In the United States, an FDA-approved generic must match its specific reference brand medicine in active ingredient, strength, dosage form and route of administration, demonstrate bioequivalence, and meet quality standards. A generic may look different and use different inactive ingredients; those differences generally do not change how it works. For questions about a particular medicine, check the exact drug and talk with a pharmacist or prescriber.
Are generic drugs as safe and effective as brand-name drugs?
FDA describes generic drugs as “FDA-approved copies of brand drugs.” That applies to products approved by FDA—not every medicine called a generic everywhere. An approved generic is evaluated against a specific reference brand medicine, and must have the same active ingredient, strength, dosage form and route of administration. It must also demonstrate bioequivalence and meet FDA quality standards. FDA’s generic-drug questions and answers explain these requirements.

Bioequivalence means the generic delivers the same amount of active ingredient at the same rate and to the same extent as the reference medicine. It does not mean the products must look identical. These requirements are the regulatory basis for expecting the same clinical benefit and risks when an FDA-approved generic substitutes for its specific reference drug. Do not assume that two medicines with similar names—or products sold as generics in different jurisdictions—are interchangeable.
Why does my generic pill look different?
U.S. rules allow a generic to differ in appearance, such as color, and in some inactive ingredients. FDA says these differences generally do not change how the medicine works. A minority of patients may have tolerability or absorption concerns related to inactive ingredients, so an individual reaction is worth discussing with a clinician or pharmacist; it does not show that generics as a class are less effective. FDA’s consumer guidance describes the permitted differences.

There is also a category called an authorized generic: FDA describes it as the same product as the branded product, apart from the brand name on the label. A tablet or capsule may still differ in color or markings. Authorized generics are marketed under the brand product’s new drug application (NDA), so they are not listed among generic approvals in the FDA Orange Book. FDA’s authorized-generics page explains the distinction.
Can generic drugs cause different side effects than brand-name drugs?
The active ingredient and the FDA-required standards underpin the expectation of the same clinical benefit and risks for a generic and its reference medicine. However, allowable differences in inactive ingredients can matter to a minority of patients—for example, if a person has a sensitivity or experiences a tolerability or absorption concern. A problem after a change is a reason to ask a pharmacist or prescriber to review the exact product and ingredients, not a reason to change or stop treatment on your own.

FDA reviews generic and brand-name medicines against quality standards and monitors approved products for safety signals after approval. If a medicine seems not to work or causes a problem, FDA advises talking with a doctor or pharmacist and considering a report to FDA. FDA’s generic-drug facts describe its quality oversight, and MedWatch is FDA’s safety-information and adverse-event reporting program.

Generic manufacturers generally do not repeat the animal and human studies that established the original drug’s safety and effectiveness. Competition among generic manufacturers can also put downward pressure on prices. FDA reports that one generic competitor can lead to price reductions of 30%, and five generic competitors are associated with drops of nearly 85%; the fact sheet does not state a publication year for those figures. FDA also reports an IMS Health Institute estimate that generics saved the U.S. health care system $2.2 trillion from 2009 to 2019. These are broad historical figures, not a price quote or savings guarantee for an individual prescription. FDA’s fact sheet provides the figures.

Not every prescription medicine has an approved generic. Availability is specific to the drug and may change. Use FDA’s Orange Book to check whether a particular brand-name drug has an approved generic. An entry is not a blanket statement that every product with a similar name is an equivalent.
