How Federal Agencies Detect Fraud in Relief Programs

Federal agencies detect relief-program fraud through layered controls rather than one “fraud score.” Before payment, they verify identity, eligibility and application consistency; automated systems compare records across government and commercial databases; analysts use anomaly, machine-learning, entity-resolution and graph models; trained reviewers investigate alerts; and inspectors general, the Justice Department and U.S. Attorneys pursue recovery or criminal and civil cases. The strongest protection is pre-award screening backed by reliable cross-program data, but agencies continue finding fraud after money has been disbursed.

What the detection system is designed to find

Relief fraud can involve a fabricated applicant, a stolen identity, false payroll or wage information, duplicate claims, an ineligible business, an organized ring or an improper payment caused by error rather than intent. A data match is therefore a lead, not proof. Agencies must determine whether a record reflects deliberate deception, identity theft, a clerical mistake, eligibility confusion or a legitimate exception.

The same framework applies across programs such as the Small Business Administration’s Paycheck Protection Program (PPP) and COVID-19 Economic Injury Disaster Loan (COVID-EIDL) program, and the Department of Labor’s pandemic unemployment-insurance programs. Each program has different eligibility rules and administrators, so the fields and thresholds vary, but the operating sequence is broadly similar.

How a relief-fraud case moves from application to prosecution

  1. Risk assessment and control design. Program managers identify who is eligible, what documentation is required, which payment channels are vulnerable and what risks must be monitored. The Government Accountability Office found that pandemic agencies did not always manage fraud risk strategically or establish adequate controls.
  2. Pre-award screening. An application is checked against internal program records and multiple public and private databases. Systems look for identity conflicts, duplicate applications, invalid or suspicious identifiers, inconsistent addresses and other signals before approval or payment.
  3. Automated analytics. Rules, statistical tests and machine-learning models search for unusual values, impossible employment or wage information, repeated contact details, suspicious timing and overlaps with other benefits. The system assigns or recommends a risk level and sends selected records to reviewers.
  4. Human validation. Analysts inspect the underlying file, supporting documents and context. They may request clarification, correct a data error, approve the claim, deny it or escalate it. Human review is essential because an unusual pattern is not automatically fraud.
  5. Referral and investigation. Potentially fraudulent matters can go to an agency inspector general or a law-enforcement partner. Investigators reconstruct ownership, applications, bank movements, communications and relationships among people and entities.
  6. Recovery and enforcement. Agencies may stop a payment, deny loan forgiveness, recover an improper payment or impose administrative consequences. The Justice Department and U.S. Attorneys can bring criminal charges, seek civil penalties and pursue forfeiture or restitution.

What data agencies use

Detection improves when records from different programs can be compared lawfully and accurately. Typical checks combine information supplied by the applicant with program, identity and payment records.

Data category Examples of what it can reveal Important limitation
Identity and taxpayer information Stolen, invalid, duplicated or mismatched names, birth dates and Social Security numbers A mismatch can result from a typo, a name change or identity theft; it requires review.
Application and eligibility fields Conflicting business dates, legal structures, employee counts, revenue, addresses or ownership information Rules must reflect the specific program’s eligibility definition.
Payment and prior-benefit records Duplicate claims, repeated bank accounts, multiple recipients using one address or benefits received in overlapping programs Households and legitimate businesses can share addresses or financial institutions.
External public and private databases Whether an identity, business or address appears to exist and whether supplied details are consistent Coverage, freshness, legal authority and accuracy differ by source.
Investigative and enforcement records Links to prior cases, shell entities, known aliases or coordinated activity Access and use must comply with privacy, due-process and evidentiary requirements.

Cross-program matching is particularly valuable. A person using one identity for unemployment benefits and another for a small-business loan may look ordinary inside either program but anomalous when records are resolved together.

How analytics turn records into leads

Rules and duplicate checks

The simplest controls apply explicit conditions: a Social Security number that fails validation, the same bank account attached to many unrelated applicants, an employee count inconsistent with reported payroll, or repeated applications with nearly identical contact information. These checks are fast and explainable, making them useful at the point of application.

Anomaly detection and machine learning

Statistical and machine-learning tools compare an application with normal patterns for its program, location, industry or time period. An unusually high claim, a cluster of applications created within minutes or a sudden concentration of payments can receive a higher risk score. A score prioritizes scarce investigator time; it does not establish liability.

Entity resolution

The Pandemic Response Accountability Committee (PRAC) reported using entity-resolution methods to determine when differently formatted names, birth dates and other fields referred to the same person across benefits programs. This matters because “Maria L. Chen,” “Maria Lee Chen” and a record with a transposed birth date may be one individual, not three separate applicants.

Graph analytics

Graph models represent people, businesses, addresses, phone numbers, bank accounts and applications as connected entities. Investigators can then see dense clusters or bridges linking apparently unrelated recipients. PRAC used graph analytics to generate leads involving potential fraud rings and organized crime.

Automated monitoring and shared tools

PRAC also described robotic processes that flag anomalies for investigators, shared code and data-science fellows working across inspector-general offices, and risk models that prioritize high-risk recipients. Automation expands coverage, but agencies still need documented logic, version control, audit trails and a way to correct bad source data.

What the PPP and COVID-EIDL experience shows

The SBA documented a screening process that compared applications with several public and private databases and checked for internal inconsistencies. It also used machine-learning tools to review anomalies in PPP data.

GAO reported that more than 3.7 million unique PPP and COVID-EIDL recipients had fraud indicators among 13.4 million recipients analyzed. “Fraud indicator” means a signal requiring attention, not a finding that every recipient committed fraud. SBA referred more than 669,000 potentially fraudulent PPP and COVID-EIDL loans to its inspector general. SBA analytics contributed to $4.7 billion in PPP proceeds not being forgiven, a recovery or prevention result rather than a measure of total fraud.

