Both use unsettling places and personal pain to shape psychological horror; Heartworm builds its story around Sam’s grief and memories, while SILENT HILL 2 centers James Sunderland’s guilt, grief, and existential turmoil. The comparison is strongest with that specific game—not with every Silent Hill entry, which has its own approach.
What are the games about?
Heartworm: grief and the possibility of contact
Heartworm follows Sam, who is consumed by grief after losing people close to her. She pursues an internet rumor about a house connected to the afterlife. Its official description frames the story around the memories people hold on to and those they wish they could forget. Heartworm’s official site and its Steam listing describe a personal, psychologically focused horror game.

SILENT HILL 2: guilt made inescapable
Konami describes SILENT HILL 2 through James Sunderland’s guilt, grief, and existential experience. The town is more than a location for encounters: Konami calls it a symbolic backdrop, enveloped in fog. That gives the game’s horror a different emotional center from Sam’s attempt to reach the dead. Konami’s SILENT HILL 2 page focuses on James and the remake.
How do their settings reflect inner life?
Both games make place feel psychologically charged, but that resemblance is a technique, not proof that their settings mean the same thing. In Heartworm, creator Vincent Adinoffi says media helps chronicle memory, while liminal areas connect otherwise unrelated spaces. He describes surreal spaces as possible glimpses into Sam’s psyche and a way to tell the story through the environment. In an interview with Adinoffi, he explains that these areas can be “a glimpse into the main character’s psyche” and a tool for environmental storytelling.
#1 Best Overall
- Fully Remade to terrify modern audiences
- KONAMI's intense psychological horror masterpiece SILENT HILL 2 is made exclusively for PlayStation 5
- The main protagonist James Sunderland searches for clues in the namesake town, after receiving a mysterious letter from his wife Mary… who has long been dead
- Experience the eerie monsters and other manifestations of James' consciousness reworked after 20 years
- Made with state-of-the-art technology to play on modern entertainment systems in 4K
In SILENT HILL 2, the town itself serves as a symbolic backdrop for James’s experience. The overlap is in how a place can carry emotional meaning, not in an established one-to-one map between a character’s mind and the environment. Other Silent Hill games use different characters, places, and fears.
What do the cameras and mechanics do to the horror?
Heartworm: distance, framing, and a camera that matters
Heartworm uses fixed cinematic camera angles and retro, low-resolution 3D imagery associated with late-1990s survival horror. It offers optional classic tank controls as well as modern controls. Exploration and puzzles are central, and Sam’s camera also functions as a light and a weapon. That makes the camera part of both the visual identity and the player’s practical response to danger. Steam describes the game as a 4–6-hour experience with multiple endings; that is the storefront’s estimate, not a measured playtime.

SILENT HILL 2 remake and Townfall: different perspectives
The SILENT HILL 2 remake uses an over-the-shoulder view, a closer perspective than Heartworm’s fixed compositions. SILENT HILL: Townfall, released September 24, 2026, takes a different route with first-person psychological horror and exploration, evasion, combat, and narrative-linked puzzles. These examples show why “the Silent Hill camera” is not a single franchise-wide rule. Konami’s Townfall page describes its first-person approach.

Heartworm has an original soundtrack alongside its visual and environmental storytelling. One useful comparison comes from Silent Hill 4, not SILENT HILL 2: series producer Akira Yamaoka described that game’s sharper, high-impact sound approach in connection with the fear of being chased and trapped. In an archived interview reproducing a PS World conversation, Yamaoka also said the series was originally conceived around “inner fear”—psychological terror. The archive is unofficial; these comments should be read as Yamaoka’s account of the series’ concept and Silent Hill 4, not as a description of every game’s sound design.

- Simon Ordell is called back to the island of St. Amelia to ‘put things right’, encountering a town lying quiet beneath a heavy fog, seemingly abandoned but not at rest
- Venturing deeper, and driven to understand his connection to the place and its inhabitants, Simon begins to discover fragments of a past rising to the surface
- Experienced entirely in first person, Simon must explore, evade, and survive using a limited set of weapons and tools, including the CRTV, a pocket television used to tune into unstable signals. Evasion is tense; combat is frenetic, while narrative driven puzzles reveal a truth that refuses to stay submerged
Is Heartworm a Silent Hill imitation?
Heartworm openly calls itself a psychologically focused love letter to late-1990s Silent Hill and Resident Evil. That makes the influence explicit, especially in its fixed-camera survival-horror presentation and retro imagery. But influence does not make the stories interchangeable: Sam’s grief, her pursuit of an afterlife rumor, and Adinoffi’s use of media and liminal spaces give Heartworm its own design and emotional focus.
Silent Hill itself has changed across entries. Yamaoka’s comments on the original idea of “inner fear” sit alongside the distinct emphasis he described for Silent Hill 4; the first-person design of Townfall is another current example of the franchise shifting its methods. Konami reported that the SILENT HILL 2 remake had more than five million cumulative players as of January 31, 2026, counting physical and digital sales, store downloads, and subscription services. It reported 17.8 million Silent Hill series units worldwide as of the end of June 2026, counting units shipped, store downloads, and subscription services. Those figures describe reach, not how frightening or psychologically effective either game is. Konami’s 2026 player announcement and series figures give the respective totals.

Choose based on the kind of psychological horror you want, rather than assuming one is a direct substitute for the other.
- Try Heartworm if you want a shorter, retro-styled survival-horror game built around grief, memory, exploration, puzzles, and fixed-camera framing. It launched on Steam on July 31, 2025; reported releases also include PS5, PS4, Xbox Series X|S, Xbox One, Nintendo Switch 2, and Nintendo Switch. Check the relevant regional store for current platform availability. GamesRadar’s coverage reports the broader platform releases.
- Try the SILENT HILL 2 remake if you want James Sunderland’s story of guilt, grief, and existential horror in the symbolic town. Konami announced it for PS5 and PC and later reported availability on PC, PS5, and Xbox Series X|S. Its camera perspective differs from Heartworm’s fixed shots.
- Consider Townfall if you want a current first-person example of Silent Hill using exploration, evasion, combat, and puzzles. Konami lists it for PS5, Steam, and the Epic Games Store.
The main distinction is emotional and formal: Heartworm turns Sam’s grief and memories into a retro, fixed-camera journey, while SILENT HILL 2 shapes horror around James’s guilt and the symbolic town. The wider Silent Hill series is a changing tradition, not one fixed formula.
