MLB teams do not follow a fixed rule requiring a starter to leave after a certain number of innings, pitches, or trips through the batting order. A manager compares the starter’s effectiveness and readiness with the best available relief option for the next hitters, then weighs that immediate matchup against bullpen workload and the rest of the series.
What a manager is deciding
The choice is a live comparison between the pitcher on the mound and the alternatives who are ready to enter. The inning matters, but so do the score, the baserunners and outs, the hitters due up, and what each pitcher has shown that day. A starter who is still effective may be the better option; a rested reliever with a favorable matchup may be better for a high-leverage moment.

There is no source-backed universal threshold—such as always removing a starter after two trips through the lineup—that settles the decision. Managers may have access to private medical information and team-specific tracking, but the public evidence does not establish a single pitch-count, fatigue, or matchup trigger used across MLB.
How the main factors fit together
The starter’s current performance and readiness
Managers assess whether the starter is commanding pitches, getting outs, and equipped to face the next part of the lineup. A starter’s inning total alone does not establish whether he should stay or go: his present effectiveness and the upcoming hitters matter alongside his workload.

The next matchup and the leverage of the moment
A reliever may offer a particular matchup or a different look, and an opener can be used to change early matchups or disrupt hitters’ timing. But a manager cannot judge the choice solely by asking which pitcher is nominally a starter or a reliever. The useful comparison is between the pitchers available for this inning and the hitters they would face.
League figures cited in MLB’s 2018 discussion of openers showed batting average/OPS of .242/.712 the first time through the order, .252/.734 the second, .266/.786 the third, and .281/.781 from the fourth time onward. Those are historical league averages reported in 2018, not current rates or a rule for when to make a change; they do not determine what the best move is in an individual game.

Who is rested and available in the bullpen
A reliever’s value depends partly on whether he is ready to pitch and how much work he has already done. A postseason roster is fixed over the series, so asking the same small group of trusted relievers for repeated high-leverage innings can affect the club’s options in later games. A fresh arm can improve the immediate matchup, but using him now adds to the workload the team must manage.

What the move means for the rest of the series
Postseason plans can include conventional starts, openers, bullpen-heavy games, and starters entering in relief. A team may use a starter out of the bullpen if his quality and availability make him one of its best options right now, while accepting that the move could affect his next start.

MLB’s historical retrospective documented 67 pitchers making 80 sequences in which they relieved and then started again on three days’ rest or less in its qualifying sample. It reported 31 such occurrences since 2010 and 50 in the earlier period it considered. The sample excluded 2020 and required the next postseason start to follow the relief appearance on short rest; it documents that these sequences have occurred, not a universal workload plan or a recommendation for any particular pitcher.
What postseason history can—and cannot—show
MLB’s 2022-era retrospective reported that selected starters used in relief allowed a .263 on-base percentage and .564 OPS over 436 plate appearances. That is a descriptive historical result, not proof that a starter in relief will outperform the reliever a manager would otherwise have used: the comparison cannot identify the alternative pitcher for every situation.

Historical analysis also illustrates why there is no simple formula for starter versus bullpen workload. David J. Gordon’s Society for American Baseball Research article estimated that its examples showed up to a .076 wOBA penalty in “Red” relief situations, compared with an approximately .030 increase associated with a starter’s third time through the order. Those are the author’s estimates, not universal effects; the article notes it cannot separate repeated exposure to an opponent from fatigue.
MLB reported that relievers pitched 49.7% of postseason innings in 2018, compared with 39.4% of regular-season innings that year. Those figures show that relief pitching had a larger role in that historical postseason snapshot; they should not be treated as current rates or as evidence that one approach is always superior.

Why a reliever cannot always be used for just one hitter
MLB’s three-batter-minimum glossary describes a general restriction on removing a reliever immediately after a single matchup, subject to exceptions. That practical constraint affects how a manager can deploy the bullpen: the choice may require considering the hitters likely to come next, not just the one at the plate. For the current rule wording and exceptions, consult MLB’s official three-batter-minimum glossary.
A practical way to evaluate a change
To understand a decision in a particular playoff game, compare the actual options rather than applying a slogan such as “always trust the starter” or “always use the best reliever.” Ask:
- Is the starter still effective and ready to handle the next hitters?
- Does a reliever offer a better matchup for this inning and game situation?
- Which relievers are available and rested, and how much work have the most trusted arms already taken on?
- How might this appearance affect the rotation or bullpen in the next game?
The answers vary with the pitchers available, the opponent, and the series situation. The choice is an attempt to improve the club’s chance in the current game without ignoring the pitching options it will need afterward.
