, and attributable ownership, and that unreviewed AI-generated code should not be deployed. Automated checks can reduce reviewer effort; they do not create accountability or cover every failure mode.

Does every AI change need a human in the loop?
Every change needs a human owner and approval before deployment. The amount of human time can vary: a low-risk diff may receive a short, focused review, while a critical change may require several reviewers and formal evidence. “Human in the loop” should mean competent evaluation of the actual change, not a ceremonial click.
Should a team set a maximum percentage of generated code?
A percentage can be used for capacity planning or to monitor adoption, but it is not a safety control. Set requirements for ownership, risk classification, review evidence, independent testing, scanning, dependency checks, and deployment safeguards. If local incident data shows a class of generated changes is unreliable, tighten that class’s controls rather than choosing an arbitrary global percentage.
A concise team checklist
- Every AI-assisted change has a named owner.
- The owner and reviewer can explain the requirement, assumptions, and failure modes.
- The complete diff, including tests, configuration, lockfiles, and build changes, has been inspected.
- Risk is classified by blast radius, security, data, reversibility, dependencies, and reviewer capability.
- Tests include negative, boundary, failure, and adversarial cases appropriate to the change.
- Static analysis, dependency auditing, and secret scanning run before approval.
- High-impact changes receive qualified security or domain review and controlled deployment.
- Model access to source, credentials, and production data is governed and logged.
- Approval, evidence, deployment outcome, rollback, and incident data are retained.
- Review requirements change when local defect and security data shows a weakness.
Limits of the available evidence
The 26.08% productivity increase comes from specific organizations and a specific coding-assistant study; it should not be generalized to every team or model. The DevGPT vulnerability counts come from a 2025 preprint using C, C++, and C# snippets, so they are not a universal vulnerability rate for all languages. The ACM/SIGAPP result measures developer confidence and perceived secure-coding proficiency, not confirmed defects. No cited source establishes a percentage of code that can safely bypass human review.
Those limits do not weaken the operational conclusion. They explain why a universal number would overstate what is known. The reliable policy is to let AI accelerate drafting while keeping human ownership, review, independent assurance, and risk-based release controls for every production change.