Timing was a major weakness. The federal government distributed funds quickly during the COVID-19 pandemic. GAO found that the SBA’s complete COVID-EIDL process was not in place until more than $210 billion of the eventual $385 billion disbursed had already gone out. Later analytics could identify suspicious payments, but preventing a payment is generally easier and cheaper than recovering it years afterward.

How unemployment-benefit fraud is identified

Pandemic unemployment systems can compare claimant identity, employment and wage information, claim timing, payment destinations and records from other benefits programs. Signals may include a claim filed for a person who is deceased or incarcerated, impossible work histories, many claimants using one address or account, or multiple claims tied to the same identity.

GAO has cited an estimate of more than $60 billion in unemployment-insurance fraud. That figure is an extrapolation from a lower-bound fraud rate, not a direct audit total, so it should not be read as a precise count of proven losses. The same distinction applies to model-generated indicators: they help locate cases for review but do not replace evidence.

What happens after an application is flagged

1. The payment may be paused

If a flag arrives before disbursement, the agency may hold the application while confirming identity, eligibility or documentation. The exact notice and appeal rights depend on the program.

2. A reviewer tests the explanation

An analyst examines the alert, source records and documents. A legitimate explanation—such as a shared business address, a corrected name or a data-entry error—can clear the record. If uncertainty remains, the reviewer may request more information.

3. The agency chooses an administrative action

Possible outcomes include approval, denial, reduced payment, denial of loan forgiveness, recoupment or referral. Agencies should document the reason, evidence and decision so the action can be audited and challenged where applicable.

4. Investigators build a case when intent is suspected

An inspector general or law-enforcement team may interview witnesses, subpoena records, trace funds and connect related applications. A referral is not a conviction. Prosecutors decide whether the evidence supports criminal charges, civil litigation or another remedy.

5. Recovery and deterrence continue after judgment

Improper funds can be recovered through offsets, repayment agreements, civil judgments, restitution or forfeiture. Public prosecutions also deter repeat schemes, while lessons from completed cases should feed back into eligibility rules and monitoring.

What the government-wide numbers mean—and do not mean

Measure Reported figure and date How to interpret it
Federal COVID-relief prosecutions At least 1,044 people pleaded guilty or were convicted at trial of federal charges through January 13, 2023 (GAO) Counts resolved criminal cases, not all suspicious recipients or total losses.
PPP/COVID-EIDL indicators More than 3.7 million recipients with indicators among 13.4 million analyzed (GAO, 2023) Indicators are leads; they are not findings of fraud.
SBA referrals More than 669,000 potentially fraudulent loans referred to the SBA inspector general (GAO, 2023) Referrals require investigation and can produce different outcomes.
DOJ-related cases At least 2,191 individuals or entities faced fraud-related charges in COVID-relief and related cases as of June 30, 2023 (GAO) A charging count is not the same as convictions or a dollar-loss total.
Improper payments Federal agencies estimated $162 billion across 68 programs in fiscal year 2024; about $121 billion was concentrated in five program areas (GAO, 2025) Improper payments include payments made in error and are not synonymous with intentional fraud.

Why detection still misses fraud

  • Speed versus verification: Emergency programs must deliver money quickly, so controls may be introduced after substantial disbursement.
  • Fragmented data: Federal, state and local systems use different identifiers, formats and update schedules. PRAC found that data-quality and system limitations reduced consistency when tracing funds to local communities.
  • False positives: Shared addresses, seasonal work, name variations and legitimate relationships can resemble coordinated fraud.
  • False negatives: A sophisticated ring can use real identities, plausible wages and separate accounts that do not trigger simple rules.
  • Legal and privacy constraints: Agencies need lawful data-sharing agreements, purpose limits, security controls and accountable access.
  • Recovery difficulty: Once funds move through multiple accounts or jurisdictions, locating and collecting them is harder than stopping payment.

A 2025 PRAC alert examined stolen or invalid Social Security numbers in SBA PPP/COVID-EIDL and pandemic unemployment programs and argued that analytics-based vetting could have mitigated substantial potential fraud. The alert cited nearly $400 billion in combined fraud and improper-payment estimates from SBA and Department of Labor inspector-general work. That figure combines program estimates; it is not one audited loss total.

How agencies can improve the next emergency program

  1. Perform a documented fraud-risk assessment before launch and repeat it as the threat changes.
  2. Put high-value identity and eligibility checks before payment, while creating an exception path for applicants who need manual help.
  3. Use common identifiers and data standards so records can be matched across agencies and jurisdictions.
  4. Combine rules, statistical tests, entity resolution and graph analysis instead of relying on one model.
  5. Keep a trained human reviewer in the decision loop for adverse actions and provide a way to correct inaccurate data.
  6. Measure precision, recovery, processing time and disparate error rates, not merely the number of alerts generated.
  7. Connect analytics teams to inspectors general and prosecutors early so evidence is preserved and referrals are usable.
  8. Document model logic, data lineage and access so auditors can reproduce why a case was flagged.
  9. Analyze root causes after the emergency and permanently retain the controls that worked.

How to compare two fraud-detection approaches

For a proposed system, ask six practical questions:

  • Timing: Does it screen before award, or only identify payments afterward?
  • Data breadth: Does it examine one program, or connect relevant federal, state, local and external records?
  • Analytic method: Are duplicate rules supplemented by anomaly detection, machine learning, entity resolution and graph analysis?
  • Human oversight: Do trained reviewers validate flags before denial, recovery or referral?
  • Enforcement connection: Can investigators receive complete, timely evidence?
  • Governance: Are legal authority, privacy, data quality, auditability and false-positive accountability defined?
