It can register eligible hotkeys with Windows’ RegisterHotKey mechanism; it installs a low-level keyboard hook when a feature needs closer observation or handling of key events. Directives such as #InstallKeybdHook and #UseHook can also affect that choice.
Two ways AutoHotkey can handle a hotkey
For an ordinary shortcut that Windows can represent as a registered hotkey, AutoHotkey can use RegisterHotKey. This asks Windows to notify the application when the registered key combination is pressed. It does not require AutoHotkey to watch every keyboard event through a low-level hook.

When a script feature needs hook-level behavior, AutoHotkey uses a keyboard hook instead. The distinction is about what the hotkey or feature requires, not a universal rule that every AutoHotkey script uses one method.

RegisterHotKey |
Registers an eligible key combination with Windows so the application can receive its notification. | Hotkeys that do not require hook behavior. |
| Low-level keyboard hook | Lets AutoHotkey observe keyboard events near the point Windows is about to post them; a hook can also suppress an event. | Hook-dependent features and hotkeys, or when the script explicitly requests the hook. |
AutoHotkey’s #UseHook documentation describes directing eligible hotkeys to the hook rather than the normal registered-hotkey route. Features including hotstrings and input hooks can require the keyboard hook. The #InstallKeybdHook directive explicitly requests installation, including in a script that might not otherwise need it.

What the Windows keyboard hook does
At the Windows API level, a low-level keyboard hook is installed with SetWindowsHookEx using the WH_KEYBOARD_LL hook type. Windows calls the LowLevelKeyboardProc callback when a keyboard event is about to be posted to a thread input queue. The callback receives information about key-down and key-up events, including WM_KEYDOWN, WM_KEYUP, WM_SYSKEYDOWN, and WM_SYSKEYUP.
This callback is not code injected into every application that might receive a keystroke. Microsoft documents that it runs in the context of the process and thread that installed the hook, with Windows communicating with that thread. The installing thread therefore needs a message loop.

The callback can pass the event along or stop it from continuing through the hook chain to the target window procedure. In Windows’ callback contract, returning a nonzero value suppresses the event; otherwise, the hook should pass it onward as appropriate. Because the callback happens before Windows updates the asynchronous key state, it should not be treated as a way to query the just-changed state with GetAsyncKeyState.
Why the hook must return quickly
A low-level hook callback should do as little work as possible and return promptly. If it takes too long, Windows can silently remove it on Windows 7 and later. On Windows 10 version 1709 and later, Windows caps the permitted LowLevelHooksTimeout value at 1,000 milliseconds. That is a maximum timeout setting, not a recommended amount of work time.

Microsoft advises applications that need low-level hooks to consider a dedicated thread. For many cases that only need asynchronous monitoring of input aimed at other threads, Microsoft points developers toward Raw Input as a better fit. Raw Input is an alternative for certain monitoring needs, not a claim that AutoHotkey’s hook and hotkey features are interchangeable with it.
What the directives mean in an AutoHotkey v2 script
#InstallKeybdHookrequests that the script install the keyboard hook. It is useful when the script needs hook behavior even if another feature would not install the hook automatically.#UseHookdirects eligible hotkeys to use the hook. It does not mean every possible hotkey or feature has the same requirements.- Hotstrings and input hooks are examples of features that need keyboard-hook behavior, so their presence can lead AutoHotkey to install the hook without an explicit install directive.
The examples and directive names here refer to AutoHotkey v2. The download indexes list AutoHotkey v2.0.28 and v2.1-alpha.32, both dated 2026-09-12; v2.1 is an alpha prerelease, not a stable release. AutoHotkey v1 and v2 have different syntax, so check the documentation for the version you use before adapting script examples.

Physical keystrokes and SendInput are not the same thing
A physical keyboard event and a script-generated event do not necessarily trigger the same hook hotkeys. AutoHotkey’s Send documentation says that SendInput generally cannot trigger the script’s own hook hotkeys or InputHooks. It also documents circumstances where AutoHotkey falls back to another send mode if another low-level keyboard hook is installed. For that reason, do not assume that sending a key from a script is equivalent to pressing that key on a physical keyboard.

- Identify what the script is doing: a simple shortcut, a hotstring, input capture, or another hook-dependent feature.
- Check whether the hotkey is eligible for Windows’ registered-hotkey mechanism or requires hook behavior. The
#UseHookreference explains the choice between those routes. - Look for
#UseHookor#InstallKeybdHookif the script requests hook use explicitly. - If the question concerns simulated keystrokes, distinguish
SendInputfrom physical input and consult theSendreference for its interaction with hook hotkeys.
For Windows’ callback behavior and timing constraints, see Microsoft’s LowLevelKeyboardProc reference. AutoHotkey’s InputHook reference provides feature-specific context for input capture.
